3 mistakes that can cost Medicare beneficiaries a fortune

Medicare is designed to help beneficiaries save money on health care. But there are three simple mistakes people can make that could end up costing them a fortune, or at least a sizable chunk of their retirement savings.
Whether you’re brand new to Medicare, approaching Medicare age or a seasoned beneficiary, beware of these common pitfalls.
Medicare Mistake #1: Not Enrolling on Time
Enrolling in Medicare can save you a considerable amount of money on your health care, but only if you enroll on time.
Many people are automatically enrolled in Medicare Part A and Part B. You may be required to manually sign up for Medicare, however, depending on your circumstances.
If you fail to enroll in Medicare Part B during your Initial Enrollment Period (a seven-month window that begins three months before you turn 65) but eventually enroll at a later date, you could be subject to a 10 percent penalty for every 12-month period that you were eligible to enroll in Part B but didn’t.
And that’s not just a one-time fee, either. The 10 percent late enrollment penalty will be applied to your monthly Part B premium for as long as you remain enrolled in Part B.
In 2022, the standard Part B premium was $170.10 per month. So even if you were just one year late to sign up and didn’t qualify for a Medicare Special Enrollment period, you’d be on the hook for an extra $17 per month added to your Part B premium. Over the course of a year, your late enrollment penalty would equal $204. Over the course of 10 years, you would pay more than $2,000 in late enrollment fees, and that’s without factoring in any Part B premium increases.
You can delay Part B enrollment with no penalty if you have what Medicare considers “creditable” health insurance coverage. Insurance provided by your employer could count as creditable coverage, but it depends on the size of the organization. Confirm with your health insurance provider whether your coverage qualifies. And don’t forget: Medicare Part A and Part D also have late-enrollment penalties of their own.
Medicare Mistake #2: Forgetting the Lack of an Out-of-Pocket Spending Limit
Original Medicare (Part A and Part B) does not have an annual out-of-pocket spending limit. That means you are
responsible for an uncapped amount of co-payments and coinsurance for Part A and Part B coverage over the course of a year.
With surprise high medical costs being the number one reason for personal bankruptcy in the U.S., this could be a vulnerable spot to be in for the average American.
Take a discectomy for example—a common procedure in older adults to alleviate lower back pain. When performed at an ambulatory surgical center, a Medicare patient can expect to pay around $2,700 out of pocket. Fortunately, there are ways to protect yourself.
• Medicare Advantage (Part C) plans sold by private insurance companies often come with no monthly premium (though you still pay your monthly Part B premium) and are required by law to include an annual in-network outof-pocket maximum of no more than $7,550 in 2022.
• Medicare Supplement Insurance (Medigap) plans cover the costs of certain Medicare copays, coinsurance and other expenses. Having protection for those out-of-pocket costs can help save you money when you get Medicare-covered care. Certain Medicare Supplement plans also include annual out-of-pocket spending limits.
Medicare Mistake #3: Not Reviewing Your Plan Options Every Year
Medicare beneficiaries have an opportunity to switch to a more affordable plan every year, but many fail to take advantage. Medicare’s annual open enrollment period is from October 15 to December 7. You can perform a number of actions during this period, such as switching from one Medicare Advantage plan or Part D plan to another.
Prior to the beginning of fall Medicare open enrollment, beneficiaries receive a notice from their current plan carrier that lists the details of any cost or coverage changes to their plan for the upcoming year. With the average beneficiary enjoying access to 40 or so different Medicare Advantage plans, it’s wise to at least spend a few minutes comparing your plan premium with the others available in your area to see if you can save by switching. ■

How to reinvent yourself after 50

Iwas recently asked to give a talk about the wonder and excitement of reinventing yourself after 50. The person who asked me said she viewed me as being a “liver” (as opposed to a kidney?) and thought my words might inspire others.
After considerable thought, I decided there are four components that influence the quality of one’s life after 50:
ATTITUDE. Your attitude colors your entire life, shaping how you perceive the world around you. A positive attitude can find the remarkable in the mundane, tolerance in the intolerable and humor in the midst of sorrow. It reminds you to embrace the person you’ve become and not care about wrinkles or what others think.
Dr. Phil summed it up when he said, “You wouldn’t worry so much about what people think of you if you knew how seldom they did.” So stop yielding to yesterday’s belief of what seniors are “supposed” to look like!
We walk around like we’ve got expiration dates stamped on our foreheads, but like our celebrity peers, George Clooney, Meryl Streep and Kevin Costner, we’re living longer and are no longer behind the scenes.
We’re a force to be reckoned with. Instead of pining over yesterday, get interested and involved in today. Reconnect with your childhood by making a snow angel, getting a squirt gun to ambush your husband, or jumping up and down on your bed. (And, don’t forget to have your medical alert button with you at all times.)
HUMOR. The more things you can find to laugh at, the happier you’ll be. Plus, laughing has many health benefits
such as lowered blood pressure, reduced stress, diminished depression and reduced aggression. Studies show that laughing more may be as important as exercising, not smoking and eating a low-fat diet. Laughter also produces an overall sense of well-being and makes your heart healthier. It’s the pressure valve that gets us through daily obstacles and challenges.
Funny things happen around you all the time if you’re tuned in. Like the time I waited for over half an hour in a crowded deli store before realizing I’d forgotten to take a number. Even though life isn’t fair, when you laugh at yourself or at your situation, you’re in control of your life.
REDUCE STRESS. We can’t avoid it but we can handle it. Playing board games regularly offers almost as much stress relief and prolonged life expectancy for people over 65 as for those who exercise regularly. Playing music can lower your blood pressure and heart rate.
Friendships are vital. Loneliness has been linked to making people more susceptible to stress, depression and other illnesses. We all need someone with whom to discuss our problems and share our good times.
PURPOSE. Become involved in something you feel passionate about. Something that makes you feel useful and necessary, gets your juices flowing, causes you to spring out of bed in the morning and keeps you riveted long past bedtime. Having a purpose reduces stress and the risk of depression.
The bottom line is to eat sensibly, laugh and play a lot, move your body and remember that age is only a number. ■

PLANNING Your retirement savings will have a great deal to do with your long-term care choices. Now is a great time to sit down with a good attorney, your financial planner and your insurance agent.
Is your retirement plan doable?

Are you approaching retirement? If so, it’s time to review your plan. The Wall Street Journal (WSJ) recently offered some tips for those hoping to retire in the next five years. Ask yourself the following:
Do you have prepaid or discounted legal services through your job?
According to the WSJ, one of the biggest regrets clients have in retirement is that they did not take advantage of this type of plan when they had the chance. If it’s available, use it to have a will and trust drawn up.
How much income will you need to retire the way you want to?
Calculate what you spend now. This might mean keeping a record of every dime you spend for a few months so you’ll have a baseline. Then make a rough retirement budget and adjust your current spending to what you expect to have at retirement.
Do you plan to travel? Are you going to work part time or pursue a second career?
By making a retirement budget and comparing it to what you spend now, you can know what lifestyle changes you’ll have to make after you stop working full time.
Where do you want to live?
Do you plan to stay put, downsize or relocate?
Factor in the costs to move and the closing costs of buying a new residence. If you plan to move to a condo, what fees should you expect to pay?
If you expect to move to another state, what will the differences be in state personal income tax, property tax and sales tax?
State and local taxes can vary widely. Some states do not charge personal income tax at all, and in other states, property taxes are oppressive. On the other hand, some states have such affordable property tax that you’ll find you can afford a bigger house than you expected.
Will you have a mortgage?
It might be a good idea to refinance your mortgage while you are still working. Interest rates are low, but may head higher. If you can’t pay off your home before you retire, consider refinancing now to reduce your payments to a level that will be easier to manage on retirement income. Remember, you’ll have your mortgage interest tax deduction to offset your taxable interest income.
What car or cars will you need?
You’ll want your overhead to be as low as possible once you retire, so it’s best to avoid car loans. If you hope to upgrade your vehicle in retirement, it might be best to do it now. Also consider whether you’ll need two cars. If one is enough, you can sell the other and tuck away the money or upgrade the vehicle you keep.
What will your taxes be in retirement?
Of course, nobody knows what the tax rates will be, but here are some points you can anticipate. If you’re contributing to a 401(k) plan, you are likely getting a healthy tax break for that. But once you stop contributing and start collecting, the tax situation flips. You no longer have a deduction, and the distributions you get will be taxable.
You might be able to avoid that harsh reality by moving some funds now into a Roth 401(k) or Roth IRA. You will pay the income due on those funds now, but your future earnings will be tax free. Plus, if you believe taxes will rise in the future, you can save by getting the tax bite over with now. Conversions can be complex, so don’t attempt them without the help of your tax advisor.
Also, if you currently have a mortgage but plan to sell your home or pay off the mortgage before retirement, keep in mind that you will lose a significant tax deduction.
What is your health insurance situation?
Will you keep the benefits you have through your employer? Of course, you won’t be able to qualify for Medicare until you are 65, and even then you will need to cover significant out-of-pocket costs.
Sit down with your financial advisor to look at what you have and what employer policies, if any, you can keep.
How is your money currently invested?
If you’re five years from retirement, you may want to shift some of your money away from growth stocks, which carry more risk, and towards something more conservative, such as income investments. But don’t move all of your money. If the market grows, you’ll want to have some skin in the game. Even this close to retirement, you still need some diversity in your investment portfolio.
What’s your game plan?
It’s easy to put off thinking about retirement until after you retire. Take some time to consider potential hobbies and lay the foundation for what you’d like to pursue in retirement, so when the time comes, you can step right into the next chapter. ■

Tips to buying your perfect home

Finding your perfect home is an exciting prospect. But there’s no greater stress than making the mistake of buying a home that you come to regret. These tips will help you prepare for the buying process.
1. MAKE A LIST OF YOUR OBJECTIVES
Think about the specific features you want in a home. Would you like a larger garage, finished basement, lowmaintenance lawn or a walk-in shower? Make your list as detailed as possible.
Go through the list again and next to each item mark if it’s a must-have, prefer-to-have, or nice but not necessary. As you begin your search, you can always add to your list or amend it. When you find a home that wows you, look at your criteria to make sure the house or condo has all or most of your must-haves, or if your priorities have changed.
2. DETERMINE HOW MUCH YOU CAN AFFORD
First, prepare a budget and figure out how much you can comfortably spend each month on mortgage and interest payments, property taxes and homeowner’s insurance. Also, consider whether there’ll be a substantial difference in your monthly utilities. Include an allowance for home repairs and maintenance as well.
Second, get pre-qualified through your bank or a mortgage company, as a lender will ultimately determine the maximum you can afford. Don’t risk getting your hopes up on a particular home until you know how much a lender will loan you.
3. CHOOSE A REALTOR
Working with a real estate agent has multiple advantages. First, they have access to the MLS system, the database in which all regional homes listed by real estate agencies appear. Second, if you find a home and want to put in an offer, your buyer’s agent will write up and submit your offer for you.
When you meet with an agent, make sure he or she is a good fit and you feel comfortable. The agent should ask you plenty of questions about your list of must-haves and be available to show homes during your hours of availability.
4. HOME INSPECTION
Once your offer has been accepted, getting a home inspection is crucial. Do your homework before hiring an inspec-

tor. Ask about their qualifications and how long they’ve been in business. Also, check with the Better Business Bureau and read online reviews.
A knowledgeable, skilled inspector will look at every aspect of the home, including windows, foundation, attic, roof, plumbing and electrical components. Your inspector should alert you to all defects, big and small. He should also note any aging components that may require repair or replacement in the not-so-distant future.
5. LISTEN TO YOUR GUT
Regardless of what the bank says you can afford, you’re the best judge of what’s really within your budget. Remember, your financial well-being and lifestyle are on the line.
Don’t get impatient. Sometimes it takes a while to find the right home. Although you may never find a home that has everything you’ve ever dreamed of, make sure it meets enough of the right criteria.
When you do find the perfect home, don’t drag your feet! Otherwise, it might get snatched up before you act.
Finally, once you make an offer, try not to get your heart too set on the home until it’s been inspected. That way, if the report comes back with costly repairs, you’ll be able to make a wise decision on whether to proceed or back out. ■

Find the docs you deserve

Decades
ago, my primary care doctor referred me to a urologist for suspected kidney stones.
After straining my urine and collecting a little vial of stones, I phoned in the results to the urologist’s nurse. In my first meeting with the doctor, I placed the vial on his desk. He leaned back in his chair, hands crossed behind his neck, and mused, “We’ll have to figure out what your pain was because young women like you don’t get kidney stones.”
“Then what are those?” I blurted out. He lurched forward in his seat to examine the stones. Thus began our “rocky” relationship.
At my next kidney stone attack, the nurse reported that the doctor was on the golf course and not answering his pager. I ended up in the emergency room, vomiting on the hallway floor. After that, I changed doctors.
As we age, we tend to spend less time jogging and more time with doctors. Our relationships with various medical professionals become increasingly important. What happens when we don’t see eye to eye with a doctor?
Complaints about doctors are more likely to be about communication than medical care.
A recent study revealed that 75% of doctors believe they communicate satisfactorily with their patients. Only 21% of those patients agreed.
As medical knowledge about how the body works continues to grow, expectations increase for doctors and nurses. At the same time, medical insurers push medical practices to accept larger patient loads. Health care may be at a breaking point, but when you are at a doctor’s appointment, your own health care is what rightly concerns you.
DO YOUR PART
Before you give up on your current team, make sure you are contributing to good communication with your doctors. Be firm but respectful of their professional knowledge. Express your concerns honestly and graciously. If these attempts don’t resolve the issue, consider another doctor in the same practice, which would cause you less disruption in terms of medical records.
In my case, I never again met with that urologist, but that was partly because I’ve never had stones again. Each episode occurred within two months of childbirth, so I stuck with just two kids and skipped further stones.
In these difficult days for health care, you may feel like you have no choice but to keep your team together. In this case, address your concerns in the way most comfortable for you: in person, by phone, by mail or email. Don’t suffer in silence because, frankly, that’s not good for your health either. ■
10 SIGNS OF A GOOD DOCTOR What you should expect from a good medical practice
1. You can schedule appointments within a reasonable time frame.
2. The front desk staff is respectful, acknowledges your arrival and makes you feel comfortable.
3. Waiting areas and examination rooms are clean and neat.
4. You’re not bombarded with pharmaceutical advertising. If there are ads and freebies lying about, sales representatives may be pushing their products. Free lunches and other perks may affect what doctors recommend for you.
5. Your privacy is protected. HIPAA laws are spelled out in all medical forms you
sign. They include rules that medical professionals not discuss patients in public places, leave records where others can see them, or share information with others without your consent.
6. Your doctor listens attentively to your concerns.
7. You can be honest with her or him. You should feel free to ask the whys about your treatment or tell them that you plan to seek a second opinion.
8. Your doctor pauses to check your understanding. According to the Patient Advocate Foundation, almost 10% of newly diagnosed patients rarely or
never understand their diagnosis. Avoid being in this group by bringing a notepad and/or a friend to appointments. Tell your doctor if you bring a recording device.
9. Your doctor is moving at a pace you are comfortable with in terms of treatment options. You feel comfortable discussing how long you should stick with a regimen that isn’t resolving the problem. Together, you discuss goals and a timeline for when to change course.
10. Your doctor coordinates with your other doctors. Healing requires healing the whole body.

What to expect from a home health nurse

The scene is becoming increasingly familiar: a knock on the door, which the patient opens to see a strange person with a couple of bags hanging off his or her shoulders.
“Hello. I’m your home care nurse.”
The population is aging. From total knee replacements to infected wounds and from pneumonia to post-operative status, the need for home care is on the rise. Insurances have created lines of limitations in the amount of days people can stay in the hospital. Wound vacs and IVs must be tended properly in order to prevent complications. In essence, home care has become like a mini-hospital on wheels.
Your first visit from a home health care nurse usually comes on the day of discharge from the hospital or the next day. Home visits can take up to two hours—you may be exhausted, but it’s important to visit with the nurse and let them get to know you and your needs.
Nurses will usually conduct a head-to-toe assessment and note any medications you’re on. They should make sure you understand proper medication, dosage, side effects and changes to report to your physician and document follow-up visits to the doctor.
The nurse works with you to complete your care plan, which typically includes how many visits are anticipated and may include such things as making sure blood glucose and vital signs are within physician-given parameters. It’s their job to educate patients on medications and wound care, changes to report to the medical staff, signs of infection and other diagnosis-specific functions.
If you need care beyond the certification period, the nurse must obtain orders from your physician in compliance with Medicare and Medicaid guidelines. Most private insurances must provide authorization for additional visits. Physical therapy, occupational therapy or social work may follow to help guide you to independence.
Openness and honesty are important. Hopefully, you and your nurse establish trust with one another and you are confident in asking questions. No questions are off limits. If you don’t know the purpose of a medication, ask. If there’s still a question, the nurse should investigate and follow up with your doctor.
Know that health providers are there to serve you. If you find that the nurse is rude, in a rush and seemingly has no “bedside manner,” speak up about it. Family members are reluctant to speak up for fear of retaliation toward their loved one.
Time is another factor. The nurse should demonstrate responsibility when it comes to arranging the visit time. Keep in mind that the nurse has other patients to see, usually in a wide geographical area, but it’s rude and unprofessional for a nurse to show up late without letting you know, or to not show up to a scheduled appointment at all.
Open communication and mutual trust with your nurse results in less anxiety and helps the healing process. Hopefully, you can look back on your recovery with accomplishment and satisfaction, knowing that you participated in the plan of care to help yourself get better. ■

Determine what level of care is right for you

Senior housing isn’t what it once was and thank goodness for that!
I worked part-time in a real convalescent hospital when I was in college. I remember walking in the front door of that nursing home and the first thing to hit you was the smell of urine. Too many patients and too few staff to adequately care for them meant the residents were miserable a lot of the time.
These days, senior care is a booming industry. Most retirement communities and nursing facilities have stepped up to meet the demand and provide excellent care, no matter how long someone plans to stay or how much care they need.
If you foresee that you or a loved one will soon need some degree of long-term care, it might be a good idea to start looking at the options. Here’s a breakdown of the major types of facilities.
Independent living. These are best for people who want convenience, don’t have specific medical care needs and can live on their own. Independent living communities usually offer meal options, housekeeping and security, and may have on-site beauty parlors and barbers, church services and transportation.
Assisted living. These are for people who need some caregiving assistance. Assisted living includes everything offered by independent living, plus help with medication management and activities of daily living, such as bathing, dressing and grooming. Some assisted living homes are apartment-like settings and others are more institutional.
Nursing facilities. Nursing homes are for people who need round-the-clock nursing care and other support services. If this is the level of home you are considering, it’s important to inquire about how emergency care is provided. How available are physicians? Are staff members certified in CPR? How does the nursing home deal with family visits? How are patients with Alzheimer’s or Parkinson’s cared for? Your local Area Agency on Aging may have some of this information available.
News of substandard care travels fast in the medical community, so ask the patient’s doctor, nurses and therapists what facilities they would recommend. Also ask social workers, relatives and friends.
FIND OUT IF THE FACILITY HAS CURRENT VALID STATE AND LOCAL LICENSES, AND LIABILITY AND MALPRACTICE INSURANCE.
WHAT TO CONSIDER
If possible, let the patients participate in the decision of where they will live even if their needs for care are far into the future. If you go facility shopping, here are a few points to pay special attention to:
• Cleanliness. Observe the facility’s cleanliness but also residents’ hygiene. It shouldn’t be hard to notice if residents are being neglected.
• Staff. Does the number of staff seem adequate to manage the number of patients? That’s not to say that employees aren’t hopping busy, but the staff-to-patient ratio should be such that all patients get bathed and receive the personal attention they need. The same is true not just for nursing care, but also general maintenance and kitchen staff.
• Services. What are the patient’s special needs? Be sure the facility provides for those needs. For example, a patient may require the use of a lift machine to get in and out of bed. Many will need physical therapy and/or a gym where they can go to strengthen their limbs. How are supplies paid for? If supply costs are added to the patient’s bill, are the costs reasonable? Are there regular increases in costs? Hidden charges? Obviously, meals are a key service. Visit facilities during meal times and observe if meals are served in a relaxed manner and if there’s plenty of food available.
GET THE UNSEEN STORY
Don’t accept any facility without first checking its financial stability, record of legal compliance and quality of management. Facilities aren’t likely to be perfect, but significant problems in any of these areas may lead to trouble finding a sufficient staff of qualified health professionals.
Find out if the facility has current valid state and local licenses, and liability and malpractice insurance.
Ask to review state inspection reports and investigate whether they have lawsuits pending. ■

Rienstra wished there was more she could’ve done for her parents when they died. She watched her father struggle with Alzheimer’s and did her best to manage her mother’s care from afar before she was moved to a hospice setting.
Witnessing the decline of her aging parents led Rienstra to HopeWest, where she volunteered her time with patients nearing the end of their lives. When she retired from her career as a social worker, Rienstra set off on a second career as an end-of-life doula.
SERVICES AND COSTS
Generally, doulas can be with a patient from diagnosis to the very end.
“Our role is to listen to and accompany people where they are,” said Rienstra.
Most doulas charge for services either hourly or as part of a package, while others base fees on a sliding scale. Doula services are not currently covered by Medicare or Medicaid.
Services may differ depending on a doula’s experience and expertise. For example, Rienstra, who started Alpenglow End-of-Life Resource & Support, provides animal-assisted therapy with her dog, JoJo. She also collaborates with a friend to provide music therapy.
Cabot uses guided imagery to help her patients visualize experiences they loved when they were well, such as walking on a beach, skiing or gardening.
EVOLUTION OF A DOULA
Originally, doulas were midwives that helped women transition into motherhood. Now the role has evolved to help both men and women transition from life with personalized support for the dying person and their loved ones.
“We’ve come full-circle,” said Mary Anne Holmes, 59, owner of Aistriu End-of-Life Doula.
Doulas are a calming and compassionate presence for those nearing the end of their life, helping to take away the fear and mysteries of death. They provide non-medical, holistic support by helping with advance directives, vigil care, providing respite and communicating with the family and medical team on the patient’s behalf.
“Having someone familiar and comfortable with the dying process as a companion is quite valuable,” Rienstra said.

“Watching my grandmother die, although very sad, was an inspiration to me,” said Cabot, 43. “She died as she lived, being very much a participant to the end. She had no unfinished business and no regrets. She had a great network of friends and allowed people to support her.”
“Lots of things are possible to immerse the patient in nature,” said Cabot. “They may need to be near a window to be able to feel the sun on their face. A bucket of sand can be the next closest thing to putting your feet on the ground. Soundtracks or a diffuser can introduce nature sounds and smells.”
Rienstra also assists patients in reviewing their life and encourages them to share and talk about important events and memories. She can also document these stories in various momento projects, which provide comforting memories for loved ones left behind.
Doula services can also help friends and family members process their grief.
Rienstra expressed the importance of family presence during the end-of-life process.
“It’s important to keep talking to your loved ones,” she said. “Hearing is the last sense to go. Tell them stories about memories and what impact their life has had on you. People are alive until they take their last breath, and can often hear, even though they may not show a response.”

What you need to know about mental health care

Onein five Americans will experience a mental illness in any given year, according to the National Alliance for the Mentally Ill (NAMI). Seniors are no exception. According to the Centers for Disease Control, anxiety, depression and mood disorders are most common in the over-55 age group. Sometimes this means you or a relative facing admission to a psychiatric or mental health facility.
WHAT TO EXPECT
If a medical professional recommends mental health treatment for you or your family member, you’ll likely be referred for one of four kinds of treatment: inpatient care, partial hospitalization, outpatient care or private therapy.
The level of care is recommended by a licensed clinical social worker who does an assessment, which is reviewed by a physician or nurse practitioner. Most assessments cost nothing, but you may want to confirm there’s no fee or that your insurance covers it. An assessment can last up to two hours and covers issues of current and past mental health status, along with relevant medical and family issues.
Some facilities are limited in which insurance they can take. However, patients who are in danger of harming themselves or others must be accepted. The Emergency Medical Labor and Treatment Act, passed in 1986, requires that psychiatric and other hospitals accept patients in emergency situations, regardless of their ability to pay.
For standard admission, know your insurance status for a hospital stay so there are no surprises on the back end. On average, inpatient care can run hundreds of dollars per day, and the average length of stay in inpatient mental health facilities is five to seven days. Some facilities require that a patient’s insurance deductible be satisfied before they’re admitted.
Inpatient care: If you or your loved one are admitted to an inpatient facility, the admission process involves a search for contraband. While most patients in a psychiatric unit will not be inclined to violence as portrayed in the movies, anything that could be considered a weapon is not allowed,
even something as small as nail scissors.
Once on the unit, patients participate in daily activities, which include group therapy, exercise and recreation, and discussion with a psychiatric nurse practitioner or psychiatrist. Individual therapy is no longer the focus of most psychiatric facilities, and groups are led by licensed clinical social workers. A team of mental health professionals will monitor your progress and work with you and your family on continued treatment. Patients are allowed visitors during limited hours. Most facilities do not offer private rooms because sharing space with another person is considered part of therapy.
Some facilities offer specialized programs for seniors. However, there’s a difference between patients with dementia and those with mental illness. Dementia patients generally won’t improve, while patients with mental illness can make great progress with the right treatment. Dementia patients may be admitted for an evaluation of their medication.
POST-HOSPITAL OPTIONS
You may be offered one of these options upon discharge: Partial hospitalization: This day program mirrors inpatient programming without the overnight stay. Patients arrive in the morning and have five or six hours of therapy, eating lunch in the facility. Transportation may be available. Many facilities offer day programs on various schedules.

Outpatient treatment: Most communities offer multiple outpatient programs, from those by traditional inpatient hospitals to counselors hosting office therapy groups. These groups generally meet for several hours in the morning, afternoon or evening, three days a week. Outpatient groups may have up to 10 participants.
Private therapy: Most psychiatrists only offer medication management, so you may see them only periodically. Counseling sessions can be as frequent as twice a week or as little as once a month, depending on one’s diagnosis and personal needs. ■

How old is old?

When
I was in my 20s, I figured by the time I reached 60, I’d be decrepit and barely able to take care of myself.
To my surprise, when I turned 60 I felt no different on the inside than I did when I was much younger. I realized mature adults are quite capable of dancing, running political campaigns, debating issues, climbing mountains and writing books (That’s what I’m doing post-retirement).
But what about the people around me? How do they view me?
According to the 2019 Aging in America Survey by Parker Health Group, younger people continue to define old age at a much younger time than we do. The survey found that Baby Boomers and members of the Silent Generation were far less likely to perceive people to be old by their 70s than Gen Xers (born 1955-1980) and Millennials (born 1981-1996).
So the younger the individual judging us well-seasoned folks, the more they might be thinking, “That guy’s too old to be doing that.”
HOLD YOUR TONGUE!
Even though younger folks think 70 is old, that doesn’t mean they think we shouldn’t be active and participating.
Well over a majority of Americans felt 80 is not too old to engage in spirited activities, including falling in love (88 percent), running a marathon (72 percent), starting a business (69 percent), getting a tattoo (68 percent), even riding a motorcycle (62 percent).
Baby Boomers and the Silent Generation were a bit more realistic about their ability to do these things at 80, but they all seemed eager not to eliminate the possibilities.
Over one-third of Americans identify gaining experience and wisdom as upsides of aging, while 30 percent credit time spent with family and friends as beneficial. Embracing a new life chapter ranks a distant third on the list (12 percent), followed by getting close to retirement (10 percent). Finally, Americans seem willing to loosen their death grip on gaining wealth and consumer goods (only 1 percent). Maybe they figure if they haven’t become rich by the time they’re old, they might as well give up.
The vast majority of respondents (88 percent) expressed at least one age-related fear. Physical health issues ranked highest among respondents (66 percent), trailed by mental health issues (59 percent). Running out of money was a top three fear for two in five adults (40 percent) while roughly half as many worry about being lonely (22 percent), being bored (21 percent) or not having the

lifestyle they expected (19 percent). Just 13 percent rank losing their physical attractiveness among their three greatest fears related to aging.
IDENTIFYING AGEISM
Americans are evenly split in their views of how the media portrays older people, with 38 percent believing they’re portrayed positively while 36 percent perceive the opposite. This leaves nearly 25 percent with neutral views.
About one-third of respondents have experienced ageism in some form. Discrimination in the workplace has affected more than one in four. Outside the workplace, one in five has faced harassment or discrimination based on age, which divided into too young (15 percent) and too old (10 percent).
What exactly is ageism? Two-thirds of respondents were able to choose at least one example from a list of five. Topping the selections was “Describing minor forgetfulness as a senior moment.” I hear my aging contemporaries use this as an excuse for their behavior quite often.
Nods were also given to social media campaigns that compare current photos of people to older ones (24 percent), birthday cards that joke about aging (19 percent), and dressing children as centenarians on the 100th day of school (14 percent). Women tend to be more sensitive to these forms of ageism than men.
WILL CHANGE CONTINUE?
Now that many are aware of the subtleties of ageism, will those who took the surveys change the way they think and act in the future?
One in five responded yes, but Millennials and Gen Xers were more inclined than Baby Boomers and the Silent Generation to expect a change in themselves.
It will be interesting to see what occurs as the world around us alters. No doubt the pandemic will impact the results, but will it dampen our satisfaction with life as we grow older? Will an increasingly aging population change our usual perceptions about staying young at heart? Will we begin thinking there are definite age limits to activities we find appropriate? Let’s not forget that views from the old and the young have always been dynamic and viewed from opposite ends of life’s spectrum. ■
