In tune with other community pharmacists who support the Conservative manifesto for health, Pharmacy Voice has pledged to call on the new Government to be true to its pre-election promises and place pharmacy at the centre of the wellbeing agenda. Visitors to this year’s Pharmacy Show, taking place at the NEC, Birmingham, on October 18-19 will no doubt relish the opportunity to discuss just how soon the new Government’s health administration intends to turn pre-election words into action. Now in its 9th year, the Pharmacy Show has become the UK’s largest pharmacy networking and training event; with its combination of free continued professional development (CPD),

It is, perhaps, indicative of the importance of the NHS as an election issue – even to a Government with such a decisive mandate – that the reappointment of Jeremy Hunt as health secretary was one of the last senior Cabinet posts to be announced in the reshuffle. In this post since 2012, Hunt for the first time finds himself across the Dispatch Boxes from Andy Burnham, his opposition shadow. But it is unlikely that his continuing role as the health chief will offer much advantage: Burnham is himself no stranger to health matters; since 2006 he has made four appearances in the health department, including as health secretary to Blair 2009-10.

clinical and business training and opinion leadership, the event has become a must-attend event in the pharmacy calendar. A signature event stream for the Pharmacy Show is the Keynote Theatre where top health officials, opinion leaders and professional pundits cover the most important issues of the day; topics being finalised for this year’s show will include the future of community pharmacy, integrated care, fitness to practice, Vanguard sites and funding – all vital topics for the new Conservative government to get right. In their pre-election manifesto, the Conservatives promised the NHS an extra £8 billion a year in funding by 2020. Further support was also given to the Five Year Forward View (5YFV), which was published in October 2014 and which sets out the direction of travel for primary care and pharmacy in England. Scottish and Welsh pharmacy staff attending the Pharmacy Show will also want to see progress on the pre-election pledges to provide the Scottish Parliament with what the Conservatives describe in their manifesto as “one of the most extensive packages of tax and spending powers of any devolved legislature in the world”. Welsh pharmacists and their teams will also be looking for clarity on the manifesto’s stated intention to pass more powers to the Welsh Assembly as part of an all-new Wales devolution settlement.

Changing customers, changing expectations Do you know what expectations your patients have when they come into your pharmacy? Do you know what it means to be a professional and act professionally? To help pharmacy teams answer these questions, the General Pharmaceutical Council has launched a ‘national conversation’ on patient-centred professionalism. In the conversation, patients, the public and pharmacy professionals will be asked what patient-centred professionalism means to them. The outcomes of this conversation will inform the GPhC’s future regulatory work, and in particular the forthcoming review of its standards of conduct, ethics and performance. A discussion paper, open to views until June 26, asks a number of questions, including: • What characteristics does someone who is professional and patient-centred demonstrate? • What are the barriers and enablers to patient-centred professionalism? • Which examples of professionalism best illustrate what patient-centred professionalism is all about? In line with Government expectations for pharmacy – as outlined by initiatives such as the Five Year Forward View in England, Prescription for Delivery in Scotland and the Welsh Assembly’s 21st Century Healthcare programme – the pharmacy regulator has set out its view that it has “never been more important to have an honest and open conversation about what it means to be a pharmacy professional today and in the future”. Duncan Rudkin, GPhC chief executive and registrar, explained: “Pharmacy professionals’ roles and their contributions to public health are growing, as are the public’s expectations, and there is every sign that this will continue for many years to come. Speaking to pharmacists at the recent Clinical Pharmacy Congress, a networking and educational event organised by the Pharmacy Show event team, clinical pharmacy personnel were told that that the regulation of education and training of pharmacy personnel, and their continuing fitness to practice will be key activities for the pharmacy regulator over the next three years. According to a survey by the GPhC, the public has expressed some concerns over their trust in pharmacies, and not every person using a pharmacy feels that they receive the information and advice or respect that they deserve. Mr Rudkin said: “Responses to this paper will help us define the essential features of patient-centred professional practice in pharmacy. This will, in turn, help us draft revised standards.”*

...page 1 continued Headline election pledges Pre-election pledges which include that British people will have seven days a week to access GP and hospital care by 2020 demand change in the way the NHS is currently organised. Given the backing of the election manifesto, all eyes are now on the Five Year Forward View and there is an expectation of delivery as per the outline published by NHS chief executive Simon Stevens. Among the plans for NHS reform mooted by the NHS England chief are new care delivery options, such as the proposed new care model of Multispecialty Community Providers (MCPs). This sets out a model of care built on the strength of ‘expert generalists’, but with proactively targeted and intensive services for registered patients with complex ongoing needs. Examples will include the frail elderly or those with chronic conditions, and will see healthcare professionals working much more intensively with these patients. According to Stevens, the pivot for achieving this vision will be a new model of multidisciplinary community-based care, and where the local situation demands it, scaled-up general practice. Pharmacists are specifically mentioned in the 5YFV and are seen as playing a “far greater” role either as part of the community or expanded practice-based team, or from a base in the current network of community pharmacies, where their role will centre on reducing unnecessary GP appointments for minor ailments. It is also possible that pharmacists are in the frame to take a leading role in delivering pre-election pledges on public health, which include action to reduce childhood obesity, to implement a world-leading national, evidence-based diabetes prevention programme, to reduce the risk of lifestyle-related cancers and to offer support for people who are struggling with addictions. With its pre-existing service specifications and experience, pharmacy also seems likely to be in with a good shout to take a lead on delivering on promises to improve the care of people with long-term conditions, as well as to speed up the earlier detection of cancer. The Government’s promise to develop “meaningful care plans” for people with dementia, and their families also suggests potential input from pharmacy teams. With so much on the political agenda, many will wonder whether the budget of £8bn will be enough to cover these plans and, given the continuing deficit, actually where this money will come from? The Manifesto details plans to harvest £30 billion in fiscal consolidation over the next two years, and explains that this is to be achieved through departmental savings (£13bn), welfare savings (£12bn) and attacks on tax evasion (£5 bn). A key question will be: Where else will the Treasury look if its targets do not yield their promised returns? Pharmacy staff attending this year’s Pharmacy Show may be among the first to find out.

Hull pharmacy wins Flu fighter carers award The team at Newland Community Pharmacy in Hull, one of the top-performing pharmacies in the Carer Friendly Pharmacy pilot, have had their work recognised by the Flu fighters awards. The annual ceremony celebrating the success of local flu fighter campaigns introduced a new award this year: the Flu fighter carers award. This award was added to highlight how teams have used the newly available resources for carers within their organisation and Newland pharmacy’s team managed to increase uptake of flu vaccinations by an impressive 150% compared to last year. So how did they do it? Newland Community Pharmacy had already been accredited as a Healthy Living Pharmacy (HLP) and provided a flu vaccination service, so they were already involved in several initiatives to support their patients. Then, after deciding to get involved with the Carer Friendly Pharmacy pilot (run by Carers Trust and PSNC), it seemed only natural for them to link together their support for carers with signposting those carers into the additional services the pharmacy team could provide there and then, such as flu vaccinations. A ‘flu helper’ was appointed so someone on the team had the specific role of discussing the benefits of flu vaccination with eligible patients who came into the pharmacy. Carers in particular are short of time, so many patients commented on how much more convenient it was to visit the pharmacy rather than needing to find time to book and attend GP appointments for a vaccination. The whole pharmacy team are trained in identifying and approaching possible carers and two designated ‘carer champions’ are on hand to talk to carers about how they are coping, the types of support them may need, and raising awareness of the resources that are available to them. Why should other pharmacies get involved in supporting carers? After getting in touch with the local carer service she came into the pharmacy with a big smile and said that she now has a plan in place and it has taken a big weight off her shoulders and is now looking at going to their carers coffee mornings so she doesn’t feel as isolated.”
1. Yvette thinks pharmacies should be used to help signpost members of the public more. “We are the front line to the community and have a wide knowledge on a medical and person level. Almost all pharmacies have a consultation room and most are active in services so have the experience to collect information and refer to the appropriate organisation.”
3. Yvette tells us she recently attended a ‘Partners in Health’ course which brings carers together with people from across the health sector and “when I heard about the Carer Friendly Pharmacy pilot I was excited to be part of it.”
2. Yvette feels a sense of accomplishment each time a carer returns to the pharmacy after being signposted to a useful service or resource and is glad that her team has now found that those patients will come in and freely talk about their situation.

Pharmacy regulator unveils three-year regulation plan The regulation of education and training of pharmacy personnel, and their continuing fitness to practice will be key activities for the pharmacy regulator over the next three years, a packed Strategy and Policy Forum was told on Day One of the Clinical Pharmacy Congress. The regulation of education and training of pharmacy personnel, and their continuing fitness to practice will be key activities for the pharmacy regulator over the next three years, a packed Strategy and Policy Forum was told on Day One of the Clinical Pharmacy Congress. Damian Day, Head of Education and Quality Assurance for pharmacy regulator the General Pharmaceutical Council, told the audience that quality outcomes would underpin the planned changes to pharmacy regulation. Looking first at revalidation or continuing fitness to practice (CFtP) Mr Day said that based on current thinking by 2018 the profession could have a new system. This would be based around a revamp of the current Continuing Professional Development (CPD) processes, peer review, and ‘other’ evidence. “On its own CPD is not enough”, he told the audience, explaining that the current system of CPD would be revamped to place more emphasis on quality rather than quantity, and that this would be supplemented by a new formal peer review, for example, through appraisal, and feedback from service users. This year would be spent researching and testing new models of CFtP, 2016/17 would be spent on pilot and evaluation, followed by consultation
and eventual roll out in 2017/18, he said. In the second part of his presentation, Mr Day outlined the regulator’s plans to overhaul pharmacy education and training standards. He said the aim would be to embed outcomes - for patients and the NHS – at the centre of the education and training of pharmacists, technicians and support staff. Explaining the changes, he said that core learning outcomes – valid across Britain to ensure workforce mobility – would be supplemented with regional variations to accommodate different regional health policies and patients’ needs. Core pharmacist learning outcomes would produce pharmacists who were professional, with strong science, research, clinical, leadership and management skills, he pledged, noting that this would demand “conversations” between universities and preregistration training providers. He also told Congress delegates that the GPhC’s education and training work programme could also be extended to include new support staff functions. He said a green paper, setting out the direction of travel for the new standards, would be published in mid-2015, followed by a consultation, and hopefully, the introduction of new standards by late 2016.

Pharmacy teams share their experiences of repeat dispensing
NHS England lays down the medicines optimisation challenge
Following the introduction of the requirement to advise appropriate patients about the benefits of the repeat dispensing service in March, PSNC has spoken to Lorna Harwood, the pharmacist at Langho Pharmacy in Blackburn, about how she has been promoting the service.
Is medicines optimisation really a routine part of pharmacy practice? Do we really look at the evidence-base for every decision, and do we always strive to improve the patient experience even when it seems “too hard”? These were the challenges posed to the Clinical Pharmacy Congress by Dr Bruce Warner, NHS England’s deputy chief pharmaceutical officer.
2. What if it’s not right for my patients? Lorna’s team were having problems with one couple who just couldn’t get their heads around it, but she learnt a useful lesson from this: “you have to be selective on patients you try it with.” 3. What if something goes wrong? Lorna recalls one GP who kept trying to change an electronic repeatable prescription
which caused a bit of hassle, but her team were quick to pick this up and explained how the process works to him. This highlights the importance of GP engagement and involvement in the repeat dispensing service: good communication between community pharmacy teams and GP practice staff is essential.
He also told the audience of NHS England’s plan to extend the number of pharmacists working in general practice. In this new model, practice pharmacists would be much more visible to patients, and would have much more influence. He said: “With their unique offering pharmacists will be seen as equals to the GP. The point is that, now, all bets are off. New models of care are on the table and CCGs are looking to do things better. But this has to be driven from the bottom up – so speak to CCGs and local decision makers and push the boundaries of what pharmacy can do.”

Making the Congress keynote speech to a packed theatre hall, Dr Warner described NHS policy in England, with its shift to prevention and advocacy of new models of care, as a “real opportunity for pharmacy”. He added: “The Five Year Forward View sets out three challenges for pharmacy to address: the need for a radical upgrade in preventative care; new models of care; the need to achieve efficiency savings. He urged pharmacists: “Go back to your trust and challenge your chief pharmacist to let you see your trust’s performance against clinical benchmarks and use them to improve your practice.”
