BEST CARE CLOSE TO HOME

With locations in Delaware, Maryland, New Jersey and Pennsylvania, ChristianaCare offers primary care, right in your neighborhood. We serve as your medical home so you can build relationships with your health care providers. For physicals and sick visits, preventive screenings and immunizations, and treatment for common conditions like diabetes, "we are there for the whole family," said Family Medicine caregiver Bridget Peters, D.O., Ph.D., with expertise in pediatrics, adolescent health and adult medicine.

LIFESAVING HEALTH CARE

After two strokes in early 2023, Everett Lacy temporarily lost his ability to speak and write but never lost his ability to connect with people. His positive attitude steered his recovery, supported by ChristianaCare rehab therapists Bridget Callahan and Bridget DiMaio, affectionately called “the Bridgets.” With their expert help, Everett regained his speech and fine motor skills. “They never gave up on me, so I didn’t either,” he said. Inspired by Everett, the Bridgets organized a fundraiser, raising nearly $41,000 to support outpatient rehabilitation for stroke survivors.

I got a second chance.”

F or Ladonna Behnke, family is everything. After being diagnosed with advanced colorectal cancer, she felt spending time with loved ones was an important goal. “Because we are a cancer treatment and research center, we look beyond the conventional,” said medical oncologist Philip Margiotta, M.D. “We were able to offer a novel targeted approach that proved to be effective for a longer period and without the debilitating side effects of chemotherapy.” That means Ladonna can do what matters most in her life.

MAJOR DISCOVERY FOR COLORECTAL CANCER TREATMENT

S cientists led by Bruce Boman, M.D., MPH, at ChristianaCare’s Helen F. Graham Cancer Center & Research Institute have discovered an enzyme called CYP26A1 that links two pathways in colorectal cancer, offering new treatment possibilities for advanced drug-resistant cases.

We are committed to improving the health — and hope — of our neighbors.” ” GAME-CHANGERS in OUR COMMUNITY ChristianaCare saved my life.” ”

Medical care accounts for only 20% of a person’s overall health — the rest is influenced by where they live, their environment and other conditions that make up their daily lives. Stark differences in these factors, known as “social drivers of health,” can lead to big differences in lifespan and disease risk. ChristianaCare is a catalyst in addressing social drivers of health — the other 80% of health care — in the communities we serve.

ADDRESSING SUBSTANCE USE DISORDER

ChristianaCare is a leader and partner in our patients' journey to recovery with community-forward programs including with Project Engage, in which peer counselors engage with hospital patients to discuss treatment for their substance use disorder (SUD). Other programs include the Project Recovery Outpatient SUD treatment program and Addiction Medicine Consult Service supporting hospital patients. We also have an expanding partnership with behavioral health units of Delaware police departments to help community members with SUD access the appropriate level of care.

Project Engage has served 13,000 patients and conducted 27,000 patient engagements, 60% of these engagements referral to community SUD discharge since the program ChristianaCare's Addiction Consult Service served in 2024 for opioid withdrawal transitioning to medication opioid use disorder.

UNIQUE RESPITE AT THE HOPE CENTER

The Hope Center Medical Respite Program cares for patients without housing who are too medically fragile to stay in a traditional homeless shelter but do not require inpatient hospital services. Through an agreement with New Castle County, ChristianaCare uses 36 beds for medical respite at the Hope Center, a transitional living facility that houses nearly 500 people experiencing homelessness. ChristianaCare also offers primary care to all Hope Center residents. Since 2022, the staff, with clinical lead Ben Golden, M.D., has supported more than 2,000 patient encounters.

BRINGING CARE INTO the HOME

ChristianaCare's virtual health services empower patients to connect with their care teams securely and conveniently, often through text or video, to stay engaged with their health. These offerings reimagine traditional care models with 24/7 accessibility and personalized support through computers, tablets or smartphones. More than 101,000 virtual visits were conducted throughout the health system in 2024.

101,096 PATIENTS SEEN VIRTUALLY IN 2024

Among these services, virtual behavioral health enables patients to receive psychological and psychiatric care from the comfort of their homes. This innovative approach — with 2,900 virtual visits per month in 2024 — ensures seamless access to mental health support, meeting patients wherever they are.

Primary Care at Home serves homebound patients with a multidisciplinary team to meet their medical, emotional and social needs. In 2024, 1,262 patients were seen, with seamless care provided through relationships with HomeHealth, palliative care and hospice programs. ChristianaCare has a home-based hospice and palliative care program in collaboration with AccentCare.

Hospital Care at Home — a service of the Center for Virtual Health — uses innovative technology to provide care for people in their homes in entirely new ways. ChristianaCare is at the forefront, recognizing that care in the home often leads to better health outcomes and lower costs — and people love it! Since its launch in 2021, Hospital Care at Home has treated 1,423 patients with safe, high-quality, hospital-level care in the home. The outcomes are excellent: Infection rates and readmissions are well below the national average for brick-and-mortar hospitals. For patient Bill Bieber and his family, Hospital Care at Home is more than health care. “It’s just the best thing ever,” said his daughter Carol Bieber.

3X NATIONAL AVERAGE

Patient experience scores are three times higher than the national average for traditional hospital stays.

COMMUNITY HEALTH WORKERS ARE HERE TO HELP

ommunity Health Workers (CHWs) are frontline liaisons, guiding individuals and families through health, social and community services to foster well-being. They sit with patients on the front porch or at the park, discussing daily struggles like grief, financial stress and medical care confusion. CHWs provide emotional support and care coordination, accompanying patients to community centers, smoking cessation classes and the gym. As trusted community members, CHWs offer culturally and linguistically appropriate services, addressing social drivers of health to achieve better outcomes and health equity for the populations they serve. $128M
