Boards of Health Collaboration Initiative 2015 Non-Competitive Request for Application

Blue Hills Community Health Alliance (CHNA 20) invites local health departments to participate in the Boards of Health Collaboration Initiative. This initiative offers assistance to groups of public health nurses in their effort to address chronic disease prevention and wellness. Up to $5,000 in grant funds are available to each group to build capacity and develop a program to address a shared community health need.

The Blue Hills Community Health Alliance (CHNA 20) represents the 13 communities of Braintree, Canton, Cohasset, Hingham, Hull, Milton, Norwell, Norwood, Quincy, Randolph, Scituate, Sharon and Weymouth. Established by the DPH in 1992, CHNAs are local coalitions of public, non-profit, and private sectors working together to build healthier communities in Massachusetts through community-based prevention planning and health promotion. The mission of CHNA 20 is to improve the overall health of local residents through increased coordination and delivery of services and the mobilization of community resources. This is achieved by promoting collaboration between CHNA 20 partners, providing support to local health initiatives and prevention programs and increasing awareness of local identified health issues throughout the communities it serves. The Boards of Health Collaboration Initiative is the first CHNA 20 program targeted to health department leaders. The goal of the project is to strengthen the capacity of public health nurses to collaboratively address a local health need related to chronic disease prevention and wellness. Applicants must submit a short application including: name and affiliation of collaboration members; one paragraph summary of the project; list of anticipated challenges as they implement the proposed project; expected benefits from efforts over the next 6-12 months; and name of the fiscal agent/conduit where the grant funds can be deposited. This is a non-competitive grant application available to all CHNA 20 catchment health departments. Applications should be submitted by February 1, 2015. Once approved, recipients will be awarded up to $5,000 in grant funds. Technical assistance will be available for groups to help guide them through the application process. Receiving a grant through the Boards of Health Collaboration Initiative does not exclude health departments from applying and receiving grants through other CHNA 20 grant programs. ELIGIBILITY: CHNA 20’s Boards of Health Collaboration Initiative is targeted to health department leaders and public health nurses working towards building capacity and addressing shared needs related to chronic disease prevention and wellness. Groups must meet the following criteria to be eligible. • Groups must serve three or more of the 13 communities in CHNA 20 (listed in RFA). • Groups must be from Boards of Health or Health Department developing and/or implementing chronic disease prevention and wellness projects in their communities. • Groups must have a fiscal agent or conduit (municipal agency, Board of Health, health department) where grant funds can be deposited. POST AWARD EXPECTATIONS: Once the application is approved and funded the following are expected from each Coalition: • Grant funds and services used within 12 months of receiving grant approval. • A one-page summary report of activities, tangible benefits and expenses associated with the BoH Collaboration Initiative grant. Please include number participants, pre/post surveys and other data where relevant. Report submitted to CHNA 20 within 30 days of the end of the 6-12-month grant period. • Participation in one or more of CHNA 20 member meetings, workshops or programs.
