It would be, perhaps, a little too optimistic to state that as Spring marches on, our lives are very nearly back to normal although that a fourth vaccination against you-knowwhat is available for certain groups implies that you-know-what hasn’t actually gone away! As I stated in the Autumn 2021 newsletter it is likely that you-know-what, just like the flu, will be with us from now on as a fact of living. I tried, in this introductory paragraph, not to mention Covid by name and almost succeeded. We reported in the Autumn newsletter that it had been confirmed that there would be a diabetes centre in the new hospital but since there are still many obstacles to be overcome (and more seem to become apparent every week) before that hospital becomes a reality (wherever it might be), for the time being the current Diabetes Centre stays where is it, although it would seem that there will still be a move in due course to the former Les Quennevais School. A thought from the famous actress the late Ingrid Bergman: “Happiness is good health and a bad memory.” Keep well and keep safe. Peter Tabb Editor peteretabb@gmail.com
Diabetic retinopathy is a complication of diabetes affecting the eyes and is one of the top five causes of blindness and sight loss in both the working age population and developed world. Retinal screening of patients with diabetes has been shown to reduce the risk of sight loss by allowing prompt identification and treatment of diabetic retinopathy. There are approximately 4,500 patients with diabetes in Jersey. This number is increasing, in keeping with the international rise of this disease.
Over the last 12 months the Jersey Diabetic Retinal Screening Programme has gained traction and fully launched. As a result the number of patients referred to ophthalmology from the service has been reduced by over 90% and wait times for our diabetic patients have reduced significantly. The RSP has also started repatriating patients from ophthalmology’s digital surveillance clinic. All these factors result in both cost and medical time savings for Jersey Health, along with improved outcomes for patients.
Retinal screening programmes (RSP) are described as complex compared with other screening programmes. This is due to various factors - including that entry onto the programme is predicated by a clinical diagnosis rather than an age, also that patients will require screening for their entire lives - and the service needs to make provision for these factors. In addition, the monitoring of any retinopathy may transfer between ophthalmology and RSP numerous times over the years. A RSP has a variety of possible outcomes, including surveillance pathways rather than a more distinct yes/no outcome. A robust RSP reduces the number of patients that need to enter ophthalmology by keeping them within the RSP until the point of treatment.
The RSP appointed a programme manager and clinical lead in March and a specialist optometrist in July. The appointment of these staff members ensured that strong governance was developed and maintained. The RSP adheres to most guidance by Public Health England (PHE), whilst taking advantage of being our own jurisdiction to make upgrades to our programme and develop a surveillance programme.

As a diabetic person you will be invited for, routine digital screening (RDS) this involves taking a 2D image of the retina to look for diabetic changes, the appointment will start with you having you vision measured then some dilating drops are used to make your pupils larger. Once your pupils have dilated then we take a photograph of the back of the eye. The drops will make your vision blurry and you must not drive for at least 4 hours after your appointment.
Once the photo has been taken, it is sent to Gloucester NHS Trust for it to be graded by highly trained specialists. The grading involves looking at the photos, in high definition, magnified to search out signs of diabetic changes. Once your photograph has been reviewed by one grader, it will be secondary graded by another person to ensure nothing is missed, 10% are further graded by a third person to ensure quality and assurance across all graders. The results are entered in the computer system and a result letter is printed in Jersey and sent to you and your GP within 3 weeks of your appointment.
If there are no diabetic changes then you will be recalled in a years time (possibly going forwards this will change to 2 years, if you have 2 consecutive screenings with no retinopathy). If diabetic changes are found at the back of the eye you will either be referred to ophthalmology for treatment or more likely into one of our surveillance clinics.
The effect of surveillance clinics has excellent benefits for both the health service and the patient. For the patient, waiting times are much shorter within the RSP compared to ophthalmology and for Jersey Health, it saves £152 per patient.
Surveillance clinics take on two forms, Optical Coherence Tomography (OCT) clinics and Slit Lamp Biomicroscopy (SLB) clinics. OCT is a scan of the retina which allows the clinician to view the retina in 3D to see if any further treatment is required. In the last 6 months of 2021 we have referred 7% of our RDS patients to our OCT clinic.
SLB clinics take patients who were unable to have clear photographs taken at RDS. Currently there is a long wait for cataract removals, and due to the older demographic of our patients many will fall into this category. The slit lamp usually allows a view of the retina even if the camera does not. In the last 6 months of 2021 we referred 8% of our RDS patients to SLB clinic and only 1.5% of those required referral to ophthalmology for further treatment for their diabetes.

Although the improvements to clinical timetabling allowing more patients to be seen and the setting up of surveillance clinics has been our primary driver this year, there have been many other improvements. Staff development has been important. The two screeners and the Programme Manager have undertaken and passed the University of Gloucester Higher Certificate in Diabetic Retinal Screening, our specialist optometrist has started additional courses with the University of Gloucester on OCT grading and both the specialist optometrist and clinical lead have been enrolled on the iTAT (International Test and Training) which provides external quality and assurance of their grading skills. Both business support officers are also due to undergo a University of Gloucester course in screening programme administration and failsafe.
Policies, protocols and procedures have been written to cover all area of the programme. Leaflets have been designed and translated into both Portuguese and Polish. An easy read leaflet has been developed for our patients with learning difficulties. Engagement with stakeholders has been advanced and a steering group established.
The main battle with our service is the high number of patients who do not turn up for their appointments or cancel at last minute, whilst we understand events do happen that mean you can’t make your appointment please do provide us with as much notice as possible on either 444590 or retinalscreening@health.gov.je so we can offer that slot to one of your fellow diabetics. Currently we waste between 10-20% of our capacity due to non-attendance.
And for 2022…there’s always more to do: improvements can always be made to ensure we are offering the best, research driven care to our patients. Research over the last 10 years has pointed to moving to a two year screening cycle for those patients who have had two normal (R0M0) grades, we are working with our software providers to bring this in, in a phased approach from April 2022. Enhanced communications with patients will come with use of SMS appointment reminders scheduled for later in the year.
It must however be noted that none of this would be possible without the very generous donation of two retinal camera from Diabetes Jersey in 2020 and the hard work and vision of my predecessor the late Alain du Chemin who also bequeathed monies to purchase a slit lamp to ensure surveillance clinics could take place. CHAIRMAN’S REPORT Staffing Levels
Being a reflective practitioner, I have considered the content of my report contained within the last Newsletter, which in many respects indicated some positivity, while also acknowledging disappointment in the number of posts still to be filled at the Diabetes Centre. The charity has still to finalise matters with Health & Community Services (HCS) for us to fund a health professional to deliver pro-active measures to prevent the onset of Type 2 diabetes. As an interim measure we are seeking to employ a health professional to support those individuals who have been identified as being ‘pre diabetes’. There still exists a need to recruit a number of permanent staff to deliver services provided by the Diabetes Service, despite the Health Minister recently declaring in the media that in general terms, there was not any pressure within HCS in regard to staffing levels. I would argue that in terms of secondary care for those 4,500 Islanders who have diabetes that there exist serious staffing issues. In late 2019, as part of its request for HCS to review the staffing levels, the charity was advised that HCS would create a part time post to provide a bespoke

psychological service for patients, but recruitment to the position has yet to be made. In the knowledge that a number of patients believed that they needed urgent support, the charity has considered the funding or part funding of services from private practice. This has since led to a recent meeting between the charity and a Director of HCS, where the offer has been made for the charity to make a financial contribution and for the latter to use its underspend for the post still to be filled, so that individuals practicing privately can provide some initial support. It is important to get this matter in perspective in that the vast majority of individuals with the condition do not require support, but many of those with Type 1 diabetes and a small nucleus of Type 2 patients who have complications do. The final draft of the Island Wide Diabetes Strategy was presented to HCS over a year ago for its consideration, prior to its submission to the Minister for his approval and thereafter, the Council of Ministers. As at 17 March, the Minister still awaited receipt of the proposed Strategy. In my view this is not acceptable. It is pleasing to see that the Retinal Screening Service (RSS) has vastly improved over the past 12 months to the extent that all of those over the age of 12 years, should have by now received an appointment to attend an examination. The charity is a member of the Diabetic Retinal Screening Group which meets twice a year. The Group receives management information on the performance of the department, which currently indicates the Service is meeting the vast majority of its pre-set targets. It is a sad reflection that the remaining aspects of the diabetes service do not appear to be so transparent, but the charity will endeavour to bring about change.
Elsewhere in this Newsletter, there are updates on three particular fundraising activities undertaken by Collette Labey, the O’Haire family and Kirsty Moseley, who between them has raised £30,000. You will also see an article which relates to the gift of £10,000 worth of ‘state of the art’ podiatry equipment from Le Gallais Real Estates Limited, which the charity facilitated the purchase on the company’s behalf. Kirsty Moseley’s update on completing her challenge Wow what a year, when I first set myself this challenge I didn’t fully appreciated how hard it would be both emotionally and physically but also that I would learn to love running and that my three children would embrace it and become involved too. It has been hard but I have enjoyed all 365 runs and each run has reminded me how lucky we have been to receive all of the help and support we have over the last 6 years since our sons diagnosis with Type one. I am thrilled with how much money (in excess of £5,000) we have raised for Diabetes Jersey and hope it will help other children and families affected by Diabetes.
A MESSAGE FROM THE PAEDIATRIC DIABETES SERVICE The Paediatric Diabetes Service has seen another successful year in 2021 and are looking forward to exciting projects in 2022. Dr David Lawrenson said that the COVID pandemic triggered numerous difficulties for the Service. Despite this, with the use of Virtual Clinics and access to digital data, we managed to maintain contact with our patients, and assist them in managing their diabetes. It is great to be doing face to face clinics again, knowing that staff, parents and children have been vaccinated against COVID. 2021 saw further growth in the use of new technology to support children and young people to manage their diabetes. Building on the initiation of insulin pump therapy in 2020, we started 6 further children on insulin pumps, bringing the number of children on pumps to 12 at the end of 2021. We intend to initiate at least 6 further patients on pump therapy in 2022, hopefully more. All of our patients are using some form of continuous glucose monitoring, either CGM or Flash monitoring systems. Dr Lawrenson said that in November 2021 Celia Devlin, Paediatric Diabetes Specialist Nurse, was recruited to the team. She joined the team officially in February 2022. She brings with her knowledge, skill and enthusiasm, and she is already proving to be a valuable member of the team. We wish her good luck and a long career with the team. We are now fortunate to have two full time Paediatric Diabetes Nurse Specialists, an enormous resource for the Island. Unfortunately we have not managed to improve access to Psychology for young people with diabetes, but we have identified this as a priority for future service development. Dr Lawrenson said the department continue to monitor their performance through audit. Previous audit highlighted that a systems error meant that not all young people were undergoing foot screening. We have addressed this issue and are pleased to say that the repeat audit shows that this has improved dramatically. The audit also shows that we continue to achieve good results in terms of measures of diabetes control. Unfortunately we do not have UK data to compare ourselves to, as the COVID crisis prevented audit in England and Wales in 2020 and 2021, but the audit does show ongoing good overall diabetes control for children in Jersey. The data for HbA1c levels for children meeting HbA1c targets for 2019-2021 is presented in the graph opposite. Dr Lawrenson finished in thanking Diabetes Jersey once again for their ongoing support, allowing the Paediatric Diabetes Service to deliver services to our patients that would otherwise not have been possible. With your help we have been able to be early adopters of new technologies in diabetes management. In terms of access to technology, children with diabetes in Jersey are receiving treatment packages that are at the forefront of treatments available in the UK and Europe.

RUNNING A MILE A DAY FOR 100 DAYS RAISES OVER £6,000 FOR THE CHARITY

Flynn O’HAIRE, now aged 7, has been diagnosed with Type 1 diabetes. His parents Edel and James, together with Flynn and his elder brother Shea, aged 9, set themselves a challenge to each run one mile a day for 100 days to celebrate the discovery of how to extract insulin a century ago. Edel said, “As a family we all wanted to give something back to the charity for everything they have supported us with and to help others now and in the future. Like all of those with Type 1 diabetes, Flynn is now dependent on insulin. Commenting, chairman Bill O’Brien said, “This is a significant fundraising effort undertaken by the entire family and the amount raised is exceptional. The money raised will be used in a way that will support all of the children with diabetes in the Island.”

Before the onset of the pandemic, representatives of Le Gallais Real Estates approached Diabetes Jersey offering to assist in purchasing specialist podiatry equipment to the value of circa £10,000 on the understanding that it will be kept at the Diabetes Centre and primarily used for the benefit for those with diabetes. The charity, working with Health & Community Services, identified the specialist apparatus required which has now been put into daily use in the Diabetic Foot Clinic. The equipment involved included a MolecuLight i : X which allows the clinicians in the podiatry clinic to quickly, safely and easily visualise bacteria and measure wounds at the time of treatment, a FLIR thermal imager for deep investigation within the tissues, a Huntleigh Digital Doppler, which is a hand held device to investigate the blood circulation in the lower legs and feet and a digital camera with underwater casing, for infection control issues for recording all foot problems. Tim Le Gallais, managing director of Le Gallais Real Estates Limited says, “Having had a personal interest in the impact of diabetes especially in the area of podiatry, we were very happy to support the charity in this particular initiative which we are confident, as is the charity, will be great benefit to those with the condition.” Diabetes Jersey chairman, Bill O’Brien agrees, “This gift provides a fantastic opportunity for state-of-the-art equipment to be used for those with podiatry issues and in many cases, will enable a speedier resolution to the issues involved.” From left to right James, Flynn, Shea and Edel O’Haire

Collette Labey together with her helpers staged yet another successful event, ‘The Forest’ themed Ball in November, raising in excess of £18,000. This was the fifth event organised by her with the 300 people attending enjoying a most spectacular evening. The total raised by Collette since 2012, is in excess of £100,000 and she intends to hold another Ball in the latter part of next year. For those people who are interested in attending and to avoid disappointment, the charity recommends that you purchase your tickets immediately they become available. Bill O’Brien, chairman of Diabetes Jersey commented, “ Over the past decade, Collette has devoted so much time and energy to support the charity in several ways. It is not just a matter of selling tickets for the event as the preparation commences some 18 months in advance. Together with friends who help in a variety of ways, Collette ensures that all of these events are very successful and for that, the charity is extremely grateful”. Collette added, “I would like to thank all those who supported the event, whether it be attending the Ball, the buying of raffle tickets or donating auction lots and, of course, everyone who donated prizes. CELIA DEVLIN - New Children & Young Persons Diabetes Nurse Specialist Celia recently joined the Paediatric Diabetes Team in Jersey in December. She will be working alongside Paediatric Diabetes Specialist Nurse, Keertee Beejmohun and Paediatric Diabetes Consultants, Dr Owen Hughes and Dr David Lawrenson. Celia previously worked in the Paediatric Diabetes Team at the Royal Alexandra Children’s Hospital, Brighton where they cared for approximately 250 patients with Type 1 Diabetes across East & West Sussex. She grew up in Jersey and left the island to study Paediatric Nursing at the University of Surrey in 2015. After qualifying, she worked in Great Ormond Street Hospital and then Royal Alexandra Children’s Hospital. She wanted to gain as much experience as possible before returning to Jersey for her career. Celia has missed the beautiful island of Jersey and is loving being home again.
