Michael came to Markle Health and Rehabilitation after having Bilateral Knee replacements. He participated in therapy in order to return to his prior level of function and was able to go home after 8 days. 12 Weeks after surgery Michael was able to return to work and officiate basketball games. Michael states therapy pushed him and he didn’t like it but he knew he had to do it if he wanted to return home and to work. Michael is happy with his choice and back to work at the school!

STATE OF THE HEART HOSPICE:

H
ighways 67 and 26 merge at a wide curve just outside Portland, Indiana. Inside that generous sweep at the western edge of the county seat stand two landmark broadcast towers. When a young girl suffering from an incurable disease came to State of the Heart Hospice, Director of Patient and Family Services Mindy Stebbins helped her create an end-of-life video. “We called it her ‘reflections of life,’ and we went over some of her best memories from
when she was younger,” Mindy explains. “She was able to talk about her family and what they meant to her. “It was something her parents didn’t see until after she had passed away. That was a very special experience for me, and it was a gift to them.” State of the Heart Hospice is a non-profit agency that serves families and patients in western Ohio and eastern Indiana. Its offices are located in Greenville and Coldwater, Ohio, as well as Portland, Indiana.

“I have a heart for hospice,” says Mindy, because “it is different from any other type of health care. Hospice is more journey-focused than it is task-focused.”
Mindy continues, “When you go to a doctor or hospital, you go for a task; you are there to get something fixed. You want to find out why something is not working, and you want it fixed. “When folks have reached the end of life, they are not looking for a cure or a fix. They are looking for comfor t. “Hospice care treats the person, rather than the disease; it focuses on quality, rather than length, of life.” Hospice care is centered on the patient’s family, involving them and the patient in decisions. For those with incurable diseases, Mindy says, “Hospice provides humane and compassionate care. The goal is to help them live as fully and comfortably as possible.” A common myth is that hospice is for older people or people with cancer. However, Mindy advises, State of the Heart cares for people of all ages. She offers another personal glimpse from her experience. “A mother was told her baby would never live, that it would die at birth. The baby actually lived for several days in the

hospital, and we were able to bring her home with State of the Heart Hospice. “That baby was able to die at home. The mother never dreamed she was going to be able to bring her baby home, or even to hold or to have that time with her baby.” State of the Heart Hospice accepts that death is the final stage of life. The staff’s goal is to enable patients to continue an alert, pain-free life, while managing other symptoms.

The patient’s last days should be spent with dignity and quality, surrounded by their loved ones. Hospice affirms life and does not hasten or postpone death. Most hospice care in the United States is given in the home, with a patient’s family serving as the main caregiver. State of the Heart Hospice cares for patients at home, as well as in hospitals, in nursing homes, or in private hospice facilities. Acknowledging the importance of professional staff, Mindy says, “We have a really great team. They give you and your loved ones complete palliative care aimed at relieving symptoms and giving social, emotional, and spiritual support.” She continues, “Our hospice care begins in Admission, where we assess the patient and get them admitted to our program. From there, our primary nurses and aides take over and handle the care on a daily basis. “We have music therapists and social workers who are involved with all aspects of the dying process, whether it is helping our patients prepare for that, or helping their families deal with their grief.” State of the Heart Hospice’s nurses are certified in hospice and palliative care. Other staff members in contact with patients, such as the music therapists and social workers, are certified and/ or licensed.

State of the Heart Hospice may offer spiritual care to meet individual religious needs. It also helps family members stay involved with patients’ health care should they be admitted to a hospital or other care facility. Especially in home hospice care, family and caregivers may need some time away. State of the Heart Hospice can offer them a break, which is often arranged in durations of up to five days at a time. Families can plan a mini-vacation, attend special events, or simply get much-needed rest at home while the patient is cared for in an inpatient setting.

Family members may also find great support and stress relief through family conferences. These conferences may be done informally on a daily basis, as the nurse or nursing assistant talks with patients and their caregivers during their routine visits. In the hospice journey, bereavement naturally follows. Bereavement counseling is provided at no cost for those who need it. Through this counseling, family members find support from a trained

volunteer, counselor or clergy member who will visit them, call them, and write to them. Family members can also find help through support groups. Bereavement services are provided for up to a year after the patient’s death. State of the Heart Hospice also offers Camp BEARable, an annual event for children ages 6 to 15, aimed at helping them deal with grief and loss issues. It is free to participants and is held at the Spiritual Center at Maria Stein, Ohio. The camp is supported financially through donations. A smaller but very vital portion of State of the Heart’s hospice services are provided by volunteers. Hospice volunteers don’t need any prior experience, Mindy says. “They just need to have a good and willing heart.” Those interested in volunteering are encouraged to apply. Volunteers may visit with patients or run errands or assist at health fairs to help raise awareness about hospice care. State of the Heart Hospice’s website assures, “Volunteering is one of the most powerful ways to make a statement of joy and honor with your life.”

LEHMAN & BIXLER PROFFESIONAL EYE CARE:

W
hen considering careers, both Dr. Scott Lehman and Dr. Scott Bixler of Lehman & Bixler Professional Eye Care knew they wanted to pursue some aspect of healthcare but were uncertain where they’d find their niche. A routine visit to the eye doctor during Scott Lehman's decisionmaking process provided the spark that led him to consider optometry. Dr. Lehman explains, “I wanted to get into healthcare... I was in pre-med, and I was trying to decide what I wanted to do.” Scott attended Valparaiso University for his undergraduate degree before enrolling at Indiana University School of Optometry
in Bloomington. He completed the program in 1985. Upon earning his degree in optometry, Dr. Lehman adds, “I joined my optometrist from childhood, Dr. Fred Sprunger, at the Family Eye Clinic. He died a year and a day after I started with him in 1986. “I worked there almost a year after that, trying to come to an arrangement with Dr. Sprunger’s family. In the end, it didn’t work out. So, I opened my own office.” The building that Lehman & Bixler Professional Eye Care calls home today was

not always used as a health care office. “It had been a car dealership until about five or six years before I bought it,” says Dr. Lehman, “and then, it was a sporting goods store. “Tom Dull had a sporting goods store here, and I bought it from him.” From there, the facility grew into the thriving business it is today. As his practice expanded, Dr. Lehman eventually took on Dr. Scott Bixler as a partner in the summer of 2001. “He had said he was interested in optometry. He talked to me in high school, did four years of college, four years of optometry school, and four years in the air force. So, it was about 13 years until he joined me, but we knew each other pretty well by then.” Before partnering with Dr. Lehman, Dr. Bixler’s choice to study optometry was influenced, he says, by “a variety of things,” including “different conversations, and an interest in healthcare with a focus on working with people and science.” Like his partner, Dr. Bixler explains, “I enjoyed science, research, studying, and I knew I wanted to do something in healthcare but wasn’t sure what.”

Dr. Bixler continues, “I remember when Trent Lehman and I were going up to Fort Wayne for a summer basketball league, and we were talking about what we wanted to do. I had originally thought about accounting or psychology. “Trent said something like, ‘I think I want to be an optometrist,’ and I started thinking about it. I’d never thought about that particular field.” Sometime later, Scott enrolled at Saint Joseph’s College, south of Chicago, and graduated with a double major in chemistry and biology; he then made plans to attend optometry school at IU in Bloomington on a military scholarship. “I thought it would be nice to limit the loans,” he says, “so I looked into an Air Force scholarship. “The catch was, if you were accepted for the scholarship, you owed them a year of service for every year of college they paid for. I didn’t know much about the military, but from talking to different people, I decided it was whatever you made of it.” Dr. Bixler served as a Captain in the United States Air Force at a base in North Dakota for four years as an optometrist before returning to Berne to join Lehman as his partner. Speaking of his military experience compared to his work in town, Scott says, “It was just a different model. “I enjoyed it, looking back. You were a part of the hospital. You built a lot of relationships.”

“For example, the OCT (Optical Coherence Tomography)–it uses light waves to look at the inside of the eye, like an ultrasound. We use that for things like macular degeneration and glaucoma to see how a patient’s doing. That didn’t come along until I’d been in practice for almost 20 years!” Dr. Bixler echoes his partner, “We (optometrists) have become a lot more comfortable with treating ocular disease than we were 20 and 30 years ago, just because of the technology and education that we have.” Though both Lehman and Bixler experienced uncertainty in deciding

I thought I’d like optometry, but I’ve found that I enjoy the healthcare aspect, too. S
ara Rhymer had joined Anytime Fitness in Decatur, Indiana while she was training for the 2009 Fort for Fitness halfmarathon event. When the owners of the gym approached Sara about purchasing the Decatur franchise, she responded with a very enthusiastic “Yes!” In fact, when the Decatur location first opened in 2008, Sara, who’d been teaching health and physical education at Monmouth and Northwest elementary schools for the past 12 years, was frustrated. She recalls, “I felt like I had missed the boat. “I was kicking myself, thinking, ‘I could have done this.’”
Fortunately, the opportunity was not lost but only delayed. When Sara lost her job with the closing of Monmouth Elementary in 2010, the offer to purchase the Anytime Fitness franchise was like “God closing a door and opening a window.” With plans to open more gyms in the Fort Wayne area, the original owners happily sold the Decatur facility to Sara and her husband Chris. “It was a very easy transition to come here,” remembers Sara, noting the benefit of her background in physical education. “Anytime Fitness is a global company.” According to Sara, current statistics as of the last week of July show the franchise company

includes 2,177 clubs in North America and 720 more locations in 20 other countries. In 2014, Anytime Fitness was ranked #1 on Entrepreneur magazine’s prestigious “Top Global Franchise” list and #2 in the “Franchise 500” list. Sara recalls the valuable information she received during her training at the corporate headquarters in Hastings, Minnesota. “The main reason people leave is they haven’t made a connection at the gym, to the staff or to the workouts.” She and her staff focus on making that personal connection with each of their members. The gym had 442 members when the Rhymers purchased it five years ago. “Right now, we have 660 members. My goal is to stay above 700 members year-round.” Another of Sara’s strategies for membership consistency involves training her clients. “We try to educate our members and help them use the gym so they aren’t intimidated or frustrated and don’t get bored,” she explains. “I really want them to learn how to use the whole gym.” New members go through an orientation that stresses safety and instruction in the use of the equipment. This process includes a 50-minute personal training fitness consultation to point

Memberships range in price from $29.95 to $40 per person per month, depending on a variety of factors, such as length of contract and corporate or student discount. “Each additional family member is $25 per month.” Sara adds, “We do have quite a few of the bigger companies who offer a discount for their employees.” Customers may choose from a one-month or 12-month contract. All payment is received via automatic withdrawal. Drop-in day passes are available during staff hours for visitors to the area, or for parents to bring a child along as a trial basis to gauge the child’s interest in a membership. The rate for students or members of the military is $3 per day, with an adult pass costing $5 per day. “We see a lot of people who are having their motorhome serviced at Fleetwood,” Sara adds. The gym contains a wide variety of equipment, which explains why the facility appeals to members of all ages, from the high school athlete to their grandparents. Their goal is to help customers broaden their exercise horizons beyond the treadmill—the most familiar piece of equipment. “The cardio equipment is used by everyone,” Sara observes. Each piece of cardio equipment is includes personal television. Wi-Fi access is also available in the gym. The facility offers both weight machines—a safer means to participate in strength training—and the more traditional barbell weights.

Clients may opt to hire one of the personal trainers at Decatur’s Anytime Fitness for more one-on-one guidance and instruction. “We try to match the trainer with the customer, according to the customer’s personal fitness goals and the time of day both the client and trainer are available.” The gym’s busiest hours are from 6 a.m. to 10 a.m. and then again in the evening from 4 to 7. The Decatur location is staffed from 10 a.m. to 1 p.m. and from 5 to 8 p.m. on Mondays, Tuesdays, Wednesdays and Thursdays; staffed hours on Fridays and Saturdays are 10 a.m. to 1 p.m. only. A
fter several years of practice, Dr. Teresa Nicholson says her expectations about a dental career have changed “very little.” She explains, “I was raised in a ‘dental’ household. I already had a good idea of what to expect from a career in dentistry.” Although Teresa’s father practiced in Bluffton, Indiana for 25 years, she wasn’t interested in entering the field of dentistry. At least, it wasn’t a serious consideration until she went away to college. Teresa realized that dentistry was not only a rewarding career, but it would also offer her more freedom than other healthcare fields to have and enjoy a family at the same time.
Teresa earned her Bachelor’s Degree in chemistr y at Butler University before attending the School of Dentistry at Indiana University. She completed her Doctor of Dental Surgery degree in 2004. Following graduation, Dr. Nicholson worked at a group practice in Evansville, Indiana for a couple years. She returned to her hometown then, and found a position in Fort Wayne. In 2011, Dr. Nicholson accepted a position at a dental practice in Muncie. Within a year, she had the opportunity to take ownership. Teresa and her husband Shawn purchased the business in January 2012, officially

establishing a new practice, Nicholson Family Dental. In the meantime, a Bluffton practitioner was winding down a lengthy career in dentistry. By early 2014, Dr. Rob Evans had decided it was time to hang up his dental instruments for good. The Nicholsons were pleased at the thought of expanding Teresa’s practice to Bluffton. She explains, “Since Bluffton is our home, we have been naturally drawn to work in the community and provide care to our friends and neighbors.” So, in April 2014, Nicholson Family Dental took over the Bluffton office formerly occupied by Dr. Evans. “I provide the typical services of most general dentists,” says Dr. Nicholson. This would include preventative care, restorative dental services and cosmetic dentistry, offered at both the Bluffton and Muncie locations for patients of all ages. “However,” she adds, “I do have a CT scanner in my office that allows me to see beyond the average dental x-rays. Not only can I plan implant surgeries with greater success and efficiency, I am also able to provide better diagnostics.”

While Dr. Nicholson’s Bluffton and Muncie offices are only separated by a couple county lines, she says there are differences between them. “The practice’s characteristics are synonymous with the cities they are in. Muncie is a transient, busier community, while Bluffton has that ‘hometown’ feel, where you know your neighbor.” Dr. Nicholson says the best part of her job is the “artistic component of working on teeth and the creativity in accomplishing the procedures.” The work is not without its challenges. “On occasion, a patient will present with a dental or medical issue in which I do not immediately have the exact answer— that is the exception and not the rule. “But, I am a ‘fixer,’ and I want to have an answer so the problem or issue is managed efficiently and effectively.” What is the most unusual dental problem Dr. Nicholson has encountered? “Many years ago,” she says, “I examined a patient who had two complete sets of third molars—wisdom teeth.” However, most dental problems at the Bluffton office are not as exotic. According to Dr. Nicholson, the most common treatments needed here are for Temporomandibular Dysfunction (TMD)—problems with the jaw and the muscles that control it—and cracked teeth. Her advice to all patients is simple. “Proper home care with routine dental visits can prevent major dental problems.” When Dr. Nicholson is not in one of the two offices, she enjoys time with family, traveling, and scrapbooking. Teresa and Shawn, who serves as the Business Manager for Nicholson Family Dental, are the parents of two girls: Grace, 10, and Olivia, 7. Dr. Nicholson jokes, “They are a little too young to assist me! But, they do some cleaning at the office and help with a few non-clinical tasks.”

I
n May of 2014, the skilled nursing and rehabilitation center on West Lancaster Street in Bluffton, Indiana was purchased by Signature HealthCARE, a corporation headquartered in Louisville, Kentucky. Formerly known as Meadowvale Health and Rehabilitation Center, the facility was renamed Signature HealthCARE of Bluffton. Signature HealthCARE owns nine nursing care facilities in the state of Indiana, as well as dozens of facilities in seven Midwestern and southern states. In 2012, the company was named in Modern Healthcare’s “Best Places to Work” and U.S. News & World Report’s “Best Nursing Home.” It was also named by Inc. 5000 as one of the “Fastest Growing Private Companies.”
The company’s vision is “to radically change the landscape of long-term care… forever!” Signature HealthCARE is governed by the principles found in its mission statement: “Our family-based organization will revolutionize the Long Term Care industry through a culture of resident-centered healthcare services, personalized spirituality, real quality of life initiatives and stakeholder education and empowerment, to earn the trust of every resident, family and community we serve.” At Signature HealthCARE of Bluffton, the bright, flower-themed décor provides a warm, inviting atmosphere to the facility, which currently offers 91 beds. The services available consist of skilled nursing care, rehabto-home care, and both in- and out-patient

The newest addition, known as “The Bridge,” is a memory care unit that features a quieter environment structured to the needs of those with memory impairment. While most of the facility’s patients are receiving longterm skilled nursing care, others take advantage of the rehab-to-home services following an illness, emergency situation or surgery. “We have excellent therapy,” says Marketing and Admissions Director Bobbie Banks. “We are known for our therapy.” Depending on the patient’s treatment plan, a short-term resident’s stay can range from two weeks to two months. “We have patients come to us short term for IV antibiotics,” Bobbie advises, noting that age is not a factor, if the patient’s condition warrants the services of a nursing facility. Signature HealthCARE of Bluffton CEO Sally Sharp has many years of experience in various capacities related to nursing home care. “One of the things I love and respect about Sally is how she interacts with the residents and staff. You can’t help but respect that,” says Bobbie.

While the government establishes patient-to-staff ratios for nursing home care, Signature HealthCARE of Bluffton goes a step further. The company uses "acuity-based staffing" that takes into account the level of care its current residents require. The equation is continually reviewed to ensure the best level of care. “Nursing homes have gotten a bad rap,” Bobbie says, “as people tend to think, ‘This is the last step before I die.’ That’s not always the case. Rather, ‘This is where I get to go to live out the rest of my life and enjoy it.’ “At Signature HealthCARE, we call it ‘Quality of Life,’ meaning ‘continuing to live life to the fullest extent possible, regardless of age or physical challenges.’

“We have a lot of group activities, but we offer individualized activities, as well. When you have group activities like bingo and crafts, it promotes socialization. They also plan outings to Walmart and to Richards or The Corner Depot for lunch.” She says her staff members are pleased when the residents make connections and friendships blossom. “Typically, we host a large community event each month. In August, we held a hog roast, featuring entertainment by the Mountain Dew Boys. In September, we’re hosting a car show.” These community events are free for anyone to attend. “ I n O c t o b e r, c o m p a n y- w i d e , each Signature facility will host a Hall of Fame Café. We will honor two residents, one stakeholder (employee), and one person from the community.” Enthusiasm bubbles from Bobbie as she relates the details surrounding this favorite event. “We go all-out to celebrate these people.” A picture of those honored will hang in the dining hall.

A big Thanksgiving Dinner is in the works for the residents and their families in November, and a Christmas Open House is slated for December. “We’re always looking at doing something different to bring people together.” Weekly staff meetings at Signature HealthCARE of Bluffton include a devotional and prayer. Church services are held throughout the week, hosted by various local pastors. People from the community host hymn sings and weekly Bible studies, and on occasion, local youth choirs perform. “We are a non-denominational, faithbased facility,” Bobbie explains. “One of the things that sets us apart from many facilities is that we employ a full-time chaplain. She is here for the residents and their families, the staff, and even the community.” The focus at Signature HealthCARE of Bluffton is resident care. “Everyone here answers call lights,” says Bobbie. “We are here for our residents.

key line from one of my favorite movies is, “There’s no place like home.” For many people, there’s just something about returning to familiar surroundings that puts a smile on your face and a spring in your step.
made the leap to “the big city,” attending the American Music and Dramatic Academy of New York and Los Angeles and graduating in 2004. For a while, Sarah worked in Los Angeles in the entertainment industry and helped support herself by teaching piano.
On the other hand, it’s sometimes difficult to be the adult you’ve become among people who knew you as a child. There is a tension that exists between the expectations of others and the private knowledge of what one is capable of offering to the world. Often, leaving your hometown is the only way to pursue your dreams.
Sometime later, she met and married her husband, Rich Humbach, in Detroit, Michigan. Together, the pair made the decision to move to the Van Wert area.
Sarah Hope Wells definitely understands this concept. After graduating from Crestview High School and Otterbein College, she

like Toledo and Columbus just a few hours away.” Back in Van Wert, Sarah and Rich began to look for a location to establish a joint venture, quickly finding a location on North Washington Street ver y close to Main Street. “We originally wanted to build a recording studio, but our location isn’t a good one for that. “We have two competitors— truck traffic on Route 127 and the railroad, which is just north of our building. Neither one is conducive to the kind of soundproof area required of a recording studio!” S arah continue s , “ We decided to pursue our primary joy in life—creating things—and opened The Bachwell Center for Music and Dance on September 13, 2014. “The building is still a work in progress, but we were able to add a few walls in the front of the space. We created a waiting area in front of the first studio area and a separate, smaller teaching area behind the studio.” When asked about the origin of the name “Bachwell,” Sarah laughs and says, “Well, my last name is Wells, and Rich’s last name ends in ‘-bach.’ So, we combined them to make ‘Bachwell,’ which sounds rather sturdy, and yet artistic!” Indeed, the storefront has an art-

deco look and feel to it, lending to its impression of both a firm foundation and an artistic bent. In this facility, Sarah and Rich began to offer piano and dance lessons to young students in the area. “Dance classes are a large part of our studio work here,” Sarah reports. “We offer dance classes for all ages. “Currently, for adults, we offer ballroom dance, line dancing, and tap classes. These are ‘drop-in’ classes—you can come and go as you like. We encourage adults of all ages and ability levels to participate in the fun.” Sarah and Rich have also taken advantage of today’s trend toward fitness. Zumba classes have been very popular, offering a way of combining exercise—something many people tend to avoid—with a fun group activity that feeds the need for social encounters. Zumba is an aerobic fitness program featuring movements inspired by various styles of Latin American dance and is performed to a host of Latin rhythms. On the other end of the spectrum, the curriculum at the Bachwell also includes yoga classes. “The yoga classes are more low-key, focusing on stretching muscles and promoting physical balance.” Sarah smiles. “They’re much less stressful than the Zumba sessions!”

Along with younger children and teens, adults can sign up for private lessons in voice, piano, guitar, and drums. “These are the instruments we’ve been able to offer thus far,” explains Sarah. “All our instructors are locally-based. “We’re still looking for instructors in bass guitar, brass and wind instruments, including trumpet, clarinet and flute, and orchestral string instruments, such as violin. We’re also on the lookout for another piano instructor.” Similarly, Sarah would like to expand her dance instruction to include adult clogging and ballet classes. “Our

ultimate goal,” Sarah confides, “is to establish a dance company made up of both adults, teens, and children—a real universal group. That would give us a chance to perform in the area and offer some artistic fulfillment to the people of the Van Wert area.” A unique event at The Bachwell is the “Impromptu Friday,” an adult afterhours “dance party” that allows locals to come and relax in a fun social setting. “This led to the establishment of the LifeTree Cafe,” says Sarah, “which meets in the rear of our establishment.” The LifeTree Cafe is open Thursdays at 7 p.m. The weekly event provides an hour-long experience packed with interesting stories, fascinating people and lively conversation “that will feed your soul,” says a brochure. “We offer refreshments, but the main entrée is ‘food for thought’,” Sarah explains. Past discussions have included such topics as: “Upsizing Life by Downsizing Stuff—Living Large in an 84 Square-Foot House”; “Modern Romance”; “How Does Prayer Work? Exploring the Mystery of Connecting with God”; and, others.
