Many people ask me how to select the right business. Others ask me how to select a business that will be very profitable. If you observe the market, you will find that many people build businesses related to jobs they’ve had in the past. A teacher starts a new school, a graphic artist starts his own design firm, a dance teacher starts her own dance studio, a hairdresser starts her own salon and a corporate computer programmer develops and launches his own software. Others turn hobbies into money-making ventures. These include photographers, authors, aerobics instructors and professional athletes. They had a passion and turned it into a way to make a living. As they say, “A man who loves his job will never have to work again.” Then there are those who want to be independent, to be their own boss. The only thing missing is the vehicle. Take an inventory: First, your skills. Write down everything you’re good at and try to match these skills with opportunities. Look for needs and problems you can solve. Second, your passion. What do you like to do? What are you passionate about? Find a way to turn that passion into a product or service. In order to maximize your chances of success, you must be a “product of your product.” You should have a product that you like so much you could talk about it all day long. That excitement should hold you over on days when you don’t want to get out of bed. You should be powerfully moved to get up every single day to work in whatever business you choose. It’s much easier to become successful and profitable if you pick a business that suits your abilities, interests, skills, lifestyle and passion. For others it’s just a matter of money. They seek opportunities where money can be made regardless of personal satisfaction. A friend of mine told me his latest business idea, dealing with funeral homes and relatives of the deceased—no mitzvah attached. I asked him if he was ready to deal with such an environment for years to come. His answer was that there was money to be made. I’m sticking with passion. Your quality of life will be so much better by dealing with something you really love. Have in mind that half to three-quarters of your day is connected to your business. At the end of the day, though, it’s really a question of personal preference. If all you want is something that’ll get you from point A to point B, you might find a used car to be perfectly acceptable. But there’s nothing quite like driving a brand-new car, and I believe that experience shouldn’t be exclusive to rich people. Capital Motors aims to make it accessible to anyone. What’s the advantage of purchasing a car through financing rather than leasing?

First, we offer up to 100% financing, which means you don’t need a down payment. Not everyone has the cash to put down for a new car, but at Capital Motors, we believe that shouldn’t mean you can’t have one. Our unique financing methods tailor the plan to each customer, taking their needs, budget, and other factors into account. Second, leasing is different from purchasing. When you lease a car, it still belongs to the company, and you’re basically renting it from them. That means the monthly payments will be higher and you will be more limited in your use of the car: a maximum driving distance, insurance policies, etc. When you purchase a car, through whatever method—our finance-only method included—the car is listed as yours from day one. You’re the boss. That makes it a much more attractive option for private customers. Are there any prerequisites or conditions to qualify for this financing program? Anyone over the age of 18, with a steady income and residency in Israel, is eligible for the financing program. This sounds too good to be true. What’s the small print? I strongly believe in transparency. There’s no small print, no hidden fees, no tricks. If you wish, at any point, to exit the deal, you can do so by closing the loan and selling the car. How often can a customer upgrade his car once he’s purchased one through Capital Motors? This is one of the advantages of purchasing as opposed to leasing: the car is yours, and you can sell it whenever you like! If you’d like to trade it in through Capital Motors, you are welcome to do so 36 months after your purchase. Can small business owners purchase a business vehicle from Capital Motors? Absolutely. We sell commercial vehicles as well as private cars, and business owners can buy a vehicle from us. It’s also possible to list the car privately under the name of the purchaser (rather than the company) and have the receipt written out to the owners of the company for maximum tax benefits. Commercial cars weighing 3.5 tons or more that are certified brand-new are eligible for a complete tax rebate, tax credit of up to 20% per year for value depreciation of the car, and other benefits. (Of course, I’m not an accountant, and you shouldn’t take this as professional advice—be sure to consult your accountant and explain that this would be for a purchase, not a leasing contract.)

How M Ads Almost Broke My Finger
…and how to make sure Facebook Ads don’t break your wallet.

e stood in pairs that cold day in the gym at the University of Virginia. It was my turn to hold a fake gun to the head of my sparring partner, Keenan. Keenan raised his hands slowly, as if in surrender, and then suddenly pushed the fake gun away from his head and took it from my hand. And he almost broke my finger in the process. I wasn’t there yet in the gym when the Krav Mega instructor said very clearly, “Whatever you do, do not put your finger inside the trigger guard or you might get your finger broken.” Oops. As president of a pro-Israel advocacy organization on campus, it was my job to make sure everything ran smoothly for our Krav Mega event. I had been rushing to get some last minute things together right before the event. When I finally got there and opened the heavy iron gym doors, I was blown away by what I saw. Normally, ‘Hoos for Israel had anywhere from 5 to 25 students in attendance at events. This time, we had over 80 students. The year was 2009, and I had just learned about this cool new thing called Facebook Ads. It let you advertise to students on Facebook based on what school they were attending. I called our sponsoring organization, the Zionist Organization of America, secured a budget of $150, and placed some ads. Voilà. 80 people in attendance at our event. Since that event I’ve come a long way with Facebook Ads. I’ve been able to: • Help medical and therapy clinics double the number of new patients they see • Increase ecommerce sales • Take a financial advisory from around 150 to 1,258 leads per year • Fill seminars and events with attendees • Help B2B businesses acquire new clients • And much, much more HOW TO RUN PROFITABLE FB ADS CAMPAIGNS Facebook Ads are not likely to break your index finger… but if you’re not careful, they could break your wallet.
If you don’t know what you’re doing with Facebook Ads, you will probably waste a lot of money. If you want to waste time and money, follow the herd and treat social media advertising as an activity to engage in simply because “everyone else is doing it,” with “likes,” “comments,” and “shares” as the criteria of your success. The objective of a Facebook Ads campaign ought to be no different from the objective of any other advertising campaign: sales. Facebook Ad campaigns that generate 100%, 200% or better return on investment start with a clear path to getting people to move through your sales process. There are two categories of businesses that might use Facebook Ads: those that sell through their website, and those that don’t. Examples of the former would be an ecommerce store selling clothing, or a fitness coach selling an information product through their website. Examples of the latter include most professional services, expensive items, and local businesses. Let’s take a look at each category in turn. BUSINESSES THAT SELL ONLINE Products sold online fall into two categories: things that people buy on impulse, and things they don’t. product page on your website. Selling products that prospects need to consider or research is more complicated. Such products generally require marketing funnels that: • Catch the prospect’s attention • Get the prospect's contact info • Develop a relationship with the prospect through carefully crafted marketing materials • Encourage prospects to attend a “conversion event,” such as a webinar or sales call whose goal is to make the sale A long sales cycle can create cash flow issues for smaller businesses. One way to address the cash flow issue is to create an impulse purchase product at a low price point (such as $7) whose purpose is twofold: • To liquidate some or all of your advertising costs • To identify people who, unlike most of your prospects, want to buy something today (so you can upsell them other products right away) Note that each step of your marketing funnel is something that you can split test to make better. You might start off spending $4 in ads for every $1 that you make for the first few weeks, and end up spending only $0.30 for every $1 that you make three months later and for a long time thereafter, just by testing variations of each element in your funnel. BUSINESSES THAT DON’T SELL ONLINE
If your business doesn’t sell online, what you are looking for from Facebook Ads is sales leads (or, if you’re local, perhaps coupon redemptions). Facebook Ads make it easy to collect sales leads because they allow you to create ads that have contact forms built right in, so people never have to leave Facebook to give you their contact information and enter your sales process. The key to generating sales leads is to figure out what problems people in your target market are having, and then offer them something that gives them value in exchange for their time with you on the phone or in person. That is usually some variation of a free consultation, but the specifics of what form the offer takes depends heavily on what you sell.

What’s the difference between using an internal and external auditor, and in which situations is each most appropriate? An external auditor extends his opinion of the financial status of a company or a private business and acts as a company’s representative vis-à-vis the tax authorities. His job is focused on reviewing and auditing financial records and transactions in accordance with relevant applicable standards. An internal auditor is a “tool” activated by management and the board, and whose work focuses on the operational side of a business: review and analysis of a company’s appropriate behavior with respect to compliance with the law, proper management, moral integrity, savings and efficiency, and whether these serve to achieve its goals. Additional responsibilities include review of a company’s assets and liabilities including bookkeeping, as well as the manner in which it safeguards its property and manages and invests its funds. Foreign investors who would otherwise have no representation in Israel rely on internal auditors to maintain their rights over their investments here. Why is an effective audit system crucial for a business? Internal audit activity (rather than a system) that works specifically in line with the needs of an organization can help improve efficiency, reduce unnecessary expenses, assist in establishing procedures and guidelines for better management, reduce the chances of fraud-related incidents and aid managers to work in accordance with applicable laws and regulations. There is immeasurable added value in an effective internal audit. Please explain in brief about the field of forensic accounting and its application. What type of establishments require this kind of service? Forensic accounting is a special type of

audit aimed at detecting and preventing embezzlement, fraud and financial irregularities. This auditing field is based on a unique combination of knowledge, practice, experience and special training and we have vast experience executing this type of audit both in Israel and abroad (for citizens or corporations with businesses abroad). Members of our staff who deal with investigative and forensic auditing have over twenty-five years of cumulative experience. We maintain discretion and apply a “need to know” approach throughout the entire process, from referral through the various stages of the investigation, and confidentiality with respect to its results. For this reason, we strictly avoid any publicity regarding our activities. Our firm provides expert opinions in a variety of areas, including evaluation and quantification of financial irregularities, financial disputes between partners, shareholders, creditor priorities and deprivation of minority shareholders, as well as special expert opinions for silent or inactive partners. Are there any measures a corporation can take to prevent fraud and embezzlement and not merely react after it’s already transpired? I don’t believe water-tight prevention is possible, but there are definitely several key steps that organizations can take to significantly reduce the chance of fraud or embezzlement: Establish proper procedures especially in processes directly related to funds like purchasing or payments (via signatory authorities). Create proper segregation of duties – Maximize the capabilities of computer systems already in use and compensate

controls when necessary. Recruitment – At the end of the day, it all comes down to the people doing the job. Although all people come into this world as honest human beings, organizations must apply a cautious approach when recruiting personnel: request recommendations from prior employers, perform social network or other online searches, verify certification if necessary, etc. Our firm has consulted for numerous corporations in risk assessment aimed at detecting loopholes and opportunities for misconduct and extending recommendations for improvement. From experience, the cost of preventing a fraud is far lower than the cost of recuperating in its wake. Is there anything a company can initiate at the outset to facilitate the auditing process later on? For sure. Defining proper job descriptions for employees together with correlating authorizations in the organization’s information systems, setting proper procedures especially for key processes in the business, using updated information systems that are compatible with the scope of the organization – these are all a good start. Many olim chadashim, especially from the western hemisphere, are involved in non-profit organizations (in the fields of charity, education, etc.). An internal auditor can advise and assist in avoiding problems. How can your firm help local and foreign investors before they make an investment in Israel? Our economic department provides professional consulting services in all areas, including corporate valuations, preparation of

What do dual (Israeli and US) citizens have to be concerned about when filing taxes? Hats On: An Interview
with Meir Learman, Manager of Fuchs Hats Beit Shemesh Fuchs Hats has been supplying high-quality hats for the haredi and yeshivish communities in Israel for around 40 years. It began as a hat factory in Yafo, the first of its kind in Israel. The first store then opened in Bnei Brak, followed by stores in Jerusalem and Kiryat Sefer, and two years ago, a branch in Beit Shemesh—with an all-English speaking staff—opened its doors.
W
e spoke to Meir Learman, manager of the Beit Shemesh branch, to learn more about the business. What types of hats do you carry? What are the differences between them? We carry hats of all kinds, serving a very wide range of communities and featuring many different styles. We import from various factories all over the world, offering all kinds of styles, including French and American. The hats we carry are generally made of wool or rabbit felt. Wool hats are more economical, while rabbit felt hats are higher quality and last longer. However, even our least expensive hats are made with top-quality wool. We offer the best of the market at very affordable prices. What does the price of a hat tell us about its quality? The price of a hat is largely based on two factors: the density of the fibers and the shape of the hat. Higher-density fabrics are more durable and less likely to lose their shape. How do you decide which type of hat to recommend to each customer? We speak to the customer and ask what they like, how long their last hat lasted, what sort of price range they’re looking for, how the hat will fit into their lifestyle, etc. We choose the hat based on their answers and preferences. One customer may like to buy a new hat every few years; another might prefer a hat that will last for many years. One customer may want a simpler hat for weekdays and a fancier one for Shabbos; another may prefer to use the same hat for both. We help each customer find what’s right for them.
What do you recommend for a bar mitzvah boy buying his first hat? Normally I recommend going with a less expensive hat, because usually the first one doesn’t last a long time—he’ll quickly grow out of it. So I recommend purchasing a simple, inexpensive one for weekdays and a higher-quality one for Shabbos. When he’s done with the weekday hat, he can use the higher-quality one for weekdays and buy a new, better one for Shabbos. Which type of hat is most popular? Our best-selling hat is the “mondial chatanim.” It’s been on the market for around 30 years, and in my experience, it lasts a very long time. My grandfather has been wearing this hat for 6-7 years and it still looks brand-new. It’s a very practical and versatile hat, with the widest range of styles. I’d say about 80-90% of the hats we sell are from this brand. The borsalino hat—a very high-quality narrow-brimmed

hat—also gets an honorable mention. What factors should a customer consider when buying a hat? The hat should sit on the line from where the hair starts to grow naturally to the back of the head. The hat should be proportional to your body. Always ask how long the hat should be expected to last, especially with younger customers who are not fully-grown yet. Any tips for optimum hat care? First, brush your hat every two weeks, even if it looks clean. Dust collects on the brim, and if you don’t brush it regularly, the dust can accumulate. Especially if it comes into contact with moisture, it can stain the hat and be hard to clean. Second, always brush your hat in a counterclockwise motion. Third, it’s best to store your hat in a hat box to prevent it from getting crushed, and if not in a box, then at least in an enclosed space to protect it from dust. What additional services do you offer at Fuchs Hats? Our main focus is service and a positive customer experience. We offer our clients free steam-cleaning of a hat they purchased here—for life. Bring it in to any of our branches whenever you like and we’ll happily clean it for you. We will also reshape your hat for free—even if a bus ran over it! We have a professional shaping tool even in the smaller branches. We also offer brushing and chemical cleaning services. What happens if a customer is unhappy with their purchase? We offer unlimited full refunds on unused returned items. Only two people have returned their hats in the past 3 years. Our company philosophy is to go above and beyond to make customers happy. A few weeks ago, a chatan came and needed a hat for the next day, but it wasn’t in stock. Dovid Fuchs, the owner of Fuchs hats personally traveled here in the middle of the night bringing a hat from our main branch in Bnei Brak, so that the chatan could pick it up the following morning. The first thing he teaches employees is to treat each customer as if he’s the only customer, and therefore the most important. It’s a family-owned business and their yirat shamayim really comes through in the way they run their business. Our customers sense this, and they always come back.

Renovating a Chalal Anyone who has lived in Israel for a while knows what a chalal (lit: space) is. Finishing your chalal is often a cost-effective way of gaining more room in your home—especially compared to selling and buying a larger place. This article will address some of the issues involved.

ry explaining this one to a relative abroad; just imagine the conversation: “Did you hear? We started renovating. We’re finishing our space”. “Huh?”. “You know, the space the contractor gave us when he built the building”. “But if he gave it to you, then why are you only finishing it now?”. “Because it’s behind a concrete wall”. “So how do you get to it?”. “We cut a doorway in the concrete wall”. “That’s great. But why can’t you just start using it?”. “Because it’s filled with dirt.” “Huh?” So what does “chalal” refer to? A better way of saying it in English is potential space that you can finish. There are several types; I’ll describe a few in brief and then re-visit the first one in more detail. (1) Buildings are often built terraced on the side of a hill. This means that apartments lower down have the potential to be dug into the side of the hill. They will already have a concrete ceiling in the chalal because that is the floor of the neighbour above, and they might also have side or back walls. (2) Chalal kaful, or two-story space. This occurs in a duplex apartment when the living room ceiling is six meters high rather than the typical three. In this case, the bedrooms are located on the upper floor, and the kablan who built the apartment purposely did not put a floor above the salon and adjacent to other bedrooms or a hallway upstairs. He will often install a window and electrical outlets up there as well. This is all done in preparation for a renovation contractor to come along at a later date, install a floor, break a wall upstairs, put in a door, and voilà, you’ve got an additional bedroom! (3) Storage room with a lowered ceiling. This room is either inside your apartment or adjacent to it. Break down a wall or two, remove the lowered geves ceiling, add a little paint, and presto, you’ve got a room of a normal height!
PLANNING PERMISSION It goes without saying that the client should check with the relevant municipal authorities for proper planning permission prior to starting such any chalal project. Assuming permits are in place, the client chooses a contractor. One of the conditions of the building permit is to use a kablan rashum, registered contractor, so make sure the contractor you choose has these credentials. The contractor assesses the job, examines access to the location and gives a price. Sometimes this is tricky because the contractor doesn’t know what’s behind the wall—whether the space will be filled with dirt or empty. TERRACED CHALAL Working off the architectural plans, the contractor performs nissur, concrete cutting with a wet saw, on the wall where a door will eventually be located. He uses this as his access

point to get in and out of the chalal. Because the building contractor built another floor on top, he filled this area with dirt, so it’s up to the renovation contractor to now get rid of it. CLEARING OUT THE DIRT If access allows, a bobcat is brought in, sometimes in conjunction with other heavy-duty digging equipment. This certainly is the quickest way to clear out the dirt. If this is not possible, the old-fashioned method is employed—one bucket at a time—to a ballah (canvas sac) for crane disposal, or to an awaiting dumpster. On one particular job my workers filled 8 dumpsters with dirt! Sometimes this chalal area will be empty and no dirt removal will be necessary. It all depends on your luck (and how the building contractor built it originally). WATERPROOFING & STRUCTURAL At this stage, the contractor refers to the structural diagrams from an engineer. The area being finished is underground, and proper waterproofing must be done to prevent ingress of water or moisture. He also needs to build concrete retaining walls to prevent dirt from the sides or back from caving in. I have unfortunately seen jobs where an unscrupulous contractor dug 20cm beyond the dirt, built a block wall, and called it a day—no waterproofing, no retaining wall, no nothin’! The structural plans dictate the steel and concrete required to build these components. The contractor needs to pump in concrete via a special pump located on the street, as a regular pump truck cannot be used. FINISHINGS Once the dirty work has been done—clearing out the dirt and creating a safe structure— the contractor finishes the area like any other renovation. He works according to the architectural plans and builds interior walls, bathrooms, plaster, flooring, drywall, painting, etc. Some challenges relating to this type of chalal are getting natural light in, finding a waste plumbing point to attach to, and ventilation. I have worked with very talented and creative designers and architects and together, we have come up with solutions to all of the above challenges and more. Chalals are certainly more challenging than most projects, but with proper planning and an experienced contractor, you can be enjoying your enhanced chalal, er, space, in no time at all.

Whats Up? Interest rates are, up that is. On November 26, 2018, the Monetary Committee of the Bank of Israel (BOI) decided to increase the interest rate from a rate of 0.10 percent to a rate of 0.25 percent. This rate has not been increased in a long time (about seven years) and caught many analysts by surprise. Considering that interest rates in the US have been increasing for a while, we may be seeing a long-term shift upwards in interest rates in Israel. By Moshe Wilshinsky

imagine you are asking yourself: Why do rising interest rates in the US concern someone living in Israel? That’s a good question. The answer is because in today’s global economy, the US is the world’s largest economy. In fact, the BOI press release cites, among other factors, what is happening in the US. To better understand where we are today a little historical context is needed because historically with such shifts, the mortgage qualification process is adversely affected. As a result of 9/11 and the stock market crash of 2001 (often referred to as the “bursting of the Internet bubble”), the Federal Reserve, which is the central bank of the US (the FED), repeatedly lowered short-term interest rates to historic levels. In 2008, the world financial markets suffered one of their worst crashes. As a response, the US and other governments around the world enacted Quantitative Easing, a monetary policy where the government buys its own government bonds, which keeps demand high thereby lowering long term interest rates (a bond’s value and interest are inversely related). This monetary policy created what is commonly referred to as “cheap money.” The cheap money enabled businesses, government bodies and consumers to borrow more than ever to acquire assets and utilize resources. This, in turn, helped jumpstart the economy, especially in the US, which, immediately after 2008, many were afraid was on the brink of an economic depression. The post 9/11 interest rate drops affected
the Israeli mortgage market as well, lowering interest rates and increasing loan amounts. Just when everyone thought rates would start increasing, 2008 brought Quantitative Easing as a response. This maintained the lower interest rates in many countries; while in Israel, even though Israeli mortgage interest rates remained low, the BOI introduced many mortgage regulations and one of the effects was limiting the size of the mortgage a borrower can get approved for. In the past few years while there were periods where long-term and mortgage interest rates in Israel increased, they would go lower as well in part because of the lower interest rates in the rest of the world, especially the US. The Fed has been raising interest rates on a regular basis for a few years and recently made it clear it is ceasing Quantitative Eas-

ing. At this point, in the US, the sentiment about interest rates is more one of wondering how far interest rates will go up than if they will drop long term. The situation in Israel isn’t as clear. Although the interest rate that the BOI just increased is the leading interest rate index, and even though it’s a short-term interest rate, the fact that they increased it may cause the market to increase long-term interest rates as well. One of the ramifications of all this is called interest rate risk. For a borrower, it means the possibility of interest rate (i.e. cost) increases and higher monthly payments on adjustable-rate mortgages (or parts/maslul, thereof). For a lender, it means less profitable mortgages. The Basel III regulatory framework, another 2008 by product, actually discusses interest rate risk and the importance of taking it into account. Regulators like the BOI base their regulations on the Basel framework. For example, how banks take into account the possibility of variable interest rates going up in the future in the approval process can affect the loan amount a borrower can qualify for. If the Israeli mortgage banks start to assume the inevitability of the BOI increasing interest rates, mortgage banks will increase mortgage interest rates in anticipation of those future increases. In the month following the BOI rate increase many things changed. The US stock markets have lost significant value, which caused the interest rates in US bonds to drop and the new governor of the BOI has been quoted as saying: “It is important that the interest rate is not raised too rapidly or aggressively.” So, in fact, no one knows if, when, or how high mortgage interest rates will rise, but it’s a real possibility and mortgage borrowers, both current and future, should be prepared because with mortgage interest rates, what goes up does not always come down. For anyone with a mortgage that has a portion of it with a part (maslul) linked to the Madad (CPI) or has an interest rate that is adjustable, now is a good time to see if it makes sense to refinance to a fixed non-linked interest rate mortgage. For anyone thinking of getting a mortgage in the future: budget for higher interest rates. You also may face more scrutiny in the approval process. Winter, Reinstatement Value, Foreign licenses By Shimon Cohen

inter is here. Now that the log fire / heater is warming you up, a few words about fire insurance.
What is it? The definition of fire in most policies is the appearance of sparks or flames. (In other words, damage caused by a hot iron would not be covered under standard fire cover). Also included is smoke damage and explosion. Spontaneous combustion is not covered, nor is theft as a result of the fire. Obviously, if you own your home you should make sure that you are covered for fire damage for your contents as well as for the actual house/apartment. (This is done either through a private insurance policy or through your mortgage). However, for those renting homes, it is extremely important to check your contract and see whether you or the landlord is responsible for this cover. Other aspects of fire cover are: • Payment for alternative accommodation. • Payment of rental (depending on amount of insurance). In our experience, most fires have been caused by unattended Shabbat or Chanukah candles and by clothing left near electric heaters – so, be warned! Insurance Tip - Kinun (Reinstatement Value) Have you ever received an insurance claims check where an amount seems to be deducted
from the payment, with the mysterious words “kinun” printed next to the deduction. Translated, this means “re-instatement value”. An example: Let’s say you pay NIS 3,000 annual premium to insure your car that is worth NIS 80,000. You have an accident, which costs NIS 10,000. The cost of the repair is covered by the insurance but now you have “used up” NIS 10,000 of the cover and therefore need to “top up” the policy. The amount that you actually pay is dependent on a) the cost of the damage and b) how much longer the policy has to run. In other words, the two extremes are: having an accident on the first or last day of the policy. On the first day you would pay maximum “kinun” because you have to ‘top up’ the policy for a full year, and on the last day there is

no “kinun” at all. When the new insurance year begins, everything starts afresh. When do you not have to pay the Kinun? 1. When there is a total loss—in this case you use all your premium to pay the total cover—or when the accident happens on the last day of the policy. 2. Bituach Hova—In claims of personal injury there is no re-instatement value. 3. Any case where the client decides not to replace or repair the damage. For example, if you decide not to repair your old Subaru, you will be paid out minus the VAT but without the reinstatement clause kicking in. “The times they are a changin’” New Conditions for driving with a foreign license in Israel 1) Non-residents must hold a valid foreign driver's license for the type of car they are driving in Israel. 2) They must be 17 years or older. 3) They are not unfit to receive an Israeli license. 4) They have not been in Israel from the date of their last entry for more than a year. 5) The definition of “last entry”: Where the non-resident was abroad for at least 3 consecutive months. Example: A tourist or student comes to visit Israel once or twice a year. As long as he/she went back home for at least 3 consecutive months and doesn’t stay in Israel for more than a year, then he/she can use his/her license every time. Problem: A yeshiva student who is here for Shana Bet. He arrives for his first year from chul. He can drive for the full year on his Foreign license. He then goes home for bein hazmanim and returns for Elul. If he doesn’t return to chul for at least 3 months, his last entry is calculated from when he arrived for Shana Aleph and he no longer has a valid license in Israel!! In a case like this, he/she must immediately convert his/ her license to an Israeli license. How to do this? The student must present his/her passport, study visa, eye exam, medical exam and foreign license to misrad harishui. N.B.: This is my understanding and interpretation of the rather complicated law. Measles
Exclusive Interview with Professor Eli Somekh, Director of the Pediatric Wing of Mayanei Hayeshua Medical Center The measles virus has been rearing its ugly head in the past year, with an unprecedented rise in cases all over the developed world, including Israel. According to the most recent Ministry of Health data, there have been more than 2,000 cases throughout Israel since the beginning of the outbreak, most of them in Jerusalem.

n this article, we have asked Mayanei Hayeshua’s Professor Somekh to provide information about the disease, its symptoms, its treatment, and most importantly, how to prevent further spread. What is measles? Measles is a serious and sometimes fatal viral infection most often occurring in children. Its symptoms include fever, dry cough, runny nose, sore throat, conjunctivitis (inflamed eyes), and characteristic rashes. Measles has several stages that unfold over the course of two to three weeks. For the first 10-14 days after infection, the patient has no signs or symptoms; this is
called the incubation period. Then the patient normally contracts a mild to moderate fever, often with a cough, runny nose, inflamed eyes and sore throat—symptoms that seem typical of most other viral infections. This stage lasts two to three days, and is called the prodrome. Next, a rash begins to appear, normally starting on the face: small red spots in tight clusters that make the skin look splotchy. The rash spreads over the course of the next few days throughout the whole body. During this time, the fever rises significantly, often reaching 40 or 41 °C. Finally, the rash begins to recede, fading first from the face and last from the lower extremities. In most cases, children will recover from the disease naturally. But the fatality rate,
even in developed countries, is somewhere around 1:1000 —higher when complications develop or when the child has other “common” health issues. Some of the complications of the measles virus include: ear infections, laryngitis, croup, bronchitis, pneumonia, encephalitis, and pregnancy complications including preterm labor, low birth weight and maternal death. An additional severe complication that may develop 7-10 years after having measles is SSPE, a terminal neurological disease. How is measles treated? Because measles is a viral—rather than bacterial—infection it cannot be treated directly once it’s been contracted. Treatment of measles normally involves managing the symptoms and treating secondary infections and other complications as they arise. This treatment is suitable for immune-compromised patients. Vitamin A deficiency has been found to increase the severity of measles, and therefore children who are found to have low levels of the vitamin and/or patients with a severe illness are treated with high doses of vitamin A. The best treatment for measles is prevention—that is, the vaccine. The measles vaccine is extremely effective with a 97% success rate, and involves minimal risk: extensive medical research has shown that there are no long-term side effects. The vaccine is normally administered at one years of age, with a second shot given in first grade.

If you have not been vaccinated and you’ve been exposed to measles, your doctor may recommend a post-exposure vaccination. If the vaccine is given within 72 hours of exposure to the virus, it may prevent the disease, and if not, it can make the symptoms milder and decrease the duration of the illness. Another option is an injection of antibodies, called immune serum globulin, which can have a similar effect if given within six days of exposure. This option is particularly suitable for people with a compromised immune system who cannot get the live measles vaccine. If most people recover naturally, and my child is healthy why should I bother with the vaccine? First of all, even when measles isn’t fatal and no complications develop, it is still an extremely unpleasant illness. Why force your child to endure that suffering when you can prevent it from happening in the first place? Second of all, and most importantly, while your child may not be severely affected by measles, he may infect others who are not as fortunate. Measles is an extremely contagious disease—90% of unvaccinated people exposed to it will contract it. It is the only virus that can be contracted from a carrier who is no longer in the room (up to two hours after the infected individual has left the room). The period during which measles is contagious lasts about eight days, starting four days before the rash appears and ending when the rash has been present for about four days. That means that your child will be contagious even before you know that she is sick—and by the time you realize that she has measles, she will already have exposed others to the illness. Children who have been vaccinated will probably not contract the disease, but there are plenty of children who can’t be vaccinated, such as infants who are too young, and children undergoing treatment for cancer, whose immune systems are compromised. Measles can be fatal to these children. Vaccination is a shared social responsibility. We vaccinate our healthy children to protect our unhealthy children who are unable to receive the vaccine. When lots of healthy children are not vaccinated, the disease is able to spread and infect people who are more vulnerable. But isn’t the vaccine risky? No. The measles vaccine is extremely safe.

There is a vast amount of high-quality research documenting this, and not a single credible study has found a link between the vaccine and autism or any other serious condition. You may have seen articles online calling this fact into question; every single one of them is based on faulty science and a conspiracy-theory mindset. Vaccines are safe, effective, and crucial for public health.
The best treatment for measles is prevention—that is, the vaccine. Some parents may be concerned about the fact that the vaccine contains preservatives or other chemicals that may be harmful to humans in large amounts. What’s important to understand is that toxicity is not a question of the substance itself, but of dosage. Even the most benign substance, such as water, can be fatal when consumed in large quantities. The vaccine contains these substances in amounts that are completely safe for human consumption and some of these preservatives, such as mercury, are no longer added to vaccines. In fact, you are naturally exposed to some of these chemicals in your regular food and drink. There is 10-60 times more formaldehyde in an average pear than in a vaccine. Your body knows how to process these chemicals and get rid of them in reasonable amounts. Why is this outbreak happening? After the introduction of the measles vaccine, infection rates dropped dramatically—almost to the point of eradication—in developed countries such as Israel. to vaccinate their children. As mentioned above, in order to protect those who cannot be vaccinated against viruses like measles, a certain percentage of the population must be immune. We call this herd immunity. The fewer children vaccinated, the more the disease is able to spread. What do Rabbanim say on this issue? Gedolei Torah from all over the world have spoken out very clearly on this matter and their opinion is one and the same: vaccinate your children. In fact, it was Rav Dr. Menachem Brayer, deputy medical director of Mayanei Hayeshua Medical Center, who initiated the joint call from a group of highly respected rabbanim urging parents and communities to vaccinate against measles. Among the rabbanim who joined the initiative are the head of the Eida HaHaredit Beit Din, Rabbi Moshe Sternbauch; Rabbi Yitzchak Zilberstein, known for his expertise on medicine and halacha; Rabbi Moshe Klein, rabbi of the Hadassah Ein Kerem Hospital; Rabbi Shimon Baadani and others. Their joint declaration states that “the Torah granted doctors the power to heal, and it is a mitzvah, even pikuach nefesh. And if one refuses [to vaccinate], it is though he has spilled blood.” Similar statements have been made by such eminent personalities as the Rosh Yeshiva of Ponevezh, HaRav HaGaon Rabbi Baruch Dov Povarsky and the head of the “Tiferet HaTorah” yeshiva in Williamsburg, Rabbi Yoel Roth. It is extremely unfortunate to note that the majority of cases of measles in Israel were discovered among the charedi communities in Jerusalem. Those who claim to avoid vaccination in the name of Torah are, as the above statements unequivocally declare, in severe violation of it. Where can I get my child vaccinated? If you or your child missed the MMRV vaccine at one and/or six years of age, it is still not too late. Contact your physician or Tipat Chalav staff immediately. The Talmud teaches that anyone who saves one life is as if he has saved the entire world; vaccinating your child means saving not just one, but thousands and thousands of lives. r. Gil Garalnick is a graduate of the Leeds University Dental School and has been practicing since 1997. After a year of vocational training, he worked as an associate for three years, and then opened his own office, where he offers his patients all aspects of general dental care focusing on implants and aesthetic dentistry. What made you decide to become a dentist? Originally, I wanted to become a maxillofacial surgeon, but after finishing dental school, I decided that going through medical school would not work well with my plans to get married and start a family. So I went into general dentistry, and haven’t looked back. Over the years, I learned from experts in all disciplines of the field, focusing on aesthetic dentistry and implants. What do your patients particularly like about you? I try my utmost to approach my patients in a calm gentle manner, and to treat them as individuals with their own needs and sensitivities. I find it inspiring to be able to diagnose and treat patients correctly, while making them feel at ease. Helping patients look and feel better is very rewarding and can even be life-changing for some patients. The smiles and ‘thank you’ patients give me when leaving the office make me feel good about what I do. Any tips for maintaining optimal oral hygiene? I always encourage my patients to keep up good oral hygiene by offering three simple reminders – brush twice daily, floss most nights, and don’t snack between meals. Your teeth need a rest too!

Preventing and Treating Winter Illnesses By Eliyahu Shechter
T
he emergency room and departments are full, and there isn’t a single unoccupied bed in the hospital…” We are used to hearing such announcements from newscasters in recent years. The Ministry of Health and the general public scramble to prepare for the onslaught of seasonal infections with the annual flu shot, antibiotics, and more. But is that the only way to prepare our bodies for winter? Does natural medicine offer an alternative, or do we need to rely entirely on vaccines and medications? In this article, I would like to propose a new approach to preparing our immune systems for winter, based on the philosophy of naturopathic medicine. I will start with a question many patients ask me: how is it that they’re always catching viruses or bacteria at work, while their co-workers—who are sitting in the same room, exposed to exactly the same germs—feel fine, and weather the winter in perfect health? Natural medicine teaches that germs don’t indiscriminately attack everyone in the environment. They attack when they detect weakness—that is, a person whose immune system is compromised. Pathogens settle in and multiply unopposed, because there’s nothing there to fight them. One might compare this to an ant invasion: when ants are swarming our kitchen floor, we might also ask, “Why are they coming in to my house and not anybody else’s?!” And the answer is that they come where they can find food. Viruses and bacteria are also looking for a place where they can flourish. Therefore, the real cause of the illness isn’t the virus or germ in and of itself, but the weakness in the person’s body or spirit. This means that in order to properly prepare for winter, we don’t need to “declare
war” on the viruses or bacteria; they’re not the real cause of illness. Instead, we should focus on the real cause of the problem by taking responsibility for our own health. By strengthening our general health, our immune systems will protect us from whatever we’re exposed to. There are two complementary paths to strengthening our systems for winter. The first is avoiding elements that weaken our health, and the second is consuming or otherwise using elements that strengthen our health. The following are some elements that weaken our health, according to “Human Nutrition and Health” by Yitzchak Ben Uri: an unhealthy diet, pollution, pesticides and chemicals in our food and water, obesity, smoking, alcohol, lack of exercise, emotional stress, medications and difficult life circumstances. By contrast, here are some factors that strengthen our health: a healthy diet, fresh air, good sleep, good water, creative physical activity and serenity. The more we adopt these principles, the healthier we will be. In fact, this type of “pre-

ventative care” is more effective when we turn it into a way of life and not just a temporary solution in the months before the winter. However, even just a month or two of following these principles can help, depending, of course, on the person’s baseline of health. I would like to make some practical dietary suggestions that can help even in the short term, to prevent or alleviate seasonal infections. They are particularly important for people who have a tendency to catch these infections. However, please note that none of these suggestions can replace a proper evaluation and treatment plan adapted to the specific needs of the patient. Things to avoid: Dairy products, sugars, processed wheat items, peanuts and peanut butter, processed snacks, and fried foods. These foods can increase the production of mucus, which stresses the body and wastes energy, making it harder for the body to fight the germs. Additionally, sugars weaken the immune system by increasing inflammation. Things to consume: Base your meals primarily on fruits and vegetables, nuts, legumes, and whole grains. These foods are light, nourishing, and neutral. They can also assist in cleansing the body of accumulated waste. You may also want to consider taking vitamin C regularly, which strengthens the immune system, and Omega 3, which is a strong anti-inflammatory. If you start to feel sick, especially if you have a fever, you can ease your body’s fight with the germs by minimizing or avoiding eating—and drinking plenty. Drink water, water with lemon, or herbal infusions that warm the body, such as cinnamon, or those that strengthen the immune system, such as ginger. If you feel weak, and you need more energy, you can soak dates in water and drink the water. This way, the body’s energies are not wasted on digesting foods and are instead devoted to fighting the virus. Herbs such as echinacea, which fight infection naturally, may also help. Of course, only do these things after consulting a professional. In most cases, given the right conditions, the body will know how to deal with various illnesses in the best, most precise and often the fastest way. Remember that G-d provided the cure before the illness; healing is within us. If we trust it, we will enjoy good health, and a happy, healthy winter. Dental Care During Pregnancy Did you know that the hormonal changes of pregnancy can affect your oral health? Dr. Eli Prenzlau explains this phenomenon and offers advice on dental care during pregnancy. By Dr. Eli Prenzlau D.D.S.
D
o you avoid going to your dentist while you’re pregnant? Relax! You can and should keep up with your dental appointments, as delaying treatment can cause further problems. Both the American Dental Association and the American College of Obstetricians and Gynecologists agree that oral health care during pregnancy is both safe and important for you. Pregnancy can trigger oral health problems, including gum problems, and dental erosion. GUM PROBLEMS During pregnancy, hormonal changes can increase the risk of developing gum-related problems. Gingivitis, or inflammation of the gums, may increase, and can cause your gums to swell and bleed. To prevent and treat this, you need to put in extra effort to care for both your teeth and gums. Make sure to: 1. Brush twice daily for two minutes with a toothpaste that contains fluoride. 2. Clean between your teeth once daily with either floss or other interdental cleaners.
PREGNANCY GINGIVITIS When left untreated, pregnancy gingivitis can develop into full-blown periodontitis: a condition where the infection spreads beneath the gum line. This can damage the tissues that hold your teeth in place, and without treatment, the teeth can actually loosen and may even fall out or need to be extracted. Some women might even develop ‘lumps’ on their gums during pregnancy. These rarely hurt, but can disturb brushing and make it harder to keep your teeth clean. They typically go away after the baby is

DENTAL EROSION The loss of the hard, outer enamel layer of your teeth is called dental erosion. This can happen as a result of repeated exposure to acid—such as after frequent vomiting, which many women experience during their first trimester. To prevent dental erosion caused by vomiting, rinse your mouth with a teaspoon of baking soda dissolved in 1 cup of water after you vomit. But don’t brush right away! The rinse will help stop the acid from attacking your teeth and will give the enamel on your teeth a chance to harden again. You can brush after about an hour. OTHER CONCERNS Dental Radiographs (X- Rays) X-rays are safe during pregnancy. The American College of Obstetricians and Gynecologists recommend the use of a protective lead apron to cover your throat and abdomen during radiographs. In my office, I use two aprons on all pregnant women, just to be on the safe side, and limit x-rays only to those that are absolutely necessary during the first trimester. Medications Safe options are available during pregnancy for local anesthetic, antibiotics and pain relief medications, as needed. Talk to your dentist regarding alternative drug therapy that can be prescribed. Always double-check with your family physician or obstetrician regarding both the dosage and time these drugs may be taken. Although treatment is safe throughout pregnancy, you may be most comfortable visiting your dentist during the second trimester. Nausea and vomiting could be a problem during the first trimester, and during the third trimester, you may be uncomfortable when the chair is reclined because of the weight of the baby. Make sure to tell your dentist if you are uncomfortable or feel light-headed during treatment, as the chair position can easily be adjusted to help you feel more comfortable. In conclusion, don’t avoid treatment during pregnancy! See your dentist (and hygienist) regularly to help keep your mouth healthy. And last but not least, may you have an easy and comfortable pregnancy and birth. Besha’ah Tovah!

Meet Chaim V’Chessed’s Newest Team Member Being here for Israel’s English-speaking community is a responsibility that Chaim V’Chessed does not take lightly. Over the past three-and-ahalf years, the once-fledgling organization has exploded into a non-stop chessed operation, with an average of 50 new cases to tackle each day.

haim V’Chessed offers assistance to English-speaking residents and tourists in all aspects of life, including special education, mental health, geriatrics, government offices, kupat cholim and health care. The organization is departmentalized by areas of service, and each department is staffed by experts in its field. Chaim V’Chessed’s medical department has been operating beyond capacity for a while. Mrs. Nechama Nulman, department coordinator, acts as the liaison between callers and their healthcare providers, hospital staff and on-site Chaim V’Chessed hospital representatives. The reps, Zevi Weingarten and Aralla Weingarten, are both native Israelis with years of experience interfacing with hospital staff, smoothing out complications and ensuring that patients’ needs are met. Zevi and Aralla circulate between Shaare Zedek and Hadassah Ein Kerem hospitals. They advocate for patients and guide them through the confusing medical terms and foreign hospital mentality. But the need for hospital assistance just keeps growing. This October, Chaim V’Chessed welcomed a new team member. Meet Mrs. Shani Wahrsager, a born Israeli who is intimately familiar with the Anglo mindset and culture. It’s rare for Israelis to work so well with Anglos. What’s your secret? I was born in Israel, but my parents are American. I grew up in the Israeli system and I have a fluent Israeli Hebrew, but I’ve lived and worked all my life with Americans. I get their mentality and I speak their language. How did you hear of Chaim V’Chessed? I’ve been working in the world of English-speaking non-profits for years. partment was a perfect fit. What is your role? I work with Nechama Nulman in the medical department. Nechama’s been handling all the behind-the-scenes medical cases on her own until now. My job, ultimately, is to phase into her current role as the in-office liaison. This will free her up to spend more time in Jerusalem hospitals. Zevi and Aralla Weingarten are phenomenal patient advocates. They accomplish so much and have affected the lives of hundreds of people. But the need is growing. Adding Mrs. Nulman to the hospital representative roster will not only increase Chaim V’Chessed’s presence in the hospitals, but will also offer patients more emotional support. You’re new to the organization. What’s your take? I can tell you one thing: I don’t like being bored, and that has yet to be a problem. It is unbelievable what goes on here. The amount of people we help every day is mind-boggling. When I told my kids about the amount of cases we handled one day, my daughter said, “What? It sounds like every English-speaker has Chaim V’Chessed on speed dial!”
