Current or Former Heavy Smoker? CT lung screening for cancer saves lives. It’s commonly known that you are at a higher risk for lung cancer if you have smoked heavily during any time in your life. In fact, smoking is believed to cause up to 90 percent of lung cancer cases and lung cancer is the leading cause of cancer death in both men and women in the United States. The good news is that CT lung screening for high-risk people can reduce deaths from lung cancer by 20 percent compared to the traditional chest X-ray. What is CT lung screening? Computed tomography (CT) lung screening is a simple radiological test done on people that are completely symptom free as a way of finding cancer before there are any indications. CT uses special X-ray equipment and computer software to obtain multiple cross-sectional images of the soft tissues, blood vessels and bones. It can distinguish between normal and diseased tissue based on changes in size, shape and appearance of the tissue. A CT lung screening can detect cancerous lung nodules at an early stage when they are more treatable, increasing survival chances.
Am I a candidate for CT lung screening? The American Lung Association and Austin Radiological Association recommend CT lung screening if you: R are a current or former smoker and 55 to 75 years old R have a smoking history of at least 30 pack years (this means one pack a day for 30 years, two packs a day for 15 years, etc.)
Together, these factors put you in the highest risk group for lung cancer and screening is recommended for you. However, the risk for lung cancer is different for everyone. If you do not fall into this group, consult with your doctor to see if you might benefit from CT lung screening.
The research behind CT lung screening. Results from the National Lung Screening Trial (NLST) published in 2010 showed a 20 percent reduction in mortality from lung cancer among participants screened using CT lung screening as compared to those screened with chest X-rays. The NLST was a randomized national trial of 53,454 current or former heavy smokers who fell into the risk group mentioned above. The group was ethnically representative of the U.S. population.
What is it like getting a CT lung screening? The CT scanner features a large ring that your body slowly passes through on a movable table. You may hear a series of whirring or spinning sounds as the X-ray tube and other components move inside the scanner housing. CT lung screening is completely pain free and noninvasive and does not require the use of injectable contrast media. It takes between five and 15 minutes to complete. After the exam is completed you will be able to resume your normal activities.
What do the results mean? A “suspicious result” means that the CT scan detected something

Abnormal findings include lung cancer or other serious conditions and also the possibility of a false positive, meaning no serious condition is present. You may need to have further diagnostic exams to find out more about the abnormal results. If you do have lung cancer, your doctor and other experts will discuss plans for treatment with you. A “negative result” means that the scan revealed no abnormalities. This does not guarantee that you do not have lung cancer or that you will not get it. Your doctor will help you determine if and when you should be tested again.
What are the risks? CT scans do involve exposure to radiation (X-rays). But radiology is advancing rapidly and ARA’s new low-dose CT scanners allow for up to 60 percent reduction in radiation dose with improved image quality. Research has consistently shown that the benefit of early treatment and potential cure from obtaining a CT lung cancer screening outweigh the risks of the radiation involved. Plus, ARA uses best practices for lung cancer screening. Your safety is paramount. The best way to prevent lung cancer is to never smoke or stop smoking now. If you are still smoking, talk to your doctor about ways to help you quit smoking.

Number one: Varicose veins can cause clinically significant symptoms including pain, swelling, blood clots and skin breakdown. Number two: They get worse with time if left untreated. Number three: Treatment is safe and effective with minimal down time. All treatments are performed in our office with a local lidocaine (common skin anesthetic). Most treatments take less than an hour.
WHAT ARE COMMON MISCONCEPTIONS SURROUNDING VARICOSEVEIN TREATMENT WITH REGARDS TO PREGNANCY? There’s a lot of misinformation suggesting patients wait until they are certain that they’ve finished having children before they treat their varicose veins. This is false. With each pregnancy, varicose veins will get significantly worse. By treating them prior to future pregnancies, the problem may be eliminated. Another common misconception is that varicose vein treatment is cosmetic. That’s not true. Varicose veins are certainly often visually unappealing, but they are also unhealthy and will continue to cause worsening health problems if left untreated.
WHAT OPTIONS DO PREGNANT WOMEN HAVE? If you have varicose veins, the earlier you get evaluated, the sooner treatment can be completed. We don’t typically

Centering Pregnancy Mothers-to-be get together in groups at CommUnityCare to receive pre-natal care, education and social support. There are 25 CommUnityCare Health Centers in the Austin area, providing health and dental care to residents of Travis County. Centering Pregnancy is one of the nationally accredited programs they offer.
Centering Pregnancy Groups Help Moms Help Themselves and Each Other. Centering Pregnancy has been generously funded from the beginning by the March of Dimes, an organization devoted to healthy pregnancies and most recently has received additional dollars from DSRIP (the Delivery System Reform Incentive Payments). Centering Pregnancy groups share this goal. Each group consists of eight to 10 women who share the same due date month. They meet 10 times during their pregnancy. The group meets in a space that has a TV, video monitor, books and videos for moms, a food area, a kids’ play area and a space off to the side where the women receive their routine pre-natal health care.
When a woman gets to a meeting, she weighs herself and checks her own blood pressure. The meetings always begin with a welcoming activity and a time for socializing. The women come together as a group to discuss topics like nutrition, healthy relationships, preparing for labor, post-partum depression or other pregnancy and family-related topics. The group leader facilitates the discussion, clarifying information as needed and encouraging group members to share their own experiences and opinions. “The idea is that women know their own bodies, and they are, in general, a wealth of knowledge for each other,” says Zander. Zander says she knew the program was successful when she got a call from a mom who had just had her third baby. She had experienced post-partum depression with her two previous pregnancies. At her six-week postpartum checkup, she told Zander, “I’ve actually been kind of depressed, but then I just called up some of the girls from the group and had them over.” Zander says the group helps women to feel less isolated.

only have a 4.8 percent pre-term labor and delivery rate. Zander says, “This is beneficial in many ways. One, for moms who’ve had babies pre-term, it can be wrenching to be apart from your baby. Two, it’s also hard to breast feed and bond with baby, and pre-term babies often have health problems. It’s also a huge driver of health care costs.” Zander is proud of the fact that 99 percent of the women in these groups come out of the hospital breastfeeding their baby. She also offers this example of the groups’ success: “We had one mom in our early groups who had been pregnant five times, and she’d had four pre-term labors and never breast fed, and after Centering Pregnancy she had a healthy term baby and breast fed for more than a year.”
Centering Parenting Provides Infants with Care until Two Years of Age. Centering Parenting groups work much the same way as Centering Pregnancy. Six to eight mothers with babies born around the same time meet as a group, and the babies receive routine well-child care visits and vaccines. There is also time for an educational presentation, games, activities and discussion. Zander loves her work and the Centering programs. “I’m lucky to do the work I do. I’m happy to help women. Since 2007, CommUnityCare has been facilitating Centering Pregnancy programs. These patient-centered groups involve the women in every way. Each group meeting includes a time for socialization, education, discussion and health care. Alicia Zander, a licensed nurse practitioner who works with the Centering groups says, “Centering works to build social support. It really involves people in their own health care, and they set their own goals. It’s a lot of fun.” Fun is important to Zander, and she thinks that’s part of what women enjoy about the groups. Zander says, “It’s great for the patients because it’s a lot more egalitarian and patient-centered. You affect change and affect people’s lives if you can relate to them on a personal level and make them laugh. It’s a much more down-toearth way to approach health care.”

Does whitening damage teeth? Studies have shown that when properly done on patients with no active decay, there is no damage. According to the American Dental Association, damage to teeth after whitening can occur with overuse of at-home whitening systems as these are administered without the supervision of a dental professional. Damage could also occur in the enamel if inferior whitening solutions are used. Some patients experience sensitivity in their teeth or irritation to the gums for a short time immediately after treatment; however, symptoms usually resolve quickly and no permanent damage occurs.
If my teeth don’t hurt, why should I have any dental treatment? Dental pain is often referred to as the worst kind of pain and often, painful dental emergencies occur in the middle of the night or on a weekend. Addressing tooth decay early, before you have pain in a tooth, is the best possible thing you can do to keep your teeth healthy and functional. Typically, by the time you have a toothache, the cost of dental care is significantly higher because the treatment is more aggressive. A small
cavity can be painlessly removed with a quick and inexpensive filling, but left untreated, that same cavity grows and goes deeper into the tooth. By waiting until it hurts, the decay can expand into the nerve and may require a root canal, or even extraction of the tooth. These procedures often require multiple appointments to get you restored back to health. The same can be said for gum disease—symptoms are “silent” until a very serious and painful event occurs. Being proactive, instead of reactive, makes treatment and its outcome more beneficial and longer lasting.
How often do I really need a dental cleaning? Every person is different, so the answer varies throughout your life due to changes in habits, increases in stress levels and health conditions, which can influence the health of your mouth. Most healthy patients who brush and floss regularly and have no active cavities will benefit from professional hygiene cleanings twice a year to remove any buildup that cannot be reached with a toothbrush and floss. If you have had bone loss you may require a cleaning every three to five months to keep your mouth healthy and prevent bacteria from causing more bone loss, which can lead to tooth loss. Once the bone is lost, it will require more frequent cleanings to ensure the disease process does not

worsen the condition of your bone and gums. Your hygienist and dentist will develop a plan that is best for keeping your mouth healthy.
How often do crowns, veneers and fillings last? While the answer varies person to person, there are certain things you can do to ensure that your dental treatments will last as long as possible. First, keep your regular cleaning appointments. This will keep the edges of your restorations cleaner and healthier than you are capable of doing at home. Second, wear a night guard or bite guard. If you grind and clench, you are placing more forces on your dental work, which may cause it to wear out sooner, decreasing longevity. Similar to tires on your car, there is only so much use and “tread-life” to your natural teeth and dental work as well. Habits, such as crunching ice, chewing fingernails, clenching teeth when you are stressed or grinding teeth in your sleep, all effect longevity. Next, brush twice daily with fluoride toothpaste and floss either in the morning or at night. Finally, if your teeth are crooked and uneven, you are placing more stress on your teeth during normal chewing. It will also be more difficult for you to keep your teeth clean. By straightening your teeth and wearing a retainer to keep things in alignment, you will be able to keep your teeth cleaner at home and have less chance of fracturing a crown, filling or tooth.

Painful Periods Dr. Lisa M. Jukes outlines the causes and treatment options for heavy menstruation and agonizing cramps. Most women experience discomfort when Aunt Flo comes to visit. However, there are varying degrees of pain associated with menstruating, as it affects women differently depending on the symptoms and causes. Heavy menstruation and severe cramps not alleviated by healthy lifestyle and use of over-thecounter pain relievers should be evaluated further by a specialist. When your body doesn’t respond to these tactics, it could potentially be more serious and even require surgery. Endometriosis, adenomyosis, uterine fibroid tumors, ovarian cysts and prolapse are some of the common diagnoses for women suffering from pelvic pain and/or abnormal bleeding. What are the signs of an abnormal period? One sign is pain that doesn’t respond to Motrin, or [the patient] has to dose frequently and take several Motrin to be able to get relief and go about their normal activity. As far as unusually heavy bleeding, changing a Kotex every hour or two, that would be considered not normal. Staining bed clothes at night or having to get up a couple times in the night to change a Kotex would also be considered not normal. If your pain does not respond to various therapies (healthy diet, exercise, pain relievers, birth control), if you are bleeding between periods, experiencing a heavy flow or if your menstrual cycle is interfering with your quality of life, preventing you from going to work or leaving the house, then it is time to seek help from a women’s health professional.
Why do heavy menstruation and cramps occur? Painful menstrual cycles can occur as young as the early teenage years, but tend to peak in a woman’s 20s and 30s. The
painful symptoms typically go away with pregnancy and then oftentimes return for women in their 40s. Pregnancy can provide relief of symptoms from endometriosis and pelvic pain for several years. Endometriosis is a condition in which endometrial type cells have implanted outside the uterus, causing severe cramps, abnormal bleeding and other pain, including pain with intercourse. Endometriosis can affect at least 25 percent of women. It can be genetic or even associated with congenital uterine anomalies. There’s a condition related to endometriosis called adenomyosis and that’s a common reason for severe menstrual cramps as well as heavy bleeding. It’s a condition where the endometrial lining has implanted in the muscle wall of the uterus. Pain can also be caused by uterine fibroids and can cause bleeding problems if the fibroid is affecting the lining of the uterus. Fibroids are benign, non-cancerous, fibrous-like tumors in the myometrium (muscle layer of the uterus). Pelvic pain should be further investigated to assure that it is not related to a gastrointestinal, urologic or musculoskeletal disorder. Heavy bleeding can even start with a young woman’s first few menses. Up to 25 percent of these adolescents may have a hereditary bleeding disorder, which can be determined by simple blood work. Later on,

heavy bleeding, bleeding between cycles or with intercourse can be secondary to overgrowth of the endometrium, uterine polyps, fibroids or adenomyosis.
What are the treatment options? The first step would be to see how the body responds to pain relievers and possibly hormonal contraceptives. If the body is not responding, the next step is evaluation and possible ultrasound in order to determine the etiology. The treatment depends on what we find and whether that person wants to have children or has completed their family. If the findings are suggestive of endometriosis based on history and physical exam, then we typically proceed with a laparoscopy, which, in my experience, I prefer using da Vinci surgery. The da Vinci robotic system gives me 3-D visualization, better magnification and use of instruments that have more precise movements. Laparoscopy allows me to see into the abdomen through small incisions in order to diagnose what is causing the pain or bleeding. I am now able to perform more complicated, minimally invasive surgeries rather than making large abdominal incisions, including single-site umbilical laparoscopic surgery. With da Vinci, patients experience less pain, less risk of infection and scarring, and shorter recovery time, among many other benefits.

Important Testing for Women Awareness, prevention and treatment for breast and bone health. As technology continues developing in the arena of women’s health, St. David’s HealthCare is proud to take part in that discussion, providing important testing options and serving as a resource to women in Central Texas. There are two important examinations women who are 40 and older should know about and take action to get tested for. Threedimensional (3-D) mammography helps to detect cancers in women earlier, while bone mineral density testing helps determine a patient’s bone health. Dr. Elizabeth Moorehead and Dr. Mark D. Akin explain why these tests are important for your health.
What is 3-D mammography technology? The latest advancement in mammogram technology to benefit women, 3-D mammography offers a comfortable design and delivers exceptionally sharp breast images, as well as superior screening and diagnostic performance. Similar to a traditional two-dimensional (2-D) exam, the 3-D mammography experience captures multiple, low-dose images of the breast from different angles which are then used to produce a series of 1-millimeter thick slices that can be viewed as a 3-D reconstruction of the breast. Only taking a few minutes more than a traditional mammogram, the 3-D exam allows physicians to see breast tissue in greater detail than ever before.
What are the benefits of a 3-D mammogram versus a traditional mammogram? Because a 3-D exam allows a physician to see the different layers of breast tissue without as much overlap as with a traditional mammogram, it helps reduce false positive rates, which minimizes the chance that a patient will be asked to return for follow-up tests. Recent studies show the use of 3-D mammography, combined with conventional 2-D imaging, has a significantly higher cancer
detection rate than conventional 2-D mammography alone. All women may benefit from the use of 3-D mammography, but the technology is particularly effective for women with dense breasts.
When and how often should a woman go in for a mammogram? By age 40, a woman should schedule her first mammogram and should return once every year following for the rest of her life. Depending on family history and if a firstdegree relative has been diagnosed with breast cancer, a women should consult with her doctor on mammogram examination at an earlier age. Breast cancer is the second leading cause of death among women, and statistics indicate that one in eight women will develop breast cancer sometime in her lifetime. The stage at which breast cancer is detected influences a woman’s chance of survival, so it’s necessary to schedule yearly exams. Paving the way for breast cancer detection, 3-D mammography can help detect cancers earlier and may improve a patient’s outcome.
What is bone mineral density (BMD) testing and why is it important? A bone mineral density test shows the amount of bone a person has in the hip, spine and sometimes other bones, and through the use of dual energy X-ray absorptiometry (DXA), the density of bones or the level of bone mass within the body is measured. This testing is the best possible way to determine your bone health and can identify low bone mass (osteopenia), osteoporosis and can determine your risk for bone fractures. For those who do have osteoporosis, the test also measures your response to osteoporosis treatment. During the test, a patient lies underneath a DXA scanner that produces two X-ray beams, each with different energy levels, and together
these beams scan the bones to determine their thickness. The test takes 15 minutes to complete, is painless and emits less radiation exposure than a traditional chest X-ray.
When and how often should a woman go in for a BMD test? Because most women reach maximum bone density by age 35, testing for low bone density is not required until the age of 50 or by menopause. After an initial BMD test is complete and your results are within normal range, the test should be repeated every three years. In some cases, women may need to repeat testing sooner. Women who suffer from estrogen, calcium or vitamin D deficiency, or those who have undergone chemotherapy, are at a greater risk for low bone density and should undergo BMD testing sooner than the recommended age. To learn more about BMD testing, women should talk with their gynecologist or primary care physician during an annual check up. Women over 35 should seek higher-level care after only six months of trying because maternal age negatively impacts egg quality and quantity.

The Health Benefits of Compression Hosiery What you may not know about the woman’s clothing staple. What is compression hosiery? Compression stockings, or hosiery, are spandex garments compressing the limbs, offering pressure against our legs, reducing the diameter of distended veins and causing an increase of healthy blood flow, velocity and heart valve effectiveness. Compression hosiery is available in a range of graduated levels of compression depending on the needs and use of them. Compression levels start from 10 to 15 mm Hg and continue up to 30 to 40 mm Hg—the lowest level providing lighter pressure, with the higher level being the tightest. Stroke Alert Know the facts so you can respond rapidly and effectively. Knowing the risks, signs and symptoms of stroke can save lives. Stroke is the leading cause of serious, long-term disability and the third leading cause of death in the United States. Each year, nearly 800,000 people will suffer from stroke and 140,000 people will die from stroke. What exactly is a stroke or “brain attack?” A stroke is a sudden infarct or area of dead tissue resulting from failure of blood supply to the brain, spinal cord or eye causing loss of function. It is equivalent to a heart attack, only it occurs in the brain and instead of destruction of heart muscle, brain tissue dies. Moreover, nearly two million brain cells (neurons) die each minute during a stroke. Hence, just as rapid treatment to re-establish blood flow is key in a heart attack, time is key for the brain. Nearly 90 percent of strokes are caused by a blockage of blood supply, cutting off oxygen supply to cells, while 10 percent are due to bleeding (hemorrhagic strokes). Blockage of an artery restricts blood, oxygen and nutrient supply to brain cells, causing an ischemic stroke. Ischemic strokes usually occur from a clot which formed elsewhere (e.g. the heart) or forms within an artery and then breaks off and travels through the bloodstream until it lodges and blocks a vessel to the brain (or spinal cord or eye). Clots which form in large arteries (vessels carrying oxygenated blood from the heart) can form within areas with buildup of cholesterol, fat and other products in the walls of arteries, causing either a clot that blocks the artery at that site (called a thrombus) or the clot can break off, travel and block the artery in the brain (embolic stroke). Finally, conditions such as chronic high blood pressure (hypertension) can cause thickening of small vessels in the brain that can narrow and cut-off supply to smaller but often critical areas of the brain. Other strokes may not be readily detected because the unaffected brain
areas compensate (known as a “silent stroke”). If the blood supply is restored and all deficits clear within an hour, the term Transient Ischemic Attack or TIA is used (though brain images would be needed to determine that the ischemia was truly transient and that no tissue death or infarct occurred).
How do I know if I am or a loved one is having a “brain attack” or stroke? Stroke symptoms will depend on the part of the brain deprived of blood supply. Common symptoms or signs include the following: R Sudden numbness, tingling and/or clumsiness or weakness of face (facial droop or slurred speech), hand, arm and/or leg (especially on one side of body). R Sudden trouble seeing in one or both eyes. R Sudden dizziness with nausea, loss of balance or coordination. R Sudden unusual and severe headache (hemorrhagic stroke). R Sudden difficulty with speech, expressing thoughts, understanding language, or confusion. Strokes not only occur in a variety of ways, but they may also stutter, that is to say, symptoms may seem to be resolving then worsening again due to blood vessel obstruction. It is therefore critical to not wait and hope symptoms subside but act fast to treat and prevent death or disability.

Transient Ischemic Attack (TIA): A Critical Opportunity. Stroke and TIA represent conditions within the spectrum of brain ischemia—a TIA may be viewed as the equivalent of angina of the brain. About 500,000 people have TIAs in the U.S. every year, though this is likely underestimated as symptoms may be ignored and underreported. Symptoms typically last less than one hour; a third of those with symptoms lasting up to 24 hours have actual brain infarcts (tissue death). Studies suggest risks of stroke after TIAs exceeds 10 percent in 90 days; up to half of these persons will have a stroke after a TIA within two days. Hence, urgent work-up for the underlying cause(s) of TIAs offers an opportunity to prevent permanent disability or death. These studies include blood tests, imaging studies of the brain and blood vessels and an echocardiogram, all of which can be performed expediently during hospital observation after a TIA.
How can stroke be prevented or treated? Major risk factors for stroke include prior strokes, poorly controlled hypertension, diabetes, smoking, high cholesterol, obesity or sedentary lifestyle, alcohol abuse, atrial fibrillation and other heart disease. Most of these risks can be successfully modified and managed. Moreover, rapid treatment of a stroke within four and a half hours with clot-busting medication can significantly help outcomes.

Laser Assisted Cataract Surgery Westlake Eye Specialists explain the advancements in cataract surgery. Cataract surgery is an inevitable part of the aging process given our increase in life expectancy as well as the desire for an active lifestyle necessitating good vision. Cataracts affect more than half of Americans over age 65. Gone are the days when cataract surgery was an inpatient hospital ordeal. Today, this procedure has become streamlined and incredibly sophisticated. The latest of these technological advances is laser assisted cataract surgery. WHAT IS A CATARACT? A cataract is clouding or yellowing of our own natural lens inside our eye. This occurs slowly as we age as a result of protein buildup in the lens. Cataracts prevent the passing of light to the inside of the eye and cause glare and loss of vision as a result. Common symptoms include difficulty with night vision, driving (glare from headlights) and reading. Fortunately, this is a totally curable condition with outpatient surgery.
WHAT DOES THE SURGERY ENTAIL? Cataract surgery is an outpatient procedure that can be as short as 15 to 20 minutes long. The entire lens is removed through microincisions and replaced with an artificial lens implant made of acrylic that usually does not have to be replaced. Surgery is
performed under IV sedation one eye at a time, and once a cataract is removed, it never grows back.
WHAT IS LASER CATARACT SURGERY? Until recently, cataract surgery was always performed with a blade. Now we have the ability to use femtosecond lasers, similar to Lasik, to assist in cataract surgery. The laser measures the patient’s eye shape prior to the procedure and customizes the incisions as a result. The incisions are then constructed with the help of the laser computer and without a blade. Usually, the entire laser treatment is finished under 30 seconds! In that time, the physician is able to create up to six different incisions to aid with the surgery.
WHY CHOOSE LASER? The laser procedure has many benefits, making it a great tool to use during surgery. Each treatment consists of an ultrasound, which helps customize incisions specifically to each patient. The incisions are then constructed in a very predictable fashion with great accuracy and seal incredibly well as a result. The laser is also effective in treating astigmatism. While blade surgery has a hard time placing corrective incisions at the right depth, the laser is able to measure the patient’s cornea accurately and place the incisions at the perfect

depth. Astigmatism is managed quite effectively as a result. Finally, the lens implant is held in position more effectively due to the accuracy of the laser.

Allow yourself a couple of weeks to assemble a treatment team, and consult a plastic surgeon before you begin treatment. The initial panic is overwhelming, but rushing can result in mistakes that you’ll regret later. What’s a common misconception in the reconstruction process? By law, if you’ve had a mastectomy and/or a lumpectomy, you cannot be denied reconstructive coverage by your insurance provider. The Women’s Health and Cancer Rights Act guarantees reconstruction and revisions for the duration of your lifetime, so it’s never too late to begin reconstruction or to refine one that does not satisfy you. What personal qualities serve you best as a surgeon?
