Alzheimer’s Disease and Dementia Caregiver Support Program 29125 Chagrin Blvd. Pepper Pike, OH 44122 P: 216-292-3999 F: 216-504-0551 W: jfsa-cleveland.org Nata Mendlovic, Director Families work with a Care Navigator and geriatrician and other professional staff, to develop a care plan that includes screening and assessment of a family member, information and referrals to appropriate community services, such as homecare, education about brain health and dementia, companion care, adult day support or assisted living, coordination of care – between medical, social services and other providers as well as communicating with families to provide support, education and peace of mind, and understanding financial and legal options available, and Caregiver Respite Program which provides limited hours of free caregiving or companionship to individuals with Alzheimer’s disease or related disorders.

Our innovative real-life suite and neighborhoods at Menorah Park and Rehab Road at Montefiore simulate life at home and in the community to help with activities of daily living and a comfortable return home.
