GENERAL DOCUMENTATION REQUIREMENTS AND EVALUATION AND MANAGEMENT OVERVIEW
Evaluation
and Management Webinar Workshop Series Evaluation and management (E&M) visits account for more than 50 percent of revenue in most primary-care practices. In recent years, Medicare reimbursement for E&M visits across specialties added up to more than 1 percent of the entire federal budget. Accurate, comprehensive, patient-centered documentation and record keeping is a fundamental part of clinical practice. Understanding the principles and guidelines of E&M coding is essential for everyone involved in billing and will ensure each service is reported correctly. The California Medical Association (CMA) is hosting a four-part series of FREE E&M workshops presented by PalmettoGBA. The workshops are 1½ hour each, spread over four-days and are designed for physicians, nonphysician practitioners and other staff involved in the coding and billing process. Attendees will receive a downloadable workbook from the Palmetto’s Provider Outreach and Education team.
This course will cover the general documentation principles including: Signatures, medical record cloning, addendums, illegible documentation, scribes, and medical necessity/ reasonable and necessary. An overview of E/M services, common documentation and coding errors and helpful resources will also be included in this webinar.

KEY COMPONENTS IN SELECTING THE LEVEL OF E/M SERVICES: HISTORY
This course will cover: History of present illness (HPI), review of systems (ROS), past family and social history (PFSH). Documentation reminders, frequently asked questions (FAQs), common documentation and coding errors and helpful resources will also be included in this webinar.
DAY 3:
KEY COMPONENTS IN SELECTING THE LEVEL OF E/M SERVICES: EXAMINATION
This course will cover: 1995 examination guidelines and 1997 examination guidelines. Documentation reminders, frequently asked questions (FAQs), common documentation and coding errors and helpful resources will also be included in this webinar.
DAY 4:
KEY COMPONENTS IN SELECTING THE LEVEL OF E/M SERVICES: MEDICAL DECISION MAKING
This course will cover: Diagnosis/management options, type of data, risk assessment. Documentation reminders, frequently asked questions, common documentation and coding errors and helpful resources will also be included in this webinar.
