Professional indemnity insurance – are you covered? By Kate Sheehan, Independent OT Are you aware of the new requirements that all OT’s (private, statutory, third sector and voluntary) must now have appropriate professional indemnity insurance arrangements in place as a condition of registration.

On 30th November 2012 Minister of State for Care Services, Norman Lamb (pictured), announced the roll out of personal health budgets. Currently, those eligible for Continuing Healthcare (CHC) have a ‘right to ask’ for a personal health budget, but from October this year they will have a ‘right to receive’ it also.

A personal health budget is an amount of money to support a person’s identified health and wellbeing needs, planned and agreed between the person and their local NHS team. The Department of Health (DOH) state that their vision for personal health budgets is to ‘enable people with long term conditions and disabilities to have greater choice, flexibility and control over the health care and support they receive’.

Implications Unlike personal social care budgets which are designed to give the recipient flexibility over who they employ to meet their daily functional needs, the health budget focused on health and wellbeing. The examples provided by DOH from the pilot scheme indicate that those eligible have used their budget to pay for therapeutic services tailored to their needs which would not be available on the NHS. This has potential to allow a recipient of the budget to pay for services such as Occupational Therapy, where goals can be set to have positive impacts on wellbeing and, as a result, minimise

hospital admission, referral to crisis services and ultimately long term care costs. An Occupational Therapist’s Thoughts In some part, it can be difficult not to be skeptical about the terminology used to promote this innovation. Patient/client centered services, empowerment and choice are surely only as good as the advice, support and advocacy which surrounds it? From a clinical point of view, CHC eligibility is difficult to gain therefore do the criteria of the CHC eligibility ‘gate keepers’ change? However, the examples provided by DOH are compelling and if the theory is applied along with robust processes developed and implemented, the budget should allow recipients to truly get what they need.

This is a result of new legislation (The Health Care and Associated Professions (Indemnity arrangements) Order 2014) on the application of patient rights in cross-border healthcare. Predictors of long-term outcomes after severe brain injury By Karen Keen, Head of Service, Independent OT Practice Early rehabilitation interventions seem to be essential for how well an individual recovers from a severe brain injury. It might even increase the chances for longterm survival, according to researchers at the University of Gothenburg.

and how fast they can return to work, is substantially improved among those who are admitted to inpatient rehabilitation care early. Being unemployed or on sick leave prior to the injury also seems to be associated with worse functional activities performance and quality of life several years after the injury. Moreover, the Gothenburg studies show that men who suffer a severe traumatic brain injury have a five times higher risk of dying 10 years after the injury; for women, the risk is eight times higher. These findings also confirm similar recent research results.

The increased risk can be attributed to illnesses and disabilities lingering on for several years after the injury. The participants reported lasting disability and low quality of life, with a complex range of physical, cognitive, behavioral and emotional disturbance.

The studies included 280 Swedish and Icelandic participants who were followed up 1 - 11 years after the injury, show a clear association between how quickly individuals get access to rehabilitation and how well they recover. Outcome was measured once, 2 - 11 years after trauma, and included hormonal testing, the Short Form-36 Health Survey, the Glasgow Outcome Scale – Extended (GOSE), and a self-report questionnaire specifically designed for these studies.

Ulfarsson discussed that the studies provide important information for survivors of brain injury, their families and friends, and the healthcare sector. One conclusion is that a severe traumatic brain injury should be considered a chronic medical condition that requires professional care and support from acute care through to specialist community neurological rehabilitation. ‘Our studies provide valuable information that will help us improve outcome predictions, optimise the rehabilitation process and

Going independent? Top tips to consider when making the leap By Caroline Pomeroy, Independent OT Why did you decide to start your OT business/start working as an independent OT?

from my post. I never really intended to only work as an associate but, I find that it works really well for me.

What is the best thing about owning your own business/ being an independent OT?

Have you found a benefit from social media marketing (twitter, facebook, google plus, etc)?

For a long time I’d been curious as to whether working independently as an Occupational Therapist could work for me. I was self employed for some time before I trained as an OT so had already had some experience of the book keeping side of things. I have always naturally sought a challenge and, because Occupational Therapists are renowned for their adaptability, if things didn’t work out I was confident that I could do something else with my skills. My curiosity led me to attend a COTSS-IP look before you leap conference which gave me the push to dip my toe in the water and try it out.

Thus far I have managed to avoid the terrors of social media. It is always in the back of my mind that I may have to embrace it fully at some point but the longer I can deny its strong influence the better. I did send a tweet once but I talk far too much to be able to say what I mean with so few characters available to me!

It’s a bit of a cliché but, having the time and resources to be able to use my skills and imagination to make a real difference to peoples lives rather than just scratching the surface to reduce their difficulties. For the first time I feel like I am doing real Occupational Therapy where I can carry out treatment programmes and see them through to the end, getting people back to work, enabling access to a toilet for the first time in a year, providing the right equipment to help someone walk their dog, facilitating sensory stimulation, increasing confidence, reducing anxiety....... The list is endless and the restrictions due to resources are few. It’s a wonderful feeling. I love my job.

How did you take your first steps to starting a business/ in independent OT?

Also, be meticulous with book keeping and time management, if these systems fail you will quickly lose control and flounder.

I contacted a number of private occupational therapy services and went through the process of registering as an associate with them. A few pieces of work began to come in, initially I did these on top of my full time post with a local authority and it was not long before I found myself working weekends and evenings to make sure everything was done. At this point I knew I could drop my full time hours by half, within a year I could no longer sustain both private and local authority work so I bit the bullet and resigned

What were some lessons that you learned in the initial stages of your business/working for yourself? What I have learnt is the importance of contracts. If you follow all of the advice and make a water tight contract and agree prices (including who will be responsible for payment) prior to carrying out any work it will save a lot of time and hard work in the long run.

What was your original goal and have you been able to achieve that vision, if not how have things changed? I always intended to set up my own business and promote my services locally. As things are, I am a sole trader working as an associate and travelling anywhere across London and the South East. This works well for me, in fact much better than I had imagined and I have no need or want to change the way things have worked out.

What are some tips that you could give to OT practitioners considering starting a business or working for themselves? • Believe in yourself and your skills, you CAN do it. • Keep on top of your CPD...there are lots of resources out there to make sure you keep up to date and ensure your skills are fresh, they do not always have to be costly. • Don’t lose touch with other OTs-networking is essential. • Make a business plan, stay focused and manage your diary and time well. • Make absolutely sure you keep a good work/life balance. Independent practice and sub-contractor for NHS Fiona graduated from Brunel University in 2008 and has worked across a variety of fields during her career so far. This includes various acute general medical hospitals, a private medium secure forensic mental health hospital, nursing and residential EMI homes and is now starting up her own independent practice, working with people with long term physical conditions of working age.

Jennie graduated from Brunel University in 2007 as an Occupational Therapist and then worked as a Deputy Manager for a charity called CMSS, which encouraged adults with disabilities to participate in and enjoy activities such as college, holidays, social activities and work. Jennie’s main role was to research and arrange holidays both in the UK and abroad for the clients who attended the centre.

Acute in patient stroke unit Sally graduated from the West London Institute of Higher Education (Brunel University) in 1994 with a Diploma in Occupational Therapy. Working at Basildon Hospital in Essex, Sally gained experience in many areas of acute inpatient and outpatient care including general medicine, neurology, surgery, orthopaedics, rheumatology and elderly medicine. She worked as a Senior 2 O.T in an elderly care rehab unit for 6 months and then a newly opened acute stroke unit where her love for working with stroke patients began. Sally then went on to work in various roles including Deputy Head O.T for the O.T dept at Hemel Hempstead hospital, Senior O.T in A&E and the rapid assessment unit and Professional Lead O.T. In July 2013, Sally took up her current post at Luton and Dunstable hospital working predominantly on the acute stroke unit. There were interesting and thought provoking seminars and some brilliant information on HCPC requirements for CPD portfolio. Overall a great experience!”

RICA BEST BUY ENERGY SAVING LIGHT BULBS - Getting the right lighting is crucial for older people and especially those with sight loss. Rica, the independent consumer research charity, has updated an essential guide to buying energy saving light bulbs which includes Best Buy recommendations from Which?. The guide was funded by sight loss charity Thomas Pocklington Trust. The guide, Choosing energy saving light bulbs for your home provides useful information about bulb types, colour of the light emitted, start-up times and types of fittings. The guide explains lumens which denote the brightness of the bulb and are more important to look for than the old measurement of wattage. The guide recommends the ideal lumens for specific areas of the home; at least 1200 on the stairs for example, but only up to 250 when fitted under kitchen cabinets. The Rica information has useful tips for buying light bulbs including: • work out what you need before you shop and write it down if necessary • go to a specialist electrical shop if you can • don’t shop by brand as it’s not the best indicator of what you need.
