Endocrinology is the science of hormones, which affect every cell and every function in your body. The glands secreting these hormones form your endocrine system, a tightly interconnected system with thousands of feedback loops. It is far more complex than any supercomputer today. Hormones control everything in your body from birth to death. Without hormones, your body cannot function. Examples of hormones: estrogen, testosterone, insulin and hormones like thyroid, cortisol, adrenal and pituitary.

WHAT ARE ENDOCRINE DISEASES?

They are diseases caused by a malfunction of one or more endocrine glands in your body. Examples: thyroid, osteoporosis, metabolic syndrome and obesity, hirsutism, menopause, low testosterone in males, andropause and impotence, polycystic ovaries, irregular or lack of menstrual periods, high and low calcium, and diabetes.

WHAT FACTORS AFFECT YOUR ENDOCRINE SYSTEM?

Aging, other diseases, stress, environmental and genetic factors do influence your endocrine system. Aging changes how hormones are produced and absorbed by your body. Genetic factors and other diseases can do the same. Stress triggers a cascade of hormones that affect your heart, kidneys and other organs. Recent research identified endocrine disrupting chemicals in our environment.

WHY SEE AN ENDOCRINOLOGIST?

Hormone treatments must be followed by a hormone specialist (endocrinologist) the same way heart disease is followed by a heart specialist (cardiologist). An endocrinologist has years of special training in diagnosing and treating your hormone imbalances. Endocrine diseases are often missed, since symptoms are often subtle and easy to brush aside. An endocrinologist starts out with a thorough physical evaluation looking for these telltale sings, then follows up with a battery of blood and other lab tests. Often, additional highly specialized tests are involved to identify the root cause of your hormonal imbalance.

WHAT IS THYROID DISEASE?

Thyroid disease affects 30 million Americans, yet half of them do not know they have it. It is called the “silent disease.” One in eight women will develop a thyroid disorder in their life; women are five to eight times more likely than men to develop hyperthyroidism or hypothyroidism. Thyroid ailments include Graves’ and Hashimoto

disease, goiter, thyroid nodules and thyroid cancer. Thyroid problems require lifelong attention. Each person has a different genetic set point for TSH, the thyroid stimulation hormone.

WHAT IS OSTEOPOROSIS?

Osteoporosis is a disease in which bones become fragile and more likely to break. Osteoporosis affects one in two women and one in four men over the age of 50 and is generally missed. Bone fracture is considered to be the heart attack of the bone and can have major consequences on your quality of life, from reduced mobility to potential loss of mobility altogether. A bone density test is the only way to test for osteoporosis. We perform such testing and provide consultation on bone metabolism and osteoporosis treatment.

Breast Reconstruction After Breast Cancer

Facing a breast-cancer diagnosis, many women feel overwhelmed as life becomes about battling the cancer. While there’s a lot you can’t control about the diagnosis, for women who desire breast reconstruction after a lumpectomy or mastectomy, there are many safe and natural-looking options. Austin breast reconstruction specialist Dr. Christine Fisher empowers women to make the choice that’s right for them. Below, she answers your questions.

What are my options for breast reconstruction? Today, there is a safe breast-reconstruction option for virtually every woman diagnosed with breast cancer. Breast implants are a traditional option that typically look best on women with moderate-sized breasts who are having both breasts removed and do not need radiation. Women desiring a more natural look and feel may choose a flap procedure, which uses living tissue from another part of the body to reconstruct the breasts that will never need to be replaced. The most common flap procedure is a DIEP (deep inferior epigastric perforator) flap reconstruction which uses abdominal tissue to reconstruct the breasts. Women who have extra tissue on their abdomen are ideal candidates for DIEP flap reconstruction, which gives the added benefit of a “tummy tuck” effect.

What is the success rate of DIEP flap breast reconstruction? In my hands, this surgery has a 99 percent success rate. It is important to choose a surgeon who performs this microsurgery frequently. I have performed 500 flaps over the last five years.

Can I keep my nipples? Women with a small tumor and no signs of cancer near the nipple may be candidates for nipple-sparing breast reconstruction. I perform a hidden-scar technique in which there are no scars on the front of the breast. The nipple and areola are left in place while the breast tissue underneath is removed.

I am going to need radiation therapy after my mastectomy. How will this affect my plan for breast reconstruction? Patients planning radiation therapy after mastectomy present a more complicated reconstructive scenario. Implant reconstruction before radiation leads to a higher rate of aggressive scarring, pain and deformity. There is also twice the risk of implant-based infection, which can lead to removal of the implant. For most women requiring radiation therapy after mastectomy, my general recommendation is to put in place a temporary implant at the time of mastectomy to “hold the space.” After radiation, we complete the reconstruction by placing a healthy, living-tissue flap in the breast.

I am a very active person. Will I be able to resume all my normal activities after breastreconstruction surgery? The short answer is yes! Once you have finished the recovery period, there will be no limitations to your activities. The DIEP flap procedure does not remove muscle from the abdomen. Both DIEP flap and implant reconstruction have been performed on high-level athletes, professional dancers and other active women with complete recovery.

Nutrition is the cornerstone of good health, and for individuals looking to combat chronic disease like diabetes or HIV, a chat with a dietitian can make all the difference. The CommUnityCare Nutrition Services team is comprised of six registered dietitians and led by Kristin Brookshire, MPH, RD, who currently serves as clinical nutrition manager. Together, the team provides nutrition counseling to more than 5,000 patients a year at 10 clinic locations throughout Travis County.

WHAT DOES IT TAKE TO BECOME A REGISTERED DIETITIAN AND HOW IS THAT DIFFERENT FROM A NUTRITIONIST? A lot of people are confused by the difference between a nutritionist and a dietitian. While both professions help individuals navigate our complex food environment, there are currently no requirements for someone to become or claim the title of “nutritionist.” In contrast, registered dietitian is a protected credential that requires a minimum of a bachelor’s degree in nutritional science (Soon a master’s level degree will be required.), as well as 1,200 hours of supervised practice in various professional settings. Once those requirements are complete, the candidate must pass a national credentialing exam and maintain that credential through continuing education.

WHAT TYPE OF NUTRITION EDUCATION DOES THE TEAM PROVIDE? When it comes to nutrient education, there’s really no one-sizefits-all approach. The dietitian team strives to provide a diverse menu of services in order to meet people where they are. Patients can choose to visit with us one on one, or join one of the many classes that we offer. This past year, we were very excited to be able to pilot our first-ever cooking class. The Healthy Chef series empowers participants by using hands-on learning to build the skills needed to cook their own healthy meals at home. The patients love it and have even been able to impact the health of their families by preparing healthy meals that actually taste good.

HOW DO YOU TEACH PATIENTS TO EAT HEALTHIER WITHOUT BREAKING THE BANK? Cost is a major concern for many of our patients, who are often forced to prioritize paying for rent versus paying for food. Unfortunately, this can lead to overdosing on fast food from the dollar menu and packaged foods from the grocery store that are high in empty calories. Empty calories are foods that are packed with calories but missing the vital nutrients that the body needs to maintain health. Our goal is to help patients see that the healthy choice can also be the inexpensive choice. A lot of times, 1 0   Guide to Good Health 2 0 1 7

that starts with busting food myths about what’s healthy and not healthy and then working with an individual’s cultural background and lifestyle to develop a plan that’s customized to them and still fits within their budget.

WHAT ADVICE WOULD YOU GIVE SOMEONE WHO IS TRYING TO TURN THEIR EATING HABITS AROUND? For lasting change, eat the elephant one bite at a time. In other words, set yourself up for success by breaking big changes down into smaller, manageable steps during a long period of time. Let’s take eating more vegetables as an example. If your big goal is to eat five servings of veggies a day but you’ve spent your entire adult life never touching a vegetable, I don’t expect you to start eating salads at every meal, or at least not for very long! Making small steps toward change might look like a commitment to add one vegetable serving to dinner three times a week. Once you’ve comfortably established the habit of three times a week, and you’ve discovered that roasted Brussels sprouts are actually amazing, then make it four days a week. Keep going, slow and steady until you reach your goal.

WHY IS IT SO HARD TO FIND RELIABLE NUTRITION INFORMATION. HOW CAN I KNOW IF SOMETHING IS LEGIT? The food-and-nutrition industry is a big money generator and companies will try to sell their product by making as many health claims as possible. Fortunately, there is some regulation by the FDA about the wording of health claims on packaged foods and supplements, but it’s definitely still confusing for the average consumer. The key to finding good information is to make sure that it has its roots in good science. We call this “evidence-based.” Health claims that are evidence-based will have the backing of several different studies over different periods of time, and with funding that has minimal conflicts of interest. For example, we don’t want a new magic weight-loss pill if it is only supported by one study that was funded by the company that manufactures that pill. It might sound crazy, but that scenario happens more often than not. Protect your health and your wallet by doing some homework upfront.

Are there differences between men and women regarding the GI tract? Women tend to have longer colons and a different pelvic anatomy than men, with more redundant colons lower in the pelvis that may contribute to trapping gas and stool and leading to ailments such as bloating and constipation. In addition, women also have different hormonal variations that can play a role in symptoms.

What are the causes of constipation and what are the treatments available? I see many women in my practice with constipation, many of whom have been suffering with lifelong constipation and assumed there was either no treatment or are experiencing daily bloating and abdominal discomfort because of it. There are a variety of causes for constipation, ranging from slow transit in the colon to structural changes in the rectum to pelvic-floor dysfunction. When I see a patient with constipation, we go through each possibility and discuss what intervention or test would best help us find the root cause and treat accordingly. This can range from over-thecounter medications, prescription medications, diet and even physical therapy.

What about bloating? how can I fix this? Every day, I see patients who are struggling with bloating and want to know how to fix it. The causes of bloating vary from common to rare illnesses and to some degree, the formation of gas is a natural byproduct of the digestive tract functioning as it should. However, some people are more sensitive to the gas they produce, such as those with irritable bowel syndrome, or are eating certain food groups that lead to more gas formation. There isn’t always a one-size-fits-all approach to bloating but dietary changes can be a good place to start. I want to provide a reasoned and systematic approach to changing the diet rather than your trying to change it alone and becoming frustrated when your symptoms aren’t improving.

When should I see a gastroenterologist for my symptoms and for colorectalcancer screening? Colorectal-cancer screening typically starts at the age of 50, unless you have a family history of colon cancer or certain types of polyps or are having “alarm” symptoms that may necessitate starting evaluation earlier, such as unintentional weight loss, blood in the stools, amongst others. However, I would recommend that anyone who is having digestive distress, whether it be bloating, discomfort, constipation, reflux or change in bowel habits consider seeing a gastroenterologist. With the help of your doctor, we can determine if there is a more serious underlying condition and then work to improve your symptoms. I strongly believe a well-functioning GI tract is a vital part in a daily sense of well-being.

Is dental care safe during pregnancy?

Can I whiten my teeth during pregnancy?

Routine dental care is not only safe but very important during pregnancy. Let your dentist know when you go that you are pregnant and also how far along you are in the pregnancy. It is best to have any necessary dental treatment completed during the second trimester. During the third trimester, it may be a little too uncomfortable to be in the dental chair on your back for an extended period of time. Any elective dental work that you want to have done, I would recommend you schedule for after delivery.

Teeth whitening is considered to be a cosmetic, elective procedure and therefore, I would not recommend it during pregnancy. There hasn't been enough data to show the effects of the whitening agents on the developing baby, so it's best to be safe and wait to whiten until after delivery.

How will pregnancy affect my mouth? Pregnancy gingivitis is a common condition that affects many expecting women. Due to all of the hormonal changes, there can be increased inflammation in the gum tissue. You may notice more bleeding when brushing and flossing. Gingivitis can precede more serious forms of periodontal disease, so it is very important to go to the dentist routinely to have your teeth cleaned. Morning sickness can create a higher risk of tooth decay. The extra acid in the mouth can cause deterioration of the hard outer enamel layer of the tooth. Rinsing your mouth with water immediately after being sick will help to decrease the amount of acid present.

Can I get dental X-rays during pregnancy? Radiographs of the teeth are very unlikely to pose a risk to a developing baby in the uterus. However, since there is such controversy surrounding dental X-rays and limiting exposure, I would only recommend a pregnant woman have X-rays taken when the benefits outweigh the risks. For example, if an expecting mother has a toothache or swollen jaw, it would be much less risky to have a single X-ray taken to help identify the problem than to let a potential dental infection continue to spread.

What antibiotics are safe to take during pregnancy? Antibiotics may be prescribed for you for a dental infection. Amoxicillin and clindamycin are both commonly prescribed and are considered to be safe to take. Tetracyclines such as minocycline and doxycycline can cause discoloration of the baby's developing teeth and also damage to the pregnant woman's liver. These antibiotics should be avoided during pregnancy.

Are the local anesthetics used during treatment safe? Lidocaine is the most commonly used local anesthetic for dental treatment. It is a category B drug (does not cross the placenta) and therefore, it is considered to be safe to administer during pregnancy.

What pain medications can I take for a toothache or after dental work? Tylenol (acetaminophen) is a category B drug and is the drug of choice for pain relief in pregnant women at any time during the pregnancy. Advil (ibuprofen) is a category B drug and is safe to take in the first and second trimesters. However, it becomes a category D drug during the third trimester and should be avoided, as it can cause premature closure of one of the heart ducts.

Minimally Invasive Gynecology Surgery

“What’s next in minimally invasive surgery? You have to see it to believe it. Soon, we will be able to safely accomplish even more through less.”

Dr. Devin Garza was one of the world’s first surgeons to use a single-site robotic approach for benign gynecology. Performing more than 1,200 robotic procedures and given the distinction of da Vinci mentor/epicenter surgeon by Intuitive Surgical Inc., he champions the use of minimally invasive surgery by training surgeons and lecturing nationally and internationally on robotic techniques. In 2016, Dr. Garza, a Board Certified OB/GYN, presented on the national stage as faculty at American Association of Laparoscopic Gynecologists and World Robotic Gynecology Congress. Dr. Garza is a referral specialist in the management of some of the most complex benign gynecology surgery, such as endometriosis resection, myomectomy and Acessa for fibroids, complex hysterectomy, pelvic-floor prolapse and abdominal cervical cerclage. What is the Biggest challenge that minimally invasive gynecologic surgeons face today? A surgical technique called mechanical morcellation* has disappeared from the surgeon’s toolbox, and the conversation is now focused on alternatives. A morcellator would slice tissue into small pieces for removal through small incisions. Problems arose when the devise dispersed pre-existing unknown cancer cells in the specimen throughout the body. Many physicians have resorted to an open surgery with a large incision and the resulting prolonged recovery time and associated risks. Rather than resorting to open surgery, I teach surgeons how to safely remove tissue using a preferred minimally invasive approach, containing tissue within a pouch, and removing often very large specimens through a cosmetic 1-inch incision. This gives surgeons a tool with which to maintain MIS management of a large uterus for hysterectomy and myomectomy. * Renaissance Women’s Group does NOT use morcellators for gynecologic surgery.

Who is an ideal candidate for minimally invasive surgery? Women of all ages and conditions benefit from MIS, and you should discuss all viable alternatives with your physician. Complex pathology can be managed safely and effectively with a highly experienced surgeon who is equipped with the most advanced surgical armamentarium, either traditional laparoscopy or the da Vinci robotic platform.

Are all surgeons trained in the minimally invasive approach? Most are, but some surgeons are more experienced than others. Obviously, you get really good at what you do a lot. For example, you would expect that the pilot of your airplane has logged many hours in the air and in-flight simulation and will continue to sharpen his or her skills and the competency to manage potentially complex situations. It’s the same with surgeons. I continue to enjoy sharing strategies and techniques that have served my patients well over many years as a highvolume MIS surgeon. The type of hands-on training that I teach throughout the U.S. and other countries as an epicenter/mentor 2 0   Guide to Good Health 2 0 1 7

surgeon prepares less experienced surgeons to perform the latest robotic techniques and safe tissue removal. Often they can apply what they learned the very next day. My hope is that a patient who was given no choice but an open surgery will now have the opportunity to have small incisions and a greatly reduced recovery time. I am convinced that as we further the knowledge of MIS, our patients benefit.

I am considering surgery. Are there questions that I should ask my physician? You will, of course, have discussed all of the options to surgery, as surgery should be the last option after failed medical management. 1. "Will this be a minimally invasive surgery?" At Renaissance Women’s Group, our MIS rate approaches 99 percent. 2. If they are not comfortable offering an MIS approach, ask: "Is there a surgeon you trust in the local medical community that can perform this particular surgery using MIS either laparoscopic or robotic?" Collaboration among surgeons is key to maintaining a high MIS rate for our patients. 3. If you are not comfortable or have unanswered questions, always ask about obtaining a second opinion.

What’s on the horizon in MIS? There continues to be a buzz around the new da Vinci Xi robotic platform, and I was very fortunate to be among the first to use it, as well among the first to perform single-site robotic surgery on the Xi. The new platform will continue to improve surgical outcomes in gynecologic surgeries, as well as other specialties with its improved vision, increased range of motion and access to complex pathology. Also, a new treatment for fibroids called Acessa /RFA (Radio Frequency Ablation) is showing promise as an MIS approach with an even shorter recovery as an option to hysterectomy and to possibly help maintain fertility. I have enjoyed seeing how well my patients respond clinically after this treatment. I am certain this approach will help manage patients with fibroids as the technology continues to advance.

Most women experience discomfort when Aunt Flo comes to visit. However, there are varying degrees of pain associated with menstruating, as it affects women differently depending on the symptoms and causes. Heavy menstruation and severe cramps not alleviated by a healthy lifestyle and the use of over-the-counter pain relievers should be evaluated further by a specialist. When your body doesn’t respond to these tactics, it could potentially be more serious and even require surgery. Endometriosis, adenomyosis, uterine fibroid tumors, ovarian cysts and prolapse are some of the common diagnoses for women suffering from pelvic pain and/or abnormal bleeding.

WHAT ARE THE SIGNS OF AN ABNORMAL PERIOD? One sign is pain that doesn’t respond to Motrin, or [the patient] has to dose frequently and take several Motrin to be able to get relief and go about their normal activity. As far as unusually heavy bleeding (i.e., changing a Kotex every hour or two,) that would be considered not normal. Staining bed clothes at night or having to get up a couple times in the night to change a Kotex would also be considered not normal. If your pain does not respond to various therapies (healthy diet, exercise, pain relievers, birth control), if you are bleeding between periods, experiencing a heavy flow or if your menstrual cycle is interfering with your quality of life, preventing you from going to work or leaving the house, then it is time to seek help from a women’s health professional.

WHY DO HEAVY MENSTRUATION AND CRAMPS OCCUR? Painful menstrual cycles can occur as young as the early teenage years, but tend to peak in a woman’s 20s and 30s. The painful symptoms typically go away with pregnancy and then oftentimes return for women in their 40s. Pregnancy can provide relief of symptoms from endometriosis and pelvic pain for several years. Endometriosis is a condition in which endometrialtype cells have implanted outside the uterus, causing severe cramps, abnormal bleeding and other pain, including pain with intercourse. Endometriosis can affect at least 25 percent of women. It can be genetic or even associated with congenital uterine anomalies. There’s a condition related to endometriosis called adenomyosis and that’s a common reason for severe menstrual cramps as well as heavy bleeding. It’s a condition where the endometrial lining has implanted in the muscle wall of the uterus. Pain can also be caused by uterine fibroids and can

cause bleeding problems if the fibroid is affecting the lining of the uterus. Fibroids are benign, non-cancerous, fibrous-like tumors in the myometrium (muscle layer of the uterus). Pelvic pain should be further investigated to assure that it is not related to a gastrointestinal, urologic or musculoskeletal disorder. Heavy bleeding can even start with a young woman’s first few menses. Up to 25 percent of these adolescents may have a hereditary bleeding disorder, which can be determined by simple blood work. Later on, heavy bleeding, bleeding between cycles or with intercourse can be secondary to overgrowth of the endometrium, uterine polyps, fibroids or adenomyosis.

WHAT ARE THE TREATMENT OPTIONS? The first step would be to see how the body responds to pain relievers and possibly hormonal contraceptives. If the body is not responding, the next step is evaluation and possible ultrasound in order to determine the etiology. The treatment depends on what we find and whether that person wants to have children or has completed their family. If the findings are suggestive of endometriosis based on history and physical exam, then we typically proceed with a laparoscopy. In my experience, I prefer using da Vinci surgery. The da Vinci robotic system gives me 3-D visualization, better magnification and use of instruments that have more precise movements. Laparoscopy allows me to see into the abdomen through small incisions in order to diagnose what is causing the pain or bleeding. I am now able to perform more complicated, minimally invasive surgeries rather than making large abdominal incisions, including single-site umbilical laparoscopic surgery. With da Vinci, patients experience less pain, less risk of infection and scarring, and shorter recovery time, among many other benefits.

What is the latest on the health risk of old amalgam (mercury) fillings in my teeth and should I have them removed? These types of fillings have raised a lot of controversy in the last decade or more. Some people have claimed amalgam fillings could be linked to a multitude of neurological diseases, such as Alzheimer’s and multiple sclerosis. The ADA, the AMA and the FDA, along with numerous other accredited public-health organizations, have done thorough independent research within the last 10 to 15 years that have found zero connections or solid evidence to support such claims. There is not a higher incidence of people manifesting symptoms of these types of diseases when comparing patients with existing amalgam fillings, patients with amalgam fillings that have been removed and replaced with alternative restorative materials, and with patients that have never had an amalgam filling. That said, these types of fillings can be harmful for very different reasons. Unfortunately, metal fillings, especially when left in place for too long, can definitely cause fracturing of your teeth and put you at higher risk for emergencies, tooth pain, tooth failure and tooth loss. Most fillings are intended to last eight to 12 years in a tooth. The larger the filling, the shorter the life span. Metal, in a humid environment like the mouth, corrodes over time. This leads to gaps forming between the restoration and the remaining tooth structure. This gap formation will likely lead to new cavities forming under it, but worse yet, it causes a weakening that contributes to the surrounding enamel beginning to fracture. If left untreated, this crack line can lead to the nerve and cause severe pain or even spread to the root, which leaves the tooth untreatable. Therefore, the biggest danger to you when it comes to having old metal fillings still in your mouth is the much greater potential for more costly and inconvenient dental emergencies and procedures.

How safe are dental x-rays? Advances in dentistry and dental technology have led to almost immeasurably low levels of radiation when you receive full mouth and checkup X-rays. In fact, modern dental X-rays taken regularly are virtually harmless, especially in comparison to sun exposure, minerals in our soil and appliances in our homes. Digital X-ray machines and high-speed film limit the radiation beam to the small area being X-rayed and requires very short exposure time (fractions of a second) to get the same results as older, traditional film. The use of lead-lines aprons protect the body from stray radiation, even though this problem is almost non-existent with modern techniques. And always remember, if dental X-rays truly increased your chances of cancer, then dentists and their staff would show a higher incidence than the general population. Wearable Fitness Devices: Do They Work? Wearable fitness devices, such as the Fitbit, are popular because they help users track steps taken throughout the day in a fun and engaging way. Though fitness trackers can range in price from low- to high-end models, the social aspect of the device is what drives many users to invest in them. The U.S. Centers for Disease Control and Prevention recommends that adults walk 10,000 steps per day, as this number is estimated to burn about 20 percent of your daily caloric intake. People tend to be more active when they work out with others. Wearable devices can help drive frequency with the social nature they provide: if you have a friend wearing a fitness tracker with you, there is a sense of community and this support network motivates users. There are 5.4 Million Skin-Cancer Cases Every Year. In 2016, 5.4 million cases of skin cancer were diagnosed in the U.S. That number far surpasses the number of breast, prostate, lung and colon cancers combined. It’s not even close. So given these grave statistics, why aren’t more people rushing to their dermatologist to get a skin exam?

There are 4 million Basal Cell Carcinoma Skin-Cancer Cases Every year. One of the most straightforward reasons people tend to downplay the significance of skin cancer is that most cases of skin cancer are not deadly. For example, of the 5.4 million cases of skin cancer in the U.S., more than 4 million of those are basal cell carcinoma. Less than 1 percent of these basal cell carcinomas metastasize or spread to other parts of the body. For this reason, they are rarely lethal.

There are 1 million Squamous Cell Carcinoma Skin-Cancer Cases every year. By the same token, only 5 percent of the squamous cell carcinoma skin cancers metastasize or spread to other parts of the body. So, that means 95 percent of the time, they are not lethal. However, if left untreated, basal cell carcinoma and squamous cell carcinoma can destroy large areas of skin, causing physical disfigurement that is permanent. So when we consider that there are 5.4 million skin cancers each year and 5 million of them are not very likely to spread or cause death, that’s pretty reassuring. So what’s the big deal?

Malignant Melanoma Malignant melanoma accounts for less than 1 percent of skincancer cases but the vast majority of skin-cancer deaths. One person dies of melanoma every 52 minutes. Melanomas are sometimes challenging to diagnose. Of all the skin cancers malignant melanoma is the sneakiest skin cancer for a number of reasons.

Melanoma is a master of disguise. Melanomas love a good disguise. Maybe you are thinking you can use color as a dependable guide to nail down a diagnosis of melanoma. Think again! Melanomas can be black, brown, tan, white, pink or red. Maybe you think the shape of a melanoma can help you detect one early? Think again! They can be round, oval, jagged or amorphous in shape. As far as elevation goes, melanomas can be flat, raised, shiny or rough. Given all these varied appearances, it makes sense to get a skin exam from a professional dermatology provider whose expertise can decide which lesions are suspicious enough to biopsy.

Melanomas grow in the strangest places While basal cell carcinomas and squamous cell carcinomas tend to grow in sun-exposed areas like the head, neck, arms and hands. This makes sense! So because all skin cancers are triggered by the sun, they should all grow in sun-exposed areas, right? Think again. The most common place for a man to grow a melanoma is the back. For a woman the most common melanoma site is the legs. Skin cancers are quiet and don’t complain much. Whether you are growing a basal cell, squamous cell or melanoma skin cancer, it is rare that you will feel anything associated with your skin cancer. At the early stages, there is usually no itch, no stinging, no burning, no bleeding, no fatigue, no weight loss. In short, you won’t realize anything is wrong with you at all!

What’s the move? Because skin cancers have a million disguises, grow quietly and painlessly and frequently grow in places you would never suspect, the only dependable way to protect yourself in 2017 is to call and schedule a full-body skin exam at Tru-Skin Dermatology as soon as possible. How does it work? Patients are given a small-dose sedative to take before the procedure and are driven home by a friend or family member. The procedure is fast and recovery time is minimal. Most patients return to regular activities the next day. Balloon sinus dilation is covered by many insurance plans and requires no cutting or removal of bone or tissue (unlike traditional sinus surgery) and is almost painless. • A balloon catheter is gently inserted into the sinus through the nose. • The balloon is inflated and gently widens the walls of the sinus passageway. • Saline is sprayed into the inflamed sinus to flush out the mucus. • The catheter/balloon is removed, leaving the sinuses open.
