While there’s a lot you can’t control about the diagnosis, for women who desire breast reconstruction after a lumpectomy or mastectomy, there are many safe and natural-looking options. BREAST RECONSTRUCTION AFTER BREAST CANCER What are my options for breast reconstruction? Today, there is a safe breast-reconstruction option for virtually every woman diagnosed with breast cancer. Breast implants are a traditional option that typically look best on women with moderate-sized breasts who are having both breasts removed and do not need radiation. Women desiring a more natural look and feel may choose a flap procedure, which uses living tissue from another part of the body to reconstruct the breasts, which never need to be replaced. The most common flap procedure is a deep inferior epigastric perforator, or DIEP, flap reconstruction, which uses abdominal tissue to reconstruct the breasts. Women who have extra tissue on the abdomen are ideal candidates for DIEP flap reconstruction, which gives the added benefit of a tummy-tuck effect. For women undergoing a lumpectomy rather than a mastectomy, I can perform an oncoplastic reconstruction in cases where the lumpectomy surgery removes at least 30 grams of tissue, with a 2- to 3-centimeter diameter of tissue removal or more. In many cases, a much larger portion of the breast is removed; this depends on the tumor size. Oncoplastic surgery then rearranges the remaining breast tissue to fill the empty space in the breast left behind by the lumpectomy. To address asymmetry between the two breasts, a breast reduction and/or breast lift can be performed on the opposite breast, a procedure that is covered by insurance. What is the success rate of DIEP flap breast reconstruction? In my hands, this surgery has a 99 percent success rate. It is important to choose a surgeon who performs this microsurgery frequently. I am a member of the American Society of Reconstructive Microsurgeons and have performed more than 500 flaps in the last five years. Can I keep my nipples? Women with a small tumor and no signs of cancer near the nipple may be candidates for nipple-sparing breast reconstruction. I perform a hidden-scar technique in which there are no scars on the front of the breast. The nipple and areola are left in place, while the breast tissue underneath is removed. What are my reconstruction options if I’m not a candidate for nipple-sparing mastectomy? If the nipples must be removed as part of the cancer surgery, women can elect to have nipple reconstruction. The new nipple can be surgically created as a protrusion at a natural location on the breast mound, or can be visually reconstructed via a three-dimensional tattoo. Three-dimensional areola tattooing adds color to the areola and the appearance of a nipple. Patients who have had surgical nipple reconstruction may also elect to undergo tattooing to add color to the areola and nipple. Tattooing can be done in my office about three months after the patient’s final surgery.

I am going to need radiation therapy after my mastectomy. How will this affect my plan for breast reconstruction? Patients planning radiation therapy after mastectomy present a more complicated reconstructive scenario. Implant reconstruction before radiation increases the risk of aggressive scarring, pain and deformity fivefold. There is also twice the risk of implant-based infection, which can lead to removal of the implant. For most women requiring radiation therapy after mastectomy, my general recommendation is to put in place a temporary implant at the time of mastectomy to “hold the space.” After radiation, we complete the reconstruction by placing a healthy, living tissue flap in the breast. I am a very active person. Will I be able to resume all my normal activities after breast-reconstruction surgery? The short answer is yes. Once you have finished the recovery period, there will be no limitations to your activities. The DIEP flap procedure does not remove muscle from the abdomen. Both DIEP flap and implant reconstruction have been performed on highlevel athletes, professional dancers and other active women, with complete recovery. I have problems with my breast implants. What can I do? For patients suffering the disappointment of breast-implant problems, reconstruction failures and radiation injury, I have the extensive experience, meticulous artistic technique and commitment to personalized care to address your concerns. Dr. Devin Garza was one of the world’s first gynecologic surgeons to adopt robotic surgery. Having performed more than 1,600 robotic procedures and given the distinction of da Vinci mentor/epicenter surgeon by Intuitive Surgical Inc., he champions a minimally invasive approach to complex surgery by also training other surgeons. Since 2012, Garza, a board-certified OB-GYN, has often presented as faculty on the national and international stage at conventions such as the American Association of Laparoscopic Gynecologists and World Robotic Gynecology Congress, and in Australia and Asia. He was elected chair of the American Academy of Gynecologic Laparoscopists robotic special interest group for 2017 to 2018, and has personally trained more than 1,500 surgeons from throughout the world. MINIMALLY INVASIVE GYNECOLOGY SURGERY Who is an ideal candidate for minimally invasive surgery? Women of all ages and conditions benefit from minimally invasive surgery, and you should discuss all viable alternatives with your physician. Complex pathology can be managed safely and effectively with a highly experienced surgeon who is equipped with the most advanced surgical armamentarium, either traditional laparoscopy or the da Vinci robotic platform, which overcomes most of the limitations with traditional laparoscopy. Are all surgeons trained in the minimally invasive approach? Most are, but naturally, some surgeons are more experienced than others. Obviously, you get really good at what you do a lot. It’s the same with surgeons. The type of hands-on training that I teach throughout the U.S. and other countries as an epicenter/mentor surgeon helps surgeons to perform more proficient robotic techniques. My hope is that a patient who was given no choice but an open surgery will have the opportunity to have a minimally invasive approach. I am convinced that as surgeons become more experienced and skilled in minimally invasive surgery, our patients truly benefit. I am considering surgery. Are there questions I should ask my physician? Importantly, you should have discussed all the options to surgery, as surgery should be the last option after failed medical management. But as it pertains to minimally invasive surgery, here are some key questions:

1. Will this surgery be approached in a minimally invasive way? Many surgeons are unaware of the potential for surgeries, such as very large uteri, removal of fibroids and transabdominal cerclage, which can be safely and routinely performed on the robotic platform by experienced surgeons. 2. I f your doctor is not comfortable offering a minimally invasive approach, ask if there is a surgeon he or she trusts in the local medical community that can perform this particular surgery in a minimally invasive approach. Collaboration among surgeons is key to more patients benefiting from a minimally invasive surgery approach. 3. Dr. Margaret Whitney is a gynecologist at the UT Health Austin Women’s Health Institute. The institute focuses specifically on complex women’s pelvic conditions, such as urinary incontinence, pelvic-floor disorders and vulvar disorders.

VULVODYNIA AND THE RISK FACTORS What is vulvodynia and what are the risk factors? Vulvodynia is a condition in which women experience pain on the vulva, which is the area outside the vagina between the mons pubis and the anus. It can include the labia, clitoris, vaginal opening and other areas. Some women have constant pain, while others may only have pain when provoked by certain activities, such as sex or wearing tight clothing. It is generally considered to be a chronic condition, but many women have resolution of their symptoms with appropriate therapy, and relapse varies from person to person. Known risk factors include young age, Hispanic ethnicity, being married and having coexisting conditions such as depression, post-traumatic stress disorder, sleep dysfunction and other pain syndromes like irritable bowel or bladder-pain syndromes and fibromyalgia. However, this is an area of medicine that is poorly understood and there are likely other risk factors and associated conditions that we have yet to identify. How many women are affected by vulvodynia and at what age does it affect women? Surveys suggest 3 to 16 percent of women report pain suggestive of vulvodynia. It is more common in younger women but can occur in women of all ages, such as during the time of menopause. Vulvodynia can occur early in life, such as with the first tampon use, and it can occur later in life after a pain-free period. Why is it important to treat vulvodynia? Many women with vulvodynia have seen multiple medical providers before they are given a diagnosis of vulvodynia, and even fewer can find effective treatment. The condition can have a significant impact on self-esteem, relationships, quality of life and ability to work. There is still much to be learned about this condition, and by treating more women, we can begin to learn more about the disease and hope to continue to offer better and more effective treatment options. What aspect of vulvodynia do patients struggle with the most? Many of our patients have been told there is nothing wrong or have been misdiagnosed. What treatment options are available? Treatment can vary depending on the severity, duration and associated conditions. There can be an initial period of trial and error, and most women benefit from a multidisciplinary approach to care. How does Women’s Health Institute use the team approach for patients with vulvodynia? We approach vulvodynia using the biopsychosocial model of care. This means we treat the whole person and keep in mind the complex issues that can arise when a patient is living with a painful condition. Our team includes associate providers, such as physician assistants and nurse practitioners, pelvic-floor physical therapists, sex counselors, social workers and behavioral health specialists, nutritionists and physicians. All members of the team play a critical role in the treatment of our patients. We meet as a team in the morning to review patient treatment plans so we can receive input from all team members. GET HELP FOR YOUR PAINFUL JAW ISSUES What does my bite have to do with the way I feel? There are two main nerves associated with the face and jaw. The information from these two nerves comprises more than 50 percent of the total impulses sent to your brain. The brain gives this area of your head and neck a lot of importance and is therefore very sensitive to any information it receives from this area. If something is even slightly off, your body knows and responds, many times in ways that create pain and discomfort. What are some of the signs of temporomandibular joint dysfunction? Signs and symptoms of temporomandibular joint, or TMJ, dysfunction can vary from person to person but may include clenching, grinding, popping or clicking in your jaw joint, headaches, ringing in your ears, ear congestion and dizziness, neck tension, stiffness and tightness, sleep issues like sleep apnea and many others. I get a lot of headaches, even migraines. I’ve seen many different professionals about this but typically the answer is just another medication. Could my jaw be causing this? Absolutely, your jaw could be the issue. Only a small segment of dentists have the knowledge and training to understand and treat the physiology of why this is happening. The answer is not another pill. By using advanced computer-based technology, Dr. Kevin Winters is able to get information about the muscles connected to your bite. Most women experience discomfort when Aunt Flo comes to visit. However, there are varying degrees of pain associated with menstruating, as it affects women differently depending on the symptoms and causes. Heavy menstruation and severe cramps not alleviated by a healthy lifestyle and the use of over-the-counter pain relievers should be evaluated further by a specialist. When your body doesn’t respond to these tactics, it could potentially be more serious and even require surgery. Endometriosis, adenomyosis, uterine fibroid tumors, ovarian cysts and prolapse are some of the common diagnoses for women suffering from pelvic pain and/ or abnormal bleeding.

PAINFUL PERIODS What are the signs of an abnormal period? One sign is pain that doesn’t respond to Motrin, or the patient has to dose frequently and take several Motrin to be able to get relief and go about her normal activity. As far as unusually heavy bleeding (i.e., changing a Kotex every hour or two), that would be considered not normal. Staining bed clothes at night or having to get up a couple times in the night to change a Kotex would also be considered not normal. If your pain does not respond to various therapies (healthy diet, exercise, pain relievers, birth control), if you are bleeding between periods, experiencing a heavy flow or if your menstrual cycle is interfering with your quality of life, preventing you from going to work or leaving the house, then it is time to seek help from a women’s health professional. Why do heavy menstruation cramps occur? Painful menstrual cycles can occur as young as the early teenage years, but tend to peak in a woman’s 20s and 30s. The painful symptoms typically go away with pregnancy and then oftentimes return for women in their 40s. Pregnancy can provide relief of symptoms from endometriosis and pelvic pain for several years. Endometriosis is a condition in which endometrial-type cells have implanted outside the uterus, causing severe cramps, abnormal bleeding and other pain, including pain with intercourse. Endometriosis affects at least 25 percent of women. It can be genetic or even associated with congenital uterine anomalies. There’s a condition related to endometriosis called adenomyosis and that’s a common reason for severe menstrual cramps as well as heavy bleeding. It’s a condition in which the endometrial lining has implanted in the muscle wall of the uterus. Pain can also be caused by uterine fibroids that can cause bleeding problems if the fibroid is affecting the lining of the uterus. Fibroids are benign, noncancerous, fibrous-like tumors in the myometrium (muscle layer of the uterus). Pelvic pain should be further investigated to assure that it is not related to a gastrointestinal, urologic or musculoskeletal disorder. Heavy bleeding can even start with a young woman’s first few menses. As many as 25 percent of these adolescents may have a hereditary bleeding disorder, which can be determined by simple blood work. Later on, heavy bleeding, bleeding between cycles or with intercourse can be secondary to overgrowth of the endometrium, uterine polyps, fibroids or adenomyosis.

What are the treatment options? The first step would be to see how the body responds to pain relievers and possibly hormonal contraceptives. If the body is not responding, the next step is evaluation and possible ultrasound in order to determine the etiology. The treatment depends on what we find and whether that person wants to have children or has completed her family. If the findings are suggestive of endometriosis based on history and physical exam, then we typically proceed with a laparoscopy. In my experience, I prefer using da Vinci surgery. The da Vinci robotic system gives me 3-D visualization, better magnification and use of instruments that have more precise movements. Laparoscopy allows me to see into the abdomen through small incisions in order to diagnose what is causing the pain or bleeding. I am now able to perform more complicated, minimally invasive surgeries rather than making large abdominal incisions, including single-site umbilical laparoscopic surgery. With da Vinci, patients experience less pain, less risk of infection and scarring, and shorter recovery time, among many other benefits.

BY KELLY NARDO We’re all trying to start off on the right foot, be that by cleaning up our diets, hitting the gym one more time a week or focusing on self-care. With all these new resolutions to prioritize, one thing that can get pushed to the back burner of our to-do lists is getting organized. We all know how wonderful it feels coming home to a sparkling kitchen, a clutter-free office and all the laundry put away, but do you ever think about the health benefits that come from channeling your inner neat freak? Let’s count the many ways in which prioritizing organization is good for our mental, emotional and physical sanity. IT REDUCES STRESS.

Are you running about the house trying to find keys or searching through a 2-inch stack of papers on your desk for a certain contract? When everything has a home, be that a desk drawer, a shelf, a hook or a filing folder, and you know exactly where it is, you’ll no longer have to stress about trying to find something when you need it most, which will make you more relaxed. IT ENCOURAGES HEALTHIER EATING HABITS.

Having clutter and disorder can lead to stress, which increases your cortisol levels. Excessive cortisol can encourage fat synthesis, increase your appetite and can affect your choice of food. Translation: Come snack time, your tendency is to lean toward something high in fat, sugar or both. When things are less cluttered, more clean and therefore inherently less stressful, you are more likely to choose an apple instead of a candy bar and keep that cheat day in check.

Do you ever clean your house and feel like a million bucks afterward? Just 10 minutes spent cleaning—a quick sweep of the kitchen and getting a load of laundry started—can give you the energy boost and the mental break you need to continue on with your day. IT ENABLES YOU TO FOCUS ON THE FUTURE.

Have you ever moved or redesigned a room and found a miscellany of items you didn’t even know you had? Make a point to get rid of the things that drag you down, especially those that do so emotionally, for instance, those old jeans from college that haven’t fit you for a few years. Eliminate items that are more likely to give you feelings of guilt than happiness, and keep the things that motivate and inspire you and make you feel like your best self. IT MAKES YOU HAPPIER.

Lastly, it’s no shock being more organized can help increase happiness. Studies have shown clutter can increase levels of cortisol, the stress hormone, and that stress can make you feel overwhelmed, fatigued, depressed or, in layman’s terms, like you don’t have your life together. At the end of the day, there are few things more rewarding than coming home to a well-organized and clean home.

A messy room can make you feel anxious, leading to lost hours of sleep. Keep your room clean and your bed made, and clear out things you don’t need to help reduce clutter and have a restful night.

Do you ever sit down at your desk and try to get something done only to realize your desk is a disaster zone? Some would say a scattered workspace means a scattered mind. Being clutter-free helps you focus on the task at hand without getting distracted. Light a candle and find a new place for the mess, whether it’s the trash can or a filing cabinet, and make your desk a clean slate. Although work processes differ from person to person, being able to concentrate on one task at a time can increase your productivity and get creative juices flowing for the next task at hand.

WHY CHOOSE AN ORAL SURGEON What is the difference between an oral maxillofacial surgeon and my general dentist? All dentists attend a four-year dental school prior to receiving their dental degree. Oral and maxillofacial surgeons then spend an additional four to six years intensively training in medicine, surgery, anesthesia and implant dentistry. Some receive medical degrees in the process but all get the same intensive core training in expert third-molar removal, compleximplant placement, jaw surgery, temporomandibular joint and trauma surgery. Following this training, most then submit for written and oral examinations to achieve board certification. My dentist says she has a specialist that comes to her office monthly and can take out my son’s wisdom teeth under anesthesia. Is her office set up to provide this procedure safely? Oral surgery offices are built from the ground up for outpatient surgery and anesthesia rather than general dental care. From anesthesia and safety equipment to patient flow and recovery, the facility is designed for outpatient surgery. All oral-surgery offices undergo onsite examination and certification to ensure office-based anesthesia standards are met. All general dental offices are different, but most necessitate that the traveling dentist bring anesthesia, surgery and safety equipment with each visit. Additionally, while complications are rare, often, patients want the reassurance of a quick unscheduled visit to check in or confirm healing is going well. With most surgeons’ offices staffed full time, there is always an expert available to provide that follow-up care and reassurance. Such accessibility is often not available with traveling providers. I have a complicated medical history. Am I a candidate for anesthesia? Almost all who can be seen in an office environment can have some form of anesthesia. The type of anesthesia and ideal treatment environment are best discussed with your surgeon at the time of a consultation visit, when your medical history can be discussed in detail.

WHY DO WE TEND TO MINIMIZE SKIN CANCER? How prevalent is skin cancer? In 2016, 5.4 million cases of skin cancer were diagnosed in the U.S. That number far surpasses the number of breast, prostate, lung and colon cancers combined. It’s not even close. So, given these grave statistics, why aren’t more people rushing to their dermatologist to get a skin exam? If skin cancer is so common, why do people downplay its significance? One of the most straightforward reasons people tend to downplay the significance of skin cancer is that most cases of skin cancer are not deadly. For example, of the 5.4 million cases of skin cancer in the U.S., more than 4 million of those are basal cell carcinoma. Less than 1 percent of these basal cell carcinomas metastasize, or spread to other parts of the body. For this reason, they are rarely lethal.

Why is it important to get my skin checked? By the same token, only 5 percent of the squamous cell carcinoma skin cancers metastasize or spread to other parts of the body. So, that means 95 percent of the time, they are not lethal. However, if left untreated, basal cell carcinoma and squamous cell carcinoma can destroy large areas of skin, causing physical disfigurement that is permanent. So, when we consider there are 5.4 million skin cancers each year and 5 million of them are not very likely to spread or cause death, that’s pretty reassuring. So, what’s the big deal? Malignant Melanoma Malignant melanoma accounts for less than 1 percent of skin-cancer cases but the vast majority of skin-cancer deaths. One person dies of melanoma every 52 minutes. Melanomas are sometimes challenging to diagnose. Of all the skin cancers, malignant melanoma is the sneakiest skin cancer for a number of reasons. Melanoma is a master of disguise. Melanomas love a good disguise. Maybe you are thinking you can use color as a dependable guide to nail down a diagnosis of melanoma. Think again! Melanomas can be black, brown, tan, white, pink or red. Maybe you think the shape of a melanoma can help you detect one early. Think again! They can be round, oval, jagged or amorphous in shape. As far as elevation goes, melanomas can be flat, raised, shiny or rough. Given all these varied appearances, it makes sense to get a skin exam from a professional dermatology provider whose expertise can decide which lesions are suspicious enough to biopsy.

Melanomas grow in the strangest places. Basal cell carcinomas and squamous cell carcinomas tend to grow in sun-exposed areas like the head, neck, arms and hands. This makes sense. Because all skin cancers are triggered by the sun, they should all grow in sun-exposed areas, right? Think again. The most common place for a man to grow a melanoma is the back. For a woman, the most common melanoma site is the legs. Skin cancers are quiet and don’t complain much. Whether you are growing a basal cell, squamous cell or melanoma skin cancer, it is rare that you will feel anything associated with your skin cancer. At the early stages, there is usually no itch, no stinging, no burning, no bleeding, no fatigue, no weight loss. In short, you won’t realize anything is wrong with you at all. What’s the move? Because skin cancers have a million disguises, grow quietly and painlessly, and frequently grow in places you would never suspect, the only dependable way to protect yourself in 2018 is to call and schedule a full-body skin exam at Tru-Skin Dermatology as soon as possible. Most importantly, Moossy emphasizes both aesthetics and functionality when discussing dental options that will be optimal in creating the desired smile. Are porcelain veneers right for me? A veneer is the ideal restoration to address issues that patients aren’t happy with, such as shape and color, alignment of the teeth, uniformity of the smile and spacing concerns. However, it does have its limitations. Great care needs to be taken if the patient is a clencher or grinder, or has gum or bone disease. In addition, it tends to be a more expensive option but lasts longer. A more fiscally conscious approach might be simply whitening your teeth followed by contouring and bonding. It is possible to obtain dramatic results with little investment. What is the black line at my gum line? This is usually the result of a previous dental restoration. Older crowns were made with a metal substructure, which is revealed when the gum recedes with age. So, newer restorations are more aesthetic in the sense that they are metal-free yet still have adequate strength. Crowns are great, just like veneers, in fixing similar issues, but are needed in certain circumstances. Crowns are ideal for the patient who has preexisting dental work or is known to be a clencher or grinder. I want longer teeth. How do I achieve this? One option would be adding length to the edge of the tooth using composite material, otherwise known as bonding. Length may also be added with the use of veneers or crowns. Sometimes patients have what is referred to as a “gummy” smile, which can easily be resolved with a gingivectomy or crown-lengthening procedure (removing gum tissue), which reveals more tooth structure, in turn, creating a longer tooth.

CHRONIC PAIN Dr. Genaro Gutierrez, what pain syndromes do you typically treat in women? Women are prone to developing chronic pain after surgery, trauma or just organically. I see women with chronic headaches, complex regional pain syndrome, pelvic pain from endometriosis and from physical activity. One exciting emerging therapy for sports injuries is regenerative medicine, enhancing the body’s own natural ability to heal itself. Can you tell us what it means to have chronic pain? Chronic pain is defined as pain that has been persistent for more than three months, or beyond the expected recovery or healing time for an injury. Unfortunately, sometimes chronic pain is permanent pain. My job is to introduce both classic and novel methods to reduce a person’s daily suffering and make their pain much more manageable on a daily basis.

What are some of the most common conditions you treat? I see a large variety of chronic-pain syndromes, starting with the very basic degenerative-arthritis pain cases of the neck, low back, knees and hips. I also see a substantial amount of neuropathy in my practice that can stem from nerve damage from chronic diabetes, chemotherapy, surgeries or trauma. We also see a lot of chronic headaches in our practice. There is a lot we can do about these issues without resorting to oral medications to calm the pain. Can you name some of these treatments? For spine pain due to degenerative arthritis, there is a really neat treatment that targets the specific small nerves that communicate pain from the small joints of the spine (facet joints) with a specialized needle that creates discreet thermal lesions to these nerves and renders them unable to communicate pain. This typically lasts for 12 months at a time, but in some cases, it can last longer. This same concept and technology can also be applied to other joints in the body. What if the patient is terrified of needles? Not to worry. Our pain center can provide safe and effective IV sedation under the care of a licensed anesthesiology provider to make sure your procedure and experience are as smooth and painless as possible.

What is spinal-cord stimulation? Spinal-cord stimulation has been getting a lot of press these days, and rightfully so. This technology has actually been available for decades but in the last couple years, there have been tremendous advances in both convenience of the therapy and improved efficacy. Spinal-cord stimulation is a minimally invasive therapy targeting specific bundles of nerves within the spinal cord in order to calm pain in the spine, extremities and even pain in the chest and abdomen. It is most commonly used for the unlucky but rare situation in which a person has undergone spine surgery for degenerative or traumatic conditions, but for whom, unfortunately, the pain never went away. I also perform this on many people that are simply not candidates for spine surgery or for any additional spine surgery. I work very closely with many of the spine specialists in Austin and Round Rock, Texas, to identify patients that would benefit from this modality. This treatment also works very well for peripheral neuropathy and nerve-pain syndromes. It is also gaining a lot of traction in chronic pelvic-pain syndromes. Is this done in a hospital? We do all of our spinal-cord stimulation cases as outpatient surgery. What are your favorite cases to treat? My favorites also happen to be the most challenging. These include complex regional pain syndrome and cancer-pain syndromes. Both are very rewarding to treat but challenging because of recurrence. My goal is always to do my best to provide people with more good days than bad days as they cope with their condition.

First and foremost, psychotherapy is a very nuanced process and dependent upon a number of factors. However, the most important factor that predicts positive outcomes is the relationship between therapist and client.

DEBUNKING THE MYTHS ABOUT PSYCHOTHERAPY What is psychotherapy? While every clinician has his or her own definition, I believe psychotherapy is a process by which an honest and open relationship is created and nurtured. Many practitioners have modernized their participation and are active collaborators with their clients, moving from clinician to educator to life coach to ardent supporter. I often tell my clients that as with any relationship, there will be ups and downs. I believe the therapeutic relationship evolves to represent a microcosm of connections that are fostered outside the therapeutic space. This being said, patterns that occur in associations outside the therapeutic relationship often become present inside the therapeutic relationship. This is a positive occurrence, as it allows therapist and client to observe and work through these patterns in a safe, nonjudgmental environment. This type of relationship makes room for increased insight and awareness, which can ultimately lead to sustainable change in many realms of a client’s life. When I start the psychotherapy process, does it mean I have to commit to coming in for years? Traditional models of psychotherapy have emphasized a long-term approach to treatment. I’m sure the classic image that comes to mind as you’re reading this is the famous psychoanalyst Sigmund Freud talking to a client sprawled out on a long couch. The answer to this question is not at all. The length of therapy is often informed by the presenting concern, the therapist’s approach to treatment and most importantly, the ongoing collaborative conversation with each client about the objectives and goals he or she wants to accomplish. Ultimately, the time frame is driven by the client making progress toward accomplishing his or her goals. Is psychotherapy only useful during times of crisis? The answer to this question is no. We have made strides in destigmatizing psychotherapy, but the idea that there has to be something wrong appears to remain deeply embedded in the societal narrative, even today. I believe it is critical to emphasize that going to a therapist can be useful, even when distress is not present. Developing self-awareness, strengthening emotional muscles, advancing communication skills and exploring values are the building blocks to navigating a crisis and maintaining change after the crisis is successfully managed. Often, the more meaningful therapeutic conversations are had when there is not a pressing crisis. Thus, frequent exploration of the topics, as listed above, can be analogous to “sharpening the saw”: The sharper it is, the better equipped one will be in coping with a crisis.

Will my therapist be angry if I express dissatisfaction about the treatment process? Clients are often worried about disappointing his or her therapist, especially as it relates to their treatment. The answer to this question is absolutely not. In fact, therapists welcome this feedback from their clients and believe it is critical to foster honesty and transparency in the therapeutic relationship. Expressing authentic feelings is the hallmark of a strong therapeutic alliance. Thus, it is crucial to disclose when dissatisfied with the course of treatment so changes can be made or a referral to another provider can be arranged. The bottom line is the therapist is at the service of the client, not the other way around. In my experience, when clients have expressed dissatisfaction in any way, it has actually strengthened, not hindered, the relationship.

HEALTHY GUT, HEALTHY YOU Your gut plays a critical role in your overall health. Sethi explains some of the common concerns she regularly hears and explains how to strengthen your gut with integrative solutions. How do you know if your gut is healthy? What are signs that you should see a doctor for digestion issues? Digestive issues can present with many different symptoms. Some easily recognizable symptoms might be gas, bloating or acid reflux. Less recognizable symptoms as related to the gut might include acne, eczema, difficulty with focus, low energy and difficulty losing weight. Ultimately, you are only as healthy as your gut is. If your gut is not healthy, you can end up with nutritional deficiencies, which can lead to your body not having the building blocks it needs to make proteins that help you heal or hormones that drive your metabolism or the support you need for a healthy immune system and well-functioning brain. It’s essential that your gut microbiome is healthy, and testing can provide insight into why symptoms or digestive issues are occurring in the first place. What are some helpful food guidelines and diet trends for rebalancing the gut? Which trends might actually be harmful? The truth is that there are going to be different eating patterns that are right for different people. It’s really hard to say there’s one size that fits all. That being said, I typically recommend as a general rule of thumb that the anti-inflammatory diet is great for most people as a starting point. The anti-inflammatory diet or the Mediterranean diet have the most evidence for longevity and tend to be plant-focused or plant-forward diets. Women often ask about the ketogenic diet. This diet has been shown to improve some inflammatory gut problems and can help with rebalancing the microbiome. However, it may not be the right choice for everyone, as it can also affect hormonal balance.

What are some preventative steps women can take to strengthen their guts? Eat a large variety of vegetables and fruits to increase the diversity of your microbiome, which are the bacteria and other species that live in our guts to help us with day-to-day functions. Eat probiotic and prebiotic foods, which include fermented foods, yogurt, pickled vegetables and fiber. Decrease your stress, especially prior to eating. If you are stressed, your digestion is affected, and no matter how good your diet is, your absorption of all that healthy food will be affected. Taking five minutes to practice a mindful meditation, prayer or breathing practice, such as the “4-7-8 breath,” prior to eating will help the digestive juices flow and decrease inflammation. Follow time-restricted feeding, which means eating during a set number of hours during the day. Consider intermittent fasting. Don’t eat after the sun goes down or before the sun comes up. And, finally, get good sleep. How do you advise your patients to apply integrative solutions at home to maintain gut health? You can’t supplement your way out of a poor diet. Choosing healthy foods is foundational. There may be times, however, when food and nutrition are not enough. Lifestyle modifications are essential, as described in my book, Built to Thrive, including reducing toxins in your environment, increasing natural movement, optimizing sleep, strengthening relationships and transforming mindset about what is possible for well-being. When specific natural solutions are necessary, many ingredients can be found in the pantry and fridge and created quickly and simply. Remedies include sage cough syrup, bath salts with magnesium and essential oils for sleep, fennel-ginger tea for indigestion, and marshmallow tea and bone broth to soothe the gut. We instruct women to use natural ingredients, which prevents microbiome disruption that many over-the-counter synthetic products can cause.

Here’s all you need to know about keeping your personal health records safe. BY JILL CASE

Austin’s Dr. Deborah Peel is the founder and president of the Patient Privacy Rights Foundation. In 2006, Peel also founded the Coalition for Patient Privacy. It goes without saying Peel is passionate about patients’ rights to medical privacy. In 1977, when she began practicing psychiatry, she noticed how many of her patients were afraid they would face discrimination if information in their records about their psychiatric care weren’t kept private. This was the beginning of her interest in medicalprivacy issues. Today, the issue is far more pressing because, as Peel says, “Instead of a system where most information would be locked up in cabinets, now we’re in a system where your records are in millions of databases [that are] unknown and inaccessible to you.” You may believe your privacy is protected by the Health Insurance Portability and Accountability Act, or HIPAA, form you routinely sign in your doctor’s office, but this isn’t true. Your medical information is available to more people and companies than you may realize. Currently, your information can be accessed by your insurance company, employers, financial institutions, government entities such as Medicare and the U.S. Department of Labor’s Workers’ Compensation division, as well as bought and sold by data brokers. Another issue that concerns Peel is the fact that patients withhold information from their doctors to protect their privacy and prevent job and insurance discrimination based on their diagnoses, not their health or abilities. According to a large patient survey conducted in 2016, 89 percent of patients admitted to withholding information from their doctors. “It’s incredibly ironic that the technology systems that are supposed to help doctors treat patients actually are driving people to lie or withhold information from their doctors,” Peel says. Through her organization and her coalition, Peel is fighting for our rights to control who can see and use our health data, even though it’s a difficult battle. “It won’t be easy, but it has to happen because the right to privacy—to choose who can know sensitive information about you—is a fundamental human and civil right to autonomy and respect, the right to freedom. DR. DEBORAH PEEL’S TIPS TO PROTECT YOUR MEDICAL PRIVACY 1. Ask doctors if the contract(s) they have with their electronic-health-records provider allows them to sell your data. To protect the data from being sold, the doctor or practice has to cross off the option in the contract, hidden in fine print, which allows the provider to sell or share your data. 2. Avoid free medical or health-care-related apps. “If it’s free, it means your data is being sold,” Peel says. 3. Be aware that just because a website or app says it “complies with HIPAA,” it doesn’t mean your information is being kept completely private. 4. Don’t post anything personal about your health on Facebook or Twitter. 5. Consider using a private search engine like duckduckgo.com instead of Google or Bing, especially when you’re searching about medical conditions. 6. Look into using a free privacy tracker such as Privacy Badger when you use your computer or device. 7. Avoid genetics testing such as 23andMe or labs, if possible. Almost all these companies sell your data. It may be “anonymous,” or “de-identified,” but it can still be identified as yours, eliminating your right to medical privacy.

GETTING YOUR MAMMOGRAM WITH ARA DIAGNOSTIC IMAGING What’s important to know about mammograms? I think the key message that we try to convey during Breast Cancer Awareness Month is that starting annual mammograms at age 40 saves lives. It is also vital to get yearly screening mammograms after that. Many women are unaware that breast cancer is very common. In fact, one in eight women will develop breast cancer in their lifetime. About one woman will die every 13 minutes. What’s really startling is that many Austin women 40 and older have not started having their screening mammograms. The great news is that when detected early by a mammogram, 98 percent of women survive breast cancer. The emphasis is on early detection, which catches cancers when they are more treatable. It’s so important not to wait to have your mammogram. While it’s always challenging to fit another activity into our busy lives, I think if women were more informed about breast-cancer screenings, they would certainly make scheduling a mammogram a priority. According the American College of Radiology, women should start getting mammograms at the age of 40 and get screened yearly thereafter. If you have an increased risk or family history, your screenings might need to begin earlier. What are some myths and facts about breast cancer? I speak with a lot of patients who share their questions and doubts. There are a few common myths about breast cancer that I’d like to clear up. Myth: Breast cancer doesn’t run in my family, so I don’t need to get mammograms. Fact: S eventy-five percent of women diagnosed with breast cancer have no family history. Myth: If you don’t feel a lump in your breast, you don’t need a mammogram. Fact: Mammograms can detect cancer several years before a lump can be felt.

TUMMY TUCKS 101 What’s the difference between a tummy tuck and liposuction? People don’t always understand the difference. Liposuction is a fat-debulking procedure that decreases the amount of fat you have. If you’re young and have good-quality skin, when we remove the fat, the skin should spring back. Skin has the amazing ability to adapt. Conversely, a tummy tuck is meant to address a loose-skin issue. It’s important to note that a tummy tuck is a body-contouring procedure, not a weight-loss procedure, which is a common misconception. We can combine a tummy tuck with a liposuction procedure to address the issue with fat and the issue with loose skin. For younger people, they can often just opt for liposuction without the tummy tuck. Sometimes patients want what we call a mommy makeover, which includes a breast-rejuvenation procedure along with a tummy tuck. That’s a great combo procedure for moms who feel their breasts have become droopy after having kids and want a lift, or a lift and augmentation, and a tummy tuck. You can lose fat in two ways: with diet and exercise or, to a certain extent, with the knife. But there’s no way to exercise your way out of loose skin. Are there any risks associated with a tummy-tuck procedure? We always want people to be healthy prior to a procedure. But for the most part, it’s a safe procedure. If a patient has a medical condition, say diabetes, we want to make sure it’s well-controlled because she could be at a higher risk for infection. Otherwise, most people will have scars that heal well. For someone who’s prone to significant scarring, she may want to consider that before having a tummy tuck. There are also varying degrees of tummy tuck, and depending on the patient, she could end up with an extended C-section-type scar, but most of the time, a tummy-tuck patient will have a hip-to-hip scar that is hidden by underwear. Is it OK to have a tummy tuck prior to getting pregnant? A tummy tuck is not recommended for women prior to pregnancy. During a tummy-tuck procedure, we tighten the abdominal muscles. But those muscles get stretched apart in pregnancy and they don’t always come back together. Worst-case scenario, if you get pregnant after a tummy tuck, you could be at risk of having the muscles and skin stretch back out. In the end, that procedure could be a waste of money for the patient.

What kind of down time can I expect after receiving a tummy tuck? Can I immediately go back to my active lifestyle? For tummy-tuck procedures, you usually have a good four to six weeks of recovery. If you try to do strenuous activity, your sutures could rupture, so it’s important to heed your doctor’s advice and take it easy during this recovery period. In the first couple of weeks, patients can expect some pain, soreness and tightness. After the two-week mark, you’re close to feeling back to yourself, but in reality, you still have a month of recovery to go, and that means no core exercises and no running. People are often very happy with the results after surgery, but it can be stressful for some patients when they can’t be as active as normal right after surgery. People who already have good habits should easily be able to get back to their goal weight after recovery. I suggest patients simply be careful and aware of what they’re eating during that period. How do I determine what procedure I need? This comes back to the importance of good communication between the doctor and the patient. A patient may come in asking for a tummy tuck when what she really wants is a liposuction procedure. That’s why it’s the doctor’s responsibility to ask lots of questions about what the patient’s concerns are and what’s bothering her before just jumping into a procedure. I want you to tell me in your words what the problem is so I can know what procedure you really want and why. Maybe you come in saying you want a breast lift, but when we start talking about your concerns, the actual procedure and the potential for specific scars, you may realize you’d prefer an implant procedure instead. Those kinds of discussions are incredibly important for me to have with each individual patient. I owe patients the honest answer.

Here’s how to know whether you may get osteoporosis. BY LAURYN LAX Osteoporosis is not just an “old-lady disease.” While osteoporosis is a disease most often associated with women older than age 50 due to the decline in estrogen, the bone-building hormone, younger women are not immune to the disease, particularly thinner females. Our bones are in a constant state of renewal. New bone is made and old bone is broken down. When we’re kids and teens, our bodies make new bone faster than they break down old bone, and our bone mass increases a la the beauty that is growing pains. Most people reach their peak bone mass by their early 20s and, as people age, bone mass is lost faster than it’s created. In regard to osteoporosis, this presentation is exaggerated as bone mass breaks down and density decreases. Osteoporosis is a condition that affects approximately one in two women. Moreover, 50 percent of these women will suffer from an osteoporosisrelated fracture at some point in their lifetime, and another one in two women will experience decreased bone mass, known as osteopenia, before reaching osteoporosis. As osteoporosis affects 80 percent more women than men, here’s how to know whether you’re being affected.

COMMON SYMPTOMS

Unlike a runny nose during a cold or experiencing joint pain with arthritis, osteoporosis symptoms are silent and often fly under the radar until a more serious injury, like a fracture or stress fracture, occurs. Common symptoms to look out for that may suggest your likelihood of having the disease include: r easily getting stress fractures, especially in the feet or hips r shin splints r back pain caused by an undiagnosed collapsed vertebra or fracture r loss of height r a stooped posture r digestive distress (chronic bloating, constipation, IBS or GERD)

DIAGNOSIS

Osteopenia is a precursor to osteoporosis, and both are diagnosed using bone-mineral-density scanning using a dual-energy X-ray absorptiometry, or DEXA, machine. RISK FACTORS

Women are at an increased risk for osteoporosis related to estrogen levels if they: r have had their ovaries removed r are going through menopause r are of Caucasian ethnicity r had their first period at a later age during puberty r experience irregular periods or amenorrhea (an abnormal absence of menstruation) r have a lower body weight r keep a highly active lifestyle or exercise excessively r have a history of disordered eating r have an overactive thyroid, known as hyperthyroidism r have a health diagnosis of arthritis, carpal tunnel syndrome, low vitamin D status, autoimmune condition, gastroesophageal reflux disease or irritable bowel syndrome r keep a vegetarian or vegan diet r have low fat intake, particularly low intake of saturated fats and fat-soluble vitamins such as vitamins A, D, E and K
