Instead, the book’s focus is on the aging-related services most often used by adults over the age of 55. Within the directory, the content is divided into 13 primary topics. In each section, you’ll find a brief description of the content, as needed, and many sections contain subcategories to help narrow your search. There is also a page for your own notes at the end of each section. Today, we do that and much more. SourcePoint is the most comprehensive aging services provider in Delaware County. We provide adults ages 55 and older with inhome care services, such as Meals on Wheels and personal care, which help seniors live independently at home. We also provide hundreds of community programs, both on-site in our enrichment center and off-site throughout the county, which promote health and wellness for older adults and family caregivers. We are a truly unique nonprofit organization, bringing a wide variety of aging services and expertise together under one roof. SourcePoint is partially funded by a local property tax levy, as well as grants and private and corporate donations, and relies on hundreds of volunteers to help fulfill our mission. SourcePoint proudly serves all of Delaware County, including Ashley, Delaware, Galena, Kilbourne, Lewis Center, Ostrander, Powell, Radnor, Shawnee Hills, and Sunbury, as well as portions of Columbus, Dublin, and Westerville within county lines. 4 SourcePoint IN-HOME CARE SERVICES

SourcePoint’s in-home care services are designed to help older adults remain living safely in their own homes with independence and dignity. We provide direct access to in-home services, as well as referrals to other community resources that can help. Care Management Our licensed professionals, called care consultants, work oneon-one with each client to customize a plan of needed services. They specialize in issues associated with growing older, and can often spot—and prevent—problems before they occur. Care consultants handle all service arrangements, including facilitating appointments and customer service with providers. SourcePoint’s in-home care services are available to all Delaware County residents ages 55 and older. Eligibility for in-home care services is based on need, not income. Services are available on a sliding fee scale, so copays differ for each client based on their own financial circumstances. In-Home Care Services include: Adult Day Care Adult day care provides socialization, support, and safety to frail and chronically ill adults, as well as adults with cognitive or physical disabilities, while allowing them to return home at the end of the day. Chore Services Help with a variety of chores, such as changing light bulbs, tacking down cords or rugs, raking leaves, shoveling snow, cleaning out garages and basements, and more. Emergency Response Systems A small help button, typically worn around the neck or on the wrist, provides a direct link to 24-hour emergency assistance.

SourcePoint 5 Homemaker Services For those unable to perform household tasks, services may include light cleaning, cooking, and laundry. Homemaker escorts are also available to accompany you on local errands. Meals on Wheels Our Meals on Wheels program provides delicious hot and cold meals delivered by a caring volunteer. In addition to providing good nutrition, a homedelivered meal provides family the security of knowing someone is regularly checking on their loved one. We offer several menu options, including vegetarian meals, restricted carbohydrates, and low sodium. Medical Transportation Rides to medical appointments, social service agencies, and adult day care centers. Mental Health Counseling In-home behavioral health services are available to those enrolled in our program. Nursing Services Registered nurses help you achieve optimal health and well-being through health screenings, medication monitoring, and more. Personal Care Personal care assistants or home health aides help with daily activities like bathing and dressing.

6 SourcePoint Respite Care Respite services are designed to provide relief and rest for family caregivers, so that stress and tension is minimized. Services can be provided in the home or at a local facility. Respite workers can also perform personal care tasks, such as help with bathing, grooming, and dressing. For more family caregiver support services, see page 8. SourcePoint can provide access to additional services, including: AARP Foundation Tax-Aide Tax counselors help prepare federal and state tax returns for individuals who need assistance. Home Energy Assistance Programs (HEAP) The Ohio Department of Development offers several programs to help lower-income residents pay utility bills and improve the heating and cooling efficiency of their homes. Information and Referral Services SourcePoint’s information and referral specialists are available to answer aging-related questions and link you to community resources that best meet your individual needs. FIRST Service Coordinators SourcePoint partners with local fire departments through its First In Response Service Team (FIRST). SourcePoint places service coordinators at select fire stations, where they work closely with first responders and local senior living communities. Service coordinators can link individuals to helpful resources and perform home safety checks, evaluating residences to help reduce the risk of falls.

SourcePoint 7 COMMUNITY PROGRAMS

SourcePoint is proud to offer hundreds of community programs to help older adults stay engaged and healthy as they age. From local cafes and caregiver support groups to Medicare classes and a bustling enrichment center, SourcePoint is our community’s premier aging resource. Age-Friendly Delaware County The age-friendly initiative encourages communities to work together to prepare for the rapid aging of our population. SourcePoint leads the charge in Delaware County, working with numerous community partners to improve six key areas: housing, transportation, outdoor spaces and buildings, health and safety services, community engagement, and communication and information. These areas affect everyone in our community, and improving upon them helps ensure an active, safe, and healthy lifestyle for Delaware County residents of all ages and abilities. Enrichment Center SourcePoint’s enrichment center fosters positive opportunities for social connection, intellectual stimulation, and physical well-being. The 44,000-square-foot community center offers a fully equipped fitness center, warm-water exercise pool, and numerous program spaces for aerobics, art, billiards, dance, lectures, music, and more. The center is also home to Cafe 55, one of SourcePoint’s community cafes.

8 SourcePoint Programs are scheduled quarterly, with more than 200 programs across six major categories: arts, fitness and wellness, learning, social connections, transitions in aging, and trips. Find the current schedule at MySourcePoint.org. A community pass to SourcePoint’s enrichment center is available at no charge to Delaware County residents ages 55 and better. With no annual membership fee, guests have access to the center, as well as numerous free programs. Select programs, including some fitness, arts, and education classes, require a fee. Thanks to the contributions of generous donors, the Edie Balser Scholarship Fund is available to qualifying individuals to help with up to 50% of program fees. SourcePoint is also a participant in the SilverSneakers®, Silver&Fit®, and Optum Renew Active™ health programs. Falls Prevention SourcePoint organizes the Stepping Up to Prevent Falls Coalition of Delaware County. Its mission is to reduce falls and fall-related injuries through awareness, assessments, education, and proven programs like a Matter of Balance, a series of free workshops available throughout the year. Family Caregiver Support Nearly 42 million Americans provide care to an older adult each year. 50% are caring for a parent or in-law, and 12% are caring for a spouse or partner. About 61% of caregivers are employed, requiring them to balance caregiving and work. Many don’t self-identify as “caregivers” and as a result, miss out on helpful resources. Caring for a loved one takes a great deal of time, effort, and hard work. But you don’t have to do it alone. In Delaware County,

SourcePoint 9 SourcePoint helps care for the caregiver by providing resources and support, including: Caregiver Coalition This committee of volunteers provides valuable feedback about which local programs and resources are most beneficial. Coalition members assist with setting the direction of SourcePoint’s caregiver program by providing feedback about current programs, as well as suggestions for new ones. Caregiver Relief Companions Caregiver relief companions are volunteers who provide a friendly visit with a homebound senior in order to give the family caregiver a short break. No personal care or meal prep, just a social visit. Classes and Workshops We offer free classes and workshops throughout the year, including Caregiver Concepts, a one-hour crash course on caregiving, and Powerful Tools for Caregivers, a six-week workshop that focuses on self-care. Monthly Support Groups Family caregiver support groups are held each month and are led by trained staff. Local caregivers are invited to join a group to share experiences, meet other caregivers, discover new resources, and ask questions. Support groups are held at SourcePoint and in various communities throughout the county. Online Forum SourcePoint’s Facebook page is also home to a private Family

10 SourcePoint Caregiver Forum, on which local caregivers can share stories, resources, and support with each other. One-on-One Consultations In addition to the group support provided in various programs, local caregivers can also seek guidance and support from our professional caregiver program coordinator. SourcePoint’s caregiver programs are funded in part by the Central Ohio Area Agency on Aging. Insurance Education Through group classes and individual consultations, our certified insurance specialists provide unbiased advice and recommendations that help you make sense out of Medicare and empower you to make sound insurance decisions. Three popular classes—New to Medicare, Find Your Plan, and Fill the Gap—are available in person and online, along with valuable toolkits. Nutrition Services SourcePoint’s nutrition program provides healthy options for Delaware County older adults, including: Community Cafes SourcePoint operates several social dining centers throughout Delaware County. These community-based cafes serve delicious, nutritious lunches on a donation basis throughout the week. Farmers Market Vouchers Each summer, SourcePoint provides vouchers for fresh foods from participating farmers markets and independent vendors throughout the county. The vouchers can be used to purchase fresh fruits and veggies, herbs, honey, and food-bearing plants.

SourcePoint 11 Meals on Wheels Our Meals on Wheels program provides delicious hot and cold meals delivered by a caring volunteer. In addition to providing good nutrition, a home-delivered meal provides family the security of knowing someone is regularly checking on their loved one. We offer several menu options, including vegetarian meals, restricted carbohydrates, and low sodium. Bulk Delivery Our meal clients have the option to receive assorted frozen meals weekly, rather than daily. This is ideal for clients who are unable to be home during typical delivery times or for those who do not want daily deliveries. Just for Paws SourcePoint’s Meals on Wheels program can also provide care to our clients’ beloved furry companions. Thanks to generous donors, dry cat and dog food is available for delivery weekly. In addition, veterinary care is available, as well as respite care for individuals who may need to enter the hospital or rehabilitation for a time and need care for their pets. SourcePoint’s Meals on Wheels program and community cafes are funded in part by the Central Ohio Area Agency on Aging. SourcePoint On the Go! Thanks to a generous donor, SourcePoint acquired a van, nicknamed “SourcePoint On the Go!” It is utilized year-round to bring resources to older adults and caregivers across the county.

12 SourcePoint WAYS TO GET INVOLVED Giving Opportunities SourcePoint is a 501(c)(3) nonprofit organization, EIN 31-1354284. We are partially funded by a local property tax levy, which covers between 75% and 80% of what we do. Most of the levy income supports vital in-home care services, which are dramatically less expensive than premature nursing home care. While the levy funds are critical to our organization, they don’t support everything we offer to the community—especially as we expand to serve more seniors! The 55-and-older population of Delaware County is growing at a phenomenal rate—it more than doubled from 2000 to 2010, and is expected to double again by 2030. As our older population grows, we rely on donations to help fill the gaps. Donations support the older adults we currently serve—and help us plan for the future. Every gift counts, no matter the size. The loved ones receiving care often have chronic illnesses that prevent them from handling daily activities like bathing, managing medications, or preparing meals. The Home Energy Assistance Program provides eligible lowincome Ohioans with a one-time payment to help meet the high costs of heating and cooling their homes. Payments are made directly to regulated and unregulated utilities. Who is Eligible? • Total household income must be at or below 175% of federal poverty guidelines. • Clients can only receive emergency assistance once a year during the heating season. How Do I Apply? • To find out if you’re eligible and to start the application process, go to energyhelp.ohio.gov or call 800-282-0880. What Do I Need to Bring to My Appointment? • Proof of income for 13 weeks and/or 1 year. • Social Security numbers for everyone in the household. • Proof of citizenship, such as birth certificate, baptism records, passport, or voter registration card. • Dates of birth for all household members. • Disconnect notice for gas and/or electric. • Bulk fuel at 25% or less of the tank’s capacity. • Case number from the Department of Job and Family Services. • Current utility statements and/or invoices. • Renters need to bring the name, address, and phone number of your landlord.
