Care homes have the potential to develop a whole range of new services that would enable them to become centres of excellence for the management of dementia and other long term conditions.

This is the ambitious vision that Professor Martin Green, chief executive of the English Community Care Association, will be sharing with attendees at the Health +Care event.

Commissioners may need to make some unpopular decisions about providing more services in independent care homes where nurses are cheaper to employ because employers don’t have to fund expensive public sector pensions. ‘Some healthcare professionals would definitely lose out from this arrangement but we need to remind ourselves that the issue is about how you shift resources around the system so that the citizen wins rather than the professional. The professional’s role is to support the citizen.

To achieve standards of excellence residential care employers need think about investing in their workforce so that it is well trained, highly skilled and properly remunerated. People need to feel that that rather than just doing a job that they have a career. ‘People who work in this sector are brilliant at what they do and we must start to recognise that as a society,’ he says.

Professor Green envisages a future where care homes will provide long-term residential care, reablement services, respite, night and day care services and emergency interventions plus support for carers. This will require commissioners to channel money from the NHS into care homes which could provide support for people with long term conditions more cheaply.

‘The bottom line is there’s going to be no extra funding in this sector so what we have got to do is reapportion what we’ve got,’ he says.

Care home owners also need to move away from relying on block contracts provided by the state. As clients gain greater control of their finances through personal budgets and direct payments they will be able to make more informed choices about the services they require.

The Care Quality Commission is considering appointing a chief inspector of primary care as part of a three year plan to radically change the way it inspects and regulates services. Proposals for this new layer of regulation for GPs were first announced by the Government in its response to the Francis Inquiry into the Mid Staffordshire NHS Foundation Trust scandal. How do you personalise a large care home for people living with dementia without additional staff or money?

The new inspection regime will scrutinise the things that are most important to people such as: are services safe, effective, caring and well led, and do they respond to people’s needs?

The answer is through individual service funds, says Helen Sanderson, chief executive of Helen Sanderson Associates, a consultancy which works with organisations to achieve person centred change.

The strategy also says the CQC will use information and evidence more effectively to predict, identify and respond to services that are failing. This includes listening better to people’s views and experience of care and to care staff to hear about their concerns.

Ms Sanderson, will be giving a presentation to the Health + Care show about a project at Bruce Lodge, a care home in Stockport where they introduced person centred practices and the principles of individual service funds to 43 people with dementia.

The CQC says it will introduce a programme for failing trusts that makes sure immediate action is taken to protect people and deal with the failure. CQC Chief Executive David Behan said: ... one solution. Thank you to our partners

Delegates attending the Health + Care show will be among the first to hear details of a new cross Government partnership framework for integration being developed by the Department of Health (DH).

now starting to show very promising early signs that services can be improved and savings made. Initial findings showcased in a report in published in June last year demonstrate that change is possible, savings can be made and services improved.

Damon Palmer, policy Lead for Health and Social Care Integration, at the DH, who is speaking at the conference, will be explaining how the framework will be used to encourage and promote enthusiasm for integration among those who are leading the way on the ground.

These three local authorities are now looking to further integrate care across their clinical commissioning groups, providers and community services to improve patient and user health outcomes and experience.

“The framework will be a key vehicle for alerting and signposting people to the work that we are involved in. It will be a springboard for our ambitions,” he says. There are already a number of sources of inspiration and good practice beginning to emerge from some of the early pioneers of integrated care. Examples of the numerous changes that need to take effect across an entire locality can be found among the Community Budget pilots. Community Budgets are designed to allow providers of public services to share budgets, improve outcomes for local people and reduce duplication and waste. Mr Palmer points to one particularly successful project - the tri-borough Community Budget pilot where three central London councils Hammersmith and Fulham, Kensington and Chelsea and City of Westminster – have joined forces to protect front-line services by combining funding streams from across their patch. Mr Palmer points out that the DH, and its partners, are committed to encouraging local experimentation: ‘We want to encourage and support those who are working together locally to build innovative models of care, tailored to local needs and circumstances. We know there are still barriers and complexities at national level, so through the national collaboration (between the DH, Monitor, Commissioning Board, Local Government Association, Association of Directors of Adult Social Services) we are considering how best to support local innovation and share learning without getting in the way of innovative service development.’ Announcing the pilots at the end of last year, care services minister Norman Lamb said the DH was keen to enable local ‘experimentation’ rather than dictate how integration should be done. The King’s Fund and the Nuffield Trust have also been doing a considerable amount of research into integration and Mr Palmer says the DH will be keen to highlight the evidence that these organisations reveal including their pilot work with a series of English integrated sites.

The DH is also currently working to put as much material about integrated care as it can on to the Local Government Association’s website called the Knowledge Hub, a professional social network which helps people in local government and across the NHS connect and share online in a secure environment. Mr Palmer says they are keen to support continuous learning and innovation: ‘We want to work in partnership with localities to understand what the problems are and what we can do to support them in terms of helping them to overcome any barriers or obstacles and challenges. ‘What we don’t want is for there to be just isolated pilots while other localities stand by and watch from a distance. We want to enable all localities to embark upon more innovative and integrated working. ‘I hope that delegates coming to the Health + Care conference will find they have the opportunity to take back to their offices different ideas and will be inspired to ask themselves why they are or are not doing different things. ‘Our message is that there is no one model of integrated care, there is no one-size-fits-all blueprint. To some extent it’s about localities finding and developing and implementing the solutions that best meet their local needs.’

The challenges of personalisation and safeguarding Self-directed support (SDS) is a term that conjures up magical opportunities. But while giving clients personalised budgets and more control over their care brings great gains it might also increases potential risk of harm, according to Jill Manthorpe, Professor of Social Work and Director of the Social Care Workforce Research Unit, King’s College London. In a presentation to the Health + Care conference on the challenges of personalisation and safeguarding Professor Manthorpe will discuss the ways in which worries about risks have changed and how new systems are emerging to make the most of SDS while still minimising risk of harm . Adults who are entitled to publicly funded social care are often very frail or have quite profound cognitive disabilities so they may have difficulties managing their own money and may not have anybody around who is able to help them to do it. So a question is could SDS be adding to their difficulties? Vulnerable adults in receipt of SDS might also attract people who are unscrupulous and want to abuse or exploit them. How do professionals manage that risk on behalf of their clients? Professor Manthorpe highlights that nobody

can ever say all services are safe so in challenging financial times health and social care professionals need to ensure systems are developed in which vulnerable people can be sufficiently supported.

‘People talk about these things more - it’s not that long ago that all these issues were swept under the carpet,’ she says.

Another area of concern is around the financial exploitation of vulnerable people entitled to publicly funded care who are unable to complain if they are not getting value for money. Voluntary and other community groups need to know who to contact if they are the first to suspect that things are not going well.

Integrating public health to improve health outcomes integrate the role of the clinical commissioning group to include adult social care, public health commissioning and social determinants of health. The council is also exploring how its leisure services might be brought together with NHS prevention services into integrated wellbeing centres.

The unitary council for some time has been pioneering new models of public health and social care integration to improve the health of its population which is generally worse than the England average.

A key aim is to address the health and wellbeing of those with at least one long term chronic disease to keep people out of hospital and independently living in their own homes.

Responsibility for health and wellbeing is already integrated across all council functions and the authority is currently working to

condition. Another area of focus are early years, maternal and child health and health inequalities in order to improve the health outcomes of the adult population in 25 years’ time. Dr Harrison says: ‘We’ve got a better opportunity now than we have had for a generation in England to integrate public health and social care in order to make best use of resources. Public sector reform is a very big challenge particularly at a time of funding cuts. The challenges are helping us to focus on how to make the best use of resources and how to achieve the best outcomes in a way that compared with times past, when we were relatively well funded, we haven’t had to do.

In order to decide how to use the two hours families helped residents put together one page profiles describing who they were, their likes and dislikes and answering the question’ If I could I would…’ Similar profiles were drawn up of staff so their interests could be matched with residents to help them make the best use of their two hours. Residents used their money in many different ways. One woman, Winifred, enjoyed being a home maker so she was paired up with Beryl, the housekeeper and they worked together cleaning and tidying the home. Other residents used their time to go outside the home going to venues such as a football match, a nail bar or a wine bar accompanied and supported by a member of staff who also enjoyed those activities. Family members are seeing a difference in their loved ones as a result of the project. Winifred for example, to the amazement of her daughters, is more settled in the home and has started singing. ‘It’s not just how we can give people dignity and respect - that’s setting the bar too low. At Bruce Lodge we are using the principles of giving people more choice and control over their day to day lives and also over resources in the way that their care home operates. It’s a very big cultural change but there is no reason why every care home should not be doing this,’ says Ms Sanderson.
