Controversy mounts over new-style premises inspections As increasing numbers of pharmacies find themselves on the receiving end of a new-style premises inspection by the General Pharmaceutical Council (GPhC), controversy is mounting over the ‘prototype’ inspection processes they are being subjected to, and the ratings or ’labels’ they are being given. Although a finalised inspection regime and standards are not expected to come into force this year, pharmacists are already mulling over the implications of the ‘poor’, ‘satisfactory’, ‘good’ and ’excellent’ labels now being awarded by the GPhC’s pharmacy inspection team. This year’s Pharmacy Show, taking place at the NEC in Birmingham on October 5-6, 2014 will provide the perfect venue for pharmacists and their teams to swap first-hand experiences of the inspections, and share inspection survival tips with their colleagues from the community pharmacy world – particularly given the GPhC’s stated ultimate intention to make public the inspection reports it produces. Much like a schools’ league table, this will enable the public to see how pharmacies in their area have been assessed.

Framework Guidance Although the GPhC’s inspectors say that their inspections since November, 2013 (when the new-style inspections were first conducted) have identified only minor issues, it is clear that the new inspections demand a much more testing and thorough safety check of pharmacy premises and processes. Currently, pharmacies and their whole teams must undergo an inspection without the benefit of knowing how the inspectors might arrive at a rating of ‘excellent’: decisionmaking framework guidance, used by the inspectors to ensure consistency, only covers the scenarios of satisfactory and good. Guidance is also still to be published on distance selling by pharmacies (including internet supply) and on the supply of Pharmacy medicines, and the GPhC says the former guidance may not be forthcoming until November at the earliest, once further research into this area is complete. Guidance on the safe supply of Pharmacy medicines will not be published until there has been consultation on the changes to the medicines legislation that will enable the GPhC to enforce the standards for registered pharmacies. It is currently far from clear whether this will take place before the end of 2014.

Culture Change The Independent Pharmacy Federation (IPF) has collated feedback from members receiving an inspection and its chairman Fin McCaul says that the new process does require “a mindset and culture change”. But he says it is also Fin McCaul something that pharmacy must learn to embrace. He says: “Pharmacists don’t know what they don’t know – and that needs to be pointed out. Pharmacy is beginning to change phenomenally and this is underpinning that change.”

What the GPhC says Excellent: To be considered as excellent a pharmacy will need to not only meet all the standards consistently well but also demonstrate innovation in the delivery of pharmacy services with clear positive health outcomes for its patients. This is effectively a ‘good +’ pharmacy. It is good and in addition demonstrates excellence. Good: To be considered as ‘good’ a pharmacy will need to be consistently good across the standards and demonstrate some positive outcomes for patients. A pharmacy therefore cannot be good if it fails any of the individual standards. A good pharmacy is likely to conduct continuous and systematic reviews of its staff, their skills, operating procedures, records, risks (particularly those relating to medicines management and the premises), and patient needs. It will be both proactive and responsive to changing needs and demands and will continually improve the quality and safety of its services to patients. This will be facilitated by a working culture within the pharmacy that is patientfocused and which actively uses the views of patients and staff to improve services. Satisfactory: A satisfactory pharmacy will meet the vast majority of individual standards. Any standards not met will only have a low risk of harm to patients/public safety, because they are unlikely to occur and/or have a relatively low impact. They will be minor in nature, even when taken altogether. It is highly likely that most pharmacies will fall within this category. This is because at the lower end of the performance scale there will be pharmacies that are not meeting a small number of individual standards but which taken together are not sufficient to warrant a label of poor; whilst at the upper end of the satisfactory scale there will be pharmacies that are good for a number of individual standards, but not consistently across all standards. Poor: A poor pharmacy is one that has not met the satisfactory standard overall. It is likely to have a range of concerns and/or standards not met either within one principle, or more often than not, across a number of principles. There may be systemic weaknesses or a collection of aggregated concerns, such as poor record keeping, a lack of monitoring and review arrangements and unhygienic conditions. A poor pharmacy is also likely to present an unacceptable risk of harm to patients and the public. This means the risk is likely to occur and/ or will have moderate to high impact. Face to face wellbeing centres – the future for pharmacy The Face to Face Wellbeing Centres project announced in Natural Pharmacy Magazine has caught the imagination of pharmacies across the country. Right now, 12 i n d e p e n d e n t pharmacies are providing extended primary healthcare in their communities by working with a British Association for Applied Nutrition and Nutritional Therapy (BANT). The pilot scheme is under way across the country to show how pharmacists’ contractual obligations to provide healthy lifestyle advice to patients can be met. The Face to Face Wellbeing Centres project has been devised by Babi Chana, chief feature writer for Natural Pharmacy Magazine, and draws on her 25 years of experience in both pharmacy and nutritional science. It brings together the nationwide membership of the British Association for Applied Nutrition and Nutritional Therapy (BANT) and the publisher of Natural Pharmacy Magazine, JHN Productions Ltd, to form an innovative partnership that will provide financial and services benefits to participating pharmacies. Announced in May, the scheme will be presented to delegates attending The Pharmacy Show in October, both at the Natural Pharmacy Magazine stand (PA19) and in the Pharmacy Business Accelerator venue. Babi Chana and representatives from the pilot scheme will share with delegates their experiences of how they have made this scheme a success in terms of revenue and customer satisfaction, and how they can enrol their own pharmacy.

The emphasis is on making individual nutritional recommendations accessible in a way that they haven’t been before,” said BANT director Sarah Green. “At the heart of this project is a desire to promote the growing recognition that healthcare at its best includes acute pharmaceutical treatment alongside long-term diet and lifestyle support. In bringing pharmacists and registered nutritional therapists together we are providing that kind of integration at community level, face to face.

Refit for the future

A rapidly changing healthcare scene is encouraging pharmacists to think about the future. Research by Lloyds Bank shows pharmacists’ confidence has risen and over 80 percent expect to provide more services over the next five years. Krishma adds, ‘A modern clinical image, high impact retail area, four new consultation rooms accommodating a variety of healthcare professionals and numerous services has really driven Eastcote’s success.’

Increasingly pharmacy design is helping to strengthen many independent and multiple pharmacy businesses. Offering an efficient environment that protects the confidentiality of patients and the public who receive pharmacy services is vital in functional pharmacy design. A recent shift has also seen many pharmacies relocating, opening in health centre settings or acquiring neighbouring premises to expand. When two local surgeries merged, moving to a new purposebuilt centre Barrons Chemist had the opportunity to relocate. The previous premises had no consultation space and was not fit for purpose. ‘It made absolute business sense to relocate,’ says owner Mayank Patel. At the time of the acquisition of the previous pharmacy scripts averaged around 2,500 a month and £3,500 counter sales.

After relocating and fitting out, NHS items stood at 7,500 items a month and counter sales had doubled. Mayank believes that the design enhanced functionality and profitability. ‘Within two weeks of opening the pharmacy had carried out more than a 100 NHS flu vaccinations because of a consultation room,’ he says. Eastcote Pharmacy owners Vikash and Krishma Patel acquired a vacant neighbouring unit enabling an expansive refurbishment to treble retail and dispensing areas. Now an open-plan dispensary allows for efficient workflow and script volumes are up by over 2,000 items a month and retail sales have increased by 15 per cent.

NHS England pledges to test SCR access by pharmacists Six years after it was first discussed, pharmacist access to the summary care record (SCR) is taking its first steps towards reality. Following an announcement by health secretary Jeremy Hunt, NHS England has pledged to progress a pilot to test community pharmacy access to the summary care record. In a letter sent in November, 2013, Mr Hunt told pharmacists that he was “aware of the important role that pharmacists play in supporting the rest of the healthcare system” and he said he was “keen” to explore how this role could be developed through electronic record sharing. He added: “I think it is important that the Department, in partnership with NHS England, looks strategically at how pharmacy can support the rest of the healthcare system, in the context of working towards more integrated care and a paperless NHS by 2018. In particular I would like to see if, when a patient gives permission, it would be possible for a pharmacist to access a GP record in order to give the best possible advice.”

Access to Records The call has been welcomed by the Royal Pharmaceutical Society (RPS), whose Pharmacy IM&T Strategy group wants all registered pharmacy professionals to have full access (read and write) to the patient health record. The group believes that information is key to reducing medicine errors, improving medicines adherence and delivering safe and more effective care to patients, but it says: “There are currently very few community pharmacies that have access to the SCR. Those that do generally have a direct IT link from a local GP practice; a system wide approach is lacking.” The RPS group maintains that access to the SCR facilitates out of hours care, potentially reducing avoidable visits to emergency care services. In a pilot in a community pharmacy in Sheffield, 95% of SCR views occurred outside GP practice opening hours and that the majority of views were to assist in the delivery of medicines optimisation services, ensuring patients get the most from their medicines. In all cases the patient consented to the pharmacist viewing their record. Ash Soni, vice-chair of the RPS English Board, said: “Access to the SCR is essential for the development of effective clinical services in community pharmacy and makes a real difference to the quality of patient care we can provide. “It’s especially important when a patient needs emergency or unplanned care, as well as when optimising the use of medicines. The rollout of the SCR programme needs to happen as soon as possible.”

According to the RPS, as well as reducing unplanned admissions, pharmacist access to the SCR would confer other advantages including: • More robust records: Currently a GP is limited in their ability to know if a patient is taking a medicine that they have been prescribed, or even if that medicine has been dispensed. With the roll out of a variety of services such as the chronic medication service in Scotland, medicines use reviews (MURs) and the new medicine services in England and MURs and the discharge medication service in Wales more information on pharmaceutical care issues is being recorded and stored in community pharmacy patient medication record systems. With SCR access, this could be transferred back to the GP as required, but currently there is no link between the pharmacy record and the GP record. • Multiplicity of prescribers and providers of care: There are now many non-medical prescribers including pharmacist prescribers operating in Britain and consequently at any time a number of different practitioners may be prescribing for a single patient. Access to and input into the electronic patient record, providing an up to date overview of a patient’s clinical condition, treatment, tests, drug history, allergies and adverse reactions is important for patient safety and for the provision of efficient patient centred care. • Transfer of information across care settings: Community pharmacists that have access to hospital discharge summaries are able to check that medication changes recommended in secondary care have occurred and are safe and effective for the patient. In extreme cases errors and delays in information transfer can result in re-admission to hospital. • Participation in research

Care Act In May of this year, the Care Bill received Royal Assent and became the Care Act and for the first time made clear a statutory basis for sharing information that will enable the tracking of patient outcomes across health and care services. The new law means that a person’s data can only be shared and analysed when there is a benefit to healthcare, never for other purposes, and that all uses will be scrutinised with full transparency by an independent statutory body. In addition, there will now be a legal basis for people to stop their data being shared if they wish to. Information sharing within the NHS in England is now being managed by the care data programme, and in the autumn there will be a phased implementation of the IT sharing plans in between 100-500 GP practices. The pilot aims to refine and test the best ways of supporting GPs to ensure patients are informed of the purposes of data sharing, implementing safeguards and enabling patient choice. The SCR was one of the key projects of the National Programme for IT. In 2008, the Department of Health first began investigating the possibility of giving community pharmacists access to the SCR. It later launched a pilot programme. However, while the pilot in NHS Bradford and Airedale was well received, the trial was cancelled in 2011 after two reviews of the SCR were published and the government announced that the by-now much delayed project would go-ahead with a focus on out-of-hours care.

Pharmacy commissioners struggle with financial woes CCGs and local authorities are struggling under the pressure of achieving public sector savings, two reports have shown. A report looking at local authority finances also reveals that two thirds of council leaders expect many local authorities to end up in financial crisis next year. The Local State We’re In report, published by PriceWaterhouseCooper (PwC), revealed nine out of 10 council leaders and chief executives believe that some local authorities will get into serious financial difficulty in the next five years, and that eight out of 10 will fail to deliver essential services.

The report also highlighted a significant decline in leaders’ confidence in protecting front line services. Although the funding for pharmacy public health services is currently ringfenced, experts believe that this may not be a permanent arrangement. Jonathan House, from the PwC local government team, said: “It’s becoming increasingly clear that two-tier local government is unsustainable in its current form.”

In its 2013-14 Consolidated Financial Report NHS England has warned of “even more challenging years ahead” as it revealed that twelve CCGs reported unplanned deficits at the end of 2013-14. Overall, at the end of 2013-14 19 CCGs were in deficit as a result of disaggregation of PCT budgets, activity pressures, and delivery of QIPP savings. The report notes that £2,015m of savings through QIPP were planned for 2013/14 but that only 85 per cent of £1,722m was actually delivered.

Small scale automation

Pharmacy automation was released in the UK around 17 years ago, originally designed as a solution for mid to large pharmacies. Now that a large proportion of these larger pharmacies have automated, ARX has been working to develop systems that can be used effectively in smaller community pharmacies. This became a reality with the release of our plug and play version of the Vmax robot: the Smart. The Smart was designed to be a cheaper, simpler and smaller automation solution, so that all pharmacies can realise the benefits of automation whilst providing patients with the very highest level of care and attention. This universal solution can now improve patient care by reducing picking errors and waiting times, as well as improving pharmacy efficiency by freeing up staff time and improving the overall service patients receive within the pharmacy. By introducing this machine we have given pharmacies the chance to become more efficient whilst improving patient care, without a huge investment. The future of all industries, not just pharmacy, is customer convenience. Customers and patients want services to be available to them when they need it. With this in mind, around 12 months ago, we released a new system allowing

patients to have access to pharmacy services 24 hours a day, the Pharmaself. The Pharmaself allows patients to collect their predispensed prescriptions at their own convenience, 24 hours a day, 7 days a week, along with having the option to buy other retail products. It also gives pharmacists the ability to provide their patients with medical advice via a webcam link. ARX sees its role as working with pharmacists and pharmacy owners to design technologies in line with what is required. A great deal of pharmacists are incredibly innovative, continuously pushing the boundaries of technology, to increase their service offering and enhance their businesses.

Dementia: Pharmacy has a role to play One in four people will suffer from mental health problems at some point in their life, and that makes it an enormous market for community pharmacy to consider. Dr Dave Branford, chairman of the RPS Pharmacy Board for England, and former chief clinical pharmacist, Derbyshire Healthcare NHS Foundation Trust, spoke at the Pharmacy Show last year in Birmingham in a presentation entitled: Dementia: what it means for pharmacy. He told the audience in the 2013 Show’s Clinical Theatre that the Prime Minister’s Challenge on Dementia would see English cities, villages and towns sign up to be more dementiafriendly by 2015. He said that the Royal Pharmaceutical Society is now considering how pharmacists can support patients with dementia and their carers in this initiative. So far, the suggestions include: • Providing evidence for activity that helps carers and sufferers, perhaps, using the HLP model • Achieving dementia-friendly accreditation • Establishing dementia champions and alliances • Public education on dementia. He told delegates: “You can have a huge impact just by helping people to understand how to take their medicines, or to help them with adherence. It doesn’t have to be complicated. If we don’t take on challenges like this, we will be ignored as a profession. Taking part in the Dementia Challenge - hopefully, this moves us along the clinical role.” The topic of Dementia will be a key focus again at this year’s show, with full details being released in the next few months.

RPS improves public health support New Professional Standards for Public Health Practice for Pharmacy aim to improve the standard of public health advice and support given by community pharmacies, for the benefit of patients, pharmacists and commissioners operating in the new NHS.
