Easing Fears and Finding Comfort Common Hospice Questions Answered

In the past few years, Americans have learned we must have some uncomfortable conversations if we want to make progress. One topic that makes nearly everyone uncomfortable is death. Ironically, talking about end

of life should include something that makes everyone more comfortable: hospice care.

Hospice is comfort-focused, wholeperson care for end of life. It combines

medical care with emotional and spiritual support. People choose hospice care so they can prioritize quality of life, but if you wait to learn about hospice until it’s time to make that choice, whether for yourself or a loved

one, you could miss out on some of its benefits. Let’s make some progress by exploring four common questions about hospice care.

Is hospice only for the last few days of life?

Despite what you often hear when celebrities have hospice care, it’s not just for the last few days. Two doctors must certify that a person meets hospice eligibility criteria, usually those defined by Medicare: one, the patient has a life-limiting illness; two, the patient is expected to live six months or less; and three, the patient has a primary goal of comfort—not cure. Patients do not need to be bedbound or even homebound to be in hospice care. When someone has hospice care for several weeks or months, rather than only days, there’s more time for hospice staff to focus on their wishes and comfort. There’s also emotional and spiritual support to help all prepare for the death and the patient’s family to cope with grief afterward.

Is hospice care expensive?

Not only is hospice care one of the most complete forms of care you can choose at the end of life, it’s also very affordable. In Wisconsin, our local nonprofit hospices have some of the most innovative services and facilities available anywhere in the country.

Hospice care is typically paid for by Medicare, Medicaid, or private health insurance. For most patients, this leaves little or nothing for them or their families to pay out of pocket, even for medicines and medical equipment they may need in the last months of life.

Do I need a doctor’s referral to start hospice?

No. Anyone can start a conversation with a hospice, whether it’s a prospective patient, a family member or friend, their doctor, or another caregiver. Choosing a reputable hospice provider will ensure that your doctor will remain involved as part of your care team, working closely with the hospice staff to create a personalized care plan.

When people enroll in hospice care, do they have to give up Medicare?

When a person with Medicare enrolls in hospice, their standard Medicare benefits continue for items and services that are not related to their terminal illness. Care that is related to their terminal illness is typically paid by the Medicare hospice benefit. Related means that the treatment, medicine, and equipment is provided by the hospice to help manage the symptoms of the terminal illness. It’s for comfort— not cure.

Hospice care can be incredibly helpful to both the patient and to their loved ones who are caring for them: their spouse, children, friends, and neighbors. This service offers much-needed support and peace of mind during a very stressful time.

Solar Energy Investment Helps Contain Costs for Retirees

The sun is a hot commodity in a senior neighborhood on Madison’s far west side. That’s because the sun’s energy is new every day, and it’s being harnessed to lower the monthly expenses of retirees living in Attic Angel’s Prairie Point neighborhood of homes.

So far, 50 Attic Angel homes have been outfitted with solar panels, with a plan for all 123 to receive the energyproducing enhancement. The cost of

the initial investment differs by home size, energy needs, and orientation toward the sun. Various incentives, like federal tax credits, can reduce the cash outlay required. Whether the goal is to feel good from going green or from saving green, the long-term savings and sustainability leave seniors with a spring in their step year-round.

“Attic Angel is a leader [in senior living] for using renewable energy,” explains

Margo Hansen, Prairie Point resident living with her husband, Dr. John Hansen. As a longtime volunteer and member of Attic Angel Association, Margo knew about Attic Angel Community’s use of solar to generate power for its nearby senior living facility starting in 2018.

costs in check. The organization initially installed solar panels on just two wings of its facility, followed by a third array of panels in 2019.

Subsequently, the Hansens were among 46 families who chose to add solar to their Prairie Point homes. Shortly after moving to Prairie Point in 2022, they installed an array of 26 panels on their home, providing enough electricity to meet all of their household needs and fully power their two electric cars. They’ll even receive a check from the energy company for excess production their home creates used by other customers.

“Every month, except January, we’ve received a negative bill,” says John.

“Which is positive!” Margo interjects. January’s cold and snowy month left them with about $42 to pay out of pocket.

In June, the month with the maximum amount of sunlight, they produced 1400 kilowatt hours of electricity and saved money in three ways:

• They used 652 of the 1,400 kilowatt hours for personal electricity use, thus avoiding $85.37 in energy cost.

• The 748 kilowatt hours of excess electricity produced was sent back to Alliant Energy, resulting in a cash credit of $52.90.

• The 652 kilowatt hours used included charging their two electric cars, so they paid nothing to energize their vehicles.

Combining their enthusiasm for such bottom-line successes, love for the environment, and concern for future generations, the Hansens contributed a major gift to Attic Angel Community to establish the MJH Solar Fund, which will provide solar energy for the remaining 77 life-lease homes in their neighborhood.

“Margo and I feel a personal responsibility to support programs that increase the use of renewable energy, decrease emission of greenhouse gases, and reduce the harmful effects of global warming on the planet,” says John. Prairie Point has become the new staple of what seniors should expect when they join a senior community. There’s no age that can be reached where a person stops thinking about the future and can’t serve as a beacon to their peers. Shine on!

For those households that don’t meet the income qualifications, Project Home’s Optimus program offers an exciting option. UNDERSTANDING SPINAL STENOSIS and a Path to Relief through

Minimally Invasive Lumbar Decompression (MILD)

Spinal stenosis is a debilitating condition that affects many individuals, particularly as we age. Spinal stenosis involves the narrowing of spaces within the spine, causing compression onto the spinal cord and nerves coming from the spinal cord that go to muscles throughout your body. This narrowing can occur in most parts of the spine but is most common in the lower back (lumbar spine), where it can significantly impact a person’s function and quality of life.

The Anatomy of Spinal Stenosis

The spine consists of vertebrae (made of bone) stacked upon one another.

These vertebrae have a hollow space that holds and protects the spinal cord and nerve roots, called the spinal canal. In spinal stenosis, the open spaces between the vertebrae within the spine narrow, placing pressure on the spinal cord and nerves.

Diagnosing Spinal Stenosis

The result of spinal stenosis is often pain, numbness, or weakness, particularly in the legs. Pain is often described as a burning pain radiating to the buttocks and legs that worsens with extended periods of standing or walking. One common symptom that can help your pain provider diagnose spinal stenosis

is called the “shopping cart sign.” Have you experienced leg pain and weakness while grocery shopping only to find relief when leaning over the cart? This is often reported by patients with spinal stenosis. The reason patients get relief when they lean over the cart is because it opens or widens the spinal space between vertebrae and relieves the pressure on the nerves.

Functional Impact: Navigating Life with Spinal Stenosis

Patients with spinal stenosis may find their daily lives significantly altered. The pain and discomfort associated with the condition can limit one’s ability to

stand or walk for extended periods, affecting everything from daily errands to enjoying recreational activities. For those accustomed to an active lifestyle, the impact can be particularly challenging, diminishing not only physical function, but also the overall quality of life.

A Ray of Hope: Minimally Invasive Lumbar Decompression (MILD)

Traditionally, surgical interventions have been considered for spinal stenosis; however, the field of interventional pain management has witnessed significant advancements, bringing forth less invasive alternatives. One such innovation is Minimally Invasive Lumbar Decompression (MILD). This outpatient procedure is designed to address the root cause of spinal stenosis without the need for major surgery.

How MILD Works: Precision and Effectiveness

MILD specifically targets the areas of the spine contributing to stenosis. Under x-ray guidance, a specialized tool is used to remove small pieces

of bone and excess ligament tissue, which mechanically enlarges the spinal canal and relieves the pressure on nerves. The beauty of MILD lies in its minimally invasive nature, requiring only a small incision and resulting in less postoperative discomfort and a quicker recovery. In many successful cases, patients are able to stand upright without pain for the first time in years within minutes following the procedure.

Prevention Falls: A GUIDE TO HOME SAFETY

As we age, our bodies undergo various changes that may increase the risk of falls, leading to injuries and a decline in overall well-being. For individuals aged 65+, falls can have severe consequences; however, many falls are preventable with the right precautions, particularly when it comes to ensuring a safe home environment.

For seniors, the importance of home safety is significant. Home is a place of

comfort and security. Unfortunately, it can also be a place where accidents happen, especially if precautions are not taken. Falls are a leading cause of injuries among the elderly, often resulting in fractures, head injuries, and loss of independence. By addressing potential hazards within the home, we can significantly reduce the risk of falls and promote a healthier, more secure lifestyle.

Understanding the common risk factors for falls is the first step in prevention.

Poor Lighting

Inadequate lighting increases the chances of tripping or misjudging distances, particularly in the evening or at night. Install bright, energyefficient lighting in key areas of the home, including hallways, staircases, and bathrooms. Consider motion-

sensor lights for added convenience or nightlights at night.

Cluttered Spaces

A cluttered environment poses a significant risk of tripping and falling. Ensure walkways are clear of obstacles and consider decluttering and organizing your living space to reduce the risk of tripping.

Slippery Floors

Smooth surfaces, especially in bathrooms and kitchens, can become slippery. Non-slip mats can be beneficial; place them in areas prone to moisture. Secure rugs with double-sided tape to prevent slipping.

Lack of Handrails

Stairways and bathrooms without proper handrails contribute to falls. Install handrails along stairways and in bathrooms and grab bars near toilets and in showers to provide additional support.

Unstable Furniture

Furniture that is not secure or appropriately placed can lead to accidents. Rearrange furniture to create clear pathways.

Proper Footwear

Wear sturdy, well-fitting shoes with nonslip soles when indoors and outdoors. Avoid walking in socks or slippers on smooth surfaces.

Inactivity

A decrease in regular exercise or movement leads to a sedentary lifestyle and weakness, both of which can contribute to falls. Engage in activities that improve balance and strength, such as walking, yoga, or

tai chi. Consult with a healthcare professional before starting any new exercise routine.

Preventing falls involves a proactive approach to home safety. By addressing potential hazards and making necessary modifications, seniors can enjoy a more secure living environment, promoting independence and overall well-being. Taking the time to create a safe home is an investment in a healthier and more fulfilling lifestyle for individuals aged 65+.
