WELCOME TO THE 70TH ANNUAL CONVENTION & EXPOSITION

Welcome IT’S TIME FOR A FRESH PERSPECTIVE

We’ve said this before, but this year, it’s even more so: you are going to have an amazing time at convention, and here are just a few reasons why.

LET’S START WITH THE NEW LOCATION We will be holding our three-day convention at the brand-new Mystic Lake Center in Prior Lake. As you walk through the doors into the atrium, you’ll be captivated by the floor-toceiling windows, vast hallways, ample restrooms (yes, these things are important), small group seating areas, easy access to the exposition, and of course, the showroom—all of which gives your convention experience a fresh perspective!

NEXT IS THE EDUCATION If you’ve been to our conventions before, you know we rely on your evaluations to make your next experience better. You’ve suggested shorter sessions and we heard you! All education sessions will be an hour in length—which, in turn, allows us to offer over 120 regular sessions PLUS 16 Minibytes! (A Minibyte is four, 15-minute related topics packaged together.) There will be sessions that are appropriate for general audiences, as well as specific to profession (i.e., HR, housing, and dietary); and target interests such as workforce, dementia, and older adult protection.

KEYNOTES What can we say…they are extraordinary! On Monday, Amy Dee, a national motivational speaker and author, will provide an uplifting keynote that will make you laugh while

discovering practical ways to develop kindness and positivity and deal more effectively with negative people and situations. Tuesday’s keynote features Stuart Gray, founder of Hospitality Rocks. Stuart will help each of us determine and clarify our specific gifts and how we can bring these to every human interaction—Stuart will renew your passion for caring!

Patti Cullen, CAE, President/CEO Care Providers of Minnesota

IT WOULDN’T BE A CONVENTION WITHOUT AN EXPOSITION Our exposition returns in a beautiful, dedicated space, filled with exceptional companies, looking to connect with you! Be sure to stop in and say hello on Monday, 12:30–5:00 PM (with a reception from 4:00–5:00 PM) and again on Tuesday, 8:00 AM– noon. Government/nonprofit tables are open on Tuesday, 8:00 AM– 2:00 PM.

NEW CELEBRATION OF CARING And finally, we want to make you aware of our new CELEBRATION OF CARING RECEPTION, which is a free event for all convention attendees. We are switching things up—again, a Fresh Perspective—and this year, we invite all attendees to join us in a celebration. We are celebrating the first ten years of our successful Leading4Life program, including this year’s graduates and alumni. We will also be celebrating the achievements of our scholarship recipients, national quality award recipients, and recipients of the Lifetime Achievement, Public Official, Chairperson, and Member of the Year awards. The Celebration of Caring Reception starts at 4:00 PM on Tuesday, November 13, complete with a DJ, complimentary appetizers, and

Sharon St. Mary, Chair, Board of Directors Care Providers of Minnesota

cash bar. We want to see you there, so come join in on the fun, and celebrate our accomplishments together! There are many other fresh perspectives available at this year’s convention—we encourage you to page through this guide to check out all the extras that the 2018 Care Providers of Minnesota Convention & Exposition has to offer: the reflection room and self-care offerings; our updated convention app; headshots; area restaurants; great ways to spend your down time; and more! But for now, our hope is that however you experience this convention—your convention—you will go back to your workplace with a FRESH PERSPECTIVE that will further your success in the senior care profession.

SEE YOU IN NOVEMBER!

LEARNING SESSIONS // 100 SERIES // MONDAY, NOVEMBER 12 // 9:30–10:30 AM

101: QAPI—Two Phases Down, One to Go The first two phases of the Requirements of Participation are implemented, now the third phase is looming. For once you have a little runway space to implement changes. Find out what this last phase means and how to prepare for it. Ms. Debi Damas, RN, Senior Product Manager, Relias

102: Resident Wellness—Thriving is More Than a Trend At The Waters, resident wellness is six-dimensional. Thriving is a way of being, an experience that is nurtured through whole, person-centered care and programming. Thriving is not just an outcome produced, it is the art of our engagement and practice. Our mission is to go beyond “activities” by creating programs and events intended to engage our residents on a physical, spiritual, and emotional level. Join us to explore bringing wellness to life at your community. Ms. Katherine Todd, DNP, MBA, RN, PHN, Vice President of Health and Well Being, The Waters Senior Living, LLC

103: Dining Strategies for Senior Living Dining strategies throughout all levels of care within senior living communities will be discussed in this session. Each stage of life is different and requires unique strategies to be in place to support our residents’ satisfaction and health. This includes the independent and assisted living residents who value menu choices, variety, and quality within their dining. Also your more dependent and/or memory-impaired residents, who deserve dignity and choices during dining to maximize their independence. Ms. Amy Johnston, District Manager, A’viands Ms. Kate Munson, MS, RDN, LD, Healthcare Wellness Manager, A’viands

104: Marketing: Be a Catalyst for a Radically Different Future Forward thinking marketers will learn tools and practical advice to implement right away. Attendees will learn the latest digital strategies to gain a competitive edge over competition, eliminate referral agencies, better understand prospects, drive top-line revenue, and increase ROI. The panel will demonstrate how one Minnesota provider is leveraging new technology to deliver sales-ready leads to the community, demonstrably impacting top-line revenue, and delivering sales-ready prospects. Ms. Monica Hunter, Director of Business Development, Tealwood Senior Living Ms. Jenni Jarrel, Strategic Account, Bluespire Marketing Mr. Matt McNeill, SVP Digital & Marketing Solutions, Eldermark Software / Simply Connect HIE

105: Bill for Services Today Rather Than Cost Reporting Tomorrow Minnesota Department of Human Services (DHS) auditors have been disallowing costs that can be billed separately since the value-based reimbursement rate setting method began. This has drawn attention to the fact that nursing facilities can bill for many items, but are not doing so. Billing for an item is a little more work, but it benefits providers by reimbursing for costs much sooner than expensing the item and putting it on the cost report. Of course, the facility will not get any reimbursement if DHS auditors disallows the cost! Mr. Greg TaBelle, Reimbursement Director, CliftonLarsonAllen LLP

106: Hospice: A Vision for Better Care at the End of Life Too many people die badly. Despite the best in medical science and technology, many people with advanced, life-limiting illnesses suffer needlessly in the final stages of their lives—and die in ways that leave their families with legacies of pain. This session discusses the current status of dying in America, the elements of good end-of-life for dying individuals and their families, and the urgency of developing better ways to provide high-quality hospice and palliative care. Ms. Lisa Abicht-Swensen, MHA, LNHA, Director of Hospice and Palliative Care, Allina Hospice & Palliative Care

107: Pest Management in Healthcare Facilities Many structural pests can cause serious health problems for affected individuals, making effective pest management an essential form of preventive healthcare. Unfortunately, medical facilities are no less susceptible to pest infestations than are homes, farms, or food handling facilities. This session will provide an overview of several common pests, emphasizing their damage and how to prevent, detect, and safely treat infestations and how to determine when professional help is necessary. Mr. James Kopco, PhD, Technical Director, Guardian Pest Solutions

108: Managing Challenging Behaviors in Mental Health This session will provide education on common mental health issues and strategies for managing challenging behaviors. We’ll take a comprehensive look at multiple mental illness categories, symptoms associated with each, and interventions. Ms. Heidi LaMeyer, Nurse Consultant, Advanced Health Institute (AHI)

112: Responding to and Investigating Complaints of Abuse and Neglect

The presenters share details of their innovative program— the introduction of Faith Community Nursing into the Cerenity WBL campus (TCU, LTC, AL). Participants will learn about Faith Community Nursing and its collaborative role with the Spiritual Care Department, with special focus on life transitions, care continuity, rehospitalizations, assessments and documentation, spiritual visits, end-of-life care, grief and bereavement resources, leadership team reflections, and interdepartmental team communication. This session will focus on how to effectively investigate and respond to maltreatment complaints. It will also address how to determine if an incident is reportable and how to manage investigations by OHFC and law enforcement. Tips on how to respond to general family concerns will also be provided. Strategies for resolving possible maltreatment issues while minimizing provider liability will be identified, as will appeal strategies and how a maltreatment finding by OHFC can impact a negligence lawsuit. Ms. Michelle R. Klegon, Attorney, Klegon Law Office, Ltd.

110: Gay and Gray: Ethics in Serving LGBT Older Adults Review issues facing LGBT older adults and how these issues affect their willingness to seek out services. This session will outline issues regarding the LGBT aging community and increase service provider awareness about the diversity of sexual orientation and gender identity among senior clients. It will also cover the ethical dilemmas providers may encounter and identify opportunities for growth with person-centered living to make your organization more LGBT culturally competent and welcoming. Mr. Robert F. Rodè, Esq, Attorney/Partner, Voigt, Rodè & Boxeth, LLC Ms. Jane Danner, LSW, Health Care Consultant, Volunteers of America National Services

111: Know Your Residents! What do facilities need to know about a resident and how can this information be used to create meaningful programming and experiences? This presentation will review the information that should be collected from residents before they join your community as well as ongoing while the relationship continues, review how to identify which information is important for different residents, and provide ideas on how all staff can use the information to create meaningful experiences. Mr. Drew Hood, Administrator, Oak Terrace Assisted Living of North Mankato, LLC Ms. Andrea Major, Administrator, Good Samaritan Society - Blackduck

113: Bridging Generations You’ve probably heard the terms Silent Generation, Baby Boomers, Generation X, and Millennials. We’ll discuss some interesting facts behind generational differences that could help us learn to understand, respect, embrace, and relate better. Ms. Peggy Scoggins, Owner, Adept Selling

114: A Fresh Look at COPD in the SNF/NF Resident We admit residents with COPD to the SNF/NF every day. With approximately 21.5% of nursing facility residents dealing with this disease, it is certainly having an impact on our nursing services. Throw in the fact that 37% have a dementia or Alzheimer’s diagnosis and our nursing time has increased. Are we assessing them frequently and preventing potential exacerbations? Are we aware of the barriers to proper COPD management? Attend and gain a focus on this illness and how we can improve our care. Ms. Cindy Fronning, RN-BC, CDONA, RAC, Director of Education, National Association of Directors of Nursing Administration in LTC (NADONA)

LEARNING SESSIONS // 200 SERIES // MONDAY, NOVEMBER 12 // 1:45–2:45 PM

201: Connecting to Dementia: How You Start... Dictates Where We End

205: Use Data Analytics to Understand Opportunities for Your SNF/NF

How we support matters. We are all just doing the best that we can with the resources and staff we have, but what if I told you that using a different lens and gaining a different perspective would change the way we are supporting each other, both staff and residents alike? I wholeheartedly believe that: “How we start... dictates where we end.” Join me for this interactive journey that will help you feel and experience the worlds in which we support. Ms. Krisie Barron, LSW, Caregiver Specialist, Embrace, Inc

Utilizing data analytics will be key for providers to understanding their past operations to help predict and ensure future success. Understanding how the financial, operational, and quality aspects of providers’ SNF/NF operations are interrelated and impact financial outcomes will allow providers to make logical and actionable decisions today to enhance operations. Mr. Matthew D. Wocken, CPA, Principal, CliftonLarsonAllen LLP

202: Soulful Aging: Growing Through the Wisdom of Elders This educational, inspirational, humorous presentation offers spiritual wisdom of elders received in 27 years of LTC chaplaincy, as well as insights from writings and poetry of other elders and those who have respected, honored, and sought to learn from them. Participants will discover healing wisdom and gain knowledge and resources aimed at helping elders on their spiritual journeys and will be encouraged to assimilate these learnings into their own spiritual journeys of aging. Rev. Scott Cartwright, M.Div., BCC, Director of Spiritual Care, Cerenity Care Center White Bear Lake

203: Dysphagia Diet Boot Camp: Liquids

206: Hospice and SNF/NF Partnerships: Balancing Quality and Regulation Partnerships between skilled nursing facilities (SNF) and nursing facilities (NF) with hospice providers represent a powerful presence in post-acute care. Together their efforts result in cost savings, decreased rehospitalizations, and most importantly, in terms of humane care, result in quality, compassionate care for people at the end of life. This session explores the benefits of SNF/NF and hospice partnerships, explains the interface necessary between hospice and SNF/NF teams, and provides guidance toward regulatory compliance. Ms. Lisa Abicht-Swensen, MHA, LNHA, Director of Hospice and Palliative Care, Allina Hospice & Palliative Care

Facilities should be in the process of adapting the International Dysphagia Diet Standards (IDDSI). Is your facility ready for change? This session will help you understand what is changing and how to tackle the transition specific to liquids. We will practice using the syringe flow method to evaluate consistency levels using a variety of thickeners, pre-thickened liquids, and standard beverages. Ms. Kristen Walker, President/Owner, Customized Culinary Consulting, LLC Ms. Debra Zwiefelhofer, RDN, LD, Consultant, Med Diet Laboratories / Foodservice Express

207: CMS National Emergency Preparedness Requirements—An Update

204: Assisted Living in Minnesota: What Is It? What Will It Be?

208: Parkinson’s Disease: More than Motor Symptoms

Assisted living providers have been under the microscope a lot lately. Consumers, the media, and legislators have all been looking at assisted living in Minnesota and identifying changes that need to be made to this model of care. This session will review the circumstances that led to such scrutiny. This session will also review what it means to be an assisted living provider in Minnesota. The speaker will discuss the results of the 2018 legislative session and its impact on providers, both now and in the future. Ms. Michelle R. Klegon, Attorney, Klegon Law Office, Ltd.

This session will include insights and lessons learned from your peers. We’ll discuss various assessment approaches, methods used to address various health and safety risks, and challenges with providing for “subsistence needs.” We’ll also discuss successes and challenges in networking with other providers and the critical importance of networking with the emergency management community. And finally, we’ll examine survey outcomes and patterns. Mr. Ray Miller, Director of Risk and Safety Solutions, Direct Supply

This session will educate providers on the natural history of PD psychosis, the impact it has on patients and caregivers, and how to identify it. It will also cover treatment strategies. Further, it will cover how PD psychosis is called out separately in CMS guidelines around reducing antipsychotics. This session will provide participants with knowledge necessary to understand the compliance and ethics regulations found in the State Operations Manual due in Phase 3 of the Requirements of Participation. Now, more than ever, the industry faces challenges that are different and complicated so the need for a solid compliance and ethics program is a must. Ms. Colleen O. Toebe, RN-MSN, RAC-MT, Nurse Consultant, Pathway Health

What do the superstars of success know that the rest of us do not? In this fast-paced and humorous session, you will learn the six essential key ingredients to the science behind success. Why do some people just seem to rise to the top in their field? Why is it that some people have all the luck? You will learn how your emotional intelligence (EQ) is more important than your IQ and how your attitude is more important than your latitude. Mr. Bruce D. Christopher, MA, LP, Psychologist & Speaker, Bruce Christopher Seminars

210: Keys to Effective Medication Management in Long-Term Care Medications are an essential component of care for seniors. Whether in an SNF/NF, AL, or IL, poor medication management is often a key factor in declining health. When used appropriately, medications effectively manage acute and chronic disease; however, medications also pose potential risks. This session will discuss the link between poor medication management and poor resident outcomes, increased patient transfers, and higher overall healthcare costs. Mr. Joe Litsey, RPh, PharmD, CGP, Director of Consulting Services, Thrifty White Pharmacy

214: The Nurse Leader—A Fresh Perspective Nurse leaders can be found throughout a nursing facility—a nurse manager, MDS nurse, staff development, infection control, night supervisor, and of course the director of nursing (DON). What makes them nurse leaders and could they be better leaders? As a DON, are you afraid that you won’t be able to retire because there is no one to take your place? This session will take a closer look at the role of the nurse leader beyond the position they hold. Attend and take a fresh look at your leaders. Ms. Cindy Fronning, RN-BC, CDONA, RAC, Director of Education, National Association of Directors of Nursing Administration in LTC (NADONA)

211: Contracting 101: Tips, Traps, and Concepts for the NonLawyer In today’s complex world, it’s more important than ever to understand what you are getting when you enter into a contract. LEARNING SESSIONS // 700 SERIES // WEDNESDAY, NOVEMBER 14 // 8:30–9:30 AM

701: Life and Love with Dementia: Activities as Behavioral Interventions The session is a therapeutic recreational approach to improving quality of life for residents with dementia and finding up-to-date activities to use as behavioral interventions. Those activities should go beyond the “3 Bs” of activities—bingo, bible, and birthdays—to create a meaningful and fulfilling life. Ms. Samantha Adams, ADC MC/PC, CNA, Recreation and Wellness Director, Good Samaritan Society - Bethany

702: The Work of Aging “ResoLute: Resident Empowered Services on Living Until The End/Eternity.” Through this presentation, the audience learns about seven main work of aging domains: life story, condition, purpose, relationships, spirituality, end-of-life, and legacy. These domains lead to discovery, reconciliation, and care plan solutions based on what matters most to a resident. This program strives to provide more time to achieve successful aging, often reducing time spent on unwanted/unnecessary medical interventions. Ms. Kelly Klund, Clinical Educator and Program Specialist, Empira, Inc. Ms. Adrienne Duncan, Clinical Educator and Program Specialist, Empira, Inc

703: Nutrition and Hydration at End-of-Life Our human nature and culture depicts food as something that brings people together and makes life better. By not eating, we see our loved ones fading from us and are left helpless, confused, and fighting our instinctive need to feed. Situations like these can be common when caring for those with terminal illnesses. In this presentation, learn about the physical changes occurring in the body, as well as how to maintain the nutrition-related goals of enjoyment and comfort at end-of-life. Ms. Marissa Wacker, RD, LD, CSG, Director of Dietary Services, St. Croix Hospice

704: Enhancing Client Experience Focusing on the clients’ experience is an approach that revolves around the resident. This presentation will explore the residents’ experience from admission to discharge. Specific topics will include an overview on client experience, cognitive biases, safe transitions, positive communication, and will include examples. Ms. Melissa Cyr, Admissions Director, Good Samaritan Society - Ambassador Ms. Mikayla Holm, Marketing and Community Relations, Good Samaritan Society - Waconia Ms. Alyssa Renk, Executive Director, Sugar Loaf Senior Living Ms. Katie Shelander, Account Manager - Minnesota & Wisconsin, Northwest Respiratory Services

705: DHS Cost Report and Audit Topics Review issues that are found during the audit process, potential resolutions to audit issues, general guidelines of how to complete the cost report, and new language from the 2017 Minnesota Statutes relating to the cost report. Presenters will cover what items are considered separately billable to other payor sources. Ms. Shelly Jacobs, State Programs Administrator Principal, Aging, Nursing Facility Rates and Policy Division, Minnesota Department of Human Services Ms. Jane M. Gottwald, Human Services Program Representative 2, Minnesota Department of Human Services

The session will conclude with a discussion on opiate side-effects and how to manage them. Dr. Robert G. Sonntag, MD, CMD, Geriatrician, Medical Director, Health Partners

707: When an Outbreak Knocks in LTC—What is Your Response? This session will describe how to identify and manage outbreaks in long-term care. 711: Prosecuting Abuse, Neglect, and Financial Exploitation In this presentation, participants will hear what happens after a maltreatment report is made, particularly when allegations are referred for prosecution in criminal investigations. Chies, MHA, FACHCA, HSE, Program Manager, Long-Term Care Administration, St. Joseph’s College

2018 NATIONAL QUALITY AWARD WINNERS

CONGRATULATIONS ...to the Care Providers of Minnesota members who received an American Health Care Association/National Center for Assisted Living quality award! The AHCA/NCAL National Quality Awards Program is a progressive program that is based on the Baldrige Criteria for Performance Excellence. Participants move from the bronze, to silver, and then ultimately to the gold level. Bronze recipients must demonstrate a groundwork for excellence by completing a self-assessment that outlays their organizational priorities and goals. Silver awardees outline their systematic approaches and demonstrate sustainable organizational and process results linked to their key customer requirements, success factors, and challenges. Gold recipients represent an elite group by meeting all of

the demands of the Baldrige criteria, they demonstrate the achievement of high levels of performance over time in the areas of: leadership, strategic planning, customers, workforce, operations, and knowledge management. This year, 22 Care Providers of Minnesota members were recipients of AHCA/NCAL’s 2018 Quality Awards—with 18 beginning their quality journey, and four more continuing on with their success. Members are presented with their awards at the national AHCA/NCAL Convention & Expo. We will also celebrate their achievement at the Celebration of Caring Reception on Tuesday, November 13, 2018, at our convention.

CARE PROVIDERS OF MINNESOTA 70TH ANNUAL CONVENTION & EXPOSITION // #CPMCON18

LEARNING SESSIONS // 800 SERIES // WEDNESDAY, NOVEMBER 14 // 9:45–10:45 AM

801: Five Key Strategies to a Thriving ED/DON Relationship

805: DHS Cost Report and Audit Topics

A thriving relationship between the executive director and the director of nursing is imperative to a successful senior care facility. When this relationship is one of trust, growth, and respectful communication, it positively affects the culture up and down your organization. You will see financial gains by improving your number one asset and your greatest expense—your staff. Irmadene teaches you how to revive it and drive it to be the standard for all relationships. Ms. Irmadene Hanson, CEO and Founder, Magnify Your Influence

Review issues that are found during the audit process, potential resolutions to audit issues, general guidelines of how to complete the cost report, and new language from the 2017 Minnesota Statutes relating to the cost report. General guidelines may include what the appropriate classification of costs based on Medicare and Medicaid principles are. Presenters will cover what items are considered separately billable to other payor sources. Ms. Shelly Jacobs, State Programs Administrator Principal, Aging, Nursing Facility Rates and Policy Division, Minnesota Department of Human Services Ms. Jane M. Gottwald, Human Services Program Representative 2, Minnesota Department of Human Services

802: Employee Wellbeing—It’s More Than Just Kale and CrossFit® The traditional employee wellness program offers a few educational sessions and challenges—usually focused around fitness and food. As stand-alone offerings, the result is usually mediocre, resulting in low participation, and attracts those who already care for themselves. Wellness efforts aren’t reaching those that need them most. Wellbeing is more than just food and fitness. A holistic approach that integrates career, social, physical, emotional, and financial leads to better outcomes. Ms. Teri M. Erhardt, Wellbeing & Engagement Consultant, NFP Corporate Benefits

803: Mystic Kitchen Tour Mystic Lake offers several restaurants and catering services for thousands of meals. Lace up your walking shoes for a behind-the-scenes tour of the many kitchens—pastry, sushi, and catering to name a few—and how they create and deliver delicious meals. Mystic Lake Center Staff

804: Senior Living Trends and the New Generation of Customers This session will focus on the aging demographic in the twin cities and the state of Minnesota and how those demographics will likely impact the expectations of baby boomers as they enter the senior living market. Senior living providers should be tailoring their service and amenity offerings to the baby boomer generation. There will also be discussion about senior housing affordability and how the market can address it. Mr. Peter Baum, Senior Consultant, CliftonLarsonAllen LLP Ms. Cindy Schroer, Director, Market Research, CliftonLarsonAllen LLP

806: Urinary Tract Infections Urinary tract infections are common in long-term care residents. This session will describe symptomatic and asymptomatic UTIs including the issues of pyuria and resident presentation. The mythology of what a symptomatic UTI is will be discussed. Appropriate assessment and evaluation of UTIs will be covered using well-known criteria. Therapy and prevention will also be presented. Dr. Robert G. Sonntag, MD, CMD, Geriatrician, Medical Director, Health Partners

807: Top K-Tags Under the 2012 Life Safety Code Learn about the most frequently cited K-Tags and have an opportunity to ask inspectors your Life Safety Code questions. Mr. Tom Linhoff, Supervisor, Health Facility Fire Inspections, Minnesota State Fire Marshal Division, Health Care and Corrections Fire Inspections

808: PASRR Level II Mental Health Screening The federal Centers for Medicare and Medicaid Services (CMS) requires that people who have or may have a mental illness are carefully screened before they can be admitted to a Medicaid-certified nursing facility. This session will cover basic information about the reason behind the screenings (PASRR (OBRA) Level II Mental screening), information about the responsibilities for nursing facilities, and recent changes in the process. Ms. Larraine Pierce, Mental Health Program Consultant, Minnesota Department of Human Services

LEARNING SESSIONS // 800 SERIES // WEDNESDAY, NOVEMBER 14 // 9:45–10:45 AM

809: Gay and Gray: Strategies to Welcome LGBT Older Adults Research shows that LGBT older adults may not use services because they worry about how they will be treated by staff and their peers. But research also shows there are ways service providers can create welcoming spaces. In this session you will complete a supportive environment assessment to gauge your program’s readiness to welcome LGBT older adults. Using case studies from Minnesota care settings and your own stories, you will explore strategies to prepare your program for all older adults. Ms. Marsha J. Berry, MA, CAEd, Director of Education, Training To Serve

810: Hospice and Assisted Living Staff: Who Is in Charge? What “matters most” to many in assisted living is an opportunity to remain in their home, even until the end of life. The privacy of one’s own “home” has been declared by many to be the most desirable location at end of life. In order to accomplish this, collaboration of the home care staff, hospice provider, and the family has been proven to be an effective partnership. How do the regulatory requirements meld together to make this happen? Who is in charge? What are the barriers and opportunities? Ms. Lores Vlaminck, RN, BSN, MA, CHPN, Nurse Consultant, Lores Consulting, LLC

811: OHFC Maltreatment Investigations and Progress Update The Minnesota Department of Health (MDH) and the Department of Human Services (DHS) engaged in an unprecedented interagency agreement in 2018 to fix the structural and systemic problems within the Office of Health Facility Complaints (OHFC) to ensure protections for Minnesota’s most vulnerable populations. OHFC investigates compliance of federal and state regulations, in addition to investigations of maltreatment including neglect, abuse, and financial exploitation as defined by MN State Statute 626.5572 in facilities licensed by MDH. Ms. Lindsey Krueger, Interim Assistant Director, Office of Health Facility Complaints, Minnesota Department of Health

812: SNFRM, SNF/NF PPR & Five-Star Readmission Measures Toolkit This exciting training will walk attendees through the various readmission measures now in effect for providers. Implementing key processes to decrease the number of unnecessary readmissions to hospitals is critical to organizations’ success. This informational presentation will provide attendees with key strategies and clinical readiness steps to position their organizations for positive performance measure outcomes. Ms. Lisa Thomson, BS, NHA, Chief Marketing and Strategy Officer, Pathway Health

813: Recruitment and Retention The words “staffing challenges” can be heard across senior care settings now and over the past few years. Although there are no easy answers, find out what one care center in north-central Minnesota has been doing to tackle this as a team effort. Using multiple disciplines and approaches, they are working hard to recruit new staff to senior care—and keep them here! Ms. Kathleen M. Dobson, Vice President Senior Services, Lakewood Health System Ms. Katie Paine-Santos, ADON, Lakewood Health System Care Center Ms. Jennifer Dolezal, Resident Experience Specialist, Lakewood Health System Ms. Roxanne Hejhal, MBA, Director of Human Resources, Lakewood Health System Ms. Stacy Line, RN, BSN, Care Center Director of Nursing, Lakewood Health System Care Center Ms. Kerry Weappa, LPN, Lakewood Health System Care Center

901: C. difficile Infections: Tracking, Prevention, and Management

905: SNPs Create Unique Opportunities for Post-Acute Providers

More than 100,000 C. difficile infections (CDI) develop among residents of US nursing facilities each year, making CDIs among the most serious healthcare complications that affect the nursing facility population. About 50 nursing facilities in Minnesota have been tracking CDI via a national database, contributing to the development of state and national benchmarks. These state and national data will be shared, as well as lessons learned regarding CDI tracking, prevention, and management. Ms. Kristi Wergin, RN, BSN, CPHQ, Program Manager, Stratis Health Ms. Kathie Nichols, RN, BSN, CRRN, Nursing Home Liaison, Stratis Health

Special Needs Plans (SNPs) are coordinated care plans that fall within Medicare Advantage. Because of the unique components of these plans, some senior providers are establishing their own Medicare Advantage plan or acting as a contracted provider to be able to capture the payment transformation opportunities the SNP presents. The SNPs allow providers to position themselves as attractive options for insurers, other providers, and patients by developing capabilities to deliver high-quality care. Mr. Cory R. Rutledge, CPA, Managing Principal of Industry, CliftonLarsonAllen LLP

Dr. Pamela Scarborough, PT, CWS, Director of Public Policy and Education, American Medical Technologies

904: Elderly Waiver Rates Evaluation and Adult Day Study Updates 2017 legislation required the Department of Human Services (DHS) to study new rate methodologies for Elderly Waiver and other related programs. A study of adult day services was also requested. A report on each study will be submitted to the Legislature in January 2019. This session will provide preliminary findings from these two, related studies, which are still underway. The session will also provide information about the implementation of new rates effective January 1, 2019. Ms. Rachel Shands, MSW, Manager of HCBS Policy Integration, Minnesota Department of Human Services Mr. Peter Spuit, MSW, Rate-Setting Policy Consultant, Minnesota Department of Human Services

906: Are You Issuing the Correct Notice of Non-Coverage? As of May 7, 2018, CMS required SNFs to issue a new SNF Advance Beneficiary Notice of Noncoverage (ABN). This session will discuss the new form, discuss what notices this form replaced, and the other forms that are still required. Attendees will be walked through real-life scenarios and have the opportunity to ask questions. Ms. Kim Conner, Business Office Consultant, Pope & Conner Consulting, Inc

907: MINIBYTES: ADMINISTRATOR INSIGHTS RoP Phase 3: Quick Tips Attend this MiniByte session to receive a general checklist and quick overview of the requirements for Phase 3 of the Requirements of Participation. Ms. Karolee K. Alexander, RN, RAC-CT, Director of Clinical and Reimbursement Consulting, Pathway Health How to Talk with Surveyors This MiniByte session will cover and discuss strategies and best practices to use when you and your team successfully work with state and federal surveyors and investigators, including responding to various demands and inquiries. Knowing how the five-star rating system works helps providers manage the overall star rating. Mr. Matthew D. Wocken, CPA, Principal, CliftonLarsonAllen LLP
