at the heart of good health Keeping on top of your good health is easier with a trusted partner, and that’s where your Primary Care Practice comes in. See them for minor illness or injury or for ongoing care for chronic conditions, such as high blood pressure or diabetes. Your Primary Care Practice will refer you to medical specialists when needed, keep you up-to-date on recommended screenings and vaccines, and create a personalized plan to help you Be Well. Stay Well.

Have you tried

Telehealth? Telehealth visits by phone or video chat are a convenient, secure way to meet with your Primary Care Practice or Specialist from the comfort and safety of your own home. Talk with your health care team to see if telehealth is right for your next visit.

Meet the Primary Care Team When you visit your Primary Care Practice, you may see a Doctor (an M.D. or D.O.), a Physician Assistant (PA) or a Nurse Practitioner (NP), all skilled and licensed to prescribe medicine, order tests and provide ongoing care for a chronic condition. Registered Nurses (RNs) may give shots or medicine and are your chief advocates in your wellness plan. A Care Coordinator may teach you how to manage a chronic condition, monitor your treatment plan and help with medication refills. Licensed Practice Nurses (LPNs) help the RNs provide basic care, and Medical Assistants may update your health history and make sure providers have the information they need for your visit. In case you didn’t know, the most important member of your health care team is YOU!

Care where? Primary care, urgent care or emergency department. Start with Primary Care!

Your Primary Care Practice should be your first call for coordinating your overall care and making health decisions, no matter where you choose to receive services. Urgent care centers are a great option when your Primary Care Practice is not available — and

emergency departments are your go-to place for serious injuries or life-threatening illnesses. If you do seek care from an urgent care center, ED or a specialist, ask them to send a report to your Primary Care Practice so your health record stays up-to-date for the best, most coordinated care possible.

We gather together – or not? Follow these bountiful tips to help keep those you love safe this holiday season:

Fewer is better. In this season of plenty, keep the guest list small enough for everyone to safely spread out. The CDC recommends we only dine this year with those who live with us.

Be virtually grateful. Gather with family and friends by video chat to share what you’re grateful for. If you live close enough, you can each make a favorite family dish and deliver servings to one another (contact-free, of course) so you can all enjoy the same meal during your call.

Dine al fresco. If you must gather — and if the weather permits — open a window, or better yet, set a table outside. The brisk fall air will help you keep your visit shorter, too — another CDC recommendation.

Count your blessings. Distance doesn’t lessen the love of family and friends. The greatest gift we can give each other this holiday season is to help everyone Be Well. Stay Well.

A hard pill to swallow

Skipping a dose — even once in a while — is not a good idea! If the cost of medicine keeps you from filling a prescription, talk with your health care team or pharmacist today. There may be other options that work just as well, or discount plans that can help!

It’s in the bag…

Use the enclosed My List of Medications to keep track of everything you take, including prescriptions, vitamins and pain relievers. Better yet, take the bottles with you to every appointment so your health care team can see exactly what you are taking.

Workouts for the cold months ahead

It’s important to keep moving even when shorter days make you want to curl up inside. Do these moves every day to improve strength, mobility and balance: ▶ SIT TO STAND Practice standing up without using your arm strength, then sit down and try again. Aim for 2 or 3 sets of 10 reps each day. Doing great? Work your way up to squats without a chair.

▶ HEEL TAP To get your blood flowing, sit on the edge of a sturdy chair with your feet flat on the floor. Holding the seat for support, lift your right foot and extend the leg diagonally in front of you. Tap your right heel on the floor, then return to starting position and repeat with the other foot. Do 15-20 reps. Doing great? Raise your arms overhead as you tap your heel and lower your arms and you bring your foot back in.

What’s cooking Apple Spice Muffins Full of warm spices and sweet apple, these wholesome muffins make a healthy start to the day. They can also serve as a delightful afternoon snack or dessert. INGREDIENTS

MAKES 12 2

tablespoons ground flaxseeds

1/4 cup water 1-1/2 cups whole wheat flour (gluten-free flour can be substituted) 1/2

cup old-fashioned rolled oats

tsps. cinnamon or pumpkin pie spice

3/4

tsp. baking powder

3/4

tsp. baking soda

3/4

tsp. fine sea salt

1/2 tsp. powdered stevia 1-1/4 cups unsweetened applesauce 1/2 cup apricot fruit spread (fruit-sweetened only) 1 The Annual Wellness Visit is not a physical exam. It’s a time to review your medical history, your current health and risk factors for illness or injury. Call to schedule it today!

1/2 cup raisins

1. In a small bowl, mix ground flax with water. Set aside for 10 to 15 minutes. The flax will absorb the water and create a thick gel. 2. Preheat oven to 350ºF. Line a nonstick muffin pan with paper liners, or spray the pan lightly with cooking spray. If using spray, gently wipe with a paper towel to remove excess oil. 3. In a medium bowl, whisk together flour, rolled oats, cinnamon, baking powder, baking soda, salt, and powdered stevia until well mixed. 4. In a large bowl, stir together the applesauce, apricot fruit spread, grated apple, flax mixture, and vanilla extract. Stir in half the dry ingredients, then add the remaining half and stir gently until combined. Add raisins and stir lightly to mix. 5. Spoon one third of a cup of batter into each muffin cup. Bake until a toothpick comes out clean from the center of a muffin, about 25 minutes. Be careful not to overbake. 6. Remove muffins from the oven and let cool in the pan for a few minutes, then remove from the pan and let them cool on a cooling rack. These muffins are best served shortly after baking, but will keep in a sealed container for several days.
