First Day Home

Randolph Community Mental Health & Substance Abuse (Lead Organization – Friends of the Randolph Seniors. Partnering Agencies - Randolph Senior Center, Interfaith Social Services): Improve the functioning and quality of life for Randolph Seniors by providing a series of workshops focused on healthy living, mental health and substance abuse. Workshops were offered at the Randolph Senior Center and resident facilities for seniors.

What Every Parent Should Know About Substance Use (Lead Organization – Quincy Public Schools Health Services Team. Partnering agency Impact Quincy, The Quincy Police Department): 5,500 copies of an informational booklet “Open Your Eyes” on teens and substance use were researched, created, printed, and distributed to the parents and guardians of all middle and high school students. The information provided in the booklet was supplemented by “Parent Nights” at each school. The booklets are currently being distributed by Manet Community Health Center’s pediatric practice.

Fitness Gram

(Lead Organization – Braintree Public Schools; Nursing, Food and Physical Education Departments): Braintree Public Schools was able to bring the Fitness Gram program into the physical education curriculum to improve student’s physical activity as well as to encourage them to value the importance of physical activity. The Fitness Gram computer program was installed in teacher’s computers, professional trainings were provided, teacher’s administered the program, and printed individual profiles were sent home to parents.

Health and Wellness Planning & Implementation

(Lead Organization – Weymouth Public Schools. Partnering Agencies - Regional Center for Healthy Communities, Manet Community Health): “Healthy Wey” is the town’s initiative to get its citizens eating healthy and moving more. Funds were used to continue the school district’s work in the development of the program design, action, and evaluation plan for the Weymouth Public School Health and Wellness program.

Hockomock YMCA Healthy Weight Intervention Program (Lead Organization – Hockomock Area YMCA. Partnering Agency - Sharon Public Schools): The Hockomock Area YMCA expanded the healthy weight intervention program, Promoting Achievable Change, into the community of Sharon. This was done by offering the program to Sharon Public School students, training Sharon wellness teachers to offer the program, and increasing our referral network to include additional pediatrician and family practitioners associated with Norwood Hospital.

New Directions Counseling & Maria Droste Group Therapy Counseling Sessions (Lead Organization – Interfaith Social Services. Partnering Agency - Maria Droste Services ): Offered free community-based group counseling to low-income residents of the thirteen CHNA 20 towns to address community mental health needs. Group therapy participants were recruited from physicians and from among InterFaith’s food pantry patrons.

CHNA 20’s mission is to work to improve the overall health of local residents through increased coordination and delivery of existing services, expanded community

actions, and the mobilization of community resources.


GRANT RECIPIENTS

2013/2014 CHNA 20 Community Grants

Over the years CHNA 20 has funded programs through its Impact Grant, Community Grant and Mini-Grant programs. Since 2009 CHNA 20 has distributed over $175,000 in grants.

Asthma Education for Child Care Providers

2013/2014 CHNA 20 Impact Grants Delivering Wellness – Innovative Programming for Vulnerable Populations from Eight to Eighty (Lead Agency – Manet Community Health Center. Partnering Agencies - Germantown YMCA, the School Wellness Committee of

Weymouth, the Mass in Motion Cancer Prevention Outreach of Weymouth, the Super 8 Motel in Weymouth, and the Arabic Community in Quincy): Delivering the Healthy Living concept adapted to multiple age groups in multiple communities of the underserved. Implemented the Red Light, Green Light, Eat Right Program for school age children, The Arabic Walking Group, The YMCA Garden Project and Weymouth Mass in Motion Cancer Prevention Program. Manet seeks to influence and make an impact on the growing epidemic of childhood obesity through offering age relevant programs to underserved populations in Quincy and Weymouth.

Guiding Good Choices - Train the Trainer (Lead Organization – Scituate FACTS Coalition. Partnering Agency - Scituate Public Schools): The Scituate FACTS offered the evidenced-based, NREPP-rated substance abuse prevention skills training course Guiding Good Choices to community members, teachers and parents in Scituate, Hingham and Marshfield. Workshop Leaders successfully completed the training and have the curriculum materials to implement trainings in other CHNA 20 communities.

(Lead Organization – Asthma & Allergy Foundation of America – NE Chapter): The Asthma and Allergy Foundation of America, New England Chapter (AAFANE) trained child care providers and preschool teachers about asthma and allergies in the CHNA 20 region. The providers learned to manage children’s asthma and allergies and to prevent and handle asthma and allergy incidents in child care settings.

Chronic Disease Self Management

(Lead Organization – South Shore Hospital Home & Health Resources. Partnering Agency - Randolph Senior Center, South Shore Elder Services): The Chronic Disease Self Management Program, “My Life, My Health” is a standardized program, developed by Stanford University. Certified instructors teach a series of classes, which includes strategies for living a healthy life for those suffering from chronic health conditions. The series offered was a workshop given two and a half hours, once a week, for six weeks, in community settings which were held at the Randolph Department of Elder Affairs and the South Shore Quincy Y.

Elder Dental

(Lead Organization – HealthCare Options. Partnering Agencies Local Dentists): The Elder Dental Program provided quality, affordable oral health care to low income older adults through a network of dentists who provide treatment at drastically reduced, sliding-scale fees. They held three annual free oral health screening clinics in the community for adults age 60 and over which offer dental screening exams, oral cancer screenings and include professional denture cleaning and labeling at no cost.

First Day Home Program

(Lead Organization – South Shore Hospital Home & Health Resources): First Day Home Program is designed to meet the immediate needs of a patient on the day of discharge from a hospital or facility. A comprehensive nursing visit is made to promote a successful transition home and to prevent re-hospitalization. First Day Home Program home visits were made to frail elders in the communities of Braintree, Milton, Randolph and Quincy.
