Q:

I am a mother of 4-year-old boy/girl twins. I have been debating with other mothers of twins whether it is better for multiples to be in the same class or a different class. I know some moms that have same-sex multiples separated so the children will become individuals, but I personally do not feel that mine have this problem. They have been in preschool together for two years and will be enrolled together in 4-year-old preschool. Their teachers say they are independent and play together occasionally, but play with other kids as well. I would like your opinion on the placement of multiples in the same class.

A:

While there certainly are some commonalties among human beings, every child and every sibling relationship is slightly different. There are no rules about raising twins that apply to every situation. What works for one family might not work for another. Your children don’t seem to be competitive or demeaning of each other. They have been together in school before and have done well being in the same class. They each make their own friends but also do not avoid each other’s company. It is probably comforting to each of them to have a twin around even when they are not playing together, much as a child feels safe when a parent is in the house, even if the parent is not interacting directly with the child. You are wise to be sensitive to the issue of individuality in twins. It can be an issue for some. However, your twins are not identical twins and are not of the same sex. No one is likely to call one twin by the other’s name. Few will have trouble telling them apart. Except for being the same age and in the same grade in school, they are not that

much more alike than other siblings. Non-identical twins generally only stifle expression of personality if there is a large and consistent difference in ability or dominance between them. If one twin nearly always sets the rules and the other twin follows most of the time, the more

feelings of their classmates as they make themselves feel good by putting others down. Encouraging and nurturing parents often fi nd that their children get along quite well and fi nd tremendous security from the family. These children will not be harmed by being in school together.

There are no rules about raising twins that apply to every situation. What works for one family might not work for another. timid twin might not learn to express his needs and wishes. If the two children take turns in leadership, they can both develop quite nicely being in class together, with the added benefit of the security that comes from having a sibling present. Some children are either innately competitive or are raised to be so. Most often these are children of aggressive and socially climbing parents. These twins may harm each other’s self-esteem if they are in the same class competing for grades and friends. These are the same children that hurt the

Santa is Coming to Birmingham Kids are getting their wish this holiday season: they can bring Santa to their hometown and watch him explore their favorite sites in SANTA IS COMING TO… ALABAMA! UAB Looks at Children and Sports Injuries UAB researchers have taken an in-depth look at patterns in pediatric sportsrelated injuries in a new study published in the Journal of Athletic Training. The study suggests that tailoring safety regulations more closely by age could impact the incidence of injury. It examined records of more than 2.5 million children ages one through 18 who were seen in hospital emergency departments for sports or recreation injuries during an eight-year study period. Among those, the five most common causes across all of childhood were basketball, football, bicycling, playgrounds and soccer. David Schwebel, Ph.D., associate dean for research in the sciences and professor in the Department of Psychology, and Carl Brezausek, M.S., instructor in the Center for Educational Accountability in the UAB School of Education, analyzed data from the National Electronic Injury Surveillance System, a database of injuries treated at hospital emergency departments. “There are huge numbers of children in the United States who play sports,” says Schwebel, who directs the UAB Youth Safety Lab. “Injuries can and do occur when children are active in sportsrelated activies, and often they could be prevented.” The study focused on patterns in injury incidents by exact age and gender. Epidemiology studies on injury often divide children into five-year age spans that fail to precisely account for developmental differences, Schwebel says. The data showed injuries related to certain recreation activities peaking around specific ages – playground-related injuries remained relevant up to age 9 – while activities such as bicycling caused injuries throughout the age span. Injuries peaked overall at age 14. Bowling caused the most injuries to children at the youngest age (age 4), and camping and personal watercraft caused the most injuries to the oldest (age 18). However, Schwebel emphasized that the study should not discourage parents from allowing children to participate in sports and recreation activities. “We should continue to preach safety in activities that are organized and activities that are unorganized. I think it’s the task of parents, coaches, school administrators and even children themselves to be wary that injuries can and do occur and that most are preventable.” Learn more at www.uab.edu.

Local Author Releases Next Story in Book Series Yvonne R. Edwards, author of the Chester Wiggles Series, which is about a rat who lives with his single mom, has released the second book Chester Wiggles: Family Love. The first story was Chester Wiggles: Last Great Adventure. It was featured at the Southern Writer’s Book Festival in Birmingham. In Chester Wiggles: Family Love, Chester continues to learn lessons about family, life and the world. Chester learns about the importance of the family loving and supporting each other. He also learns about the discrimination that women face in the workplace. This story also introduces a Jewish family, a Japanese man and a child with Down syndrome. The author is an alumna of Mississippi State University, the University of Southern Mississippi, and Mississippi University for Women. She has worked as a reporter for Upscale magazine and has written for other print media.

UAB Children’s Tourette Clinic selected Center of Excellence UAB Children’s Tourette Clinic was named one of 10 “Tourette Syndrome Association Centers of Excellence” nationwide by the Tourette Syndrome Association. Centers of Excellence will be awarded up to $50,000 per year for three years in an effort to improve or build high-level care for individuals living with the disorder. Tourette Syndrome is part of a spectrum of hereditary, childhood-onset, neurodevelopmental conditions referred to as tic disorders. These conditions affect both children and adults, causing them to make sudden, uncontrollable movements and/or sounds called tics (e.g. head bobbing, arm jerking, shoulder shrugging and grunting). Here are some things families need to consider while searching for volunteer opportunities: n What is the family’s goal in volunteering? n Is there an age limit? n Is the activity age-appropriate? n Does the opportunity meet required service hours for school-aged children? n Does the experience prepare their child for service in the future? Does it inspire a long-term commitment to service? n Is the parent prepared to answer questions from the child after the experience? n Does the organization provide an overview and explanation of the organization and their services?

he holiday season is a time where many families think about helping others, either to start a new tradition or to begin a regular activity together that will last all year. The possibilities are endless, so it’s sometimes overwhelming to choose. Before venturing out, there are things to consider before volunteering, says Antiqua Cleggett, public relations and fundraising manager for Hands On Birmingham, a United Way of Central Alabama program that connects people with organized and meaningful volunteer opportunities. “Depending on your children’s ages, there are lots of ways to help, both onsite and even from your own home,” she says. “Most of all, kids need to understand the importance of helping others. That lesson starts at home.” Many families, either individually or through churches and other agencies, choose to serve food at a homeless shelter, collect and deliver items for food banks, purchase food and school supplies for backpacks, cleaning/landscaping work or make Christmas cards for children who are in the hospital. 16 | birminghamparent | december 2014 / january 2015

It’s always recommended to call or schedule a visit to see if the activities offered are a good fit for your family so that everyone will benefit. For example, consider that some agencies have a limit of how many people can be in a kitchen preparing food, and children younger than a certain age must be with an adult. Most agencies are happy to provide a tour or answer questions. “We have a lot of groups that make get well cards for the kids, provide and decorate small goodie bags with crackers, cookies and water for parents to take with them to the hospital each day, kids who collect and bring pop tabs from cans, and groups that come and cook dinner for families who are staying with us,” says Stephanie Langford, special events and marketing manager for the Ronald McDonald House in Birmingham. “It is so rewarding to see how grateful these parents are whose children are in the hospital. What they are going to eat for dinner is the last thing on their minds, and this is a wonderful thing to do for them.” For kids who have an interest in helping animals, the Alabama Wildlife Center offers several opportunities, says Scottie Jackson, director of education and outreach. Young children at home can get a kit from AWC and raise mealworms for the birds, and families can come and make bird seed and peanut butter feeders, which are a “huge help.” Jackson says. “Although there is an age restriction for who can come in and help us in the clinic without an adult present, we have groups who come sweep, clean or work in our gardens. How to Pick a Pediatrician By Lori Chandler Pruitt

A

s new parents, you’ll spend lots of time with your baby in the first year of life. But there’s another person who will see your baby a lot as well – your pediatrician. Fortunately, there are lots of great choices. “Pediatric care in the Birmingham area is excellent, no matter where you go,” says Dr. Dan Carter, a pediatrician with Greenvale Pediatrics in Hoover, one of 12 Children’s of Alabama practices. “There are so many great pediatricians, and there’s no shortage.” Pediatricians have special training in the health and illnesses of youngsters through adolescence – 32 | birminghamparent | december 2014 / january 2015

and most are certified by the Article Body American Board of Pediatrics after passing a comprehensive examination covering all areas of health related to infants, children, and young adults. Parents should choose a pediatrician before the baby is born to establish care from the beginning, at the hospital, since the pediatrician coordinates efforts with the OB/GYN and attends to mom and baby in the hospital the day of birth. Many first-time parents ask friends and family for their recommendations – and that’s a great resource because friends and family can give honest and detailed information about their own experiences. Parents also can take

advantage of open houses or “meet and greets” held once a month or more at pediatric practices, so that parents can meet all the doctors in the practice, the office staff and get other information. “These are great ways to come in and meet everyone in the practice, tour the facilities and get all your questions answered,” says Dr. Kelli Tapley, a pediatrician with Birmingham Pediatrics at the Women’s and Children’s Center at St. Vincent’s, whose practice also has delivery privileges at Brookwood and Trinity medical centers. “You will spend a lot of time at the pediatrician’s office, especially in the first few years, so a good fit is important.” With a list in hand, call the office of each and inquire about the doctor’s background and training, as well as general office procedures. Make sure the doctor of your choice is taking new patients. Next, arrange for an interview during which you can meet the doctor and ask some additional questions. The demeanor and helpfulness of the front office staff is vital to a good experience as well. Many pediatricians are happy to have an in-person interview with expectant parents; although a phone interview also may work well. From the American Academy of Pediatrics, here are some things to consider:


What are the doctor’s present hospital appointments?

?

Where does he/she have hospital admitting privileges? If it becomes necessary for your youngster to be hospitalized, where would he be admitted? Is the pediatrician’s office conveniently located for you? Is it easily accessible? Are the office hours convenient for your own schedule? What is the after-hours procedure if your child needs to be seen? What is the doctor’s policy on taking and returning phone calls? Is there a nurse in the office who can answer routine questions? Is the doctor in a group practice with other physicians? Does another physician cover for the doctor at times? Who handles phone calls when the office is closed or during vacations? Do you sense a genuine interest by the doctor in the problems of your child, including particular health disorders he may have? Do both the physician and the office staff appear amicable and

courteous? Do they demonstrate compassion and patience? Or do you feel rushed in the office, as though the doctor is eager to move on to the next patient?

How are visits for acute illnesses handled? Can you make an appointment on short notice if your child needs to see the pediatrician because of a sore throat or an infection, for example? Does the doctor communicate clearly, using layman’s language (not medical jargon) to explain illnesses and treatments, and does the doctor make an effort to ensure that all your questions are answered? What are the doctor’s usual fees for sick visits, routine examinations and immunizations? What is the office policy regarding the processing of insurance forms?

In what insurance and managed-care programs does the doctor participate? If your child should ever develop a complex illness that necessitates the care of one or more specialists, will your pediatrician coordinate care among all the doctors providing treatment?

For parents whose child will be born with special needs, it’s a good idea to check with support groups, websites and parents who also have children with special needs to get good recommendations, Carter says. “If you know your child will be born with a disability, the earlier you can become connected with a pediatrician, the better, so that he or she can coordinate with the OB/GYN.” Despite such an important decision, most of the time the choice comes down to whom you trust to care for your child. “The number one consideration for most parents is the level of comfort and trust you have in your pediatrician,” Tapley says. What Type of Exercise Benefits People Living with Osteoarthritis? By Darin Tessier, MD

steoarthritis, or degenerative joint disease, affects an estimated 20 million Americans. The disease gradually wears away the articular cartilage, or protective covering of the joint ends. Once the cartilage is gone, it exposes the bone causing pain and inflammation. That’s why the American Academy of Orthopaedic Surgeons (AAOS) and the American Orthopaedic Society for Sports Medicine (AOSSM) are promoting the benefits of a balanced fitness / exercise program to prevent, and ease the symptoms of, osteoarthritis. So how do you start exercising if you have arthritis? You can begin by talking with your doctor to determine what kinds of exercise are good for you. The AAOS recommends exercise in 30 to 45 minute intervals, three to five days a week with the following goals: n reduce pain and inflammation associated with osteoarthritis n improve flexibility and range of motion n enhance muscle strength and endurance n improve balance and coordination n maintain a healthy weight Also, talk with your doctor about whether you should avoid exercise during acute pain episodes. Some health care providers advise patients to use complete rest during these situations. People with arthritis should avoid high-impact aerobic activities, such as running, jumping, tennis, or any sport that involves repeating the same movement again and again. Instead, low-impact activities, including walking, riding a bike, or swimming, are recommended to

improve cardiovascular health and control weight. The type of activity you choose will depend on the type of arthritis you have, joints involved and their stability, amount of inflammation, any joint replacements and other physical limitations. Begin slowly and eventually work your way up to about 30 minutes of aerobic activity three times per week. You

Try to choose exercises that you enjoy and fit into your daily schedule. Don’t let arthritis slow you down. In the long run, being inactive can lead to muscle atrophy and further increase joint instability. Exercise can not only help your joints feel better, but it also may decrease anxiety, improve your mood and promote relaxation.
