Name: Erick Jones Sex: Male Gender: Male Age: 49 Height: 5’11 Weight: 220 lbs. Medical History: High blood pressure, had a hernia removal surgery, was in the ICU with kidney failure and internal bleeding. Wears glasses, has gouty arthritis, Rheumatoid arthritis, and tendonitis. Lifestyle Behaviors: Sleeps between 6-7 hours. Eat of everything (fish, chicken, red meats, vegetables, white rice, yogurts, grains) 8 cups of water daily. No since I got arthritis but has started swimming at least 3 times a week. SIts in office. Medical Concerns/Family History: Diabetes, High Blood pressure, cardiac problems. Both his mom and dad have cardiac problems. Purpose of Consultation: How to prevent heart disease.


Probability of Erick getting heart disease; 5/20 (based on full family) 1/7 (immediate family)

Medications: Zocor (Brand Name) Simvastatin (Ingredient). It can treat high cholesterol and triglyceride levels. This may reduce the risk of heart attack, stroke, and related health conditions. The usual dosage range is 5 to 40 mg/day. Nitrolingual (Brand Name) Nitroglycerin (ingredient). It can treat and prevent chest pain (angina) in its oral form. When used rectally, it can treat pain caused by tears in the skin around the opening for bowel movements (anal fissures)

Nutrition Plan: You need to eat three meals a day, with snacks in between. For snacks you can eat oranges (Reduce blood pressure, cholesterol, and heart failure), dark chocolate (Reduces blood pressure), Almonds (Reduce LDL and fatal arrhythmias), Pomegranates (Reduce atherosclerosis). For meals you should have the regular veggies such as kale and garlic ( Reduces blood pressure and plaque) and once in awhile have red wine with your dinner (Boosts HDL, reduces unwanted clotting).


Lifestyle Recommendations: You should sleep eight hours daily, and if you feel tired only take a 15 minute nap during work or at home. Start swimming more often and if you feel up to it take a walk just around your street.

Alternative Therapies: Drinking green tea can be very beneficial, the antioxidant epigallocatechin gallate (EGCG) has the ability to protect the heart. Yoga helps you become more sensitive to insulin, which is important for controlling blood sugar. Doing Yoga can also help people lower their levels of harmful LDL cholesterol. Goals: Because you have Gouty arthritis, Rheumatoid arthritis, and tendonitis, tyr swimming 3-4 times a week, an hour to an hour and a half for each session. Based on your diet, decrease the amount of red meat you eat and replace it with chicken and fish, plus try drinking up to 9 bottles of water daily. Stop drinking sugary drinks (soda, juices, etc.) and replace it with water and green tea.
