This annual event in the pharmacy calendar saw the great and the good in pharmacy leadership come together with grass roots pharmacists and their staff to discuss the issues of the day. From debating the professional issues related to the new contract, stocking new products and supporting community pharmacies to offer new NICE guidelines on Atrial Fibrillation, the Pharmacy Show once again proved its worth among pharmacy’s premier networking events as the place where important conversations take place.

Sue Sharpe, Chief Executive of the Pharmaceutical Services Negotiating Committee commented “the great difficulty for community pharmacists is that they are mostly too busy to take time out to get ideas, learn about new products etc., but they can do that all here. This is an extraordinarily valuable show.” As the industry’s leading edge gathering, the Pharmacy Show is known for its innovations and this year’s new features included the public health & wellbeing forum, where experts in health and wellbeing discussed issues such as the pharmacist’s role in preventing skin cancer to new advances in point of care diagnostics. The second innovation for 2014 was the Local Pharmacy Innovation Theatre, a forum in which Local Pharmacy

Refocus pharmacy on patients – wherever they are It is time to develop a contract that encourages patient-centric clinical care, said Mark Koziol, chairman of the Pharmacists Defence Association, on the opening day of the Pharmacy Show. In a presentation entitled: ‘In the community: is the future as retailers or healthcare professionals?’ he challenged pharmacy to develop and embrace a new contract that centres on establishing clinical relationships with patients. Describing the new contract settlement for England for 2014-15 as Mark Koziol, Chairman of the “the quickest route to do Pharmacists Defence Association more for the same money”, he told contract negotiators to secure funding for wider roles in the community that are independent of the core pharmacy contract. In a separate but related challenge, he also called for a new approach to regulation that viewed pharmacists as healthcare professionals rather than shopkeepers. This also would require some sense to be made of the supervision rules and it would need pharmacists to have access to the patient record. For their part he said that pharmacy owners would have to end their love affair with commercialisation, and transform their premises into professional locations that inspire professional confidence. Relationships must be refocused from a premises-based supplier to a more clinical relationship based on pharmaceutical care delivered to patients “wherever they are based.” He explained that as the patient’s medicines champion this would mean taking responsibility for patients’ medicines needs and having real accountability. It would mean making dynamic changes to patients’ lives and not just writing a chitty for the doctor to action. This would involve pharmacists in tasks such as taking bloods, titrating dose and changing prescriptions, he said.

It’s a really stimulating experience. The great difficulty for community pharmacists is that they are too busy to take time out to get ideas, learn about new products etc, but they can do that here. This is an extraordinarily valuable show.

Could we educate people to go to pharmacy before visiting A&E? This was the challenge laid down before conference delegates attending the Pharmacy Show’s new Local Pharmacy Innovation Theatre on day one of the event. In a new development for the Pharmacy Show, the Local Pharmacy Innovation Theatre aims to provide a forum to showcase local pharmacy service development and innovation with a view to sharing best practice across the country. In his presentation, Andrew Lane, owner of AI Healthcare, told a packed theatre of a poster campaign running in Slough to drive people into pharmacies as an alternative to A&E. He told the audience that evaluation showed that 85% of people had understood the campaign message and that 51% said they would change the way they used healthcare services as a result. Some 17% said the campaign had promoted them to visit the pharmacy, with a potential saving on A&E costs of £740,000. A study of 1000 A&E visits over four years

found that most A&E visits are made during the day, when pharmacies are open. Most of the visits were made by people aged between 25-35 years old or with children aged under five years and most (over 64%) followed selfreferral. Of those visits, a significant proportion were for minor ailments including colds and flu, ear infections, cuts, bruises, thrush and cystitis. As a result of their visit, 34% left A&E with just advice and guidance, and 39% required no follow up care. He challenged the audience to find a way to drive these A&E consultations into pharmacy instead, and in the process improve uptake of pharmacy services. “Could we educate people to think pharmacy first? We need to get the message out that we are available without an appointment.”

Q&A Fontem Ventures As part of their attendance at The Pharmacy Show 2014, Puritane™ worked to communicate the impact of electronic vaping products (EVPs) to the consumer pharmacy world. On the second day of the show, a key discussion event asked pharmacists if they support the introduction of EVPs, yes or no, the result was an overwhelming yes. The following interview between Marc Michelsen, Director of Communications and Public Affairs at Fontem Ventures, who own Puritane and Dr Dawn Harper looked at how EVPs can fit comfortably into a pharmacy sphere. D: Dr Dawn Harper M: Marc Michelsen M: What made you decide to come here today? D: I am a fan of electronic vaping products (EVPs) because I am still very much a practicing NHS GP, so I see first-hand what smoking does to people, and I see them as the way forward. I think they will help people to give up smoking and I think they are a much healthier option. It is all about raising awareness making sure people understand what they do, how they work and dispelling some of the myths around them as well. M: There are still some healthcare professionals like yourself who question the long term effects of EVP, what would you say to them? D: They are relatively new, so we don’t have any long term studies, but I think most people don’t appreciate that actually it isn’t the nicotine in cigarettes that kills you; it is all the other toxins. There are around 4,000 toxins in a single cigarette, and it is those and the tar, and so on, that damages your lungs and cause all the diseases. So, we don’t have long term studies but they are almost certainly a lot healthier than the alternative. M: Unfortunately, people can’t vape here at the NEC, what are your thoughts on vaping in public places such as this? D: I think it’s a shame that people can’t vape here at the NEC. I guess one of the reasons for that is that some of these devices look very like the ‘original’ cigarette, so it can be difficult for people, just by looking, to tell the difference. Of course you only have to go close to somebody and the smell makes it very obvious whether you are dealing with an EVP or a cigarette. Lots of places actually do let you vape, you can vape in most public places and that’s not a problem. I guess it’s etiquette if you are in somebody’s house, to ask if you can vape there - but actually I am little disappointed that people can’t vape here at the NEC. M: Coming back to what you just said, do you think EVPs actually impact people passively? D: Absolutely not, the thing with EVPs is that there is no smoke, so you don’t get the passive smoking - it is pure vapour and you are only inhaling the vapour when you take a drag on the device. It is not like a burning cigarette which is giving off smoke all the time. M: EVPs contain nicotine and there are lots of stories about nicotine and its potential dangers, your thoughts? D: It is not the nicotine that kills you, it is everything else. Nicotine is addictive, there is no question about that. But you can take nicotine every day of your life, and it is not going to kill you. It’s the other things in a standard cigarette that can kill you. M: Aside from health, do you believe there are other benefits to EVPs? D: Obviously as a GP health is the big one for me. But yes, we’ve forgotten I think just how much cigarette smoke stinks; remember when you used to go to a bar before the smoking ban and your clothes would smell the next morning. Of course you don’t get any of that, so they are more sociable as well I think. M: As a doctor, what do you believe the benefits of EVPs are over, let’s say, nicotine replacement therapy products? D: I think it is twofold. First of course, you are going through the ritual that you are holding something in your hands, just like you would with a cigarette, and there is a lot of habit about smoking. People say that they miss the action of actually smoking. The bigger thing is that you get the hit of nicotine in the drag of the EVP, just like you would by dragging a cigarette. So rather than having that slow absorption from gum or a patch, it is much more like the original thing, so you get that hit of nicotine as you take a drag.

M: There are a number of stories in the media about EVPs exploding, about children getting poisoned and so on - I suppose I’d like to know what your views are on manufacturing, manufacturing standards and quality? D: One of the reasons that I am here with you today, is that I know that Puritane use very well regulated factories to guarantee a quality product - and I think that is the way forward. Certainly when I talk to my patients about EVPs I tell them to go to a reputable pharmacy to buy them - so they go to somewhere like Boots to buy their products. They do contain nicotine so the issue with children is an important point actually, and it is important like with any other medicine, just like with bleach in the kitchen, you keep it out of the way of children. M: From a medical point of view why do you think EVPs have become such a phenomenon? D: Well there are around 2 million people who use EVPs in the UK. I’d like to see that number doubled quite frankly, as it would mean less smokers, and certainly EVPs are a part of that. I think they are relatively new, they are catching on, people like them because as I say they are very like smoking. I have got a lot of friends and colleagues, dare I say, doctors who ought to know better, but unfortunately still smoke who now use them, and certainly patients who have found them very useful. M: …And there seem to be quite a lot of your colleagues who talk about the potential health benefits of EVPs as well? D: Yes, the fact that you have not got the tar and you’ve not got the toxins - so I think that’s why doctors are generally quite pro them. M: Is there a risk in your opinion that children will get addicted to the nicotine? D: There is no evidence that that’s the case actually. Certainly the vast majority of EVP users in the UK are existing smokers,

who are either trying to stop smoking or are just actually using these products as an alternative. They continue to use them just as they would have if they’d smoked a cigarette. M: There is an interesting statistic from some research that Puritane did that 68% of people would prefer their family or family members who smoke to smoke EVPs. What do you think about that? That’s nearly 70%. D: I’m surprised it’s not higher actually; my experience is that most relatives of smokers know full well the risk that their loved ones are putting themselves under so they would prefer them not to smoke. The other side is actually the smell; a lot of people don’t like the smell. I know certainly when I go to do a house visit on people who are smokers, the whole house smells – since the smoking ban we’ve forgotten that smell. M: As a practicing GP, do you recommend EVPs as an alternative? D: Absolutely, yes. I often talk about this sort of thing; it’s a little bit like the little black dress, what looks great on one lady is not necessarily going to work on another. And so we use a whole variety of options to try and help people in our smoking cessation clinic in my surgery and certainly EVPs is very much part of that. M: Final question for you, when you first started on TV, health awareness was more niche than it is now, do you think that people are more health conscious, and that’s why EVPs are growing as much as they are? D: I certainly think the anti-smoking message is out there now and people are aware that it is not just about lung disease; there are so many other things that smoking causes, so I think that message is out there. The fact that most surgeries now have smoking cessation clinics I think shows people feel that they can come to us and ask for help. I think we are more health aware as a nation – it’s good, but we still have a long way to go, we still have a lot of smokers in this country, we need to help them, but we are getting better.

“Excellence is achievable” There is a point It is possible to achieve an excellent rating following a premises inspection, NPA head of pharmacy service Leyla Hannbeck told delegates in a busy Business Theatre. Her presentation was entitled: The inspector called - real life experience of a General Pharmaceutical Council premises inspection. Out of 1,200 inspections, only one pharmacy has been rated as excellent. Only 3-4% of all pharmacies are expected to achieve an excellent rating, the General Pharmaceutical Council has said.

In an area with high levels of respiratory disease, an excellent pharmacy would: • Identify locally prevalent conditions

• Conduct relevant research and audit (e.g. of adherence levels, device technique) • Have relevant displays

There is a point in point of care testing – but only if the test makes a difference to condition management, said David Trevor, vice president and country manager of Nicox Pharma. Speaking in the Public Health and Wellbeing Forum, a new feature of the 2014 Pharmacy Show, Mr Trevor told delegates that his company’s Adenoplus test offered a different kind of diagnostic result: one that does make a difference to condition management.

But Ms Hannbeck told the audience that excellence is achievable, and such pharmacies demonstrate an innovative approach to care as well as partnerships with GPs and other healthcare professionals. An inspection must be a “show me, tell me story”, she said.

inspections, and for inspectors with different personalities, for staff to be nervous, and at the end of the inspection, a disappointing rating. She advised: “If not happy with the rating, talk to the inspector, or the regional inspection team, or even the head of inspection at the General Pharmaceutical Council.”

An adenovirus immunoassay test, Adenoplus offers point of care diagnosis for viral conjunctivitis, a condition which accounts for 38% of all conjunctival infections. Mr Trevor told the audience that the test could reduce misdiagnosis of the condition and reduce unnecessary prescriptions for chloramphenicol. The 12-minute test offers 90% sensitivity and 96% specificity to the adenovirus. Once viral conjunctivitis was diagnosed, people with the condition could be given appropriate advice and given support. Condition management advice includes not sharing towels, discarding make up and isolation. Support options include: cold compresses and comfort eye drops, he said.

To prepare staff for an inspection, pharmacies can do the following:

• Regularly track the impact of interventions • Have referral processes in place

• Go to events on inspection/regulation

• Link advice with recruitment to other pharmacy services for comorbidities and related risk factors, e.g. flu vaccination Experience with the inspections to date has highlighted that pharmacies can have problems with protecting patients’ confidentiality, for example, when delivered medicines may be received by someone other than the patient. Ms Hannbeck told the audience: “There are lessons here for the whole pharmacy team, including the delivery drivers.” She also noted that inspectors will want to talk to the whole team, to test scenarios, to observe staff and to look at documentation. Pharmacy teams should be prepared for long

• Ensure SOPs are in place and are demonstrated • Engage staff in role play • Hold team meetings to explain the new standards • Ensure staff know where important information is located • Chart when good things happen and show the inspector She told the audience: “Sell what you do and promote yourself, and focus the conversation on your positives. Don’t wait to be asked, volunteer the information.”

She explained that pharmacists should look outside the business, at the local population and its health needs, as well as the competition (GP owned pharmacies, Internet and multiple pharmacies). Understanding the PESTLE situation for the pharmacy (Political, Economic, Social, Technological, Legal and Environmental) would also help the pharmacy owner to understand the opportunities and threats for the business. NHS prescription data can help with this, she said. Understanding the business’s own strengths and weaknesses comes by looking in at the business; by asking customers and staff for feedback and using the 7S framework to determine effectiveness and competitiveness. Ms Kennedy Filer reminded the audience that additional services contribute about 29% of income to the pharmacy, versus the 2-3% contribution made by dispensing.

Using the 7S framework will lead a business to examine its structure, systems, style, staff, skills and shared values, she said. Businesses might also want to move between the different strategies below: Products and Services Existing

In the Local Pharmacy Innovation Theatre, another first for the Pharmacy Show 2014, delegates heard North East London LPC explain its new professional pharmacy practice, the Selfcare Pharmacy. The new Selfcare Pharmacy specialises in offering whole person care – or care that takes into account the wider determinants of health for a person. To deliver this model of care, the pharmacy team combines therapeutic and psychological tools in interventions designed to support the management of long term conditions and their comorbidities. Assessment is followed up by a selfcare plan for treatment, which is developed following negotiation with the patient using health coaching techniques, and with priorities set by the patient. The treatment pathway can also include referral to specialist advisors. Mr Patel explained: “We do what we are good at, but we work with others to deliver a complete solution for the patient.”

During a Heart Rhythm awareness promotion in the pharmacy, the Selfcare Pharmacy used ECG and blood pressure monitors, plus questionnaires documenting wider determinants of heart health, to refer 250 (out of 2,800) patients to the GP. Mr Patel said that by taking charge of clinical outcomes, the pharmacy had established a clinical relationship. “We gained respect for the pharmacy’s skills and repositioned pharmacy as the first port of call for pharmaceutical and general health issues.” Mr Patel added: “Every professional has to justify their existence. Pharmacy must be seen to be part of the solution to the wider determinants of health.”

Professionalism – how do we empower pharmacists? Day one of the Pharmacy Show saw four key opinion leaders in pharmacy discuss the issue of professionalism, and how it can be enabled in pharmacy. Sharing their thoughts with a busy Keynote Theatre hall were Duncan Rudkin, Chief Executive and registrar of the General Pharmaceutical Council, Mark Koziol, Chairman of the Pharmacists’ Defence Association, Mike Holden, Chief Executive of the National Pharmacy Association, Ash Soni, president of the Royal Pharmaceutical Society and Gary Paragpuri, Former Brand Director and Group Editor, Chemist & Druggist (C&D) (pictured left to right). The leaders said that factors such as the pharmacy contract, education and training, and leadership models all had a role to play in empowering pharmacists to professionalism. Taking up the issue of the contract, Mark Koziol used the platform of the debate to reiterate his call for individualised clinical care. He said that individual patient care delivered by an individual pharmacist is the best form of professionalism: “The job role has to be about really looking after patients. But, this is not a volume-led process and contract change will enable pharmacists to reclaim their professional territory.” He told the conference that with the community pharmacy professional membership now standing at 95% employees, pharmacists have become disconnected from the pharmacy, when these are owned by non-pharmacists. Agreeing with his point about the contract, Mike Holden from the NPA said: “It concerns me that professionalism is not always general practice and that it is not always enabled in pharmacies.” He added: “We need incentives in the contract framework but regulations and standards could also be empowering, and if we fully grasp this, the professional could be in a good place.” On the same subject, the RPS’ Ash Soni added: “Most of us are professionals and behave as professionals, but the contract does get in the way – it stops us providing value.

Professionalism is having the autonomy to behave in a way that recognises and allows people to demonstrate their skills. In community pharmacy, we give away this accountability and responsibility by referring back to the GP.” In a question from the audience, the panel were asked about the contribution to professionalism made by pharmacy education and training. Mr Soni responded that while the profession needs good scientists, it also needed people who can demonstrate care. Echoing this point, the GPhC’s Mr Rudkin said: “Across the professions there is a realisation that there is more work to be done on values. There is a role for us to push that agenda and we will be consulting on standards for Universities in the New Year. Get this right and this is the greatest safeguards for patients.”

But, in a message to legislators, Mr Soni said: “Being too risk averse will drive out capability. We need to move from a blame culture to one where we make mistakes and are able to learn from them.” Systems and regulations were also discussed by the panel and Mr Rudkin said that while systems were important, the experience at mid-Staffordshire NHS Foundation Trust had shown that reliance on systems can fail – catastrophically. “On their own systems will not get us anywhere - no one party can deliver this. There is a role for organisations to set the tone and control the environment for individuals. Without this, the best professional is a lone voice crying in the wilderness.” Mr Holden added: “We need champions in Government to create the opportunities. We don’t need 10,000 more GPs when there is an oversupply of pharmacists.”

Are you future-proof? Pharmacy Show looks Delegates at the Pharmacy Show were challenged by NHS commentator and speaker Roy Lilley to future-proof their businesses. In a presentation on day two of the Pharmacy Show, entitled: Truth, Dare, Kiss or Promise? he told pharmacists and their staff to dare to do something different. He said: “Your industry is looking over a cliff. If you are waiting for prescriptions to walk in, your business is over. It is time to rethink your business model.”

In the clinical theatre on day two of the Pharmacy Show, pharmacist Helen Williams took delegates through the community pharmacist’s role in supporting new NICE guidance on Atrial Fibrillation and on lipid management. The consultant cardiovascular pharmacist told the audience that the new AF guidance, the first for eight years, introduced a greater focus on patientcentred care, stroke prevention and symptom control. Delegates also learned of new protocols for more proactive anticoagulant use and control in all patients except for the very low risk. The audience also learned of the need for more active discussion with the patient.

He told the audience that they must adopt the 3B philosophy: • Be ready • Be open to new business models • Bring yourselves to work, so you can see your offer from the customer’s point of view He said that users of the health service were increasingly falling into the category of Savvy Patient. “The savvy patient will want to use technology for their healthcare services, including the supply of medicines. The savvy patient will change the NHS, and the savvy manager will realise that they have to change services to suit the patient.” Hospitals will become invisible, he said, as patients demand to access services using technology and not face to face. He warned the audience in the show’s main Keynote Theatre that the NHS funding gap is likely to get
