Rhinoplasty: dramatic yet natural
Rhinoplasty is the most complex of the common plastic surgery procedures. Contemporary techniques achieve a balance between obtaining dramatic results and maintaining a natural appearance. Nasal breathing problems are also addressed so that Dr. Steven patients can look and breathe better. Cosmetic rhinoplasty must be individualized to suit each patient’s personal aesthetic goals and physical characteristics. The procedure can be more or less aggressive depending on the patient’s goals while maintaining familial and ethnic traits to maintain balance with the whole face. Common goals include reducing a hump on the bridge of the nose, decreasing the overall size of the nose, straightening deviation, elevating the tip of the nose and making the nostrils smaller. Two basic rhinoplasty techniques exist: open and BEFORE AFTER closed (also known as endo-nasal). Some surgeons use only one technique, but in general, open rhinoplasty is

used for more aggressive procedures and procedures requiring refinement of the nasal tip. Closed rhinoplasty may be more commonly used for surgery being performed just to improve breathing or when reducing a hump of the nasal bridge without other aesthetic changes. The most significant recent development in the field of rhinoplasty is the nonsurgical rhinoplasty, in which injectable fillers – like Radiesse™ and Restylane™ – are used to enhance the nasal appearance. Asymmetries and irregularities may be smoothed out. A small hump in the bridge may be camouflaged and the nasal tip can be elevated. Nonsurgical rhinoplasty is performed in the office, with no anesthesia, like most cosmetic injections. The injectable fillers are not permanent, so the achieved changes are not either, but they generally last for several months. Prospective rhinoplasty patients need to be sure they find a surgeon who is a board certified plastic surgeon with a special interest in rhinoplasty; the surgeon should have extensive experience with the procedure. Dr. Paul With today’s modern technology Mikhli and advances in pain management, replacing a missing tooth can be as easy as 1, 2, 3: • First appointment is to evaluate the area of the missing tooth, which includes a 3D scan to assess the bone volume and plan where we want the new tooth to be. From that information, we fabricate a surgical guide that precisely directs where the implant needs to go to support the ideal tooth position. • Second appointment is when we place the implant. We have a surgical guide we use for this appointment that can take less than 30 minutes in some situations! Local anesthetic is always used, which will eliminate any pain, but this can be combined with Nitrous Oxide (“laughing gas”), Valium, Xanax or sedation by an anesthesiologist, which will control any anxiety. For post-op discomfort, all that’s needed is Tylenol or Advil at the patient’s discretion. I tell my patients they can go to work the same day but should avoid strenuous exercise for a few days. • The last and third appointment is to check on the healing and fabricate the crown. typically passed since placing the implant, and statistically, 95 percent of a patient’s bone has fused to the implant. Anesthetic is typically not needed for this appointment. A scan of the implant head is taken and a crown can be made while the patient waits. Once the crown is placed on the implant, there are no restrictions and the patient can now eat whatever he or she likes. So, pick up the phone and let’s get that broken or missing tooth replaced. Millennials and Cosmetic Surgery Millennials, followed by Generation Z, are the most technically inclined generations thus far and are more likely than past age groups to embrace all kinds of tech, including advancements that improve their physical appearance. Millennials (those currently 22-37 years old), specifi cally, have created what’s been referred to as a “prejuvenation” trend, utilizing the many noninvasive treatments such as Botox, lasers, facial peels, micro needling, etc., to prevent signs of aging before they appear. The new approach of controlling the aging process through self-care can be a very positive thing. There is a fl ip side for millennials and their preoccupation with tech, which comes in the form of social media. According to the Pew Research Center, 88% of 18-30 year olds said they were active social media users. One of the known effects of regular social media use is it’s tendency to amplify insecurities about one’s appearance. Looking at edited Instagram pictures of celebrities, fi ltered and perfectly tweaked selfi es on Snapchat, or how-to makeup and hair videos on YouTube can all feed into an idealistic view of how real people are supposed to look. A 2018 American Academy of Facial Plastic Surgery survey showed that 72% of plastic surgeons surveyed had seen a signifi cant increase in cosmetic surgery and injectable treatments in patients under 30 years old, that is up from only 54% in 2017 and 25% in 2013. There is also a new pattern emerging of patients referring to pictures on the internet as the reason for changes, be it an unfl attering shot of oneself on Facebook or the appeal of the fl ower wreath, doeeyed, tiny-nosed selfi e taken with a fi lter on Snapchat. Valid but sometimes unrealistic notions of how one can improve their look in social media posts is a new factor in choosing plastic surgery. If you were wondering, rhinoplasty is the No. 1 surgical procedure currently being requested. People 40 years and older generally held the belief

that constantly taking pictures of oneself or spending money on plastic surgery were both acts of vanity; for millennials, it’s the norm. Many celebrities and infl uencers on social media regularly and openly discuss or show the results of cosmetic procedures. Plastic surgery is now very mainstream and no longer something being kept secret, yet another way social media has added to the increased business of treating millennials. Ultimately, whether it’s making changes to boost selfconfi dence or having treatments to slow down the effects of aging, younger people are now utilizing new advances in cosmetic surgery in large numbers. Most physicians are very much in line with these new trends, so long as the patient’s expectations and requests are realistic.
