FERPA:
Access to My Student’s Records
USI recognizes that families play an important role in student success, and we want to assist you in that role. We also recognize and act in full compliance with the regulations set in accordance with the Family Educational Rights and Privacy Act of 1974 (FERPA), as amended. FERPA, also known as the Buckley Amendment, is a federal law that protects the privacy of student education records. FERPA applies to all schools that receive funds under an applicable program of the Department of Education. ,Understanding USI’s policies may be helpful in discussions with your student. The University expects your student to take full responsibility for his or her education, including the academic policies in the USI Bulletin (bulletin.usi.edu), Class Schedule and the Student Handbook

(USI.edu/deanofstudents/code). Parents and families will find it helpful to acquaint themselves with the policies and procedures in these publications and can view additional information at USI.edu/registrar/ academic-records/privacy.

Common Questions: • Can I access my student’s academic records? • Can I receive my student’s grades or request their transcript? • I pay the bill for my student. Why can’t I see his/her academic records? • My student needs a Verification of Enrollment (for insurance coverage, good student discount, etc.). How can an enrollment verification certificate be obtained? View answers to these questions at USI.edu/registrar/academic-records/ privacy/ferpa-information-for-parents.
Parents may access a student’s education record at USI under the following circumstances: • The student completes a “Permission to Release Education Record Information” form in the Registrar’s Office. This form entitles a parent or authorized person named by the student to obtain specific information upon written request and proof of identity. The student may rescind this form at any time. • The parent establishes the student’s dependent status by presenting the University with the most recently filed tax return showing the student was claimed as a dependent and by also completing the “Request for Confidential Data of a Dependent Child” form in the Registrar’s Office.

Registration, Payment and Financial Assistance A college education is an investment in your student’s future. USI has resources to help you through the financial and registration processes. The Registrar’s Office is responsible for maintaining the accuracy and integrity of academic records at USI. They coordinate many University services including registration, reporting early alerts, nonattendance, progress reports and final grades, class schedule publication and the degree audit system. Additional services include schedule revisions, release of USI transcripts, enrollment verification (for loan deferment, insurance coverage, etc.), undergraduate readmission, various record-keeping functions (change of address, change of major, etc.), application for graduation and conferral of degrees. Student Financial Assistance The Student Financial Assistance Office has a student-focused team that helps students and their families seek, obtain and make the best use of all available resources to help finance the costs of attending USI. In partnership with internal, federal, state and other organizations, team members coordinate the administration of all student financial assistance awarded to ensure equity and consistency in the delivery of funds to students.
Bursar’s Office The Bursar’s Office is responsible for assessing student fees, issuing billing statements for student and vendor accounts, collecting payments made to the University and processing account refunds. Billing statements from USI are accessed through the myUSI portal by selecting the TouchNet icon and logging into the University’s payment gateway. A student can set up authorized user(s) who are able to view billing statements and make payments on the student’s account through TouchNet. The University sends statements on or after the first of each month with a due date at the end of the 100% refund period.

FAFSA Online Complete The 2020–2021 Free Application for Federal Student Aid (FAFSA)
is now available! Complete it online today at fafsa.gov. The state of Indiana FAFSA filing deadline is April 15, 2020. Usage of the IRS Data Retrieval option during the initial filing or when subsequent corrections are made to the FAFSA application is strongly recommended.
for Financial Aid Awards Check Financial aid is calculated once your student is admitted and we
receive the FAFSA. Awards may be viewed via the student’s myUSI. In addition, paper award notices are mailed to new students.
Direct Loan Election Make The Direct Student Loan will be packaged as an offer to eligible
students upon receipt of the FAFSA. Beginning July 1, your student will need to log on to myUSI and accept, decline or reduce the Direct Loan. First time borrowers must complete Direct Loan entrance counseling and must electronically sign the Master Promissory Note online at studentloans.gov.
for Direct Parent PLUS Loan (optional) Apply Beginning July 1, parents who are interested may apply for the Direct Parent PLUS Loan online at studentloans.gov. For more information, visit USI.edu/financial-aid/aid/loans/direct-plus.
for Additional Requirements Check In addition to FAFSA, some students may be required to submit
additional paperwork and/or complete other processes to finalize their financial aid package. Your student should check myUSI to determine what might be required.
for Disbursement Watch Financial aid is paid to students’ billing accounts beginning the middle of the second week of each semester for students whose paperwork/ requirements are complete and processed. It is paid one semester at a time based on the number of credit hours students are enrolled in at the time of each disbursement.

Freshman Year Initiatives
USI has several programs designed for new students. These programs support them as they adjust and become integrated into USI’s culture and student body. Living-Learning Communities At USI, first-year students with similar interests, goals and academic programs can be placed on designated floors within the residence halls. These living-learning communities (LLC) help students create a strong link between their lives in the residence hall and learning experiences in the classroom. Students who reside in an LLC enjoy the same amenities as other residents, yet they benefit from a different living experience. Students on the floor work together to develop a community that relates to a common theme. Themes can be selected from these topics: Global Community, Honors, Business, Education, Liberal Arts, Exploring Majors, STEM, Nursing and Health Professions.

First Year @ USI Newsletter We only have one day for Orientation, but we have so much more information to give your student! First Year @ USI will build from his/her Orientation experience and help your student successfully transition to USI. First Year @ USI is emailed weekly over the summer and during the first six weeks of the fall semester.

Welcome Week
Student Involvement Fair
Welcome Week is your student’s first taste of being a student at USI! It is a week full of activities and events designed to help students meet one another, adjust to new surroundings and start on the right foot in their academic career at USI. All students are expected to attend the Eagle Experience activities, and all new freshmen are required to attend the UNIV 101 program on Saturday.
Attending the Student Involvement Fair, held during the second week of the semester, is a great way for your student to connect with other USI students and student organizations. Encourage your student to find student organizations and activities that are of interest to them.

The Source Newsletter The Source is a great way for your student to learn about events and opportunities on campus. The Source is emailed weekly during the academic year. Students are automatically signed up to receive the newsletter through their student email address, but they can opt out at anytime by clicking unsubscribe.
Emerging Leaders The Emerging Leaders program offers USI freshmen the opportunity to develop leadership skills, meet fellow leaders and contribute to the University community. The program focuses on preparing individuals to succeed as student leaders so that, in subsequent years at USI, they can be active campus leaders.

Tips for Connecting Your Commuter Student to Campus
Commuter students make up a large part of the campus population at USI. Parents who understand the unique challenges of commuter students can help make the college experience better for their student. 20

Encourage your student to stay on campus during the day. The more they stay on campus, the more people and places they have the opportunity to meet and see.

Advise your student to study on campus. USI has study areas all across campus. Computers, desks, printers and private rooms are available, and your student’s studying won’t have to compete with chores, siblings, pets or other at-home distractions.

Promote an on-campus job. Student workers normally work 12 to 15 hours per week, and USI employers respect that school is your student’s Number One priority. Plus, when the University is closed, student workers don’t have to work.

Suggest that your student venture back to campus for evening or weekend events and meetings. Activities Programming Board is an organization that provides entertainment and fun, outside of the classroom, during the day and in the evenings for students.

Encourage involvement. With more than 140 student organizations, it’s easy to find one that fits every student. Your student will feel connected to USI, learn valuable skills, meet other students and faculty and have fun!

Be aware of campus news and events. Get to know more about USI. Check out the website and sign up to receive Parents & Families Association news at USI.edu/familyconnect. If you acknowledge the importance of what’s happening at USI, your student will too. University of Southern Indiana

Public Safety
Security and Parking Departments Public Safety is a 24-hour, seven-daysa-week operation and is comprised of a Dispatch Center, Security and Parking Departments. Services provided by Public Safety include emergency medical care and non-emergency requests such as campus directions, general campus information, vehicle jump starts, student/faculty/staff escort service, tire air-ups, vehicle unlocks and parking assistance.

RAVE Alert System USI offers a free text messaging service called RAVE Alert that provides critical and timely messages to the campus community. These notifications are sent via email, text and voice message in order to provide valuable information about emergencies, severe weather and other incidents impacting the University community. Everyone with a USI email address is automatically enrolled in the RAVE Alert program. Students can opt in to receive text messages, test the RAVE Alert information, or sign up a parent or family member to receive alerts by going to the Eagle Apps section of myUSI. For information on how

Annual Security and Fire Safety Report USI’s Public Safety Department is committed to assisting all members of the University community in providing for their own safety and security. Health & Wellness Services
University Health Center The University Health Center is a Deaconess clinic that provides medical services and health-related information to students, faculty and staff. It is located in the lower level of the Health Professions Center. The goal of the Health Center team is to help students and staff maintain or improve their health, allowing them to reach their highest potential both in and out of the classroom. Services available at the University Health Center include, but are not limited to, acute and minor care, immunizations, allergy shots, laboratory testing, prescriptions (including a limited on-site pharmacy), specialty referrals and physicals.

Office hours are Monday through Friday, 8 a.m. to 4:30 p.m. When the campus is open, but classes are not in session, the Health Center is open Monday through Friday, 9 a.m. to 3 p.m. Services provided at the University Health Center do not replace the need for a primary care physician. The Center’s services may be billed to insurance, billed to the patient or students may enroll in the Office Visit Plan (OVP) for a fee each semester. The OVP is required for students living in campus housing and optional for all other students. The OVP provides unlimited office visits to the University Health Center including a flu vaccine for no additional fee. Prescriptions, procedures, breathing treatments, lab work, STI testing, and radiology services are not covered under the OVP. For OVP enrollment

If your student gets sick when the office is not open, he or she can call the University Health Center RN on call at 812-450-8063 to speak with a nurse. Counseling Center Counseling Center services help students to understand their emotions, overcome problems, make effective decisions and maximize their educational experiences. Appointments are typically 45–50 minutes once a week or every other week. If a student has needs that are beyond the
role and/or scope of services available, an appropriate community referral will be provided. Information shared in counseling is confidential and only disclosed in the rarest of circumstances. All services are supported by tuition and student fees, and students do not pay extra to access counseling. However, students found responsible for certain violations of the Code of Student Behavior may be charged an additional administrative fee. The Counseling Center celebrates all aspects of diversity and seeks to support the development of a healthy University community. The Counseling Center is also available to speak with concerned family and friends about how to assist a student who is in distress and needing support. Recreation, Fitness and Wellness Center The Recreation, Fitness and Wellness Center (RFWC) is a place to get fit, meet other students and have fun. Students can exercise in the fitness center, boulder or rock climb in the climbing center, check out bicycles for the day, play volleyball and basketball on the outdoor or indoor courts or run on the indoor track. A number of group exercise classes are offered, as well as ping-pong, billiards and video games. Intramural and recreational sports and events are also offered. Students may play in intramural sports leagues or one-day activities, giving them the opportunity to compete individually or on a team. Intramurals provide a great way to meet new friends, as well as interact with old ones. Students may choose to take part in Outdoor Adventure Programs, which include camping, rock climbing, canoeing, caving, white water rafting, ice skating,

snow skiing and hiking. Tents, sleeping bags, sleeping pads and lanterns are available for check out as well. Encourage your student to find the complete schedule of events and hours of operation at USI.edu/rfw.
Sexual Violence Prevention and Response Sexual violence is an issue of concern for both students and their families. Although our campus is generally very safe, sometimes assaults do occur at USI, as at any college campus. USI is committed to creating and maintaining an educational environment free of violence in which all members of the USI community are treated with respect and dignity. All forms of gender-based violence, harassment and non-consensual sexual activity are prohibited. Students need to be aware that alcohol is often involved with assaults. Someone who has been drinking may not be able to give

consent for sexual activity. We encourage students to intervene if they think someone is in an unsafe situation, either by direct action or by letting a University official know immediately. We ask that you encourage your student to program the 24-hour Public Safety emergency number 812-492-7777 in his or her cell phone for easy access in case of an emergency.
Substance Use
Any student who has been assaulted is encouraged to report the assault as soon as possible to USI Public Safety, the Dean of Students Office or to the Vanderburgh County Sheriff’s Office. Reporting may be done anonymously through both Public Safety and the Dean of Students. Confidential reporting is available at the USI Counseling Center and the Albion Fellows Bacon Center, a local community domestic violence and sexual assault agency, which can be reached at 812-4247273, 24 hours a day.
All parents should talk to their students about drinking and drug use. There are plenty of alcohol-free activities on campus: movies, athletics, student organizations, intramural and club sports, outdoor adventure trips, music and more. Students who are concerned about their use of substances, or about a friend, can talk to someone at the USI Counseling Center at 812-464-1867. Parents and students can find additional resources at USI.edu/rfw/aod.
Many USI students choose not to drink, and most USI students who do drink tend to do so in a reasonable manner. However, some students do engage in high-risk drinking. USI’s policies prohibit alcohol and drugs on campus, and we need parents to be partners in our efforts to keep students safe.

Academic Resources and Support Services
Academic Skills has everything your student needs to enhance his/her success as a college student. This office offers academic support in a variety of academic areas for all students. Learning assistance programs include: Math, Study Skills, and Subject-Based Tutoring No appointment is needed for most tutoring, although appointments are available with the Learning Assistance Specialist and study skills tutors. For writing assignments, peer writing consultants can provide students with guidance and feedback at any stage of the writing process. If students cannot visit the Writers’ Room (ED 1102), they can email their papers to the Online Writing Lab at owl@usi.edu for feedback.
Supplemental Instruction This academic support program provides regularly scheduled out-of-class review

sessions to all students in a targeted course. Facilitated by an SI leader (a student who has successfully completed the course), the sessions provide an informal environment to review notes, discuss readings and prepare for exams.
Academic Peer Coaching Students may request appointments with academic peer coaches who are also college students themselves. The academic peer coaches can assist students with study skills enhancement, including note-taking and test-taking strategies and successful time management. The academic peer coaches can also assist new students with motivation and overall approach in order to help students make successful transitions to a university setting.

Academic Advising
Career Counseling
Academic advising is an ongoing educational process that connects the student to the University. Faculty and staff academic advisors represent and interpret University policies and procedures to the student and help the student navigate the academic and organizational paths of the institution.
Students who are exploring or rethinking their major and career options are encouraged to utilize University Division Advising’s career counseling services. Through interest assessments and other resources, staff members help students clarify their interests and skills and identify the major and career options that closely match their goals. Appointments are tailored to students’ specific needs and concerns. Students may schedule a career counseling appointment by calling University Division Advising at 812-4651606 or visiting us on the first floor of the Education Center, room 1142.
USI students have certain responsibilities in regard to their academic progress, including: • Being knowledgeable about policies, procedures and requirements published in the USI Bulletin and class schedule • Determining a course of study and satisfying the requirements defined in the USI Bulletin and degree audit • Scheduling and appearing promptly for appointments with an advisor, when necessary • Preparing for an advising session by having a list of questions and courses (and alternatives) • Following through with appropriate action after the advising meeting • Accepting responsibility for the academic decisions to be made
David L. Rice Library Named for USI’s first president, the David L. Rice Library is the go-to spot on campus for students to study, access their online course materials, get items their instructors put on reserve, check out a laptop, iPad, or Galaxy tablet, collaborate with classmates, find a good book to enjoy, charge their electronic devices or recharge themselves with a nap! Parents can relax too, knowing that the library offers many services to help your student succeed, and that she or he can get great advice. Check out USI.edu/library.

Honors Program The Honors Program at USI is designed to offer enhanced opportunities for students who show promise of outstanding academic achievement. Participation in the program exposes students to a wide range of intellectual and academic experiences, provides enriching extracurricular activities, promotes rewarding interaction between students and Honors Program faculty and gives students the opportunity to form lasting friendships with other students committed to academic excellence. The Honors Program Office, located in Orr Center room 1070, has a student lounge designed to provide students with a space to study, meet other students or take a break and enjoy fresh coffee. Student Support Services (SSS) Student Support Services (SSS) is one of eight grant-funded TRIO programs offered through the U.S. Department of Education. At USI, SSS helps motivate 140 eligible students to successfully complete their degree. SSS provides

students with free, comprehensive services through classroom instruction, academic skill development, academic assistance and career development, personal and financial aid counseling, mentoring with upper classmen, one-on-one tutoring and experiences designed to enhance the collegiate experience. To receive assistance, students must be U.S. citizens or permanent residents of the U.S. and: • Meet limited income criteria (receive a Pell Grant); and/or • Be a first-generation college student (neither natural parent residing in the household with the student has a four-year college degree); or • Have a documented learning or physical disability recognized by the American Disabilities Act and be registered with Disability Resources. Students must show an academic need and be willing to take full advantage of all support services offered by the program. Disability Resources (DR) Disability Resources (DR) coordinates services and academic accommodations for USI students with disabilities to ensure equal access to facilities, programs, services and resources at the University. Students with disabilities seeking accommodations (such as extra time on exams, accessible textbooks, note takers, etc.) are encouraged to submit documentation of their disability to DR at least 60 days prior to the date they will need accommodations. Veteran, Military and Family Resource Center The Veteran, Military and Family Resource Center at USI assists veterans and their families as they navigate the educational benefits offered by the Department of Veteran Affairs and the United States Military branches. The Veteran, Military and Family Resource Center is located among many other student service offices and leisure activity locations in the Education Center. Their lounge features a computer lab, specialized counseling, educational programs, workshops and seminars to help veterans complete their degrees and reach their goals successfully. Study Abroad
An important aspect of your student’s education is hands-on career development. The Career Services and Internships office offers a wide range of services for students.
The University offers students the opportunity to learn more about themselves and the global community through study abroad opportunities in over 60 countries. With advance planning, it is possible for students in virtually any major to participate in an overseas program. Programs offer a variety of experiences, from specialized courses taught entirely in English to direct enrollment in a foreign university. Students receive USI credit for the coursework completed during the summer, academic semester or academic year.
The office maintains a Job Locator program and assists students in securing part-time, temporary and summer employment. Eagle Career Launch, an online system exclusively for University students and alumni, lists full-time, internship and parttime job vacancies in the tri-state area and elsewhere. In addition, the Career Services and Internships website provides jobsearch related tools, information on local and regional job fairs and other valuable employment-related resources. The Multicultural Center supports organizations that encourage academic achievement and personal development of its members. Membership is open to all races, cultures and ethnic backgrounds. • The Black Student Union sponsors activities on campus such as the MLK Jr. birthday holiday observance, the Black History Month Celebration, performing arts productions and social activities. • The Hispanic Student Union/ Latinos Unidos (HSU/LU) promotes programs that enhance and increase knowledge and appreciation of cultural diversity such as the Latino(a) Leadership Conference, a Salsa Dance Night and Dia de los Muertos. • The South Asian Union coordinates Indian Culture Night, Global Communities, Diwali Lecture and the International Food Expo. Religious Life Religious Life provides religious guidance and worship opportunities for students, faculty and staff. While our primary goal is to encourage students to stay active in the faith life of their choice while at USI, we also house the Spiritual Diversity Project that aims to educate and coordinate a greater understanding of the diverse religions on campus and within the community. Religious Life offers services and information for multiple denominations. Athletics Nothing screams school pride like cheering for USI’s Screaming Eagles athletic teams! USI is a member of the NCAA Division II and the Great Lakes Valley Conference (GLVC). Student-athletes compete in 17 different varsity sports including basketball, cross country/track, golf, soccer, baseball, softball, tennis and volleyball. Admission to home regular season athletic events is free to USI students. USI invites parents and family members to get involved by becoming members of the USI Varsity Club, USI’s athletic booster club. Performing Arts USI offers students opportunities to share their talent via the performing arts. While majors and minors take numerous classes in acting, dance, technology and design, any student interested in theatre may audition for productions by the USI Theatre Program. Currently, four main stage shows are produced each academic year. During the season, the “Repertory Project” brings members of the Actors Equity Association — the professional union of stage actors and stage managers — to the USI stage. This opportunity allows USI students to work alongside professional actors and with a professional production stage manager on one of the main stage productions

that take place in the USI Performance Center located in the center of campus. A second performance space, the Mallette Studio in the Liberal Arts Center, features independent student productions and second season offerings. USI’s sponsored groups include the Chamber Choir, Women’s Choir, Designed by Grace Gospel Choir and Jazz Band. Students audition for inclusion into these groups. USI Campus Store The USI Campus Store is located on the first floor of University Center West and has textbooks (new, used, digital and rental), art and school supplies, logo wear and golf items. In addition to being an authorized Apple Reseller to students, faculty and staff, the Campus Store provides UPS shipping services and sells stamps. Other services include photocopying, faxing and check cashing. Students can order online or call to have an order shipped. Children’s Learning Center USI operates the Children’s Learning Center with full-time professional care for two-yearolds and part- and full-time care during the day for children ages three to five who do not meet the kindergarten entrance cutoff date. The center is accredited through the National Association for the Education of Young Children (NAEYC), is state licensed, is on Level 4 on Paths to Quality and has five stars on the IDEM 5 Star Program. Childcare is available for a fee to faculty, staff and students. Hours of operation are 7 a.m. to 5 p.m., Monday through Friday when the University is open. Eagle Access Card The Eagle Access Card is USI’s photo identification card (ID) for students, staff and faculty. In addition to being an ID card,
the Eagle Access Card is a convenient, easy and safe way to access products and services without carrying cash. By activating the declining balance feature of the card, students make the ID card a debit card as well. Campus Housing Resources USI offers two options for on-campus living arrangements: campus apartments and residence halls. Both are located within walking distance of the academic buildings for an added degree of convenience. • Campus apartments are available to all students with the exception of students with families. Each apartment comes fully furnished and has a full-sized living room and kitchen. • Residence halls are open to freshmen only. The halls are designed as suites that combine the benefits of traditional residence hall living with popular apartment-style housing and include two bedrooms and two baths with a common living area for four students.
Additionally, on-campus housing offers the following services to our residents: • Professional Residence Life staff are available 24 hours a day/7 days a week. • Resident Assistants live on site in order to serve as a resource for students, provide programs and activities and assist during crisis situations. • Around-the-clock security regularly patrols campus housing — these officers also are first responders in case of medical emergencies. • Shuttle bus service from the apartments to the campus is provided. • On-site laundry facilities that use coins and/or flex money on your student’s Eagle Access Card are available. • 24-hour emergency maintenance is provided. • Programs and activities are provided only to on-campus residents, such as the Eagle’s Night Out, Block Party and Residence Life sponsored programs. • Screaming Eagle TV, an on-campus movie channel, plays first-run blockbuster movies.

Historic New Harmony: Experience, Explore, Create
Since 1985, Historic New Harmony, a department of USI’s Outreach and Engagement, has encouraged cultural and educational programs related to New Harmony’s important historic resources. Historic New Harmony is a unified program of USI and the Indiana State Museum and Historic Sites. Opportunities shared by USI faculty, staff, students and New Harmony include research projects, service learning courses, professional and personal development retreats, New Harmony Theatre summer productions and New Harmony Gallery of Contemporary Art workshops and exhibits. USI nurtures New Harmony as a living laboratory where students, teachers,
