Golden Construction is delighted to complete this project with quality, cost, and efficiency in mind. We are excited to extend our services to accommodate all materials, equipment, and people involved in this project, per all regulations and mandates provided. In order to comply with the RFI date of April 30th, 2023, Golden Construction has made this proposal our top priority. For this proposal, Golden Construction has determined a Design-Bid-Build delivery method and a bid cost of $7,546,472.00. The scheduled duration of the project will be 428 days. The project will be completed in full on April 27th, 2023. Here at Golden Construction, we take pride in the quality we produce for our clients. As a general contractor, we selfperform around 45% of work on our projects. This includes concrete, carpentry, and some mechanical work. Golden Construction would place our best management staff on this project. We believe in great, quality work delivered in a timely manner. Golden Construction was founded in 1962 in Northeast Ohio. Our family business started with small residential and commercial construction, but as we grew so did our projects. We built on the Kent State campus, in downtown Kent and across Portage county.
OUR HISTORY
Here at Golden Construction, we have been building in Northeast Ohio for the past 60 years. Founded by William Baden Sr. with his brother David Baden in 1962, the pair had a vision of building up the surrounding area of Kent, Ohio. Born in a blue collar family, with a dad in construction and a mom raising him and his brother. Taking an interest in their dad’s career ventures at a young age they began to shadow him when he could and even do some home projects of their own while dad worked late. After graduating high school, William and David helped neighbors with renovations and even some local businesses. From kitchen table to side street office in downtown Kent, they saw their business exponentially grow as clients referred his business to others around town and the surrounding portage area. In, 2018 Troy Baden took over the family business as Vice President. The Baden family, and their board of directors have found a special place in Portage County within the residential space and have thrived locally with the completion of many hotels within the Greater Northeast Ohio.

Golden Construction We acquired the new construction on Kent State Campus of the Design Innovation Hub. This project was a large one for our company but we did a fantastic job moving through it. This shows the diversity in our building wonderfully. For complete comparison, we also completed a Holiday Inn in Kent, Ohio. This hotel was a great project for the company and we take great pride in it. We fast-tracked our schedule for this project and it went flawlessly, following our slogan “Built In A Flash”.
B. SELF-PERFORMING CAPABILITIES
Concrete, carpentry, mechanical, excavation and wood framing are our self performing capabilities. Our contractors are the best of the best in the area. Our trades are extremely talented and well-rounded in everything we do. We take pride in the years and years of experience our trades have accumulated. These capabilities help us keep a budget and maintain wonderful craftsmanship.
C. POTENTIAL HIGH-RISK SCOPE
Locking the Job Site Every year, roughly $1 billion is stolen from job sites yearly. To prevent us from becoming a victim and saving money on insurance, we will put up cameras and fence the entire property. COVID-19 Since March 2020, COVID has been raging throughout the world. Due to this, we must monitor all of our staff at all times. To keep workers from infecting each other, we must enforce and follow all CDC guidelines and regulations. COVID-19 vaccines will not be required for our crew and staff, but highly encouraged. Supply Chain Disruptions Since COVID-19, supply chain issues have become more and more common as the pandemic progresses, and due to the nature of a fast tracked schedule, we will be planning to order materials are early as 12 weeks before hand. This ensures their arrival to the job site along with site and building acclimation. A Dying Workforce Over the past 10 years, many people are swaying away from blue-collar jobs. This shortage will affect construction for the future by not having enough people to work. Many people no longer have an interest in these jobs and want an easier job that involves no physical work. We hope to continue searching for more laborers and operators to teach them the tricks of the trade. Environmental Protection Since the hotel is within 15 minutes of lake Erie, we must take precautions with weather, runoffs from this project, and the trash involved with this project. Golden Construction will be purchasing all pertainable EPA permits for environmental safety and concerns.

Golden Construction Site Safety Every day, our crew risks their lives working how they do. As a company, safety has become a huge priority. This includes but is not limited to, hard hats, Hi-Vis work wear, knowledge and comfortability of the site. Ensuring our crew remains safe, they go home to their own families. This is not an expectation but a requirement of every project we produce. There is also an emergency safety plan that employees are taught and trained in
D. TECHNOLOGY EXPERIENCE & TECHNOLOGY UTILIZED Augmented Reality Using AR, viewing your site can become a whole new experience. After putting on the virtual reality goggles, you can view your job site and begin to conduct new tasks in the blink of an eye. This technology can measure, change and show possible alterations to a project.
Hammertech Safety Use of Hammertech Safety Software throughout the duration of the project to track and acquire permits, record staff and site safety, inspections, and all additional areas of required safety. This assures that our site, staff, and trades will be within compliance to remain as safe as possible throughout all phases of construction.
Micro-Drones Microdrones are a surveying technique that uses aerial technology to map areas. These micro-drones are an extremely new technology that can help planners understand the terrestrial area.

Vice President, Troy Baden, will keep the client informed and involved in the construction process and assure that all expectations are met and/or exceeded in collaboration with the PM, PE, and Superintendent. Project Manager, Autumn Sariano, will be in charge of executing the entire project with high responsibility and satisfaction. She will coordinate on-site with corporate office activities, organizing and overseeing job site administration, field supervision, schedule progress and assisting in the procurement of subs and suppliers. Superintendent, Olivia Williams, will be responsible for the supervision of field activities such as, scope reviews of bid packages, working alongside the project manager, developing site utilization program, field construction and directing field staff. Project Engineer, Elania Delisio, will be responsible for coordinating construction and administrative requirements for bids, submittals, document control and assisting the program manager, superintendent and staff. Estimator, Jacob Herbst, will prepare estimates for this project. He will conduct all pricing for materials, labor, and technology. Scheduler, Rachel Manuel, will monitor real time progress to adjust and produce two week look ahead schedules and six week look ahead schedules. Observe subcontractor performance and contractual documents.

Golden Construction CLARIFICATIONS AND ASSUMPTIONS 1. Job site work hours are Monday through Friday from 7am to 4pm, and the job site gate will be locked after hours. 2. Saturday’s will be reserved only to make up critical path items. No work will be performed on Sunday’s. 3. Bid will only include equipment needed for self-performing activities. 4. All Specifications, RFI responses, submittals and all other supplemental documents are included. 5. No overtime work will be allowed over the duration of this project. 6. Insurance costs are per subcontractor based. 7. The owner will be responsible for additional test and inspection. 8. Any change orders and additions to the project scope after notice to proceed will be billed to the owner. 9. All work needed for utilities and site preparation are included. 10. State of Ohio taxes and Lake County Ohio is applicable for all materials and supplies for this project. 11. If the project scope increases, the fee and overhead also increase on the basis of requirements dictated by the project schedule and job conditions 12. All contract documents shall be signed by the Owner and contractor provided in the agreement. 13. Proposed estimate is as estimate of the pricing of the current market and labor prices at the time of the submitted bid.

Golden Construction ACCESS/EGRESS
The entry to the construction site will be off of Water Tower Drive. This will allow the crew and delivery route to come in and out without traffic. The delivery route is a loop to drop off material, supply and equipment for maximum efficiency.
SITE SECURITY
A gate is located at the entry off of Water Tower Drive, which will be locked at the end of each work day. There will also be security cameras located on site that will be able to monitor the entirety of the construction site and material storage. Fencing will only be along the road, because of the tree line and major highway are along the other sides.
MATERIAL LAY DOWN
Material storage will have two places on site. One will be located at the front of the site by crew parking and the other will be on the far end. These areas were chosen to allow easy access to the construction site and the delivery route has a direct line to each of them, and provides adequate room for storaged material. All materials will be remain secure and covered from the weather.
PARKING
Crew parking is located at the front of the site to minimize personal vehicles on site. They will be out of the way of the construction equipment. Overflow parking will be located across the street in the empty field.
TEMPORARY FACILITIES
Management trailers are located near the front of the site to oversee the construction site and deliverables. This will allow for easy access for all workers, contractors and easy egress. Portable restrooms are also located next to them.
TEMPORARY UTILITIES Temporary utilities will be placed next to the construction site so minimize distance and easy access to the crew.

Golden Construction VI. SAFETY PLAN To follow all safety protocols and procedures of Occupational Safety and Health Administration, OSHA. Per regulations, we must treat all employees with the utmost regard for their health and safety while working under Golden Construction. We aim to keep everyone healthy and sending them home every single day.
I. SAFETY AND HEALTH INDOCTRINATION & SAFETY RECORD AND PERFORMANCE
a. Per OSHA guidelines, we must fill injury and illness record keeping forms 300, 300A and 301 to be filled out within 24 hours of the incident. Also following OSHA standards, these records must be kept on our company file for 5 years. b. All work safety, health and project information papers will be posted near the entrance of the job site. This is accessible for all employees to view. c. At the end of every fiscal year, our company sends our injury claims to OSHA in accordance. Our safety performance is documented and assessed at the end of every year. OSHA completes our total recordable incident rate, TRIR and days away from work, restrictions or transfers, DART. The Superintendent or on-site foreman will complete these forms, all to be handed in form submission to OSHA. These files are accessible to the public and given to each company per bid or project acceptance.
II. PERSONAL PROTECTIVE EQUIPMENT, PPE
a. Per OSHA guidelines, PPE is worn to lower the chances of being exposed to hazards, chemicals and more. All crew members must adhere to all PPE regulations of all our self-performed work for our company. Anyone entering the job site must comply with all rules and regulations. OSHA complies with CDC guidelines following the COVID-19 Pandemic. Not following company policy of PPE will result in disciplinary action. This includes but is not limited to: *(Subject to Change) 1. Hardhats on Site 5. Respirators 2. Steel toed boots 6. Gloves 3. High Visibility Clothing 7. Earplugs 4. Flags on Equipment 8. Face Masks (COVID-19) Per OSHA Standard 1910.132(a), (f), & (h), “Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.”
III. HOUSEKEEPING
a. All grounds of the site must be kept in the best condition every day before the entire crew leaves for the night. All valuable tools and equipment are to be locked and stored in a Conex box. All motorized equipment and vehicles are to be shut off and locked. The on-site foreman are to take pictures of every piece of equipment and machinery once a week. Golden Construction believes in picking up every day to ensure a quick and ready start for the next day. All trash and recyclables must be dispensed respectfully to our trash and dumpsters.

Golden Construction b. OSHA Standard 1926.25 (c) states, “Containers shall be provided for the collection and separation of waste, trash, oily and used rags, and other refuse. Containers used for garbage and other oily, flammable, or hazardous wastes, such as caustics, acids, harmful dusts, etc. shall be equipped with covers. Garbage and other waste shall be disposed of at frequent and regular intervals.”
IV. CORRECTING SAFETY AND HEALTH VIOLATIONS
a. In the possibility of accidents and violations, we take steps at Golden Construction to inform you of your rights and intervene to investigate. We follow all of OSHA regulations and rules. In the event of these happening, our disciplinary action is as states: 1st offense: A written warning and mitigation for the situation will be applied on a as need basis. 2nd offense: Depending on the situation and how it occurred, some employees will be asked to leave work for the remainder of the day. Following will be mitigation with our HR employees with a written and verbal warning. 3rd/ major violation: Employee that violates their 3rd chance will be asked to leave for the remainder of the project; subject to be dismissed from company. That being stated, we are a family-based company and understand the severity of accidents. We also offer a dispute meeting based on the accident and severity.
V. FALL PROTECTION
a. As substandard 1926.501 states overall the duty to have fall protection for all employees working 6 feet above the ground and near open edges. This includes but is not limited to, installation of guardrails, safety net systems, and personal fall arrest systems. Here at Golden Construction, we believe in always triple checking harnesses and all hoisting equipment to ensure safety of our employees. b. Insurance - Insurance will be covered in our quality plan section on page 32.
B. TOP THREE SITE RISKS
I. Falls near and around edges/ Working at heights Falls account for around 50% of all construction fatalities nationally.
“The overall rate of older adult deaths from falls increased 31% from 2007 to 2016 (3.0% per year) (Figure 1). II. Material Handling Hazards in material handling on the site can include the crew and staff being hit by materials, transporting materials from the manufacture to the site, exceeding load limits, falling caused by improperly stored materials, poorly maintained lifting devices, damaged racking and storage units. III. Moving objects/equipment/vehicles on site While working on site, many vehicles and equipment can be scattered all throughout the site. This can make people fade if they are not wearing the correct PPE for our site. This could result in serious injuries or death. These equipment and vehicles move slowly as they are expensive and hard to repair, we intend to transport all equipment and vehicles properly with flashing lights and at a low speed.

The paperwork posted on the bulletin board outside of the construction site includes: Workers’ compensation, unemployment, minimum wage, minor labor laws, public employee risk reduction, workplace domestic violence, fair employment practices law, equal employment opportunity, minimum wage, family medical leave act, U.S. employment and reemployment rights Act. I. CCIP, Contractor Controlled Insurance Program Under a CCIP, (contractor-controlled insurance program) all subcontractors will be under the same insur ance and covered the same. CCIPs help everyone stay covered and safe. As a large-scale company, this option fits us best as it can be controlled in every setting. This CCIP is applied so it is a umbrella cover age of $15,000,000.00. a. General Liability Insurance “For contractors, general liability insurance protects your business from claims against injuries, damages to proper ty, and more.” - $1,000,000/per occurrence - Medical Expense Limit: $5,000 - $500,000 aggregate limit - $250,000-500,000 deductible - Endorsements: Additional insured for all subcontractors, flood, and theft. b. Workers’ Compensation Insurance “In Ohio, all employers with one or more employees must, by law, have workers’ compensation coverage. Cover age for Ohio employers and their employees becomes effective when BWC receives: a completed application for Ohio Workers’ Compensation coverage (U-3) and $120 (minimum) nonrefundable application.” Claims made policy - $1,000 deductible - Endorsements: Medical Benefits Reimbursement c. Umbrella Coverage Will cover anything that exceeds limits of (a) and (b). - $1,000,000/per occurrence - $5,000,000 aggregate limit - Endorsements: Additional insured, flood and theft.
B. OTHER INSURANCE & MISC. I. Inland Marine Insurance

“Builder’s risk covers all work on the project while construction is occurring. This lasts until the owners/buyers move into their building. Under the Statute of Limitations in the state of Ohio, owners have the right to file lawsuits for 8 (eight) years against defective building methods that are occurring in the building post completion of the project.” - $1,000,000/per occurrence

At Golden Construction, we promise to hold our subcontractors in a hold harmless agreement. This endorsement prohibits the insurance from receiving money on a claim they paid for, by a negligent third party. This agreement protects both businesses from legal action.
VII. Waiver of Subrogation This endorsement prohibits the insurance from receiving money on claim they paid for, by a negligent third party.
VIII. Fire Hydrant Permit
In Ohio, you must obtain a permit to tap into water lines through fire hydrants. This is a yearly permit that must be renewed. $150/week (52 weeks) + Application $300= ~$8,500
IX. EPA Protections & Procedures
Once a week, a foreman or project manager must complete an SWPPP, Storm water Pollution Prevention Plan. EPA provides documents to fill out to ensure environmental safety and sustainability. Every month, we report to the EPA what we have documented. This includes pictures, videos, diagrams, and safety logs.

I. QUALITY CONTROL POLICY
Committed to providing the most effective and efficient services throughout all of the building. All levels of employees are expected to watch and look for quality work and find what is not up to speed. SUPERINTENDENT Communicate with the project managers, project engineers, and all other involved parties that are on-site what is expected of them relating to quality and consistency from the executives and owners. C. SAFETY ENGINEER Run tests and inspections as often as needed to maintain the quality of safety that is expected per the vol- ume of the site and the number of workers and equipment active at said times.

SCHEDULER Estimating proper durations and communicating with those in charge to adjust and overcome any issues relating to the supply chain and our schedule of building with each material/equipment/tools. E. PROJECT ENGINEER Obtaining the information related to quality and consistency needed by the superintendent and involved parties in order to successfully maintain the level of expected quality and perform daily site inspections/ walks.
III. DOCUMENTATION OF INSPECTIONS AND TESTS
The assigned field engineer is to check processes, quality, and consistency with every site walk; this is included with reporting to all subs about any issues that arise or concern so that they can be mitigated as soon as they arise.
IV. QUALITY CONTROL PROCESS
Quality assurance paperwork will be filled out and turned in every day. This includes pictures, videos, and logs as needed. This is in addition to daily safety inspections, briefings, and reports. All of this information will be stored on a secure server, viewable by the owner and other vested parties per request, along with monthly checkpoint meetings. All building materials and equipment are to be tested prior to use. Our company also follows protocol with the material written below: i. Pre- Mobilization Meeting 1. To take place prior to any work in the morning of each day. All team members and subcontractors will review, question, and carry out their responsibilities per any new diagrams, plans, or specifica- tions brought to them that day. Additional meetings to be scheduled as needed. ii. Initial Inspection 1. As standard with any of our projects, we will do an inspection at the beginning of each phase of construction. A follow-up inspection will be performed by different personnel to ensure safety, quality, and completion. This is extremely important and must not be skipped. All inspections follow OSHA pro- tocol. More videos, pictures, and logs may be filled as the inspections continue. iv. Final Inspection 1. Project Description Each team is to provide a project proposal for the recently built Hampton Inn Madison. Located at 171 Water Tower Dr. Madison, OH. Team’s Company Description Each team can create a Company with a name, logo and description of their choice with the following parameters: You are a General Contractor located in Ohio. Your team is coming off a project of similar size and scope. You must self-perform Concrete (CSI Division 03), Carpentry (CSI Division 06), General Trades (CSI Divisions 08, 10 and 12 excluding glazing, windows and curtain walls) and can self-perform other items if you wish. Quotes for mechanical, plumbing, and fire protection will be provided. Deliverable Instructions 1. Proposal
Teams must bring Three (3) hard copies of their proposal in a three-ring binder turned in on November 30th. An electronic copy of your proposal must be submitted via email as well. The following items should be included in your proposal. (The Roman Numerals should be the titles of each tab) I.

Company Description i. Company Experience, History and Mission Statement ii. Past project experience that is similar to the current proposed project. iii. Self-Performing Capabilities and why they are important b. Discussion of five potential high-risk scopes of work that are to be performed (These risks can be financial, schedule, quality, or safety related). c. Team’s experience with technology and new technology will be utilized d. An organizational chart of the proposed staffing of the project. The chart should include at a minimum a Project Manager, Superintendent, Safety Engineer and Estimator. e. Provide a brief job description and responsibilities of each team member with how many months they will be involved on the project. This allows the contractor March – May to perform pre-construction actives such as obtaining shop drawings, procuring long lead material items and submittal submission. a.
