With the support of EHSCP, we are currently working in your area of Edinburgh delivering holistic, person-centred support for your patients. Our work has proven, long-term results for those who actively participate and has widely impacted on the wider delivery of healthcare services in the regions where we have worked.

Walking the road to recovery with patients Health Case Management Our Health Case Managers (HCM) provide in-depth support to people ready to make changes in their lives. Many of our current clients are experiencing multiple, life-impacting issues, that may be beyond a GP’s remit or ability to “fix”. Health Case Management isn’t counselling or support work. We spend time with the person; we listen, we ask questions using our Holistic Assessment™ tool (see next page) and together we identify the challenges the person may be facing. Personal plans are then made – not by the HCM, but by the patient themselves. In so doing they are finding a path to recovery that addresses needs they have identified themselves.

The issues experienced by patients may require that their families need to be involved in the process and we can accommodate that too. We are accepting referrals from GPs in the following areas: • Bingham • Craigmillar & Niddrie • Magdalene • Portobello • Muirhouse • Pilton

The Holistic Assessment™ tool Walking the road to recovery with patients Each HCM / Link Worker is trained in Community Renewal’s methodologies including the Holistic Assessment™ tool. The goal of the assessment is to find out how the individual can help themselves. A conversation takes place which naturally leads the person to divulge information about their life and bring to light any challenges they are facing. The Link Worker / HCM then asks them to score those challenges on how much they are affecting them and together they discuss and prioritise solutions. The Link Worker doesn’t tell them what to do or does anything for them. They provide advice on the resource available and, where possible, encourage the individual to make contact themselves.

Link Workers Our team of Link Workers are also trained in the Holistic Assessment™ tool, so a patient referred to them will also benefit from that practice. Our Link Workers also have established connections with partner agencies and therapeutic services in the area so they can signpost patients to the most appropriate service which allows them to achieve the objectives they set for themselves. Community Renewal has proven success managing Link Worker services as evidenced in the evaluation of our delivery of the service in Moray. Lothian Unscheduled Care Service (LUCS) We are accepting referrals from Secondary Care Providers supported by GP’s of patients from across Edinburgh who regularly use out of hours services. This will be an in-depth service working with up to 15 patients using our tried and tested person-centred, holistic methods as outlined previously.
