Getting to grips with GP clinical pharmacy Important conversations will take place at this years’ Pharmacy Show as key opinion leaders and stakeholders get to grips with NHS England’s plans to employ clinical pharmacists in GP surgeries. Echoing a project launched in Scotland a month earlier, in July NHS England Chief Executive Simon Stevens announced £15 million in funding to recruit and employ clinical pharmacists in GP surgeries. The aim of the three-year initiative, in which GP practices will receive up to 60% funding for the pharmacist role, is to give patients the additional support of an expert pharmacist. Examples of the benefits patients can expect include extra help to manage long-term conditions, specific advice for those with multiple medications and better access to health checks. What could a clinical pharmacists’ role be in a GP practice? •
Providing clinical advice and expertise on treatments.

Developing bespoke medicine plans for individual patients.
•
Establishing ongoing professional relationships with individual patients.
•
Assisting with communication across a patient’s care pathway, including with GPs, hospitals and social care teams.
•
Monitoring patients with complex long term conditions such as hypertension or diabetes.

Managing repeat prescription requests.
•
Increasing the uptake of new medicines.
•
Managing medicines shortages by suggesting suitable alternatives where appropriate.
•
Supporting innovation and clinical research where appropriate.

Mentoring newer pharmacists.

Now in its ninth year, Pharmacy Show has become the premier event in the pharmacy calendar and one of the profession’s most important occasions where agenda-setting conversations take place. With the subject of integrated care, and practice pharmacy on the discussion table, this year’s event is set to be the most timely yet. Key among the questions already in circulation about the initiative are the implications for the community pharmacy network if the idea gains currency. The concern centres on the question: what role is left for community pharmacy? The future of community pharmacy It is fair to say that community pharmacies do – and will in the future - continue to encourage a different type of patient to those presenting in general practice: the worried well patient rather than the sick. In a world

The aim of the Face to Face Wellbeing Centres Scheme is to help maintain and improve the health of the UK populace by offering professional nutritional advice to consumers from BANT through community pharmacies across the UK. Since last year’s launch a growing number of pharmacies have engaged with the scheme providing nutritional advice and support to consumers within the pharmacy with marketing support provided by the team at JHN Productions, publishers of Natural Healthcare in the Community.

Are you a pharmacy technician? Where do you see yourself in 5 years time? What does the future look like for you and the pharmacy technician profession? What patient centred care will you be delivering? As NHS England launches a £15million scheme to fund and employ pharmacists in GP surgeries, APTUK, your professional leadership body asks, what can pharmacy technicians do to support the general practice and community teams? What part will pharmacy technicians play in improving patient care, particularly those already working in and with GP practices? As the challenges to an over stretched NHS increase with an ageing and growing population, it is recognised that greater emphasis will be on public health and disease prevention. As pharmacies become ‘the first point of contact’ for minor ailments and long term conditions, where do pharmacy technicians fit into this vision? As the professional leadership body for pharmacy technicians, APTUK have been representing the profession at a number of events looking at how all the healthcare professions can work as part of a single service to move to ‘one system’ that is easy to use by patients and that makes the best use of all resources. APTUK suggests and emphasises that pharmacy technicians have a role to play in supporting these services, as professionals who are an integral part of medicines optimisation and medicines supply. Pharmacy technicians are already helping patients by supporting many of the locally commissioned community enhanced services, such as smoking cessation, care home medicine supplies, weight management, out of hours services etc, delivered in the pharmacy to improve the public’s health and well being. APTUK are also delighted that pharmacy technicians, along with pharmacists, now have access to a patient’s Summary Care Records to help them provide safer care and to help reduce avoidable medication errors.
APTUK wholeheartedly support the need for training and personal development for each pharmacy technician, and recognise the need to support trainees, qualified pharmacy technicians and those responsible for training and developing staff. As a result, APTUK have developed the Foundation Pharmacy Framework (FPF). This framework is available to all, with additional supporting resources available for our members. More information can be found at www.aptuk.org along with information on how to access a trained FPF champion for individual support and coaching. APTUK are also supporting the CPPE Declaration of Competence (DoC) system that is seen by the community pharmacy services commissioners as a quality marker for delivering consistent and safe enhanced services to patients. The DoC is a self assessment framework that is used as a tool to reflect on your own competence and learning required to deliver specific services.

But with the advent of internet pharmacy, hub and spoke dispensing models and robot/vending machines, the challenge facing community pharmacy is to add value to the faceless but increasingly prevalent internet supply route alternative. lf patients gain all the clinical advice they need from their practice clinical pharmacist, and are happy to leave the logistics of supply to a technological or technician-led solution, what is there left for community pharmacies to do? In its response to Simon Stevens’ announcement, multiple pharmacy group representative Pharmacy Voice CEO Rob Darracott has already warned against new models of care putting “other important developments for community pharmacy on the back-burner”. Lobbying for the existing network model he says: “Strengthen[ed] links between general practice and the community pharmacy network… will greatly amplify the benefit to both GPs in terms of workload reduction, and ultimately to patients who will receive more integrated care and enhanced access to qualified healthcare professionals across the whole care pathway. Greater use of the network can make a positive difference to the issues facing general practice and A&E now, and over the coming winter.” Return on investment GPs with an eye on the eventual need to fund the practice pharmacist role will also want to understand the potential return on their investment. From year four of the pilot scheme, GP practices will be expected to pay 100% of the practice clinical pharmacist costs, and these will include the costs of training, supervision and changes to the practice infrastructure. But, GPs question whether pharmacists will have gained sufficient risk-management skills in just three years to safely take over the appointment-busting patient consultations which currently hijack the GP’s day. GPs also suggest that the length of time it will take to introduce large numbers of appropriately skilled pharmacists into
primary care will mean that practice clinical pharmacists cannot be an immediate solution to the current workforce crisis in general practice. Neither is it clear at this point what salary band the clinical practice pharmacist role will attract as the role gains currency with GPs. It seems likely that the practice clinical pharmacist skill set should command a premium compared to that paid to community pharmacists. But, will this result in a net migration of pharmacists from community and/or hospital pharmacy, and simply destabilise those sectors of practice? Or, will the community pharmacy operators simply up the pay ante in a competitive counter-strike? And, if so, where does this leave the independent sector which has less financial leeway? Is a premium necessary at all, given the considerable surplus forecast in the pharmacist workforce? Taking place on October 18-19 at the Birmingham NEC, this year’s Pharmacy Show is perfectly timed for pharmacists to come and hear the great and the good thrash out the key issues of the day. SCR access: added value or added headache? In a move that has taken over a decade of lobbying to achieve, from autumn 2015, pharmacies in England will have access to the patient summary care record. But now the wish is about to become reality, will SCR access allow pharmacies to add value to the patient consultation, or have they simply opened up a can of worms of added governance, and increased their professional liability – and for no very good reason? In June, health secretary Jeremy Hunt allocated £7.5 million for community pharmacy training that will be spent on supporting access to the summary care record (SCR). This is a centrally held electronic record containing key clinical information including a patient’s medication, known allergies and any adverse reactions to medication. At the end of March 2015, over 94% of the population of England had an SCR available to be viewed by care professionals. Learning of the SCR access and funding, the pharmacy bodies are unanimous in their welcome, and in a joint statement Pharmaceutical Services Negotiating Committee (PSNC), the Royal Pharmaceutical Society and Pharmacy Voice said: “We strongly believe by extending summary care record access to community pharmacists, patients will receive better, safer and more accessible care.” Their comments centre on a proof of concept (POC) evaluation in which 140 pharmacies across five geographical areas were enabled to access SCR. In a six month pilot, over 1,900 SCRs were accessed by the participants, and at the end of the POC pilot the pharmacies involved expressed a strong desire for access to be maintained. Yet, reading the POC evaluation a little more closely, it is also evident that while some pharmacies accessed the SCR on a regular basis, most did not; more than half of respondents to the POC survey said they accessed the SCR less than once a week, and on average POC participant pharmacies only accessed the SCR 2.9
times a month. Among the issues cited for low or nonusage of their SCR access, pharmacists cited already good lines of communication with local GP practices, governance issues (specifically, the labour-intensive process for monitoring locum access) and technology issues which took time to resolve. Importantly, the POC report showed that almost one in five (18%) SCR views resulted in the avoidance of potentially moderately or majorly harmful prescribing errors. Although good for patients, this, too, has ramifications for pharmacy professionals – namely, that pharmacy’s professional indemnity insurers will demand the SCR is accessed on every dispensing occasion. How this fits into the already stretched community pharmacy day is a question that Pharmacy Show delegates will naturally be very keen to see answered. Pharmacy Show delegates will also want to understand whether SCR access is actually something their patients want. While the POC evaluation highlights positives for patients, such as reduced overall waiting time and help with identifying ‘mystery’ medications, there are also negatives, and in the POC report some participant pharmacy professionals flagged up patients’ confusion and uncertainty when they were asked for permission to view their SCR.

The future for electronic prescriptions Electronic prescriptions currently account for around a quarter of all prescriptions issued in England but given recent reports, it would appear that this quarter of prescriptions generates far more than its fair share of complaints. With plans just announced to move the Electronic Prescription Service (EPS) in England over to a predominantly paperless system, is it any wonder that the EPS will be high on the discussion agenda at this year’s Pharmacy Show? In a statement detailing the next phase for the EPS, the Health and Social Care Information Centre (HSCIC) has told pharmacy teams that the EPS is now envisaged as “the default option for prescribing, dispensing and reimbursement of prescriptions in primary care in England”. It has explained that while paper prescriptions will continue to be available for special circumstances, “the vast majority of prescriptions will be processed electronically”. In practice, this will mean that patients will no longer need to nominate a specific dispenser (although they still can, should they so wish) and that their electronic prescription will now be available to take to any dispenser. To obtain their medication, all patients will be offered a scannable ‘token’ to present at a pharmacy. HSCIC says that it expects these new changes to increase the proportion of prescriptions sent electronically to around 90% where a practice is live with EPS. Some prescriptions will remain unsuitable for transmission by EPS, for example those where a patient has elected to receive their prescription on paper or where there are constraints on the prescribed drug. Further developments will look to address these restrictions and maximise EPS volumes.
