NHS England plans new five-year strategy
The Pharmacy Show is organised by CloserStill Media Healthcare Limited George House, Coventry Business Park, Herald Avenue, Coventry CV5 6UB.
NHS England is developing a five year strategy for the NHS which will be called the NHS Five Year Forward View (5YFV). Likely to be published in October, the same month as the Pharmacy Show, it is expected to be a highly influential document setting out Simon Steven’s (NHS England chief executive officer) vision for the NHS.

Up in Smoke! At the Pharmacy Show, healthcare commentator and journalist Roy Lilley is chairing the big debate “Should pharmacies sell electronic cigarettes?” on Monday, October 6th. Ahead of the debate, Roy penned his thoughts on the issue for this newspaper. It's Herbert's fault. In 1963, a Korean war veteran, a 40-a-day smoker; Herbert A Gilbert from Beaver Falls, Pennsylvania, filed a patent for a product he described as a "smokeless non-tobacco cigarette".
E-fags? Inhaling stuff that god never put in fresh air can't be a good idea, can it?
Are they or aren't they? Do they or don't they? E-cigarettes are they a good thing or bad? Cancerous or not? From what I can see you might as well flip a coin.
It's emerged Boots have been working with Imperial Tobacco to sell e-fags. Other chemist shops will follow, no doubt.
According to the University College London: "For every million smokers who switch to e-cigarettes, more than 6,000 lives a year could be saved". Really? University College London is alleged to have £750k tied up in tobacco investments? Mmm. A DH spokes-bod said: "More and more people are using e-cigarettes and we want to make sure they are properly regulated so we can be sure of their safety. We have already set out our intention to change the law to ban the sale... to children under 18". "The UK has an existing licensing system for higher strength products and those that claim to help people quit. We are also bringing in new EU rules to cover lower strength products which will ban most advertising, limit nicotine levels and set standards for ingredients, labelling and packaging." Well, that's OK then. Maybe it's worth pointing out that conventional smoking kills 100,000 people in the UK every year.
I consider myself extremely fortunate that tobacco has never played any part in my life and I have no idea how difficult it is to give up smoking. If smokers knew how they smell and how irritating it is when they 'pop-out' for a fag, they might try a bit harder. Who knows?
Donchyathink pharmacies, selling e-fags gives them a veneer of respectability? Boots, lending their brand, wraps the fags in a cloak of semi-medical decency. 'It's OK to smoke these... I got them from the chemist'. Not good methinks. If pharmacies sell these things why don't they go the wholehog and sell Marlboro on the grounds a craving for nicotine is a medical condition and they are providing a customer service... like a Nurofen for a hangover... why not a hair of the dog... sell Vodka? E-fags will hamper the growth of smoking cessation products and services. Why got through the pain of giving up when an electric puff of harmless propylene glycol, vegetable glycerin, nicotine and flavourings will do the trick. Patches and gum are generally lower margin products than the very profitable e-fags; a market worth £193m, that increased by 340% last year. There's loadsamoney in this. Boots say; "... customers wishing to buy the product will have to have a discussion with a pharmacist to ensure they are sold 'appropriately." I guess that means in exchange for money.

The New Medicine Service – the way forward or wasted money? Official evaluation confirming the value of the New Medicine Service will be a key topic of conversation at this year’s Pharmacy Show, taking place at the NEC in Birmingham on Sunday, October 5th - Monday, October 6th. Pharmacists have given a mixed response to news that the service has been found to significantly increase patients’ adherence to new medicines and that it will save the NHS approximately a net amount of £20 per patient in terms of patient outcomes. Practical considerations for community pharmacies such as workload and working environment have been recognised by the evaluation, which finds that local staffing and resource issues impact on the provision of NMS; pharmacies with higher numbers of staff are better able to manage and deliver the NMS service without placing excessive burden on staff.
the intervention is conducted by a pharmacy that is co-located with a general practitioner. “Co-location enabled closer working between pharmacists and GP to confirm and refer patients, possibly leading to a triangular relationship between patient, pharmacist and GP, rather than two separate bilateral relationships.” Supermarket pharmacies had the lowest effectiveness, which is said to be due to “the less developed relationships with patients and a different business prioritisation”, according to researchers.
“Patients have a lot to gain from the New Medicines Service (NMS) - as this study shows – and it is excellent news that 90% of pharmacies in England are now offering this service” Dr Maureen Baker, Chair of the Royal College of General Practice Problems with supply of drugs is also mentioned in the report, leading pharmacists to question whether resolving these, rather than funding the NMS, would be a better use of NHS funds. The research also finds that the likelihood of medicine adherence following an NMS may be almost double if the service is conducted by a small multiple compared with an independent pharmacy. “Staffing, work planning, the use of technology and scheduling affect pharmacies’ ability to deliver NMS,” conclude the researchers from Nottingham University, which was commissioned by the Department of Health to conduct the evaluation. They continued: “Small independents were most likely to report struggling with this.” Patients were also found to be more likely to be adherent if

The world of wellbeing! This year sees the introduction of a Public Health & Wellbeing Forum at the Pharmacy Show that will host a series of sessions highlighting pre-emptive healthcare advice, OTC products and patient services. Wellbeing has also been the focus of a large piece of work for Pharmacist Support – the profession’s independent charity – who in 2013 piloted a wellbeing service to help those in the profession understand and combat stress. This service was born from a legacy that was received in memory of pharmacy owner Robert Wardley. He understood the pressures of working in pharmacy and wanted his gift to be used to support those struggling with stress. Following extensive research the charity piloted a series of wellbeing workshops and an online e-therapy tool. Three separate types of workshops were developed and tailored to meet the needs of pharmacists, pre-registration trainees and pharmacy students. Attendees of each were taken on a journey through the science of stress and anxiety and their signs and symptoms and were then equipped with techniques to help deal with feelings of pressure and to manage their time better. As a secondary intervention workshop delegates were also given the option to self-refer for more in depth support offered through an online e-therapy service. This was a Cognitive Behavioural Therapy based package that clients accessed from home. These sessions encouraged awareness and understanding of how our thought process affects our
behaviour and emotions and aided e-therapy users to develop a greater perspective and balance in their personal and professional life. Additionally, learning techniques to challenge unhelpful behaviour and negative thoughts aided clients in breaking vicious cycles of depression and anxiety and to feel in greater control of the challenges they face. Evaluations of both workshop and online sessions indicated that they had had a significant impact on service users – highlighting a reduction in anxiety and depression and an improvement in wellbeing and general functioning. With a successful evaluation of the pilot complete, the charity now looks forward to rolling this service out across Great Britain. To celebrate the launch of the Wardley Wellbeing Service, the team at Pharmacist Support will be incorporating wellbeing elements into the stand at the Pharmacy Show for attendees to experience. Up in Smoke! Cinderella of the health family Pharmacists have long been the Cinderella of the health family. Begging to be allowed to contribute more; med's reviews, LTC management and the like. They've never been taken seriously. Stuck in the canyon between the condoms and sun-cream they earn a good living without the palaver. They could recruit and run teams of community nurses and win AQP contracts to manage diabetes and COPD, leg ulcers and the like. But they never do. You just see the occasional head bobbing up and down behind the shelves in the dispensary. Flogging plastic fags doesn't fill me with optimism about their intent and purpose.
Let's face it; the future of pharmacy is on-line. Amazon-style, next day delivery of your prescription to your office or home. Cut the cost of the high street out of the supply chain equation. It's bound to happen. Boots? There's nothing you can buy in Boots you can't buy elsewhere. It has a confusing retail offering. I'm never sure why I am in Boots, or why it exists? It is owned by a private equity company and locates its HQ outside UK tax jurisdiction in Switzerland. There have been protests outside their stores. Flogging Mickey Mouse Marlborough won't endear them to their detractors.
I may be wrong but my guess is, if Boots persist with e-fags and other pharmacists follow them, they'll all see their reputations go up in smoke.
