• “Germ” is a simple name for “pathogen,” anything that can produce disease. In reality, most germs – up to about 99% -- don’t harm you, and in fact, some are good for you! Some bacteria in the stomach aids in the digestive process, while other germs on your skin help keep the bad ones from getting in. We need “good” bacteria to produce yogurt and cheese. Scientists believe that being exposed to certain germs is good for you because it builds up the body’s immune system, increasing your body’s ability to defend itself. • Germs are categorized into four main groups – bacteria, viruses, fungi, and protozoa. Bacteria are one-celled organisms, so small that 1,000 of them can fit end to end on the tip of your pencil’s eraser. Some bacteria measure around 1,000 nanometers (a nanometer is one-billionth of a meter.) They get their nutrients from their environment, whatever that may be, and can reproduce outside of the body or inside where they cause infections.

GERMS (continued): • While most bacteria are harmless, the harmful ones are responsible for strep throat, tuberculosis, pneumonia, and even cavities in your teeth. • In order for a virus to reproduce, it needs to be inside living cells, which act as the host. However, they can live for a short time on surfaces such as a countertop or doorknob. Measles, chickenpox, shingles, influenza, and the common cold are examples of viruses. While antibiotics can knock out bacteria, they have no effect on viruses. • Fungi derive their nutrition from people, plants, and animals. They thrive in warm, damp places. Athlete’s foot, ringworm, thrush, and candida are all the result of these multi-celled organisms. • The single-celled protozoa often live in food, soil, water, or insects, and enter the body by these means. Malaria is transmitted by infected mosquitoes, while giardia is spread through water. • Americans miss 50 million days of work because of colds. Did you know that the speed of a sneeze is more than 100 mph (161 km/hr), spreading 5,000 droplets with 10,000 bacteria a distance of up to 12 feet (3.7 m)? • If you ask people where the highest concentration of germs is, most will probably say the toilet! However, studies indicate that cell phones are 18 times dirtier than public toilet handles, with one in six cell phones having E. coli bacteria on it. About 75% of Americans use their phone in the bathroom, but how many people wash their hands after using their phone? It’s recommended that cell phone users clean it with an antibacterial wipe once a week. Do the same with an IPad or tablet.

DEAR DR. ROACH: I have been diagnosed with multiple myeloma in the smoldering stage, based on a bone marrow biopsy and blood tests. Every three months, my hematologist checks my blood tests and tells me that when I feel bone pain, it will be time to begin chemo. While I am in the smoldering stage, is there anything I can do to prolong the shift to the full-blown stage? I am 79 and otherwise in good health. My only symptoms are some fatigue and lack of energy. -- W.K. ANSWER: Multiple myeloma is a type of cancer of blood cells -- the plasma cells, which are responsible for making antibodies. Most, if not all, cases of myeloma have a precursor stage called MGUS, monoclonal gammopathy of uncertain significance. About 3 percent of all people over 50 have MGUS, and about 1 percent of people with MGUS will develop MM per year. “Smoldering” MM is the diagnosis when the bone marrow biopsy shows evidence of MM, but there are Continued

Signs of MM include myeloma in the bones (on X-ray or CT, called lytic lesions, since they cause holes in the bones), anemia, high calcium, poor kidney function and high viscosity of the blood, which predisposes a person to strokes. In addition to looking for physical symptoms, your hematologist is searching for any of these findings. In addition, the amount of immunoglobulin in the blood predicts risk for developing overt MM (the higher the immunoglobulin, the higher the risk). A level over 1.5 g/dL puts you at high risk. Although trials are ongoing, there are no generally accepted treatments to prevent progression to MM for people with MGUS or smoldering myeloma. *** DEAR DR. ROACH: I had colon cancer nine years ago and had 12 inches of my colon removed. Quite a few times since then, I have lost control of my bowels, or have had a large movement that I couldn’t stop. Last night, I had one in my sleep. This is the first time that has happened. I take Metamucil, which was recommended by my internist, but it doesn’t seem to work. ANSWER: Fecal incontinence is a common problem, but one that is so embarrassing for some people that they won’t bring it up with their doctor. It can lead to social isolation, and is the No. 2 cause listed for nursing-home placement. Incontinence has many possible causes and, therefore, many types of treatments. Your internist is probably right about fiber, as it is very helpful in people with liquid stool, but it can make things worse in people with a stricture, such as from radiation, a common cause of incontinence. If you do try fiber, adjust amount so that you are having one or two soft but formed bowel movements daily. Other types of treatments depend on the underlying problem. Biofeedback, medications, surgery and nerve stimulation are all treatments that have been useful in some people. GERMS (continued): Your kitchen sink also is more germ-laden than the bathroom. In fact, the number one germiest place in your home is the kitchen sponge. There are typically more than 500,000 bacteria in the kitchen sink, about 1,000 times more than your toilet. Because bacteria from raw meats and other foods thrive in the sink, you should be scouring it every day with a disinfecting cleanser. Two minutes a day in the microwave will keep your kitchen sponge free of E. coli and salmonella. (Be sure to wet it first.) Dish towels can be as bad as sponges for harboring staph and other bacteria. What’s the germiest place in your child’s school? The drinking fountain, with 800 times the germs of the toilet seats! Even the lunch trays, faucets, and computer keyboards at schools have more germs than the toilet seat. Playground equipment ranks high on the list of germy spots, with little hands gripping slides and swings. What’s the worst spot in the playground? Naturally the sandbox! And one study revealed that more than half of the swimming pools tested were positive for E. coli. Don’t swallow the water! The Centers for Disease Control tell us that 65% of women wash their hands after using the bathroom, compared to 31% of men. For those who do, even the public soap pumps are home to bacteria carried from the stall to the sink. The most cost-effective means of controlling disease is washing hands. But most folks do it incorrectly, washing for only six seconds. To be effective, hands should be lathered with soap and scrubbed for 15 to 20 seconds.

TO ADVERTISE CALL 716.807.7729 don’t take it to the park mid-afternoon. Go early in the morning or late in the evening when the park is more likely to be deserted. Even with the best of precautions, dogs can bite. Should your dog bite, remain calm. Remove it from the premises. Take it to your car if out somewhere or to another room if at a home. Crate it if you can, then return to the site and attend to the person who has been bitten. Apologize for what has happened and care for the wound. Wash it with warm, soapy water and examine it carefully. Any breaks in the skin warrant medical attention. Encourage the person to seek treatment at your expense. Offer to take them to the doctor’s office or call an ambulance if the bite is severe. And, make sure you pay for whatever medical care is necessary. This will show that you are taking responsibility for what has happened and may prevent legal action down the road. Collect all of the right information. Write down your contact information and give it to the person who has been bitten. Get their contact information and that of any witnesses. Call your vet for a copy of your dog’s medical records and the police to see what further steps should be taken. Dog bite laws can vary from place to place, but most will require you to show proof of your dog’s rabies vaccination and quarantine it for a certain period of time. Do whatever is asked of you, else you could wind up in a lot of trouble.

Should your dog ever bite someone, it is important that you take action right away to keep the crisis under control and avoid legal action. The best method, of course, is to not let bites happen. Most dogs do not bite unless provoked. Know your dog and what triggers its defensiveness. Perhaps it’s animals. Perhaps it’s kids. Learn the triggers and try to keep your dog out of situations where it could be provoked. For example, if your dog is not fond of young children,

Mind your manners and be sympathetic. No matter how it may have been provoked, your dog did bite and anything you say could be used against you later in a court of law. Depending upon your behavior and the severity of the bite, the person may or may not press charges and file a civil suit. Should action be taken, hire an attorney. They will be able to advise you on the best way to proceed. While your dog may have been provoked, proving so could be difficult. If this is not the first time your dog has bitten someone, you may have a situation on your hands. Authorities could designate your dog dangerous and require you to quarantine it for a longer period of time or euthanize it. If the injury was severe or fatal, you will most likely face criminal charges. Whatever the situation, whatever the outcome, you should make it your mission to prevent the situation from ever occurring again. Whether this means you bring in a trainer for your dog, install a fence, relocate it or put it to sleep does not matter. Dog bites are serious, and you do not want history to repeat itself. Talk to your vet and your attorney in the event your dog bites and heed their advice.

GERMS (continued): • About 80% of infections are spread by hand. In fact, more germs are spread by shaking hands than by kissing! About half the population washes their hands after sneezing or coughing, and only 17% after shaking hands with strangers. • Dining out tonight? Consider the fact that restaurant menus have 100 times more bacteria than a toilet seat, and might be wiped down just once a day, perhaps with a dirty rag. It’s a good idea not to lay your silverware on top of the menu, and you might want to wash your hands after you order. And how about those ketchup bottles? No one knows what the previous person touched before handling the ketchup. If you like a wedge of lemon in your water, bear in mind that 70% of lemons’ exteriors carry disease-causing microbes. • Can you identify the most germ-laden object in a hotel room? It’s the TV remote! Carry along a package of disinfectant wipes on your next trip to take care of this. • Before loading your fresh produce into the top of your grocery cart, consider that a baby with a dirty diaper may have been sitting there just moments before, or perhaps a package of raw meat. The cart’s handle might be loaded with up to 11 million germs Those antibacterial wipe dispensers found at the front of most stores will help alleviate this problem! • Before you lock yourself up at home to avoid germs, remember that a healthy immune system that comes in contact with bad germs in small quantities will recognize them as bad and will fight them off. Alcohol-based hand sanitizers are an exceptional way to fight germs.

September 26 has been designated as Shamu the Whale Day, so Tidbits is taking this opportunity to give you the facts on killer whales and Shamu in particular. • In June of 1965, a salmon fisherman was caught in a storm near British Columbia’s Warrior Cove. He cut his net loose and headed for safety. The following morning, he discovered two killer whales trapped in his abandoned net, one male about 21 feet (6.5 m) long, and a young calf. The calf escaped shortly afterward, but the bull was offered for sale at $25,000. When there were no takers at the steep price, the fisherman finally settled for $8,000 cash from the owner of the Seattle Marine Aquarium. The whale was named Namu after a nearby British Columbian community. It took 18 days to make the 400mile (644-kg) trip down to Seattle, with Namu enclosed in a three-sided pen. • Four months later, the Seattle aquarium owner captured a female killer whale in Puget Sound, a 14-foot (4.25 m), 2000 lb. (900-kg) female meant as a companion for Namu. She was named Shamu, meaning “Friend of Namu,” however, she was anything but that. After it became obvious that the two didn’t get along, just two months later, Shamu was sold to San Diego’s Sea World, an attraction that had opened the previous year, 1964. Namu lived just a year in Seattle before drowning after becoming entangled in the cables of his pen. • While at Sea World, Shamu was trained to perform, and was a star for nearly six years until 1971, when she latched on to and refused to release the leg of a female employee who was riding Shamu at a publicity event. The whale’s jaws had to be pried apart with a pole, and Shamu was immediately retired from performances. The orca died four months later of septicemia.

The program allows it’s customers to purchase a free lunch card for vets. Customers enter the amount of their choice, sign the thank you card and hang it on the shop’s board. When veterans come to the shop they can pick a card and use it for a free lunch and Submasters in return will supply the vets with a free beverage for every card purchased.

members as well. “These brave individuals are putting their life on the line for us everyday, and just like our vets have done in the past, they never rest in their endeavors. Can you imagine if they were to only serve once a year? They’re there for us night and day, 365, we should do a little more for them than just a quick thank you one day out of the year.” Daniels said the support for the program was overwhelming last year and there were even a few cards left over, the proceeds from which went to sponsor a free Thanksgiving dinner for homeless vets through the help of another organization. “I was taken aback at the show of appreciation we saw and hopefully we can match, or even surpass those numbers this year”

Her home for the last 46 years has been the Miami Seaquarium, although plans are in the works to return her to her native habitat. • In the wild, an orca can swim at a speed of 28 mph (45 km/hr), although their average pace is about 6 to 8 mph (9.7 to 12.9 km/hr) Their diet varies from fish to seals, sea lions, sharks, rays, octopi, and squid, with some feeding on penguins and sea turtles. The average killer whale consumes about 500 lbs. (226.8 kg) of food a day. • A newborn killer whale weighs between 265 and 350 lbs. (120 to 160 kg), and measures about 8.5 ft. (2.6 m) long. An average male will reach a length of about 22 ft. (6.7 m), and can weigh up to nearly 12,000 lbs. (5,543 kg). The smaller adult females can weigh about 8,200 lbs. Heeeere’s Carson! I was eating a grilled octopus at Dimitri’s in Philadelphia when I learned that the Eagles had traded Sam Bradford to the Vikings in exchange for a first-round pick in the 2017 NFL draft. All I could do was take a deep breath, because I knew I had to pace myself. The news made a walk across the street to the New Wave, a legendary Philadelphia sports bar, mandatory. I cannot honestly say whether or not Philadelphia has the best sports fans, but I can tell you that they come off like the perfect sports-fan stereotype you often see caricatured in sitcoms and film. In Philadelphia, the mere notion of a casual sports fan is abhorrent. You don’t have to paint your face, but you must own at least one article of officially licensed clothing and you must get to the stadia off of I-95 at minimum once per year or you risk expulsion. To recap, tonight we learned that Sam Bradford, a former top pick in the draft, was traded to Minnesota, elevating the Eagle’s first-round pick from this year, Carson Wentz, to the helm. Wentz, billed as a “smart” mobile quarterback with a cannon arm, put up very impressive numbers at North Dakota State. The Eagles worked very hard to explain this rationale to their fans, and for the most part the strategy seemed to work as most Eagle fans condoned the pick. Well, at least they didn’t boo. Bradford was never really beloved in Philadelphia. Fans appreciated his talent but really disdained his fragility (yeah, and that’s how they talk at the New Wave ... “You know something? I really disdain Bradford’s fragility ... wanna shoot some darts?”). The guy always got hurt. So what do the Eagles do? They elevate a guy that got his ribs cracked open after completing 12 passes and missing three pre-season games to the starting position. He will be playing for rookie head coach Doug Pederson, a guy who brings great bonafides as a former Eagles QB and from his years of coaching the Kansas City Chiefs under former Eagle’s head coach Andy Reid. For good measure, Pederson also brought along former Mizzou QB standout Chase Daniels to back Wentz up. If Pederson thought Missouri was the “Show Me” state, he has to know that Philadelphia brings that mentality to a completely new level. Vegas has them at 100-to-1 odds to win the Super Bowl, but they are loaded with talent, having signed Darren Sproles at running back and Rodney McLeod for their secondary. They have a great set of tight ends in Brent Celek and Zach Ertz. Still, even with Bradford, this season was considered to be a rebuilding year. What do they have now? A quick survey at the New Wave sets the tone. “All I can say is this, the Eagles just better win ... I’m not into this ‘rebuilding’ (expletive),” says a fan who hails from Pennsauken. “Let’s see what this guy (Wentz) can do.” That about sums it up. Looking for a great storyline (and great meal) to follow this season? On the whole, you’d rather be in Philadelphia. Mark Vasto is a veteran sportswriter who lives in New Jersey.
