Armed Robbery On Highway 231. Do You Know Him On November 13, 2011 at approximately 5:30 A.M. a male entered a local store on Highway 231, near the Mall, and committed an armed robbery. It appears he is wearing a dark hoodie, dark jeans and a black bandanna on his face. He does not look up at the camera, however maybe someone would recognize his walk, sneakers or knows who committed this armed robbery. He is approximately 5 feet 10 inches to 6' feet 4 inches. He weighs approximately 170 to 190 pounds. He carries a silver handgun that is displayed.

Original Case Report

Narrative On the above date and time, I responded to 8408 Marcia Rd in reference to a Theft Complaint. I spoke with the Victim Barbara Westfall. Mrs. Westfall stated on 09/21/2011 at

roommates. While Ms. Norris was there, she was looking around the yard for things to scrap. At approximately 1700 hrs that day, Ms. Norris told Mrs. Westfall that she needed to leave in order to get to Dan's Pawn located at 3214 E. Business 98 by 1800 hrs. Mrs. Westfall stated from 09/01/2011 to 09/06/2011, Ms. Norris was at the residence everyday and has not been back since the 6th.

Mrs. Westfall stated on 09/23/2011 at 1000 hrs, she went to Dan's Pawn and located several pieces of her jewelry. Located was the Heart shaped Charm which has diamonds around the edge, and the women's part of the complete wedding set which was made out of gold from Guam. Mrs. Westfall stated the clerk was familiar with Ms. Norris but would not say if she had pawned the jewelry she found.

approximately 1900 hrs, her husband went into her jewelry box to get his wedding band, but discovered it was missing.

Mrs. Westfall then examined the contents of the jewelry box and determined that numerous pieces of other jewelry were also missing.

Mrs. Westfall stated the last time she saw the jewelry was around the first of September when her and her husband moved in with some elderly friends. When she moved in the residence, she placed the stand up jewelry box in her closet and was covered up by the rest of their belongings. Mrs. Westfall stated on 09/06/2011, Jamie Leigh Norris (Suspect) was at the residence. She stated Ms. Norris is the daughter-in-law of her

Mrs. Westfall said she also spoke with Ms. Norris' mother today who informed her that something funny was going on with Ms. Norris and that she is scapping everything she can get her hands on. Mrs. Westfall was also told by the mother that Ms. Norris has a severe "Roxy" addiction.

I was able to collect several finger prints from the outside of the jewelry box which were submitted into evidence.

Mrs. Westfall stated she wished to pursue charges against Ms. Norris or whoever is responsible for the theft of her jewelry.

Jamie Norris was arrested, after investigation 11/19/2011.

―Order whatever you want baby, I’ve got this one‖ Narrative On the listed date time I was working a special detail at the listed Address (waffle house #988) when I observed a gray Ford 4 door car park behind the business in the parking lot of the Flower Shop.

A white male, later identified as Michael Ford, and a white female entered the Waffle House and ordered food.

Ford then left the restaurant and went back to his vehicle.

I was advised by an employee, Toni Yourkwitz, that Ford failed to pay for his meal and said that he told her he was going to get his money from the car. I observed the vehicle leave the parking lot behind the business and go north on Hwy 77. I performed a traffic stop on the vehicle in the parking lot of the Holiday Inn and made contact with the driver, Michael Ford. I advised Ford why I had stopped him and explained that the waitress said he had not paid for his meal. Ford said he did not have enough money to cover the bill.

I advised Ford that if he could not satisfy his bill then he would be charged with obtaining food with the intent to defraud. Ford stated that he had the money and asked if he could return to the Waffle House. I followed Ford back to the restaurant and went inside with Ford.

The total bill was approximately $21.50 and Ford did not have the amount required. At this time I advised Ford he was under arrest for obtaining food with the intent to defraud. Toni advised me that when Ford sat down to eat he told the white female that he was with "order whatever you want baby, I’ve got this one".

Toni provided me with a taped statement. Ford provided me with is id, fl id card, and after a license check it was determined that he did not have a valid license and I advised Ford that he would also be charged with driving with no valid Dl.

―Get My Baby Back‖

Narrative on 11/19/2011 at 1938hrs i responded to the parking lot of 524 north Tyndall Parkway (Abalone Apartments) in reference to a report of domestic violence. Upon arrival I made contact with Sharion Davis who advised that her god-daughter, Sharon Peeples, had been beaten by her boyfriend, Laportea Howard. Sharion states that Sharon had called her earlier this morning, at approximately 0300hrs (11/19/2011), advising Sharion that Laportea had beaten her, locked her in the bathroom, and then tried to take her five-month-old baby away from her (x). The couple began to argue due to Laportea's involvement with other women and implications of infidelity on Laportea's behalf. It was Sharion's belief that deputies made contact with her god-daughter and did nothing to protect her (Sharon) or document the incident.

However, sharion later advised that when Sharon told her this, she was skeptical and had difficulty believing Sharon. It should be noted that myself and deputy Perry were dispatched to a call in reference to possible domestic violence at 0300hrs (11/19/2011) in the area of 214 north Tyndall Parkway (Abalone Junior Apartments). The complainant advised dispatch that she wanted deputies to "get her baby back." no other information was provided to the communications section due to the uncooperative nature of the anonymous complainant.

As myself and Deputy Perry were attempting to locate the individual, the communications section advised that the caller told us to disregard because her baby was returned to her (cad case # 2011-078159). [it was later learned that the caller, identified as Sharon Peeples, provided an incorrect address to the communications section] as myself and Deputy Perry were speaking with Sharion and her god-daughter, Sharon, it was alleged by Sharon that a Deputy responded to her residence this morning [shortly after having called law enforcement at 0300hrs] and retrieved the infant from Laportea. Sharon described this deputy as a short, white male but could not remember his name or describe him any further. Sharon did not realize that she was speaking to the deputies who responded to her call earlier in the morning. With Sharon's god-mother present, Sharion Davis, I explained to both ladies that no contact was made with Sharon by myself, Deputy Perry, or other deputies. After Sharon realized that we were aware of her fabrication of events, she became uncooperative and provided very few details about what actually had transpired insisting that we leave.

While speaking with Sharon, I observed several bruises to her face and forehead accompanied with some lacerations to her forehead. As my investigation continued, I learned that this morning's altercation became physical when Laportea repeatedly hit Sharon on the face and chest with a closed fist. In addition, Laportea shoved Sharon to the ground and kicked Sharon striking her on the stomach. I was advised by all parties, including Laportea, that Sharon was more than four months pregnant and that Laportea is the father of Sharon's child. Sharon and Laportea advised that they had been dating for the past nine months and the couple has been living together as a family unit for the past four months. I would like to emphasize that due to the progression of Sharon's pregnancy, her pregnant belly was easy to notice and could not have been overlooked. Sharion did advise that she would be taking her god-daughter to the hospital because she was complaining of pains and bleeding from her vagina after having been kicked on the stomach by Laportea. As I spoke with Laportea, I noted lacerations and bruising to his face and arm. Laportea advises that Sharon initiated the physical confrontation. However, Laportea states her actions against Laportea were a matter of self-defense. It seemed that Laportea was more concerned about when he could be bonded out of jail than the welfare of Sharon or his unborn child.

Deputy Perry obtained sworn taped statements from Sharion and Sharon. photographs of Sharon's and Laportea's injuries were collected and submitted as evidence.

Sharon was provided a domestic violence pamphlet and signed for its reception. In addition, a domestic Violence notification form was added to this case file. A copy of this Report will be forwarded to the department of children and families due to the presence of x during the incidence of domestic violence. At 2035hrs Laportea was arrested and transported to the Bay County Jail. A check for wants/warrants reveled that Laportea has a local warrant for "v.o.p.

Possession of a suspended driver license."

“Because I needed transportation and because I am Homeless” Narrative On 11/19/2011, I responded to W HWY 98 & Thomas Drive, in reference to a theft of a bicycle. Upon arrival contact was made with the victim Daniel XXX, his wife Tonia XXX, and their two sons X & X. They were all on the sidewalk headed West, towards Front Beach Rd. Tonia and her two boys were on bicycles and Daniel was walking. Mr. XXX flagged me down at this location and advised me that someone had just stolen his bicycle, while they were all inside of Taco Bell eating. He advised that his bicycle was a white mountain bike that has a red rear shock support. He further stated his bike had a scene of high rise buildings on the frame and that this bike had a combination locking device on it; however the bike was not locked up when it was stolen.

Daniel advised he did not see who took the bike, but did advise a older white male, wearing camouflage shorts with a blue jacket, had just left Taco Bell, prior to him discovering that his bike was stolen. During this conversation it was steadily raining, so I advised Mr. Stanford and his family to return home, so that his family did not have to stand in the rain. I explained XXX that I would make contact with him at his residence after I attempted to locate the suspect and bike. Based on the description provided by Mr. XXX, I went into the wooded area West of CVS attempting to locate the suspect and bike. The reason I went into the wooded area, was because the description Mr. XXX provided, matched the description of the suspect from a theft which occurred at CVS on 11/18/2011. The case number for the theft at CVS was 2011-078084 and the suspect from that theft, ran into the wooded area by CVS. While I was in the wooded area, I located a abandoned bicycle that matched the description of the stolen bike. This bike was loaded up in my patrol car and returned to Mr. XXX. Once the bike was returned, Mr. XXX confirmed the bike was his by unlocking the combination lock on this bike. Mr. XXX did sign a property receipt for the listed bike.

During this time a call was entered by dispatch, in reference to a theft, that had just occurred at the Express Lane located at 7624 Front Beach Road. The store clerk at this location, gave dispatch a description of a white male wearing a blue jacket and camouflage shorts as the suspect.

This clerk also advised dispatch, that the suspect fled on foot into the wooded area by CVS. Deputy Loturco handled the call at the Express Lane using case number 2011-078284; however we both searched the area for the suspect.

The suspect was later located in the wood line of the listed wooded area and was placed under arrest for the theft that had just occurred at the Express Lane.

While assisting Deputy Loturco, I witnessed him advise this subject of his Miranda rights. After doing so, I witnessed this subject admit to Loturco, that he stole beer from the Express Lane. This subject also admitted to the theft that occurred at CVS on the previous date.

This subject was identified a Steven K Hoffman. Once deputy Loturco was done asking Mr. Hoffman questions, I asked him why he had just stole a bike from the Taco Bell. Hoffman stated "Because I needed transportation and because I am Homeless".

He was charged for the theft of the bicycle and the complaint affidavit for said charge was signed by Mr. Stanford.

A taped statement was also collected from Mr. Stanford in regards to this incident. Mr. Hoffman was later transported to the Bay County Jail by deputy Loturco and was charged for all three thefts, using three separate case numbers. 2011-078084 (M.Wombels), 2011-078284 (S.Cyr), and 2011-078284 (A.Loturco).

When they went to his brothers employer, they were told “brothers” can’t work together…. “so I found work at a nearby concrete plant, that lasted for several years” Billy told The COUNTY PRESS. When I heard of work on Florida beaches after the oil spill, I headed west on my “mule” (my bike)….I never made it to Pensacola, but landed in Panama City, where I met some good friends….the travels and Billy’s artwork, next week…(and yes, Billy is Thankful for his friends in Panama City this week!)

Brother & Sister Fight Belligerent Trespasser Narrative on the above date and time, I responded to 252 n. star avenue, (Callaway sub-station) in reference to verbal disturbance. Upon arriving on scene, I made contact with the listed victim, Ms. Christina Jeanine Minor, who advised that the listed defendant, Mr. Robert Lane Griffiths did grab her during an altercation.

Christina indicated that the offense occurred at her residence. it should be noted that Christina and Robert are siblings, and both reside at 6940 Hugh Drive, their mother's residence. Christina stated that she arrived home shortly after 1700 hrs, and grounded her son for failing to follow her instructions. around 1730 hrs, her son wanted to go to church and was riding his bicycle, when she began arguing with Robert in how she raises her son. Christina stated that she and Robert were arguing in their mother's room when Robert began yelling and cursing at her. according to Christina, she attempted to leave the room when Robert grabbed her arm and turned her towards him against her will. Christina stated that this issue has been on-going and she was in fear for her own safety. Christina stated that she has been observing aggressive behavior from her brother over the past four days; however, failed to notify law enforcement of the behavior displayed by Robert due to the fact that her mother requested for her not to.

A fcic/ncic records check was conducted on Robert, and revealed that he had two prior convictions of battery and is currently on community control in Bay County, Florida for felony battery. a sworn statement was obtained from Christina and I contacted Sgt. saint and advised him of the situation. it was determined that Robert would be arrested and charged with felony battery (domestic violence). this is the third time the defendant will be charged with battery, an according to f.s.s.

784.03(2) any second or subsequent charge of battery shall be charged as a felony.

Christina was provided with a domestic violent pamphlet and a copy of the victim notification form. Christina did sign a victim notification form as well as receipt for domestic violence pamphlet. Christina did not have any visible marks or bruises, and medical assistance was denied. Christina did appear excited; however, she was coherent and was making sense during her interview. at approximately 2045 hrs, deputy proctor and I went to 6940 Hugh Drive in order to obtain a taped statement from Robert and to place him into custody for battery (domestic) second or subsequent offense. Robert was read his Miranda rights from a pre-printed Miranda card and did provide a sworn statement post Miranda. Robert advised that he and his sister did have an argument about the way that she raises her son; however, Robert denied ever grabbing his sister and holding her against her will. Robert was arrested without further incident. Robert further advised that he is currently on felony probation for the offense of felony battery out of Washington County. the battery (domestic violence) offense does violate the terms and conditions of the Robert's community control. Robert did admit to having a beer; however, he did not appear to be intoxicated. Robert appeared to be calm and coherent during the interview. These offenses did occur in Bay County, Fl. Recorded statements were obtained from both Christina and Robert. Both taped statements were emailed for transcription. a copy of Robert's statement was burned to a cd and submitted into evidence also.

Narrative on the above date I responded to 325 Leila Avenue in reference to a trespasser.

The trespasser was Angela Deeanne Voyles. Myself and Dep ut y Ro s well arrived on scene and located Angela Deeanne Vo yles standing on the victim's (Brenda Faciane) property yelling at her.

Angela was extremely intoxicated and seemed to be under the influence of some sort of narcotic.

I placed Angela into handcuffs for trespass after warning due to fact that myself, Deputy Roswell, and Deputy Khanna had just responded to 325 Leila Avenue less than ten minutes before for the same thing. (case number 2011-077870) Angela was gone when we arrived the first time, but she began calling the victim's home phone looking for her boyfriend Eli. Brenda ignored the phone calls and asked me to answer her phone the next time Angela called to tell her to stay away and to stop calling. Angela called back and I answered the phone. I relayed Angela that Brenda did not want her on the property and to stop calling.

Angela obviously did not take my warning seriously and returned to the property less than 5 minutes after I told her not to.

Angela was extremely belligerent and cursing at all deputies on scene. Angela was then transported to the Bay County Jail during which she continued to curse at me. She was charged with trespassing. I completed a written trespass warning which Angela refused to sign.

Slapped & Punched Girlfriend

Narrative on 11/17/2011 at 0105hrs I responded to 3807 Biltmore Dr #b in reference to a domestic physical call. upon arrival I made contact with Stephanie Marie Carney (victim) who stated that her live in boy friend Timothy Oneal Andrews (defendant) had slapped, and punched her in the face, she appeared very calm, and was not crying. I noted that Stephanie's nose was bleeding, and that she was holding a wash cloth, and had appeared to be cleaning her face, which I instructed her stop doing until photos could be taken. I then made contact with Timothy who was sitting in the bedroom on the bed, Timothy also appeared very calm, and gave no resistance to this deputy. Timothy stated that he had slapped Stephanie by accident when he grabbed her by the arm, and she had slapped at in the dark in their bed room. Deputy R.Lansford arrived, and assisted in taking a taped victim statement from Stephanie, which was up loaded for transcription, Deputy Lansford also took several digital photos which he provided to me, and I placed into evidence. at 0115hrs I placed Timothy under arrest for battery domestic violence. while Deputy Lansford was assisting with above items, I transported Timothy to the Bay County Jail, where I completed booking sheet, and a complaint affidavit on timmothy for battery domestic violence. I then relocated to meet Deputy Lansford to pick up the photos, and taped statement, at this point I learned that Stephanie not only had a bloody nose, and red face, but that the inside of her lips had been split by her teeth, and her left eye was puffy, but had not bruised.

Stolen Pair of Miss Me Jeans Narrative On 11/16/2011 at around 5:30pm, I was working off duty security at Dillard's in Pier Park. I was notified by the security officer, Joshua Duty, in the camera room that a female had possibly concealed a pair of Miss Me jeans after entering into the fitting room. Josh advised me that he watched the female take in four pair of jeans and exit with only three in her hands. Joshua made contact with the associate working in the clothing area that the jeans are kept in and had her check the fitting room to see if the jeans had been left inside.

The fitting room was empty and the jeans were unaccounted for. I made my way to the area and watched the female exit the store through the west exit with her boyfriend.cI made contact with the female and male as they were walking along the sidewalk outside of the store.

I asked the female, identified as Aiste Palukaityte, if she had made any purchases while inside the store and she said that she had not. I asked her if she would step back inside with me for a minute and she said that she would. Upon entering back into the store, I asked Aiste if she had anything concealed inside of her purse.

She opened it up and allowed me to look inside and I saw that nothing was there. While looking in her purse, I noticed that Aiste's shirt stopped just above her waistline. I then noticed two different buttons to her pants and realized that she was wearing the Miss Me underneath the jeans that she wore into the store.

I asked Aiste if she could show me the button under her outer jeans and saw that it was indeed a different pair. She said that she was sorry and offered to pay for the jeans that she was wearing. I asked her to return to the dressing room and remove the Miss Me jeans, which she did.

The jeans were checked and the price was found to be $106.49. The manager on duty requested that a retail theft charge be filed against Aiste for the incident. Aiste was very cooperative and did not offer any resistance.

She was issued a Notice to Appear for retail theft and her court date was set for 12/09/2011 at 8:30am. She was then released to go and escorted out of the store.

Failed to Complete Basic Driver Improvement Course Narrative on this date while on patrol in the area of Cherry Street and N Berthe Avenue I spotted a silver Chevy Impala bearing Florida tag turning left ahead of me onto Cherry Street from N Berthe Avenue. The vehicle tag indicated that the registration had expired September 2011.

I conducted a traffic stop on the vehicle at 6913 Cherry Street and made contact with the driver, Khalilah Shaffeqah Gaines and advised her why I was conducting the traffic stop. Gaines stated she knew the tag had expired, but has not had the money to renew the registration. a check of Gaines' driver license in fcic/ncic indicated that it was cancelled indefinitely on 9/19/2011 due to failure to complete a BDI(basic driver improvement) course. Gaines advised she did not receive notice of the suspension so she did not know. Gaines stated she failed to change her address on her drivers license, but she advised me that all mail was being forwarded to her new address of 6913 Cherry Street apt a. dhs mv notes notice was given to Gaines of the suspension on 8/29/2011. I issued Gaines a criminal citation (#2937rwh) for driving while license is suspended/revoked/cancelled with knowledge. She was released at the scene with a court date of 12/2/2011. Gaines was given a verbal warning for her expired registration.

Narrative On 11/16/2011 at 1913hrs I responded to 9724 Front Beach Rd Coconut Motel in reference to an information call.

Upon arrival I made contact with Randy Murrah (complainant) who stated that every time the people in room #114 opened the door he could smell gas, and thought the people in the room might be making meth. Sgt.J.Sumerall arrived a few minutes later, and I advised him what I had been told by Randy.

The Sgt, and I then went room #114, and knocked on the door, a man answered the door, we asked if we could come inside, the man stated that this was not his room. it was at that this time both Sgt.Sumerall, and I smelled the strong odor of an ammonia type chemical smell, and one of the occupants darted into the bath room. at this point we made entry ordering the occupants to get on the floor, Sgt.Sumerall made his way to the bathroom to get the man who had darted into it. we then had all the occupants come outside, and lie face down on the concrete, at this point all the subjects were cuffed, and fisk searched for weapons, and detained. Sgt.Sumerall had dispatch contact the on call SID investigator Bryan Chavers, who arrived a short while later.

The occupants were identified as Virginia A. Lewis, Ricky L. Young, Bobby Cadle, James T. Young, and Daniel R. Pascoe. Capt bell spoke to Verginia who's room it was, at which point Verginia signed a consent to search. the following subjects were arrested by investigator B.Chavers; Vergina was arrested on a violation of probation warrant, all the male subjects were arrested on manufacturing meth, and Daniel had an additional charge of possession of cannabis less than 20 grams.

The five subjects were then transported to the Bay County Jail, and booked on the afore mentioned charges, per Investigator B.Chavers.

Principal of Investment Ponzi Scheme Sentenced to Federal Prison and Will Pay $19 Million in Restitution as a Result of Attorney General’s Office Investigation Tallahassee, Fla. –Attorney General Pam Bondi today announced that David Lewalski, the principal in the investment company Botfly LLC, was sentenced to a maximum sentence of 20 years in federal prison. The Attorney General’s Office filed a lawsuit against Botfly and its principals in 2010, stopping the firm’s ponzi scheme and preserving $3.8 million in domestic assets and $1.1 million in foreign accounts for investors. As a result of the state’s litigation, Lewalski agreed to a consent judgment with our office ordering him to pay $19 million in restitution to consumers, which represents the full amount of their net losses. ―Exploitation and deception of more than 500 Floridians totaling millions of dollars is despicable and will not be tolerated,‖ stated Attorney General Pam Bondi. ―I am pleased that our office stopped the Botfly Ponzi scheme before it collapsed so that victims will receive some compensation for their losses.‖ Lewalski pled guilty to running a fraudulent investment scheme which targeted victims with promises of high returns. Botfly LLC was actually a Ponzi scheme that used funds from recent investors to pay interest and principal payments to existing investors. Botfly supposedly invested victims’ funds in foreign currency trading, but the evidence revealed the funds were spent on expensive personal items such as a Ferrari, two Porches, two Corvettes, a Ducati motorcycle, luxury hotels, jet charters, clothing and jewelry.

Attorneys and investigators with the Attorney General’s Office spent months working with the Florida Department of Law Enforcement (FDLE), the United States Department of Justice and the U.S. Postal Inspection Service to assist with the criminal investigation that led to Lewalski’s arrest and conviction. The case was prosecuted by the U.S. Attorney’s Office for the Middle District of Florida in Tampa.

Bogus coupons Incident # 11-12906 On 11-14-11 at about 2301 hours I was dispatched to Wal-Mart located at 2101 S Hwy 77 in reference to the use of coupons.

Upon arrival, contact was made with Kristian in the management office. WalMart employees advised four male subjects came into the store with coupons for several items that had higher savings amount then the product was worth.

Kristian gave me an example of a Purina cat food coupon that said save $8.00 The item is originally $7.47. Kristian advised the coupons scanned and went through but were not legitimate due to them possibly being photo shopped or altered somehow.

Kristian wanted a report to document the incident in case the people responsible are pursued in civil court by Wal-Mart for the lost money.

I watched video of the subjects making the purchase and all were white males appearing to be in mid 30s in age.

They also all had a envelope that they carried the coupons in. They all made purchases at different registers and then they all walked to the same vehicle possibly a Dodge mini van burgundy in color.

The van backed out of its space and went the wrong direction down its aisle to drive away from the store. Kristian also advised they all used Georgia food stamp cards to make purchases. Kristian advised me that the Callaway Wal-Mart and Panama City Beach Wal-Mart had also just dealt with the same people and have began to deny them purchases with the coupons. The video was not obtained due to the asset protection associate not being in the store at the time of my arrival.

Reporting Officer ADKINS, J, Incident # 11-12849 On 11-13-11 at 20:33 I was patrolling the 2500 block of Minnesota Avenue when I observed a white Ford Taurus bearing Florida tag 621-HSE turn into the parking lot of 2507 Minnesota Ave. I further noted that the vehicle tag expired on 92011. Upon seeing the expired tag I conducted a traffic stop on the vehicle and made contact with the driver Jamond Brundidge. Mr. Brundidge advised that

he borrowed the vehicle from a friend and did not know that tag was expired. I issued Mr. Brundidge a citation for expired tag less than 6 months.

It should be noted that during that traffic stop I made contact with a passenger in the vehicle Joseph Lee Rochelle Jr. who had an active warrant for failure to pay fines. Mr. Rochelle was taken into custody and transported to the Bay County Jail. A copy of the warrant has been attached to the case file. The owner of the above vehicle was contacted and it was released to her care. Reporting Officer HOBBS, L Incident # 11-12890 On 11/14/11 at approximately 1732 hours, I was dispatched to 1609 Pennsylvania Avenue in reference to a theft. I made contact with Jennifer Collins. She stated at approximately 1600 a white female arrived at her home and asked if she needed any meat. She recognized the female from a meat delivery company she uses called Stlak Steak House, but she did not know her name. She advised the female she did not want the entire delivery she normally gets, but she would be interested in a box of hamburgers. The female advised she had some and got them from a white pick up unknown make or model. Collins advised she gave the female her EBT food stamp card, and the female advised it would be $17.00 for the box. The female returned to her vehicle copied the card, returned it to Collins, then left the home. Collins stated she noticed the brand on the box of hamburgers had changed. It was now Northstar foods, and the vender that the female represented was Your Choice meats. Collins stated she had dealt with female in the past, so she did not think twice about it.

Collins stated at approximately 1730 hours, she called the EBT company to get the balance of the card. She discovered Your choice Foods and withdrew $99.01 for the order she just received. Collins stated she then called the company she thought she was buying from Stlak Steak House, and she discovered the female she

was dealing with had recently been fired from the company for credit card fraud.

Collins stated the operator that she spoke with could not release her information at that time, but she would return her call on 11/15/11. Collins stated she would come to the police station with the information as soon as possible.

I attempted to make contact with a representative of the steak company with the number provided by Collins, but I was unsuccessful.

Incident # 11-12762 I was dispatched on 11-11-2011, at 0938 hours to 409 West 4th Street Lynn Haven Florida 32444 in reference to a delayed burglary to a residence. Upon arrival I made contact with the complainant Shelley R Crooms on the front porch of his residence. Crooms stated that two 4 foot high white in color wood rocking chairs were taken from his front porch of his residence.

Crooms stated that he last saw the rocking chairs on 11-10-2011 at approximately 1300 hours. Crooms said he noticed them missing from the front porch at 0930 hours.

Crooms stated that he was home all the afternoon and evening watching television and did not hear anyone on his front porch. Crooms advised me that he was the victim of another burglary to his residence which was reported on 11-102011 case number 11-12747.

I gave Crooms a business card with the case number for this incident. I advised him to call if he discovered any other items missing. I advised Crooms that I would have his residence placed on extra patrol for the next 3 weeks.

I spoke to the neighbors at 410 and 407 West 4th Street and they stated that they did not see anyone unusual in the neighborhood yesterday or this morning around Crooms residence.

Incident # 11-12757 On 11-10-11 at about 2200 hours I was dispatched to 1301 Alabama Avenue, in reference to a disturbance that took place in the roadway in the area of 15th and Alabama.

Upon arrival, contact was made with the complainant Kenneth Larson. Larson told me while walking to the Beeline on 390, he encountered a male at 15th and Alabama Avenue. Larson told me the male said “hey are you Kenny Larson�. Larson said he walked towards the male and the male swung at him barely nicking his chin due to Larson drawing his head back. Larson said he knew the person only as David.

Larson said David is friends with Brent Adams.

Brent dated Larson's sister a while back and this is how they are acquainted. I then made contact with David Gardner at his residence of 1502 Alabama Avenue. David had a strong odor of alcohol coming from his breath. David said he and Brent were drinking at his house and Brent was jumped a while back by Kenny and his son.

David said Brent was his best friend and wanted to instigate a fight between Kenny and Brent so Brent could fight him fair. David said when he called Kenny's name in the roadway, Kenny walked up to him and flipped open a small pocket knife. David said he then attempted to swing at Kenny but missed and then chased Kenny towards Hwy 390.

David said he stopped pursuing and returned home.

Brent was not at the residence when I arrived. I then made contact with Kenny Larson and he advised he wanted a report written and did not want to file charges at this time. Kenny said he never displayed a knife and only faked as if he had a knife to scare David from attacking him. Kenny signed a declaration of intent for battery.

Reporting Officer ADKINS, J Incident # 11-12763 On 11-11-2011 at 1235 hours, I was

dispatched to 3439 S Highway 77 Lynn Haven Florida 32444, Hardees Restaurant in reference to some lost luggage. Upon arrival I made contact with a Vickie S Hart.

Hart advised me that she was riding in her friends 2005 Nissan Titan truck tag number 494YVP southbound on State Road 77. As they approached the intersection of Mosley Drive the tail gate on the truck opened and Hart's 21 inch multi-colored suite case fell out the back of the pickup on to the roadway. Hart stated a rusty blue vehicle stopped and a male picked up her suite case and was last seen heading north on State Road 77. Hart and the other passengers in the pickup truck could not give me any further description of the vehicle or the subject that took the suite case from the roadway.

Hart provided me with a detailed list of items that were inside the multi-colored suite case which are attached to the property section of this report. The total dollar figure for all the items including the cost of the suit case is $3,627.00 dollars.

Hart was given a business card with the case number for this incident.

At this time no one has returned the suite case to the Lynn Haven Police Department.

I checked with Hardees restaurant Manager and she advised the camera system records inside the store not outside. I telephoned Hart and advised her on the status on the cameras at Hardees restaurant. I also advised Hart if she discovers any further items missing to call the Lynn Have Police Department.

Reporting Officer POOLE, MILES Incident # 11-12776 On 11-11-2011 at 2015hrs, I conducted a traffic stop on a gray Chevrolet Monte Carlo bearing FL tag at 26th St and Cottonwood Cir for having a cracked tail light. The driver, Johnny J. Telfare, was identified from my past dealings with him and later verified through DAVID. A short time after initial contact, Telfare informed me that his DL was suspended. Dispatch confirmed the suspended DL through DHSMV and a citation charging Telfare with DWLSR w/knowledge was completed with a courtdate of December 7 2011 at 830am. Telfare signed/accepted the citation and was released at the scene. He left on foot, leaving his car to be picked up by his sister at a later time. He was found to have

in his possession his DL, which was seized for the return to the State of Florida.

Reporting Officer POINTER, S Incident # 11-12807 On 11/12/11 at approximately 1639 hrs. I was dispatched to WalMart 2101 S Hwy 77 Lynn Haven, FL 32444 concerning Retail Theft. Upon arrival I made contact with APA Kevin Ethridge.

Ethridge told me he observed a white male later identified as James Ray Fernandes remove four packs of Power Pro fishing line from the shelf in sporting goods. Fernandes than walked to the housewares area and concealed the packages in his pants. Fernandes then passed all points of sale without attempting to pay for the merchandise. Ethridge stopped Fernandes as he was walking out the general merchandise doors. Ethridge provided me with a receipt for the merchandise which totaled approximately $ 107.88 and a written sworn statement which are attached to the report.

I notified Fernandes that he was under arrest for retail theft. I placed Fernandes in double locked handcuffs and placed him in the back seat of my patrol car. I then completed a complaint affidavit charging Fernandes with retail theft and a booking sheet. I transported Fernandes to the Lynn Haven police station where I transferred custody to Ofc. S. Pointer. Ofc. Pointer transported Fernandes to the Bay County Jail without incident. A copy of the complaint affidavit and booking sheet is attached to the report.

Incident # 11-12813 On 11-12-11 at 17:36 I was dispatched to 635 E. Baldwin Avenue in reference to hit and run accident. Upon arrival I made contact with one of the drivers John Smart III who advised me that he was stopped for the traffic light at the intersection of SR 77 and Baldwin Road in the southbound lane of traffic. Mr. Smart went on to say that a dark colored Jeep Cherokee then collided with his vehicle from the rear. After the accident the driver of the Jeep Cherokee then fled the scene traveling west on Baldwin Road. I noted that the back of Mr. Smart's vehicle was damaged as a result of the accident.

I next spoke to Sonia Gann by telephone who stated she witnessed the accident. Ms. Gann advised that she was traveling south on SR 77 behind a black Jeep Cherokee bearing what appeared to be Florida tag . Ms. Gann went on to say that she observed the driver of the Jeep Cherokee collide with the back of Mr. Smarts van and the leave the scene west on Baldwin Road. After learning of the possible tag number a check of the vehicle registration was conducted with no record being found.

On 11-13-11 at 03:56 I was contacted

by Lynn Haven Police Officer S. Pointer in reference to him having located the suspect vehicle at 905 W. 26th Street.

Upon arrival I noted a black Jeep Grand Cherokee bearing Florida tag parked in a parking space at the above address. Upon looking at the front of the vehicle I was able to observe damage as a result the accident mentioned above. I further noted

the presence of maroon transfer paint on the bumper of the suspect vehicle which matched the color of Mr. Smart's vehicle.

I next ran a check of the tag and learned that the vehicle owner lived at 905 W. 26th Street Apt. 43. I then made contact with the occupant Anthony Gilyard who admitted to driving the vehicle earlier in the evening when the accident occurred.

Mr. Gilyard advised that he was traveling south of SR 77 when his telephone rang and he took his eyes off of the road. Upon looking down at his telephone Mr. Gilyard advised that he ran into a maroon van parked at the intersection of SR 77 and Baldwin Road for the traffic light.

Mr. Gilyard advised that his drivers license was suspended so he left the scene traveling west on Baldwin Road. A check of Mr. Gilyard's drivers license revealed that they were suspended.

A taped statement was taken from Mr. Gilyard and has been added to the case file. I further issued Mr. Gilyard two criminal citation for leaving the scene of a crash with property damage and driving on a suspended license with knowledge.

I will notify Mr. Smart that Mr. Gilyard was located and no further action will be taken at this time.

Reporting Officer HOBBS, L Incident # 11-12752 On November 10, 2011 at approximately 1756 hours I, Officer Charles Enfinger, was dispatched to Walmart at 2101 South Highway 77 in Lynn Haven, Florida in reference to a retail theft. Upon my arrival, I made contact with Walmart Loss Prevention Associates Kevin

Ethridge and Chanel Jackson.

Ethridge and Jackson did witness Diedra Faler Howard and Louis Edward Taylor commit the offense of retail theft by taking possession of or carrying away of baby clothes and a crib set with intent to deprive Walmart of possession, use, benefit, or full retail value by concealing a crib set and baby clothes inside a shopping bag and passing all points of sale attempting to exit the store on the grocery side.

Ethridge and Jackson provided written sworn statements as well as a receipt for the stolen merchandise totaling $80.28.

The statements and receipt are attached to this report. Jackson also issued a trespass warning to Diedra Howard for Walmart.

The trespass warning was given to dispatch and a copy is attached to this report.

Howard's vehicle was turned over to Matthew E. Tucker at her request. Tucker was sitting in the vehicle with the minor child, Corbin Chandler. DCF was called in regards to the minor child. Howard stated this child was her grandchild and was 3 months of age but could not or would not provide any further information about the child. Tucker and Taylor did not even know the child’s name and were not able to provide any information about the child either. DCF worker Anjanetta Bryant responded and took custody of the minor child.

Diedra Faler Howard and Louis Edward Taylor were transported to the Bay County Jail without further incident.

Incident # 11-12966 On November 15, 2011 at approximately 1942 hours I, Officer Charles Enfinger, were dispatched to 705 East Baldwin Rowe Circle regarding a domestic violence battery. Sgt. Hernandez arrived moments prior to my arrival and made contact with Michael Montgomery, waiting at the clubhouse at Baldwin Rowe. Sgt. Hernandez supplied a supplemental report documenting her interactions. When I arrived, I made contact with Traci Christman at the residence.

Chr i st ma n to ld m e Mic h ael Montgomery was her ex-boyfriend, and they had a minor child in common: Alexander Montgomery, age 2 years old.

Christman said Michael Montgomery was still living with her at 705 East Baldwin Rowe Circle and she had been trying to get him to move out. She said she hadn’t started any eviction process because she wanted to get custody of their child established first. She said that she had filed for custody, but at this time they were in mediation and no court, judge, or magistrate had made any indication yet as to who had custody of the child. Christman said she wanted to move back to New York, but had to get custody settled first and sell her residence after she evicted Michael Montgomery.

Christman said that on this particular day, she had asked Montgomery to move out again. She said after several minutes of arguing Montgomery got up off the couch and walked into the kitchen, taking her car keys from the counter where she had placed them earlier. She told him he could not take her car, and attempted to physically remove the keys from his hand.

Montgomery then walked back into the living room and picked up their two-yearold son, Alexander Montgomery, and said he was leaving with their child. Christman attempted to block Montgomery’s path of exit in an attempt to not allow him to leave by standing directly in front of the front door. Montgomery then exited through the door to the garage, at which time Christman again attempted to remove the keys from Montgomery's hand.

Christman also said she was holding on to their son Alex when Michael Montgomery pushed her down and closed the door on her leg.

Montgomery then got into Christman’s 2003 Mazda SUV and drove away.

Christman denied hitting, punching, kicking or pushing Montgomery, but said she did pull on his arms and try to pry the keys out of his hand. I did observe a raised bruise on Christman’s shin, which Sgt. Hernendez later photographed. The photographs as well as Christman’s written sworn statement are attached to this report.

Christman also said she had given permission for Montgomery to use her

but he made it out to the car with their son.

After locking himself and their son inside the vehicle, he called the police. Then he drove around to the clubhouse area in Baldwin Rowe to wait for police to arrive.

Montgomery said when Christman was trying to take the keys away from him she had scratched his arms. Sgt. Hernendez photographed his marks and those photographs are attached to this report.

Michael Montgomery’s written sworn statement is also attached to this report.

Montgomery denied hitting or punching Christman, but he said he did push her in his attempts to leave when she was trying to take the keys and/or the child away from him.

After careful consideration, I determined Traci Christman to be the primary aggressor. Both Christman and Montgomery stated she had initially attacked Montgomery to gain possession of the car keys. Both Christman and Montgomery stated she persisted in blocking Montgomery’s path of exit while persisting in attempts to physically remove the keys and the child from his grasp. Traci Christman was arrested for battery

vehicle up until the point at which they began arguing, when she changed her mind.

After speaking with Christman, I made contact with Michael Montgomery.

Montgomery said he had been using Christman’s vehicle with her permission when she called and asked him to come home early.

When he came home, they got into an argument and she told him to get out.

Montgomery said the keys never left his possession, and he had told her at first he was not leaving. Instead, he sat on the couch with their son Alex Montgomery in his lap and told her he wanted to spend some time with him before bedtime. At that time, Christman threatened to punch him in the testicles if he did not give her the keys. She attempted to take the keys away from Montgomery and during the struggle she punched him in the testicles.

At that point, Montgomery decided to leave. He said Christman tried to stop him,

(domestic) and transported to the Bay County Jail without further incident.

Prior to transporting Christman to the jail, I provided Michael Montgomery with a domestic violence notice of rights and remedies.

Montgomery signed a consent and release form for receiving the brochure and the consent and release form is attached to this report. I also provided Montgomery and the Bay County Jail with a crime victim notification form, and a copy is attached to this report.

Sgt. Hernandez made contact with Carol Spence from DCF regarding the minor child. Documentation of that contact is in a supplemental report written by Sgt. Hernandez. Traci Christman did give permission for Michael Montgomery to remain in possession of her vehicle and car keys until her release from jail.

Montgomery returned to the residence with their minor child Alex Montgomery.

―It Is in Park‖ Narrative On this date, while on patrol, at approximately 0133 hours, I was driving west over the Hathaway bridge and noticed a truck sitting in the break down lane on the eastbound side of the bridge. I responded to two different calls while in the beach area and upon crossing back over the bridge eastbound, I observed the truck still stopped in the break down lane. Suspecting the driver was having vehicle problems, I stopped to assist. As I got closer to the truck, I noticed that it was stopped very close to the crash wall and suspected it had in fact struck the wall. As I exited my patrol car and approached the truck, I noted the motor was still running. The vehicle had stopped only inches from striking the wall. Upon looking inside the driver side window, I observed a white male sitting in the driver seat with his head slumped down. I knocked on the window and the subject jumped and looked out the window. I identified myself as the subject lowered the window. I immediately noted a strong odor of an alcoholic beverage coming from inside the vehicle. The subject stated that he was very tired and had stopped. However, he was not aware of where he was at that time.

I then requested and received his Florida driver's license, which identified him as Todd Boone. While speaking with Boone, I noted his speech was slurred and that he had a strong odor of an alcoholic beverage emanating from his breath. I also noted that his vehicle was still in drive. I asked that he place the vehicle into park for our safety, to which he stated "it is in park" and then placed the vehicle into the park position. I then requested Boone exit the vehicle.

As he did so, he used the door handle for support and had a difficult time standing once on his feet. I then had him step to the rear of his vehicle. As he did so, he staggered as he walked. I then asked Boone to submit to a series of sobriety evaluations.

The first evaluation was the Horizontal Gaze Nystagmus. After instructions, I conducted the evaluation and noted that Boone had a total of six points as well as moving his head with the stimulus. The next evaluation was the four finger count. Boone attempted to start the evaluation two times during instructions, despite being asked to wait. During the evaluation, Boone counted his thumb to his pinkie finger, from one to five. I then gave the instructions a second time and Boone performed the evaluation in the same manner as before.

The next evaluation was the finger to nose. Boone swayed and had difficulty maintaining balance during instructions. During the evaluation, Boone touched the bridge of his nose with his left index finger and the side of his nose with his right index. Due to being on a slight slope from being on the bridge, I did not attempt the one leg stand or walk and turn evaluations.

Based on my observations, I placed Boone under arrest for DUI. I then called for a rotation wrecker to remove Boones truck from the bridge. Gulf County Wrecker service arrived and took possession of the vehicle. Deputy Brooks Adkison arrived on-scene and transported Boone to the Bay County Jail. Upon arrival at the jail, I made contact with Deputy David Hopkins, the intoxilyzer operator. We started the observation period at 0340 hours. I then read Boone the implied consent warning and he agreed to submit to the requested breath test. After the observation period ended, Boone provided two breath samples which registered, .123% and .124% respectively. I then read Boone his rights and he acknowledged understanding, but declined to sign the waiver to answer questions. Boone was issued a Florida Uniform DUI citation and his license surrendered.

Florida Man Arrested Two Times In One Night Donald Gartner of Port Richey, Florida, downed a six-pack of beer and a handful of Oxycontin Sunday night and proceeded to get arrested for causing a ruckus at his neighbor's house. He found himself released from jail a few hours later, but wasn't done raising hell. Shortly after being released, he was arrested again for trying to break into cars in the visitors' parking lot of the jail he was just released from. Gartner's substance-fueled trouble began when Pasco County Sheriff's Office deputies found him crawling out from underneath bushes at his neighbor's house, according to The St. Pete Times.

His shorts were sagging low on his waste and on backward. A witness claimed he had been peeing in the bushes. Witnesses also claimed he banged on his neighbor's door, broke the glass panel of a porch light, and then ripped the light's electrical wiring out of the wall. The intoxicated 48-year-old was arrested for criminal mischief and booked at the Land O'Lakes Jail at 7:38 p.m. Gartner was released around 10 o'clock that night and was waiting in the visitors' parking lot of the jail for a ride, but apparently he got impatient. A woman drove up in her Acura. Shortly after she got out, Gartner tried to get in. She yelled, and he explained he thought it was his car. Shortly afterward, however, he tried breaking into yet another car. A deputy was watching this time, and less than two hours after his release, Gartner was taken back to jail.Gartner was charged with two counts of attempted auto burglary. His bail was set at $10,000, and he's still sitting behind bars.

The Gulf County Sheriff’s Office will be conducting vehicle safety checkpoints and DUI check points during the months of October and November 2011. PANAMA CITY BEACH POLICE DEPARTMENT Offense/Incident:

Animal Cruelty Date: November 21,2011 Location: U.S. Highway 98 @ Highway 79 On Friday night (November 18, 2011) the Panama City Beach Police Department responded to the area of Back Beach Road, in the area of Highway 79, after an officer heard what he believed to be a motor vehicle crash. After arriving on scene, witnesses advised that a white pickup truck was driving westbound on Back Beach Road in a reckless manner and struck a raised curb within the intersection.

This crash caused the vehicle to rotate violently a couple of times; which caused a small black dog that was riding in the open bed of the truck, to be thrown over the side.

The dog’s leash got caught on something in the truck’s bed and caused the dog to be suspended along the outside of the truck. The driver, later identified as Mr. Mark Andre George Dixon with a listed address of 330 Placido Place, Panama City Beach, failed to remain at the crash scene and drove off at a high rate of speed while his dog continued to hang and continually slam against the side of the truck for several miles.

According to witnesses, Mr. Dixon then made a high speed u-turn on U.S. Highway 98 which caused the dog’s leash to break away and the dog fell onto the paved portion of the median. The dog died the next morning as a direct result of injuries sustained from Mr. Dixons’s reckless driving and negligent care.

A request for two misdemeanor capiases have been filed with the State Attorney’s Office for animal cruelty – Florida Statute 828.12(1) and leaving the scene of an accident involving property damage – Florida Statute 316.061

The Army released suicide data today for the month of October. Among active-duty soldiers, there were 17 potential suicides: one has been confirmed as suicide and 16 remain under investigation. For September 2011, the Army reported 16 potential suicides among active-duty soldiers. Since the release of that report, one case has been added for a total of 17 cases. Three cases have been confirmed as suicide and 14 cases remain under investigation.

During October 2011, among reserve component soldiers who were not on active duty, there were 12 potential suicides: none have been confirmed as suicide and 12 remain under investigation. For September 2011, the Army reported six potential suicides among not-on-active-duty soldiers. Since the release of that report, two cases have been added for a total of eight cases. Three cases have been confirmed as suicide and five cases remain under investigation.

Maj. Gen. David E. Quantock, director of the Army Health Promotion and Risk Reduction Task Force, knows how the tragedy of suicide affects our soldiers, civilians, and families. He joins the task force as the former commanding general of the U.S. Army Maneuver Support Center of Excellence, Fort Leonard Wood, Mo. “Our people are the Army and their health and well-being are top priorities. This is very important work and I can assure you that the Army team is fully engaged and is totally committed to it,” said Quantock.
