Substance Use Disorder (SUD) Guest Speaker Dr. Beau Nelson Psychologist, Buddy-to-Buddy Program 734-780-5725 (cell) claytonn@umich.edu Guest Speaker Summary • Problems with finding care for substance use disorder outside of the VA lead veterans and their families to the treatment being costly and many of the practices are not evidence based care that supports service members and their families. • There are two different types of treatments: Inpatient and Outpatient care. o Inpatient treatment facilities are what most think of when people say they check in for 90 days. This process includes a detoxification process and counseling with begin after the detox has been completed. o Outpatient treatment is when a patient will receive treatment from a rehabilitation facility but they go home. They are required to check in everyday to receive medications and counselling. o Deciding which option is better depends on the patient and their care provider. Inpatient treatment provides a low risk or relapse and access to therapies that will help build a life outside of drug use. And the detoxification process can be monitored by a health care professional. Outpatient treatment has a higher percentage of relapse and if detoxing there can be more complications.

Treatment for National Guard and Reserve soldiers can be tricky; they must self-identify or they must go to leadership stating that they have a problem and would like help prior to any positive drug testing results that has been conducted by the unit. If they wait until after they have tested positive by a unit administered test they will be subject to any UCMJ punishments. If the soldier is working they can utilize their private insurance to get care. There are a few other community resources to assist those who are need of substance use care; 12 steps programs are available for those who have problems with nicotine, narcotics, and/or alcohol. For many identifying the signs of a problem is not easy. Most of the time the signs are recognized by friends, family members, and co-workers before the person themselves sees any harm being done to those around them. What that means is using medication along with other traditional therapy sessions. Here are a few that used regularly with great success: Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), Rational Emotive Behavior Therapy (REBT), Dialectical Behavioral Therapy (DBT), Eye Movement Desensitization and Reprocessing (EMDR), and Seeking Safety (And Other Trauma Focused Therapies). There are sometime that medication is needed to support the counselling that the patient is receiving. Medications can be used for helping decrease symptoms of withdrawal, relapse prevention, and co-occuring (treat drug use and mental health issues). There are various medications to help with various substance use disorders. Some are taken prior so that is the substance is ingested the patient will feel ill and will not want to use that substance any more with continued use. The most effective tools while treating a patient with a substance use disorder is having family support, along with getting educated about other community resources, having family there is an effective method to reduce relapse.
