As part of the employment process, new employees must complete the following: •

Provide acceptable proof of immunization/immunity under the Immunization of Personnel policy (HHN-CP-000).

Attend an appointment with an Employee Health Nurse within 30 days of start date.

Complete the Physical Health Profile with Employee Health.

Failure to complete the profile/immunizations through Employee Health Services within 30 days of hire will result in the notification of Human Resources for further actions Directly Affected Employee Health PROCESS Within the first week of hire, new employees, FacilicorpNB personnel will be advised that as a requirement they must complete the following steps as part of the condition of employment: •

Human Resource will schedule an appointment for the new employee with the Employee Health Nurse.

Human Resources will provide the new employee with the immunization form and ask them to have this completed with their health care provider.

The Employee Health Nurse will review the immunization form for completeness.

If immunizations are required, the Employee Health Nurse will provide these to the new employee.

The Employee Health Nurse will complete the Physical Health Profile with the new employee. •

Complete physical screening includes: height, weight, BMI, blood pressure, pulse, glucometer and cholesterol testing.

Check contributing risk factors for cardiovascular disease, lung disease, and diabetes.

If risk factors are identified, the Employee Health Nurse will review strategies and information that may help reduce personal risks.

Explain the ergonomics assessment process to all employees.

Explain blood and body fluid exposure protocol to all new employees.

Title Contraindications/Limitations If an employee is rehired or transfers within the Regional Health Authority within 12 months immunizations and the Physical Health Profile is not required to be repeated, and the Employee Health Nurse will review the existing documentation. DEFINITIONS Employee - a person, including a salaried physician, who is hired by Horizon Health Network for a wage, salary, fee or payment to perform work but does not include an independent contractor and its representatives. Others - individuals, other than employees, contractors, Medical Staff, students and volunteers, who provide services to Horizon, paid or unpaid, and/or require access to Horizon facilities, information or networks. Acceptable Proof of Immunization – completed health assessment form signed by a physician or nurse practitioner, an immunization record form or a provincial immunization record.
