For far too long spinal manipulation has only been associated with the management of painful spinal conditions, and the joint cavitation has been viewed as the holy grail of expert practice. Spinal manipulation has a well-established evidence base in contemporary musculoskeletal practice. However, its clinical objectives are all too frequently attributed to the restoration of joint position or movement of specific spinal segments believed to be misaligned or hypermobile. This limited outlook does not embrace increasing evidence supporting neurophysiological mechanisms affecting pain and sensory motor control after manipulation.

Normal human movement is reliant upon effective sensory motor control. The sensory motor system is responsible for the regulation of posture, joint stability and movement. Manipulation is an effective treatment tool for gaining entry into the sensory motor system to promote normal movement. The capsuloligamentus and myofasical tissues contain mechanoreceptors, which are capable of stimulation by the forces applied through manipulation. Several authors have reported clinically significant changes in post intervention proprioception immediately after lower quadrant joint manipulation and mobilisation. Absent, reduced or inaccurate sensory information will compromise the sensory equation of the sensory motor system. This system is critical for all components necessary for individuals to move and experience the environment in which they wish to function. All activities of daily living and athletic performance are reliant on afferent input from the periphery to the central nervous system. The spine has a rich source of sensory receptors that convey proprioceptive information from each spinal level about the positional status of the individual. Insult or dysfunction of spinal structures whether due to previous pain or poor postural habits can compromise its mechanical contributions to sensory motor control of the kinetic chain. Distorted sensory information can affect the planning and organisation of motor behaviours at its higher centres. Poor sensory input at the receptor level has the potential to disrupt processes that are reliant upon its inclusion for voluntary movement, postural adjustments and motor learning.
For example, using this approach it would not be unreasonable to assess the spinal articular system of patients presenting with persistent ankle instability. Most therapists would examine the ankle in detail but not consider the regional interdependence of motor control for the whole system. Spinal manipulation with a goal ordinated motor relearning programme provides a rich prioprioceptive training environment. Although, repeated practice is necessary to facilitate long-term motor recovery, an integrated approach should include targeted manipulation, mobilisation and exercise therapy. This concept of treating the whole person is not new, but is often forgotten in the sprint to rehabilitate the patient. All regions of the body are interdependent both in terms of physiology and anatomy. The effects in one location are often related to distant locations associate with the patient’s primary complaint. Conceptually, regional interdependence has an abundance of evidence including case reports, descriptive reports and controlled randomised trials. There have been several reports of patients complaining of primary low back pain with hip or knee involvement. Many studies report clinically significant changes in symptoms and movement behaviors when treatment is directed at distant anatomical locations. In the upper quadrant, studies have reported post intervention treatment success after manual therapy for the thoracic region of patients presenting with primary neck pain and shoulder impingement. The role of regional interdependence and sensory motor control should be given serious consideration in all musculoskeletal conditions with disturbed motor behaviours. Aberrant motor behaviours could at least in part be caused by dysfunction of the sensory equation of the sensory motor system. Spinal manipulation is a time efficient and effective treatment modality for stimulating mechanoreceptors and facilitating normal movement behaviours. Therefore, an integrated approach for the clinical management of disordered movement should include spinal manipulation and joint mobilisations employed to prepare patients for exercise therapy and functional activities. The Challenge of Choosing the Correct Rehabilitation Technology to Treat Your Patient The use of technology in neurological rehabilitation has finally come of age. Therapists and GPs are increasingly employing the rehabilitation triad of hands-on physiotherapy, technology and exercise to maximise the rehabilitation potential of their patients. The challenge now for therapists and GP’s is selecting which of the many emerging technologies are the most clinically appropriate for their clients. Rehabilitation technology can be used to accelerate the rehabilitation process by minimising impairments and promoting functional recovery. This may include increasing strength, decreasing spasticity and improving motor control. Alternatively technology can be used as a substitution for lost physical function and thereby permit activity otherwise not available to the patient. For example the recruitment of weak or paralysed muscles required for normal walking. Technology can also enable intensive, repetitive, task-orientated exercises for upper limb function or provide real-time feedback vital to motor learning in the absence of a supervising clinician.

How important is Soleus to managing midportion Achilles tendinopathy? Seth O’Neill, Physiotherapy Lecturer, University of Leicester Achilles Tendinopathy (AT) commonly affects runners with prevalence rates around 10% reported but with some reporting rates as high as 50%. Endurance runners are seemingly most at risk. AT is general accepted as being an active degenerative process, with some inflammation biochemical pathways involved. These changes eventually lead to tissue alterations, tendon pain and dysfunction. One of the main risk factors in otherwise healthy athletic individuals is repetitive high tendon load activity, such as running. It may be that one of the Plantarflexors is more involved than another.
IS THERE AN ARGUMENT FOR SOLEUS INVOLVEMENT? Points to consider include that Soleus is an important force producer, especially for endurance athletes. Despite the common misconception that Gastrocnemius is the main force producer during running it is in fact Soleus. During running the Soleus muscle produces up to 50% of the total vertical support force, which is made possible by the fact it makes up 52% of the volume of the posterior calf. The actual forces it produces are around 8 times body weight but in comparison the Gastrocnemius produces forces around 3 times body weight. Most clinicians and researchers seem to have assumed the muscles always work in unison, however Gastrocnemius and Soleus can and do work independently. Soleus is primarily the “endurance” muscle of the calf and as such has a higher level of slow twitch muscle fibres making it more likely to be essential for endurance running. The existing literature around calf function and AT is really very limited. Evidence has shown people

with AT have an earlier offset of Soleus EMG activity during running compared to controls. Whilst this timing alteration may be an effect of pain and not a precursor to injury it certainly shows different adaptations to induced tendon pain. There is currently a strong suspicion that Soleus loading is necessary and essential to the resolution of Achilles tendon pain. In order to effectively rehab Soleus muscle deficits we need to use knee bent calf raises -standing or seated, and incorporate concentric and eccentric loads and progress to eccentrics in isolation as this allows heavier loads to be used. Rehab obviously needs to be graded and gradually increased over time. There is sufficient evidence to substantiate the involvement of Soleus weakness in Achilles Tendinopathy. However, further work needs to be completed to determine if it is a prospective risk factor. Obviously not all patients will behave as the evidence suggests, so I will be expanding on these concepts and research findings at this year’s Therapy Expo.

In reality, flexibility and range of movement are very much movement specific. As Ben Cormack of movement specialists Cor-Kinetic says “Flexibility is a component of successful movement. We need to be able to achieve the right amount at the right time in the right movement.” With that in mind, the usefulness of lying a runner down on a couch in order to assess whether they can achieve the required
Further research is needed to better understand the extent to which conventional analysis of a patient’s range of movement is hindered by manual tests conducted on a couch and the possible solutions. For now physiotherapists should look to integrate a variety of techniques to analyse movement.
WHAT DOES THIS MEAN?
Movement is typically presented to us as the product of individual structures like muscle, ligaments, tendons and bones. For example, if you are unable to touch your toes you are told that your hamstrings are “too tight” or “too short”; if you find it difficult to lift something heavy, you are told a particular muscle(s) is “too weak”; if you get tired too quickly your muscles are said to lack endurance; go over on your ankle a few times and you are told certain ligaments are too loose, and so on. Modern research suggests that movement is not a product of how short, long, weak or strong a muscle, ligament or tendon is. Just as bones would be nothing without muscles pulling them, muscles would be nothing without the brain communicating with them. It is the brain that decides how and to what extent we move by comparing a host of sensory feedback at that moment with a stored memory bank of every movement you have ever performed in your life. Quite a powerful little hard drive when you think about it. The feedback systems are proprioceptive, visual, vestibular, and include visual horizon, orientation against gravity, joint compression, angle and torque, tissue tension and length. The feedback is obviously very different when you are lying static on a couch, compared to the dynamic movement of running. We have obviously removed the need for balance, acceleration and deceleration etc. Range of motion itself is traditionally seen as a skill. Flexibility is seen as a skill. We are

I was unable to even sit up by myself and had to re-learn all the simple tasks you take for granted all over again,” states Justin. “It would be fair to say I was a fit and active 33 year old when I suffered the stroke; I loved my job as an electrician, I was a keen golfer and would also coach my son’s football team at the weekend. When I was unable to get out of bed it was devastating to not be able to do my daily activities and only be able to see my children when they visited me in rehab.” He has since become the first patient in the world to be fitted with a new product called MyGait Cuff Soft to treat drop foot. The most advanced cuff-based stimulator on the market, it offers two-channel stimulation – meaning more than one muscle group can be stimulated at one time. In addition, the new technology allows for adaption during sub-phases of gait to improve the way patients walk. “I’ve tried all sorts of treatments for my drop foot – from orthoses right through to other FES devices on the market and can honestly say the new MyGait Cuff Soft is worlds apart. It’s more comfortable than anything I’ve tried and is slim enough to wear under any of my jeans & trousers, even the narrow ones – which is a first for me! It fits really nicely, is easy to put on and off and has allowed me to go out and do so much more. I have

my confidence back and have lost the fear that comes when you’re unstable and immobile.” The technology works by providing an electrical impulse that is delivered to the nerve which stimulates the muscle into movement. A heel switch sensor communicates wirelessly with the cuff and electrodes to stimulate the appropriate nerve at the right time and frequency during the walking cycle. A unique advantage of MyGait system is that in addition to the dorsiflexor muscles, other muscle groups can also be stimulated using a second channel. This makes it possible to provide additional support when walking. Current ReWalk designs are intended for people with paraplegia, a spinal cord injury resulting in complete or incomplete paralysis of the legs. The system uses patented motion sensing technology along with battery-powered motorized legs powering knee and hip movement which is controlled by proprietary onboard computers and software. The systems allows the user to sit, stand, walk, turn and has the ability to climb and descend stairs. ReWalk users are able to independently operate the systems. Doctors Saved Her Life… Functional Electrical Stimulation Helped Her Walk Again… Kate Weaver was 29 years old and 39 weeks pregnant when she realised she was having a stroke. Kate who is a nurse, managed to warn her husband about her sudden condition, before she lost her speech and ability to move when she felt a weakness through the left side of her body. Doctors saved her life using a procedure that had never been tried on a pregnant woman at their hospital before, and days later delivered her baby son, Toby. Although Kate’s speech returned within four hours of her stroke, she remained partially paralysed and had almost no movement in her left side when she left the hospital. This was the beginning of her long road to recovery, including learning to walk again and regain use of her left arm whilst looking after her young baby. After her stroke, Kate had two personal goals: to wear high heels again and to start running again. Like so many of our patients Kate had to undertake her own research to find technology that would help her. When Kate initially told me her difficulties over the phone I was confident FES (Functional Electrical Stimulation) would help. She described a very familiar picture of tripping and poor confidence. She was unable to clear the ground with her left foot and often caught her toes.
This meant that she did not feel safe carrying Toby around her home or out to the car. It was a huge leap of faith for Kate to come all the way from Shropshire to visit our specialist clinic in Northampton for an FES Assessment. When she arrived at my clinic, I could immediately see that she would benefit from the rehabilitation triad – hands-on therapy, exercise and technology. Her left hip was weak and she struggled to transfer her weight onto her left leg and her left foot was catching the ground with every step. I assessed her with the Walkaide FES system that uses houses the stimulator in a cuff that is worn just below the knee. She had an immediate response and was able to clear the ground and walk confidently around the clinic and outside over uneven ground. She was delighted. As a nurse, the ‘Turn 2 Us’ charity supported Kate and purchased a Walkaide for her. She then embarked on daily use of the Walkaide and a course of hands-on therapy with plenty of exercise as home work. The result, Kate has recovered enough she now no longer uses the Walkaide and returned to work. Kate recently ran a 5km park run in under 40 minutes only 18 months after being told she might never walk again.
