Mini-Grants Program Start-up Substance Abuse Prevention Coalition Initiative Non-Competitive Request for Application
Blue Hills Community Health Alliance (CHNA 20) invites your community to join the many others in Massachusetts working to reduce substance use. We know that starting a new coalition can be overwhelming without the right organization or funds to execute your ideas and plans. Through our new Mini-Grants Program, CHNA 20 is offering assistance to start-up substance abuse prevention coalitions located in one of the 13 communities we serve. Up to $1,500 in grant funds, one-on-one technical assistance and customized training are available to each new coalition working towards building capacity and expanding community actions.
The Blue Hills Community Health Alliance (CNHA 20) represents the communities of Braintree, Canton, Cohasset, Hingham, Hull, Milton, Norwell, Norwood, Quincy, Randolph, Scituate, Sharon and Weymouth. Established by the DPH in 1992, CHNAs are local coalitions of public, non-profit, and private sectors working together to build healthier communities in Massachusetts through community-based prevention planning and health promotion. The mission of Blue Hills Community Health Alliance (CHNA 20) is to improve the overall health of local residents through increased coordination and delivery of services and the mobilization of community resources. This is achieved by promoting collaboration between CHNA 20 partners, providing support to local health initiatives and prevention programs and increasing awareness of local identified health issues throughout the communities it serves. The Start-up Substance Abuse Prevention Coalition Initiative is a CHNA 20 program targeted to newly formed coalitions not eligible for resources available to more developed coalitions. The goal of the initiative is to help new groups lay a foundation and increase capacity to eventually have access to state and federally funded substance abuse prevention grants and technical assistance programs. Applicants must serve one or more of the 13 communities in CHNA 20 and submit a short application. The application includes a one paragraph summary of the history of the coalition, names of group members and affiliations, a list of strengths and challenges of the coalition, expected benefits from your efforts over the next 12 months and name of the fiscal agent/conduit where the grant funds can be deposited. Applications are due by July 31, 2014. Once approved, recipients will be awarded $1,500 in grant funds to be used for community outreach, marketing and planning. We are making accessible technical assistance expertise for new coalitions to help guide them through the start-up process. Up to 10 coalition members from each funded coalition will also be invited to participate in a regional substance abuse prevention conference scheduled for the fall of 2014.

ELIGIBILITY: CHNA 20’s Mini-Grant program is targeted to newly formed substance abuse prevention coalitions working towards building capacity and expanding community actions. Coalitions must meet the following criteria to be eligible.

Applicants must serve one or more of the 13 communities in CHNA 20 (listed in RFA). Applicants must be from a community based coalition developing and/or implementing substance abuse prevention programs in their communities. Coalitions who have received state and/or federal grant funds directly are not eligible for the CHNA 20 Mini-Grant program. Coalitions must have a fiscal agent or conduit (Municipal Agency, School Department, Board of Health, Non-profit or other public entity) where grant funds can be deposited.
POST AWARD EXPECTATIONS: Once the application is approved and funded the following are expected from each Coalition:

Grant funds and services used within 12 months of receiving grant approval. Grant funds of up to $1,500 used for outreach, marketing and other community action efforts. Access to up to 15 hours of technical assistance (TA) services from CHNA 20 for capacity building and planning purposes. Ability to invite up to 10 members of the Coalition to participate in a regional Substance Abuse Prevention Conference and Training. Access to a series of post-conference Coalition leadership meetings. A one-page summary of activities, tangible benefits and expenses associated with the grant submitted to CHNA 20 within 30 days of the end of the 12 month grant period. Participation in one or more of CHNA 20 member meetings, workshops or programs.
