National discussion on patient- Pharmacy regulator unveils centred professionalism begins three-year regulation plan at Clinical Pharmacy Congress The pharmacy regulator, the General Pharmaceutical Council, launched a national discussion on patient-centred professionalism at this year’s Clinical Pharmacy Congress. Delegates were asked to give their views on what patient-centred professionalism means to them and what were the barriers and enablers to acting professionally and giving patient-centred care. Duncan Rudkin, the Chief Executive of the General Pharmaceutical Council (GPhC), led the first conversation about what patient-centred professionalism looks like in pharmacy today, in a full Leadership Theatre on Day Two of the Congress. He explained that professionals acting professionally and putting patients’ needs at the centre of everything they do makes the most significant and positive contribution to quality improvements in pharmacy and in managing risks to patients. The regulator therefore wants to have a conversation with

patients, the users of pharmacy services and pharmacy professionals about what patientcentred professionalism means in practice. Mr Rudkin explained that the outcomes of this conversation will inform the GPhC’s future regulatory work, and in particular the review of its standards of conduct, ethics and performance. Mr Rudkin then asked delegates to share their views and experiences of patient-centred professionalism. Key themes emerging from the discussion included the importance of having conversations with patients about their treatment and care and the need for further training to give pharmacy professionals the skills needed to provide patient-centred care. Visitors to the GPhC’s stand at the Congress were also encouraged to join the discussion. Over 100 delegates shared their views through filming vox pops, writing their comments on postcards and posting on social media using the hashtag #professionalism4patients. The regulation of education and training of pharmacy professionals, and their continuing fitness to practice, will be key activities for the pharmacy regulator over the next three years, a packed strategy and policy forum was told on Day One of the Clinical Pharmacy Congress. Damian Day, head of education for pharmacy regulator the General Pharmaceutical Council, told the audience that quality outcomes would underpin the planned changes to pharmacy regulation.

Looking first at continuing fitness to practice (CFtP) Mr Day said that based on current thinking, by 2018 the profession could have a new system. This would be based around a revamp of the current Continuing Professional Development (CPD) processes, peer review, and ‘other’ evidence. “On its own CPD is not enough”, he told the audience, explaining that the current system of CPD would be revamped to place more emphasis on quality rather than quantity, and that this would be supplemented by a new formal peer review, for example, through appraisal, and feedback from service users. This year would be spent researching and testing new models of CFtP, 2016/17 would be spent on pilot and evaluation, followed by consultation and eventual roll out in 2017/18, he said. In the second part of his presentation, Mr Day outlined the regulator’s plans to overhaul pharmacy education and training standards.

He said the aim would be to embed outcomes - for patients and the NHS – at the centre of the education and training of pharmacists, technicians and support staff. Explaining the changes, he said that core learning outcomes – valid across Britain to ensure workforce mobility – would be supplemented with variations in the three countries of Great Britain to accommodate different national health policies and patients’ needs. Core pharmacist learning outcomes would produce pharmacists who were professional, with strong science, research, clinical, leadership and management skills, he pledged, noting that this would demand “conversations” between universities and pre-registration training providers. He also told Congress delegates that the GPhC’s education and training work programme would cover the whole pharmacy team, including support staff. He said a green paper, setting out the direction of travel for the new standards of education and training, would be published in mid-2015, followed by a consultation, and hopefully, the introduction of new standards by late 2016.

Will 2015 be your year of the biosimilar? They may be complex, expensive and subject to intense pharmacovigilance, but by 2018 biological medicines will be everyday treatments, Warwick Smith, director-general of the British Generics Manufacturers’ Association, and Dr Virginia Acha, executive director of research, medical and innovation at the Association of the British Pharmaceutical Industry said in a strategy and policy forum presentation entitled: 2015: The year of the biosimilar. Despite offering cost savings in the region of 20-30%, as well as choice and competition in the market, prescribers can be reluctant to order biosimilars, the speakers admitted. But prescribers can be confident that the products are subject to regulation and licensing that supports their comparison to the originator product. The speakers said: “Prescribers should look at the scientific evidence and regulatory support for these products, consider the experience in use and try it for themselves in a controlled way.” They said that education and understanding, experience and use, sustainable pricing and rational decision-making would lead to clinical, economic and patient benefits.

Making the Congress keynote speech to a packed theatre hall, Dr Warner described NHS policy in England, with its shift to prevention and advocacy of new models of care, as a “real opportunity for pharmacy”. He added: “The Five Year Forward View sets out three challenges for pharmacy to address: the need for a radical upgrade in preventative care; new models of care; the need to achieve efficiency savings. He urged pharmacists: “Go back to your trust and challenge your chief pharmacist to let you see your trust’s performance against clinical benchmarks and use them to improve your practice.” He also told the audience of NHS England’s plan to extend the number of pharmacists working in general practice. In this new model, practice pharmacists would be much more visible to patients, and would have much more influence. He said: “With their unique offering pharmacists will be seen as equals to the GP. The point is that, now, all bets are off. New models of care are on the table and CCGs are looking to do things better. But this has to be driven from the bottom up – so speak to CCGs and local decision makers and push the boundaries of what pharmacy can do.”

CPPE is funded by Health Education England to provide CPD opportunities for all pharmacists and pharmacy technicians providing NHS services in England. CPPE was established in 1991 to address inconsistent delivery of learning, both in quantity and quality, to community pharmacists across England. We were also expected to enable greater provision through open learning, alongside face-to-face courses, and achieve economies of scale while maintaining local sensitivity through our network of local tutors. In 2006, 15 years after CPPE was established, the Department of Health (DH) embarked on a thorough review of the learning opportunities provided by CPPE. The evaluation found that CPPE is an effective provider of high quality learning resources to pharmacists and pharmacy technicians across England and it confirmed CPPE as a centre of excellence in what we do.
