Salford will be discussing Visual Cueing and its place in the rehabilitation of walking and turning phases in Stroke patients. Sports Injuries are always an important area to look at within the Therapy field. This stream will include speaker Professor Tim Professor Tim Watson Watson, who considers the evidence base for the use of Electrotherapy within the acute sports injury time frame. A 30 minute physiotherapy session could save the NHS £2,000 per lumbar surgery patient

The research review published on physical therapy site Co-Kinetic.com based on a study published in Spine Journal, revealed that patients who had received pain education rated surgery as significantly more satisfactory on several levels compared with those who had not received pain education from a physiotherapist.

The pain education group had one 30 minute physical therapy session which involved information about the neurology of pain including the decision to have surgery, the nervous system’s physiology and pathways, peripheral nerve sensitisation, calming the nervous system and issues affecting nerve sensitivity. The session was accompanied by a £5 neuroscience education booklet for patients to read in their own time before their operation.

Patients that had received pain education also spent 45% less on medical tests and treatment, despite having similar levels of pain and disability. The one-time physical therapy session resulted in a saving of more than £2000 in follow-up appointments and further treatment per patient.

However despite these findings, at one year, there were no actual statistical differences between the two groups in regards to back pain, leg pain, fear-avoidance, pain catastrophisation and function except a saving of £2000 in further treatment.

The study consisted of sixty-seven patients, scheduled for surgery, who were randomly assigned to receive either the usual surgeon-led pre-operative education, focusing on procedures and anatomical description of the surgery (control group) or the pain education (experimental) group.

The findings suggest the neurology of pain and its effects should be more closely managed to ensure the best possible results post op and to reduce the cost of surgery for the NHS and private hospitals.

Patients following an eccentric exercise program combined with heavy slow resistance loading is most effective in the management of tendinopathy By Tor Davies, founder of the sportEX journals An incredible 45% of patients do not respond significantly to eccentric exercise programs, because many musculoskeletal practitioners are not aware of the latest evidence base and continue to use old protocols as the main foundation of treatment. Currently the management of tendinopathy is a fastchanging area of musculoskeletal medicine and has been a particular clinical research focus in the last couple of years due to its high incidence among endurance athletes. This has led the team behind the physical therapy website Co-Kinetic.com to commission and publish a new digital supplement dedicated to consolidating the most recent evidence base and turning it into principles for practical application. What we found: Tendinopathy is an area in which there are many different opinions and discussions ranging from aetiology (degeneration versus inflammation) right through to when and where a patient should be imaged. In recent years the continuum of tendinopathy had been (mostly) accepted with three stages recognized as reactive

tendinopathy, tendon disrepair and degenerative tendinopathy. However, recent work suggests that chronic tendinopathy may in fact be an active process of ongoing tendon degeneration with an inflammatory response underlying this. Further research will need to be built on for both of these conditions in the future, however they are something to consider when managing your patient. A review of Achilles and patellar tendinopathy loading programs compared clinical outcomes and identified potential mechanisms for effectiveness. They concluded that combined training (eccentric-concentric progressing to eccentric) for Achilles tendinopathy as well as heavy slow resistance loading (eccentric-concentric isotonic) for patellar tendinopathy had the highest level of evidence for improving neuromuscular function. There have been advances in the research surrounding the pathophysiology of tendinopathy but focus on treatment appears to a degree to be in the incunabulum stage. Ten years ago, the management of tendinopathy was distinctively different to what you will see today. Finding high quality clinical trials providing proven clinically significant advancement following treatment is tricky, despite the apparent plethora of theoretical research looking at tendon pathology, function and rehabilitation.

What does it mean? Further research is need to better understand the links between tendinopathy and its underlying causes, and also the effects these have on different areas tendinopathy can occur within the body, whether that be Achilles, Pateller or any other. For now physiotherapists should look to identify these underlying causes before deciding on treatment and explore new treatments, including combined training.

Physios should be better used to ease pressure on the NHS In a recent statement, the Chartered Society of Physiotherapy (CSP), called for the assignment of physios in vital medical areas to relieve pressure on the NHS. To lower the burden that the NHS faces each year, this expansion would see physios across the UK take over elements of the NHS workload, making life easier for the health service. Currently, physios already have a vast input into the treatment of patients, from assessing, diagnosing and treating injuries without any form of supervision from the NHS, they have proven that they’re well adept to do more. Additionally, practitioners who have undergone training can also prescribe various medicines for patients without supervision from a doctor. The proposal by the CSP wants greater power and opportunity given to physios in order to assist doctors and nurses in the UK. The increased level of power will allows patients to self-refer to a physio, instead of attending an appointment with a GP.

Doing so would allow quicker treatment for patients, whilst reducing the number of people who are currently waiting for appointments with their local doctor. Physios would, in turn, keep more people out of hospital through the rehabilitation of their injuries, giving them longterm support with any conditions that need such treatment whilst aiding them to self-manage. Doing so should help to reduce the increasing demand on both the social and health care budgets while helping patients to be and feel in control of their condition and live more independent lives.
