Letter from the President Looking back on 2015 gives me great pride, and it should bring you great pride, as well! Because it is thanks to your support—as a patron, volunteer, donor, or taxpayer—that SourcePoint has been able to change and grow to better suit our community during 2015. Thank you. No matter how you may be engaged with SourcePoint, you have no doubt noticed that we are ever-changing. We never rest on our laurels. Rather than taking a reactive approach to the needs of our aging community, our custom is to innovate—to look forward into our community’s future and prepare for its changing population so we may proactively meet its needs. This forwardthinking approach has helped us lead the development of aging services in Delaware County since 1992. And it is that proactive view, combined with years of careful study and planning, which led us to achieve these vital milestones during 2015: • The development of a new annual report card to monitor and enhance the quality of our in-home care services • A renewed relationship with Liberty Township Fire Department which will allow for an elder care specialist to be embedded within their emergency response staff • Thorough review of our enrichment center programs and activities, leading to a variety of new offerings including personal training, massage therapy, higher-intensity water exercise programs, and the reintroduction of Saturday morning operating hours • A long-planned renovation project which provided new program spaces for classes such as art, music, and education • The establishment of two new dining centers located in Sunbury and in Delaware’s second ward • New bi-monthly caregiver support groups and expanded caregiver service hours • Thorough board and management review of our governing documents, as well as our organizational vision and values • Implementation of the 2014 board’s decision to rebrand the agency under a new name, logo, and tagline Organizations like ours must continually change and grow, either proactively or reactively, to serve those who need us. To grow in response to a community’s needs means that sometimes the quality or availability of service may suffer—that when a family needs us the most, we might have nothing more to offer than a spot on a waiting list. But we prefer to plan and change by anticipating the community’s future needs so we can be there when you or your aging parent needs our support. With all that was achieved in 2015, we know we are well positioned to be there for you and your family both now and in the years to come. Thank you for making it all possible. Our Impact SourcePoint is the primary resource and starting point for professional expertise, referrals, and programs for Delaware County adults who want to thrive after 55. We provide in-home care that helps older adults live safely in their own homes, including services such as Meals on Wheels, help with chores, rides to medical appointments, emergency response systems, and more. SourcePoint provides free resources and support for family caregivers. We are also the source for both fun and fitness—our 44,000-square-foot enrichment center is designed for adults age 55 and older to focus on health and wellness, education, creativity, and recreation amongst friends.

*Clients pay for their services based on a sliding fee scale. This ensures that local tax levy dollars are used wisely based on a client’s ability to pay as well as their level of need.

Our Impact 2015 by the numbers: • 199,883 nutritious meals were served in our Meals on Wheels program • 147,000 pounds of free groceries were distributed • 66,347 visits were made to the enrichment center • 30,390 free farmers market vouchers were distributed to local adults • 10,249 visits were made to our fitness center • 6,438 inquiries were made to our Information & Referral Specialists • 2,141 home visits were made by our professional care consultants • 1,465 individuals were served through our in-home care program • 965 individuals received free insurance counseling • 714 rides to medical appointments were provided by volunteer drivers • 562 new members joined the enrichment center • 296 family caregivers received free support

Outreach & Awareness Initiatives to improve awareness of our organization and services in 2015 included: • Distribution of nearly 240,000 issues of our keystone publication, My Communicator, throughout Delaware County to offer vital service information and local resources for people ages 55 and older. My Communicator is distributed in the Columbus Dispatch, Delaware Gazette, and at 57 local businesses and community sites. • Release of the new 2015-2017 edition of our highly sought-after SourcePoint Services Directory, a resource guide for local senior services. • Provided information and support for nearly 33,000 unique individuals who visited our website to get help and information about our program offerings and services. • Developed and organized the new Team SourcePoint, our community impact team which participated in six charitable fundraising and awareness events throughout Delaware County. • Engaged with the community at 20 local community fairs and festivals, CareVan visits, and through 12 presentations to local senior housing locations. • Planned and hosted profession-specific events for pharmacists, social workers, and local clergy to help educate and familiarize them with local senior resources.

Board of Directors SourcePoint’s Board of Directors is responsible for policy development, financial oversight, and the overall direction of the organization. The board employs an executive director to supervise staff, implement policy, and oversee day-to-day operations.

Development Donations to SourcePoint topped $267,000 Thank you for helping make 2015 a banner year for growth in giving at SourcePoint. Your donations help us offer greater quality and a wider variety of choices throughout our various programs and services, while allowing us to be less dependent on local taxpayer dollars.

A few highlights from the year: • Nearly $114,000 was contributed through congregate dining center donations and inhome client giving in support of our nutrition programs. • Memorial and tribute giving reached nearly $11,000, with hundreds of people recognizing and honoring loved ones through special gifts to SourcePoint. • More than 160 individuals made a contribution to our new enrichment scholarship program. The $4,800 given will serve scores of people who will benefit from being able to attend a class or enjoy a session in the warm-water exercise pool. • Corporate sponsorships increased in 2015 to $44,500. Sponsorships are critical for many of our events and we are grateful for support from our community partners. • We also saw increased giving and participation in our Rock the Boat and Meals on Wheels Boxed Lunch Fundraisers. These two events not only raised support for programs, but they introduced individuals in the community to the valuable work of SourcePoint and the many services we provide.

Community Grants One important aspect of SourcePoint’s role in Delaware County is the development and maintenance of a comprehensive, coordinated system of care in our community. The needs of any community’s older population far exceed what a single organization can provide. SourcePoint recognizes that needs exist in our community which we are not able to solve—such as the growing demand for senior transportation, or the availability of affordable hearing screenings. To assist in supplying those essential services which SourcePoint itself does not offer, our board of directors awards grants to local nonprofits and service providers to support those services. SourcePoint’s grantees endure a rigorous application and screening process, and board members and executive staff carefully evaluate applications and choose the most vital and deserving programs to receive support. By being attuned to the community and investing in the work of other agencies, SourcePoint’s grant program not only helps ensure a broad variety of services, it also strengthens the stability of senior services providers throughout the entire community to ensure a quality continuum of available care for those in need. In 2015, SourcePoint provided $569,490 in grant awards for 19 programs throughout Delaware County. Funding for these grants was provided by the Delaware County senior services levy.
