Re:

Physician Documentation Optimization Program Update

Good Afternoon, In late 2014, Baptist Health embarked on physician documentation optimization and introduced the ‘Physician-Led Appropriate and Intelligent Documentation’ (PLAID) Program – a consolidated program encompassing all physician-facing EHR improvement projects for the acute care setting. Since the program kicked-off back in December 2014, there has been tremendous progress made within each of the sub-projects: Cerner Physician Advancement Project (Specialty Playbook/Dynamic Documentation implementation for Cardiology, Hospitalists, General and Orthopedic surgery) o Next Steps – Migrate all Cardiology, Hospitalists, General and Orthopaedic Surgery physicians into Specialty specific positions on July 14th, 2015 o Physician Impact – There will be a new specialty specific workflow Mpage upon entry into a patient’s chart. (Screenshot below) o Dynamic Documentation (notes generated as the chart is reviewed) training and use at this time is optional. o Physicians interested in learning more about Physician Advancement and leveraging Dynamic Documentation should call Clinical Informatics at 202.1111.

Clinical Note Style and Standards (CNSS) o Workgroups were formed to develop documentation standards for Progress Note Content, Quality Requirements, as well as Regulatory and Revenue Cycle Requirements. o Clinical Notes Style and Standards documentation guidelines were reviewed and approved by the Health Information Management Committee. o Next Steps – Pilot CNSS with the Cardiology Progress Note will begin August 3rd, 2015  Web-based training will be provided. Individual training will be available on-site, but is optional. Dragon Voice Recognition Optimization Project to support ED PowerNote optimization and Physician Advancement projects o Next Steps – System-wide physician device/hardware upgrades beginning the week of July 13 th, 2015. o Training will begin at the end of July, 2015 for EDs and for Playbook physicians. Question related to the PLAID Program or to obtain a copy of the Progress Note Style Guide, please contact Sara Meinke, Physician Informatics Manger at 904.233.9493.

To:

All Baptist Health Physicians and Extenders

From: Louis E. Penrod, MD Chief Medical Information Officer Re: Anatomic Pathology Reports Since the Cerner LAB/RAD upgrade on 6/14/15, there has been a change with where Anatomic Pathology reports are located in Cerner. Issue: Anatomic Pathology reports are being produced in a new file format that is not being displayed in the same manner as previously, causing concern that this work is not being done or getting lost. The processing of specimens and generation of results is happening with the same turn around as before the conversion. Interim Solution: Anatomic Pathology reports documented beginning 06/14/2015 are located in results review.

Historical Anatomic Pathology reports (generated before 06/13/2015) will be found in Clin Doc.

Currently, both Cerner and our internal analysts are currently working on solutions that will make the reports more easily accessible. We will continue to send out updates on these issues as solutions become available. Due to the technical complexity, we anticipate a few more days until we have a final solution. Thank you for your continued patience. If you have any further questions or concerns, please feel free to reach out to Clinical Informatics at 904202-2763.
