What is the Standardized Safety Questionnaire (SSQ)?



SSQ is an acronym for Standardized Safety Questionnaire. SSQ is a system designed for ranking, managing, and evaluating a contractor’s safety. As a contractor it is free of charge to you although if your Operator requires document uploads, you must join CAP to utilize this tool

Fall Protection Compliance Program





Policy  The requirements of this program shall be in effect and enforced under the safety program at any time an employee of Employer is required to perform work exposed to a fall 6 feet or more to a lower work level.

Purpose  The purpose of this program is to ensure compliance with OSHA’s 29 CFR 1926 Subpart M, Fall Protection requirements.

Scope  The requirements of this program shall be in effect at any time employees of Employer are required to perform work with the potential to fall 6 feet or more to a lower work level.

Fall Protection Compliance Program 

Program  Enforcement of Fall Protection Compliance with the fall protection program and procedures are a condition of employment while working at Employer  To ensure the safest work environment possible, and to prevent accidents and injuries, action will be taken immediately if a violation of the fall protection program is observed. Management personnel at all levels are responsible for the enforcement of the company safety rules.  Any violation of the fall protection program will result in disciplinary action, up to and possibly including termination. 

  

Fall protection will be mandatory when exposed to a fall of 6 feet or greater.

Only approved anchor points will be used as part of the fall protection system.

Only full body harnesses will be used as part of the personal fall arrest system.

Self retracting lifelines shall be used as part of the personal fall arrest system whenever feasible.

All components of an individuals personal fall arrest system, (harness, lanyard, anchor points) shall be inspected prior to use.

Should the pre-use inspection identify any defects of fall protection system components, notify your supervisor immediately and remove the component from service.

Fall Protection Compliance Program 

Duty to have Fall Protection  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be tied off using all required fall protection.

Training 

Employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas:  







The nature of fall hazards in the work area The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used.

The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.

The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection if necessary.

The OSHA fall protection standards.

Training 

Certification of training  The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the
