Pharmacy Show challenges pharmacy to raise its game

Labour shadow health minister Lord Philip Hunt used the platform of the biggest ever Pharmacy Show to challenge pharmacy to raise its game. Opening the 2013 Show, being held at the NEC in Birmingham this weekend, the senior minister told pharmacists that they still needed to “pack a punch to make their voice heard”.

NHS England is expected to start redevelopment of the community pharmacy contract in England within weeks, deputy chief pharmaceutical officer for England Clare Howard revealed to the Pharmacy Show in Birmingham at the weekend. In a Pharmacy Show exclusive, Ms Howard said pharmacy teams and their representatives will be asked for their views on the future of community pharmacy within the next few weeks in order to shape a new commissioning framework for community pharmacy. Work on this new framework, which could result in a new contract for community pharmacy, is expected to start in the Spring, 2014. Updating the audience on the work of NHS England and medicines optimisation, the former community pharmacist told the audience that pharmacy could expect more Clare Howard alignment between the community pharmacy and general practice contracts - now both contracts are within the same directorate of NHS England. She revealed that over the next few months, NHS England will be developing a range of measures/indicators on which the new contract framework will be based. She told the audience that the likely features of the new contract will include: patient engagement, improved outcomes, value for money, partnerships, and safety and assurance. She hinted that service, rather than supply would be a key basis for the new contractual framework, and that a ‘friends and family test’ for community pharmacy could also be introduced. When asked if the new contract would include a national Minor Ailments Service, she said: [England has]

a different environment to other countries with the MAS, but we are looking at it. We will have to work with our CCG colleagues, and make sure that [the community pharmacy and general practice] contracts align.” She told community pharmacy that it would have to “mobilise” to meet the objectives of NHS England or else commissioning opportunities would be missed, and she warned it was likely to be a “long journey”. NHS England’s objectives include preventing premature deaths, enhancing the quality of life of people with Long Term Conditions, helping people to recover from ill health and injury and giving people a safe and positive experience of care. Although medicines are specifically mentioned only once in NHS England’s activity domains – in the context of patient safety - Ms Howard told pharmacists that medicines are recognised as implicit in most other domains. As an example, she cited post discharge medicines reconciliation as a way of helping people recover from ill health/injury, as well as medicines adherence support to help reduce premature mortality. She said that alignment with the GPs’ contract, which is currently subject to its own consultation, will be imperative. She told the audience: “Pharmacists have an important role working with other healthcare professionals to optimise medicines. We lead in medicines, but we cannot resolve medicines issues in isolation – you must work with other healthcare professionals and patients.”

Speaking to an audience that is expected to rise to 7000 visitors by the end of the show, Lord Hunt said that he recognised the “huge asset” that pharmacy brings to the health service. But, he warned that this was not the case in the Local Professional Networks or in the CCGs. “I have seen the role of pharmacy develop and be recognised but I am not sure about the rest of the NHS,” he said. He told Show visitors that Labour would acknowledge the potential of the community pharmacy workforce if it is elected into Government at the next election. Over the next 30 years we will see increasing demand for health Lord Philip Hunt services but less people paying for the costs of that demand. He said: “The need for more efficient systems is inescapable. We must realise the potential of community pharmacy.” He also pledged to repeal the Health and Social Care Act, particularly the provisions that enforce competition, and said his Government would improve service integration. “The NHS will be a big feature of debate,” he said.

Over 6,800 pharmacy and allied healthcare professionals (an increase of 10%) attended the UK’s largest CPD-accredited training, education and sourcing event for the UK community and hospital pharmacy industry on September 29th & 30th. Pharmacist delegates attending both days of the conference and exhibition was also up by 15% year-on-year securing The Pharmacy Show’s place as the largest gathering of pharmacy professionals in the UK. The theme of this year’s show was ‘Face up to the Future’ and the profession’s leadership vigorously debated its role in the new NHS structure. The profession was confident and positive about its future to continue its frontline role of managing patient’s medicines and safety, eliminating wastage and making it well-placed to deliver frontline clinical and public health services.

The optimism within the pharmacy profession was demonstrated by the appetite of pharmacies to invest in new technologies, services and products on the exhibition floor where a record 400 trade suppliers reported brisk trading – 94% of the exhibit space has already been booked for 2014.

Howard, told a packed keynote theatre that she will lead a consultation exercise starting within the next few weeks to shape a new commissioning framework for community pharmacy. Lord Philip Hunt, Labour Shadow Health Minister and opposition health spokesman in the House of Lords, commented after delivering the opening keynote address: “Community pharmacy is a great asset; it offers so much and I want to explore how we can build on all the good work that is being done by pharmacists.” Hunt told delegates that pharmacy has an opportunity to play a key role in Labour’s ‘whole person care’ agenda in its manifesto for the next election, but its voice needed to be heard.

There are a lot of challenges facing our industry and it’s increasingly difficult to do the day job, but it’s still exciting to see the direction that pharmacy is going in. If we can move forward and embrace the changes, we can deliver great healthcare,” said Vandna Patel, Superintendent Pharmacist at family-run independent pharmacy chain in south east London. Ash Soni, vice-chairman of the English Pharmacy Board added: “The Pharmacy Show brings all aspects of pharmacy together and so there are lots of people here. It makes it the ideal place to talk to pharmacists about how they can tap into potential income streams in the NHS, expand what they do, provide new services and make changes for the better. It’s a really positive show.”

Meet customers’ needs and enjoy repeat custom

Exhibitors were equally positive after two-days of busy trading on the trade floor. “The Pharmacy Show is the best show of the year for our business. We took more than £200,000 of orders in the first day. Ash Soni We’ve also met new customers and generated lots of new leads. It’s a fantastic show,” said Paul Tobin, Head of Business Development atWaymade.

ARX sees its role as working with pharmacists and pharmacy owners to design technologies in line with what is required. A great deal of pharmacists are incredibly innovative, continuously pushing the boundaries of technology, to increase their service offering and enhance their businesses.

Giving the example of a customer browsing the pain fixture and eventually buying a packet of generic paracetamol tablets, she challenged the audience to come up with a more value-adding approach to the sale.

One in four people will suffer from mental health problems at some point in their life, and that makes it an enormous market for community pharmacy to consider, Dr Dave Branford, chairman of the RPS Pharmacy Board for England, and former chief clinical pharmacist, Derbyshire Healthcare NHS Foundation Trust, told the Pharmacy Show in Birmingham in a presentation entitled: Dementia: what it means for pharmacy. He told the audience in the Show’s Clinical Theatre that the Prime Minister’s Challenge on Dementia would see English cities, villages and towns sign up to be Dave Branford more dementia-friendly by 2015. He said that the Royal Pharmaceutical Society is now considering how pharmacists can support patients with dementia and their carers in this initiative. So far, the suggestions include: Providing evidence for activity that helps carers and sufferers, perhaps, using the HLP model Achieving dementia-friendly accreditation Establishing dementia champions and alliances Public education on dementia.

He told delegates: “You can have a huge impact just by helping people to understand how to take their medicines, or to help them with adherence. It doesn’t have to be complicated. If we don’t take on challenges like this, we will be ignored as a profession. Taking part in the Dementia Challenge - hopefully, this moves us along the clinical role.”

Meet the unique needs of your customers and they will go away and talk about your pharmacy in a positive way, Anita Singh, new business manager at AAH Pharmaceuticals told the Pharmacy Show in her presentation to the OTC Academy. Speaking on the subject of ‘Engaging your customers, growing conversations and delivering positive outcomes for your pharmacy’, she told the audience that today’s customers are quite Internet savvy. She warned that if pharmacies are not up to speed or savvy themselves a golden sales opportunity will walk out of the pharmacy door. She said it is all about asking the right questions and having the right products and services on sale. “Not easy, but worth it!” she said.

Dementia: Pharmacy has a role to play

She said the Which? report findings demand attention from all members of the pharmacy team; from counter assistants who do not make best use of the 2WHAM medicine sales protocols, to pharmacy managers/owners who do not make themselves available to give advice, or who do not offer counter assistants any CPD opportunities. She said pharmacy staff should work together to review the pharmacy medicines sales protocols, collaborating with counter assistants to establish protocols that give customers the best possible support with their medicines taking. Ms Kingsbury told the audience: “Counter assistant training cannot end with 2WHAM. Counter assistants need to know what to do next, after they have supplied the medicine, so they can tell customers what to do next. 90% of coughs

Small scale automation Now that a large proportion of these larger pharmacies have automated, ARX has been working to develop systems that can be used effectively in smaller community pharmacies. This became a reality with the release of our plug and play version of the Vmax robot: the Smart.

She said that counter assistants, who are typically drawn from local communities, and have superb communication skills, a great willingness to help and a thirst to learn. They are, therefore, a great resource for pharmacy. But, time and time again, pharmacy is shown by reports like that from Which? as not Vanessa giving good advice. “The OTC market Kingsbury is significantly growing, and that is good for the NHS, as well as pharmacy. If we can get more people to self-treat, that is fewer people in the GP surgery, which is good for taxpayers, and good for pharmacy income streams. We all need to take advantage of all relevant OTC sales.”

Do you: • Wait for them to bring the product to the counter and sell them the product in the normal way? • Engage them in conversation about the purchase of the till? • Try and speak to them before they pick up the paracetamol and find out what they are looking for? Suggesting that the last option, earliest possible intervention, is the most value-adding approach, she said: “You have to be prepared to ask open questions. Customers won’t come to you - people are shy - that’s why they use the Internet!” Giving advice to drive footfall Ms Singh says pharmacies can use calendar seasons, eg, the cough and cold season, to determine the range in store, and your approach to merchandising and promotion, and use campaigns, such as Stoptober. When customers are in store: • Actively listen – pick up on what the customer has to say to you • Check understanding • Explain the options • Close the sale

They say that more proactive marketing of the pharmacy’s successes – to customers, local healthcare professionals and commissioners – is needed if pharmacy is to prove itself ‘Fit for Purpose’ in the new NHS. Speaking to a packed Show auditorium, Michael Westcott from show organisers CloserStill Media challenged panellists from organisations including the Pharmacists’ Defence Association, the National Pharmacy Association, the Independent Pharmacy Federation, the General Pharmaceutical Council and the English Pharmacy Board. He urged the leaders to find solutions that will position pharmacy for success at a time described as a “critical juncture for pharmacy in a fluid, uncertain landscape”.

Panellists summarised that pharmacy had to become much better at evidencing and selling its professional value to local commissioners. Speaking as vice chairman of the RPS English Pharmacy Board, Ash Soni said: “We know that we are good at what we do - but we have to prove it.” Challenging pharmacists to act, he said: “When we do something, we must think about evaluating it.”

NPA CEO Michael Holden said it was far from clear what the NHS in England was trying to do in pharmacy, and, therefore, it was hard for pharmacy to know what it needed to achieve. He said: “If it’s about optimising the value of medicines, we need a sustainable model to support that”. As an example, he said that self-care could be taken to a whole new level. Pharmacies could also be redesigned to become a hub for social care. “We need to align the business model for our premises, workforce and activities to support this,” he told the audience.

PDA chairman Mark Koziol also urged pharmacies to think about roles beyond the physical limits of their pharmacy premises. He said: “In some way, GPs are a ‘busted flush’” as

Panellists comprising NPA CEO Mike Holden, Locum Voice founder Lindsey Gilpin, and president of the British Pharmaceutical Students Association Katy Parsons were invited to give their thoughts on the key issue facing the pharmacy workforce today.

Should the pharmacy workforce be capped? Stepping up first on the issue of the pharmacy workforce, Katy Parsons said that the debate should really focus on managing the number of students, in line with the changing profile of pharmacy jobs - rather than capping numbers. Explaining her stance, Ms Parsons told a full Show hall, she said: “If we are searching for more roles, we will have more jobs to do, and student numbers should be managed in line with that, but with the number of pharmacy schools almost doubling in recent years, we need to be constantly engaging with students.” Speaking from the audience, Manor Pharmacy owner Graham Phillips said that high numbers of pharmacy graduates could be managed if there were new roles as prescribers and in public health, or if students graduated with the expectation of also seeking careers outside pharmacy practice. He said: “I have been frustrated in the past by the lack of pharmacists. We have all known the ‘corpse with a certificate’. We need to plan around the future: this is going to be very different to the past.”

However, turning the debate to practicalities, IPF chairman Mr McCaul said that pharmacy needed new money and needed it fast, but could it be sure of this coming? Doubtful of this materialising, panellists suggested that GMS funding reallocated to pharmacy for recast services could be a more realistic solution. However, GPs will need to see replacement income before they will release income to CP, Mr Soni warned.

Pharmacies should step up and take a stewardship role in antimicrobial prescribing, an expert panel debate at the Pharmacy Show in Birmingham has said.

Pharmacies must be proactive if they want to retain the USP of the P medicines category, expert panellists and the audience told the Pharmacy Show in Birmingham at the weekend.

Speaking on a panel also comprising Locum Voice founder Lindsey Gilpin, president of the British Pharmaceutical Students Association Katy Parsons, and Numark CEO John D’Arcy, NPA CEO Mike Holden said that pharmacies should be challenging every prescription for antibiotics that they dispense. “As a supplier, we should be asking ourselves: does the prescription comply with guidelines? As this is the biggest public health challenge facing this country and the world, we need to be engaging with prescribers and patients on a regular basis.”

Describing plans by the General Pharmaceutical Council to allow the self-selection of P medicines as “appalling”, the panel discussion urged pharmacists to “make their voice heard”. Speaking from the audience, Manor Pharmacy owner Graham Phillips said: “We need to embrace the P category - embrace it and grab it – instead of open display we should be having meaningful interactions with patients.”

Adding her view, Lindsey Gilpin said: “There is always more than we can do. We say: ‘Finish the course’ but do we ever tell them why? Are we really doing enough to prevent the microbial spread, or are we just relying on antibiotics? Graham Phillips, owner of the Manor Pharmacy Group, said: “Many prescribers tell their patients only to fill the prescription if they think they need them. We can help reinforce this message by telling patients: ‘Take these only if you need them’.”

Lindsey Gilpin said that in her view it was “immoral” that schools of pharmacy were training students that could not become pharmacists, and she called for Universities to be made to take responsibility for funding pre-registration places. “They are the ones benefitting from the funding,” she said. Michael Holden, CEO of the NPA also advised owners that a surplus of pharmacy employees should also not be seen as an opportunity to cut wages. Mr Holden said: “We need to defend the value of the profession.” Locums and locum representatives in the audience, however, suggested that the reality is locum rates as low as £11-12 an hour. Mark Koziol from the Pharmacists’ Defence Association said: “We’ve got to separate commercialism from the pharmacist’s clinical role.”

Panel members, which included Locum Voice founder Lindsey Gilpin, president of the British Pharmaceutical Students Association Katy Parsons, NPA CEO Mike Holden, and panel chairman John, D’Arcy (CEO, Numark) said that consultations, with patients at the centre, must be at the heart of medicines supply now and in the future – a point picked up by the BPSA’s Katy Parsons who said: “Pharmacies will be very different in 10 years’ time, and this could see pharmacists engaging with patients in very different ways”. Innovation must be allowed to flourish, added the NPA’s Mike Holden, although we “must make sure that it is safe and appropriate for the patient, and pharmacists must be empowered to make clinical decisions”. Panellists agreed that as key members of the supply chain, counter assistant education was vital, and Mike Holden urged pharmacists to assess counter assistants’ skills on an ongoing basis – “so we won’t have another Which? report on our hands”, he said. Panellists also felt that pharmacy as a profession could take a proactive stance, and could actively engage in wider discussion about the future of profession, starting with undergraduates in the schools of pharmacy. Lindsey Gilpin said: “There are too many people with vested interests and that holds us back. We need to work together, recognising the workforce.” To achieve this, funding must be aligned to what we want to do, said the NPA’s Mike Holden. “Vested interests will always surface if you have blood, sweat and tears and Pound notes invested in a pharmacy, and you struggle to get a return on your investment.” From the audience, the PDA’s Mark Koziol added: “How do we empower pharmacists? Give them prescribing rights. That will open doors, open eyes and will motivate young pharmacists.”

Alcohol dependence: is community pharmacy missing a trick?

Differentiating your offer is the key to business success, Numark director of commercial operations Raj Nutan told the audience in the Pharmacy Business Accelerator theatre at this year’s Pharmacy Show in Birmingham.

Word about the high profile published medical research findings on blood pressure reduction and performance boosting stamina impact has spread across the sporting world. Already used widely by international rugby teams, premiership soccer teams and elite European cycling teams, the shots are an important new extension to Sports Nutrition and Blood Pressure treatment ranges.

System-buy propositions are inherent to the success of companies like Boeing, said Mr Nutan, which do not just sell engines, they sell product reliability and aftercare to their customers. He said: “Look at your resources, your staff and their relationships with customers. If these can be used in a unique way, combined with a unique product offering that your customers want, you will move all your transactions to a system buy, and that is where the most value lies.”

Just by asking three questions, pharmacies can help reduce levels of problem drinking and improve customers’ health and quality of life, consultant pharmacist Professor Steve Bazire told the Pharmacy’s Show’s Clinical Theatre in a presentation on community pharmacy’s role in alcohol dependence.

Professor Bazire said: ”This is not an easy conversation to have”, but you will be helping to identify those people who might benefit from counselling or further therapeutic interventions. Even a reduction in alcohol consumption will improve health.

The Numark business expert advised pharmacists and staff in the audience that their aim should be to make every transaction a ‘system buy’. He explained that ‘system buys’ offer the highest value to the customer, and should yield the greatest benefit to the pharmacy.

Professor Bazire said that as a result of NHS reorganisation, the issue of alcohol misuse has fallen off the radar.

He explained that dispensing a generic medicine, without any other intervention, should be seen as a ‘commodity buy’. In a commodity buy, there is no added value or unique selling point.

He said that only half of GPs correctly identified alcohol dependence, despite its implication in around 8,000 deaths per year.

However, just by adding the wrap-around of pharmaceutical care, the commodity buy of the dispensed generic can become a ‘service buy’, which has more customer value, he says.

He said: “If you consider societal as well as physical harm, alcohol causes much more harm than ecstasy. It is a health time-bomb waiting to go off.”

Integrated Health and Social Care By Helen Liddell, Head of Medicines Management, Leeds South & East CCG and Committee Member for the Primary Care Pharmacists Association (PCPA) The Nicholson challenge means we need to deliver care in a more cost effective manner. With an increased elderly population and a greater burden of population with long term conditions the drive is to treat more patients out of hospital in settings closer to home. service utilisation by reducing the likelihood of their health deterioration. Computer programmes have been installed to pull out various patient data sets and profile the population –‘population predictive risk profiling’. Multidisciplinary teams discuss a patient case load identified from this system and look at care in a new way across health and adult social services. These teams include District Nurses, Community Matrons, Practice Nurses, GPs and Social Services. Pharmacists also attend team meetings when it is practical. Due to limited resource availability and timing it has not always been possible for all practices to be supported in this way, however, where they have it has proved a valuable experience and some significant interventions have been made.

One shift in policy direction is linking health and social care so that the patient becomes the centre of datasets and services that cross organisation boundaries. Patient treatment is thus intrinsically linked with how they live and whether they are able to take their medicines. Homecare and other patient support services are an essential component of Medicines Optimisation. This article looks at how Medicines Management Services have begun to link into this way of working, with pharmacists being integrated in the primary care health and social care teams giving greater potential for medicines interventions and patient benefit. At Leeds South and East CCG GP practices have been incentivised to identify patients who are at the highest risk of being admitted to hospital and to develop interventions across health and social care settings in order to prevent

Some patients identified through the profiling system were less suited to interventions than others, however medicines interventions were made in around 50% of patients identified and in some cases such as Mrs W below very significant medicines management input has been demonstrated: Mrs W was on 35 medicines. 15 interventions were suggested including: reviewing antipsychotic medicines to help control diabetes, stopping medicines where there was reason recorded (e.g. creams, hypnotics and vitamin and mineral supplements) rationalising treatment for pain and treatment for nausea - suggesting approaches to reduce number of medicines but taking account of medication history, suggesting thyroid test, trial stopping quinine in line with BNF recommendations, stopping sumatriptan as contraindicated in Ischaemic heart disease, checking inhaler technique and considering ACE Inhibitor for heart failure. She ended up on 21 medicines and felt considerably better after the changes had been made.

Ahead of the meeting the practice pharmacist looks up patient records for current medication, drug and clinical history. Suggestions for medicines optimisation are made that ensure harm is minimised and patient outcomes maximised. The pharmacist will discuss this at the multidisciplinary team meeting or if time does not permit attendance a list of interventions is supplied for the team to discuss From our experiences we have learnt that this predictive risk profiling system can be used more efficiently by medicines management teams when also targeting those having diseases associated with high medicines use. We have chosen COPD as our area of focus and this is in line with current CCG priorities. Identifying patients in this way has allowed an even greater number of interventions and outcomes to be realised across patients.
