_ _ Tangata ako ana I te kaenga, te turanga ki te marae tau ana– A person nurtured in the community contributes strongly to society. Wow, where has this year gone! We have been super busy over here in the Wairarapa. We have had the opportunity to train 3 new staff recently, all of whom are doing the level 4, certificate in sterilising. This has been an amazing opportunity for us as a department as all 3 are brand new to sterilising and extremely engaged in their learning. There are some exciting times ahead at Wairarapa DHB as the relationship between SSD and theatre is also being improved upon largely due to new fresh faces. I have really enjoyed the emails going out around the country lately. As a network we can collectively improve
our service by not isolating ourselves in our own little corners of the world. Following the email trail regarding ronguers, I presented this, along with Shelaghs article from last Supplyline and have been able to remove from our shelves all of the ronguers that could not be dismantled for cleaning. Normally this would have been a real struggle, however with concerns from sterilising professionals from around the country being aired it made my life easy! I can’t often say that.
I hope you all enjoy the read, I hope to see a lot of you at conference in Christchurch this year. Stay safe, stay well and stay engaged. Nga mihi Tracey Kereopa
President’s Message Hello everyone. The year is marching on. Here we are in June. Soon we will be on the countdown to Christmas and summer. Why is the year going by so fast? Talking of things coming up, it will soon be time for the NZSSA conference which is being held in the Christchurch Town Hall in September. It is so exciting as it is our first conference in two years. Our trades are all excited to see you again as is your executive. For those of you employed by DHB’s your Directors of Allied Health Scientific and Technical are once again behind you attending. Online registration is open on the website, and there is an early bird cut-off date of the 31st July. Whenever a member brings up an issue, I try to raise that to the attention of all of you and your managers by putting the issue on the website. In the last edition of Supplyline I wrote about my journey with non-detachable rongeurs. I had looked at mine following a query from another DHB. It is wonderful to note that you are all taking this seriously. I have been copied into numerous emails stating that rongeurs are being replaced the length and breadth of NZ. It is wonderful to see what we can achieve by communicating with each other. In this edition of Supplyline you can view some of the photos which were taken at the Toi Ohomai graduation ceremony for our first batch of Diploma graduates (including myself). It was such a special day. I look forward to the next graduation ceremony for our certificate and diploma graduates.

Letter To The Editor Hi Tracey We are giving away our PJIM RFID Tunnel – purchased in 2017. 1. The purpose of purchasing this was to be able to monitor sterile implantable (screws, plates, trials etc) coming in and out of ADHB from loan companies. However, when the plan to purchase was made in 2016 – OR was not included in the plan and only to find out later that there is already a system in place at ADHB particularly managed by Orthopaedics. 2. Currently Smith And Nephew, Stryker, DePuy Synthes (Johnson & Johnson), Zimmer & Lima Orthopaedics use the system within New Zealand. You can use this system when ordering loan sets from these companies We only have Smith & Nephew in this system. The others are not in our system as we have another system/program for receiving loan sets order, arrival and return of loan sets that was already in place, thus we do not need the RFID tunnel system, otherwise, this is work duplication. About PJIM RFID Tunnel
and are also available with conveyor runners on either side for ease of use. Tunnel readers can identify many hundreds of items in boxes, trays or tubs, or on conveyor belts, quickly and reliably. Our tunnel readers are fully shielded and are well-suited for tracking of orthopaedic implant loan kits and performing stock reconciliation. All you need to do is complete the transfer process and arrange a courier to pick up the machine from ADHB. The purchase price was 97K and I am sure one CSSD out there would find this machine beneficial. I heard that negative feedback from Maureen this morning, but Waikato boasted about this machine and was the point of reference when a plan to purchase this machine was in mind by the previous CSSD leadership. I hope there is someone out there who is interested will contact SATO and find out the real story of this machine.

Over the past six months, the world of reusable medical device sterilisation has been in turmoil over the short supply of sterilisation wrap. This has been caused by the high demand in raw polypropylene materials associated with the increased demand for PPE manufacturing across the world. Covid-19 has certainly flipped the world onto its head! Consequently, we have had to source sterilisation wrap from other suppliers to make up the shortfall. This has created some confusion regarding whether Performance Re-Qualification is required or not. What follows may assist facilities in effectively meeting the standards whilst mitigating the wrap shortage crisis.

Importantly, one must note that a health service organisation (HSO) is required to complete an evaluation under section 10.5 of AS/NZS 4187:2014 (Assessment of Change). This section states “if the effectiveness of the process(es) might be altered adversely as a result of the change, then all or part of IQ, OQ, PQ shall be performed.” It is up to the HSO to make this determination on the information that has been provided to them. The assessment should be well documented and follow robust processes. Part of the HSO’s process is to request from the supplier, a copy of their manufacturer’s technical specification information. This data should be used to identity whether there is a need to requalify the sterilisation process in preparation for the change from one wrap supplier to another.

Using ISO:11607 as the reference, assess and compare the technical specification from the existing supplier versus the alternate supplier. Section 6.4 states the manufacturer must be able to provide “stability testing which demonstrates that the sterile barrier system maintains integrity over time.” The supplier must also demonstrate under Section 5.2, that the material is “impermeable to satisfy the requirement for a microbial barrier.” Also of significance, consider sterilisation wrap properties including grammage, basis weight, strip tensile, elongation and hydrohead. These properties should be listed in detail on documentation provided to you by your supplier. Where the HSO can confirm that the properties of their current sterilisation wrap do not differ from their alternate sterilisation wrap, Performance ReQualification may not be required. Minor differences in the sterilisation wrap properties, would indicate the HSO may only be required to perform a partial Performance Re-Qualification to determine performance suitability.

President: Shelagh Thomas Thank you for electing me as President of the Association. I am greatly honoured and humbled. Since entering the world of sterile sciences in 2006, it has become my passion. I quickly realised, that the technicians were the underdogs of the perioperative arena. My focus is to ensure that you have all been recognised for your knowledge and skills. I want to see that you are all afforded the same opportunities for professional development and progress as other health professionals. I started my career as a registered nurse and have had many fantastic opportunities with my nursing qualification. Nursing took me to amazing places to work and I met amazing people. Ultimately, it led me to Sterile Sciences. When I am not at work, I have seven grandchildren vying for my attention. I love to travel, though now my wings have been clipped and it is travel around NZ. Last year I achieved the Diploma In Sterilising Technology. I say no more study but who knows what might take my interest. There is still work to do, however this will be my last term of office as president of the association. For the next three years’ I will be seeking to grow someone into the role to lead you into the future.
Secretary: June Isted I have enjoyed working on the executive for the past two terms and very excited to represent all the NZSSA members once again for the next term. As an executive member, I have had the pleasure of organising the satchels and goodies for the past five conferences and have just finished organising these for our upcoming conference in September. I stepped in as Secretary last year when Jenny stood down and still learning this role with the help from Shelagh and Alison. I am looking forward to the next term and the knowledge I will learn in the role as

secretary. As one of your executive members, I am more than happy for you to contact me direct at work if you require some advice or help. A little about my work history and myself for those who do not know me. I have worked in the Sterile Service Unit at Hawkes Bay hospital for the past 13 years. Four of these years as Team Leader, twenty months as Acting Manager and the past four years as Manager. I completed the Certificate in Sterilising Technology Level 3 in 2010 and went on to do my Certificate in Advanced Sterilising Technology Level 5 in 2013/14. At some stage I will be doing the diploma but have been too busy to even consider this at the moment. Our perioperative unit is currently undergoing expansion, with our unit changes starting in September through to March 2022.
Executive Member: Maureen Scott Hi my name is Maureen Scott and I am the Manager of the Sterilising Unit at Waikato Hospital. I have held this position since late 2015. I am also the Professional lead for the sterilising unit at Thames Hospital on the Coromandel Coast. I have worked in the sterilising unit for around 32 years. Over this time I have seen many changes and feel it is now an exciting time to be working in this profession. I have been on two visits to Fiji with the Cardiac team to help with Cardiac Surgery on behalf of the Friends of Fiji. This was an amazing experience where I got to meet and make new friends. I play golf as much as I can but find that work gets in the way on occasions, but I guess we can’t have everything. I also enjoy playing tennis even though I know I shouldn’t be after having a Knee replacement, but hey life is for living and enjoyment. I have four beautiful grandchildren with two of them living in Australia, hopefully all going well, I will get to visit them later in the year. I thoroughly enjoy my role in the unit and the staff that I work with are an amazing team, without them I couldn’t be the manager that I am today.

I am an Executive member of the New Zealand Sterile Scientific Association (professional association), of which I have contributed in various roles. I am an Educator Assessor for the NZSSA, having also graded papers for the Open Polytechnic for 6 years. I am currently part of the team who are proactively negotiating for equal pay for Allied Health Professionals, including Sterile Services. I am extremely passionate about Sterile Services which has led me to be proactive in bringing about improvements in practises within the industry here in New Zealand.
Executive Member: Jenny Carston Kia Ora My name is Jenny Carston most of you will probably know me as I have previously been on the Executive Committee. I have been a sterilising technician for the last 23 years, obtaining my Advanced Level 5 Sterilising Certificate and then went on to achieve my Diploma last year. Over the years I have watched how things have changed and how sterile processing has become more complex. I am passionate about our profession and have a vision to raise our profile nationally. Where can I see the NZSSA going in the future? With more members obtaining registration, I see greater recognition of our specialty and better understanding of the importance of our role.
Executive Member: Paul Moody Paul Moody is a Senior Product Development Manager at Fisher & Paykel Healthcare with over 20 years of experience designing and manufacturing medical devices. The past 5-6 years have included learning about the art of reprocessing – almost exclusively thanks to the members of the NZSSA – and sparked an interest in sustainable medical devices. Born in West Auckland, he grew up in Hong Kong before studying Mechanical Engineering at Canterbury University and Business at the University of Auckland. After a few years of building tin sheds in Southern California he returned to Auckland
I have been involved with the Sterile Service industry for twenty one years – a career which I am passionate about. As such I am a member of the NZ Sterile Sciences Association and completed my Certificate in Sterilising Technology in 2002, and my Advanced Sterilising Technology qualification in 2018. My experience working for Southern Cross Healthcare, my current employer, spans 17 years. In this time I have worked in various roles including healthcare assistant and Sterile Services team leader. Five years ago I was appointed as National Sterilising Services Manager for Southern Cross. Based in Christchurch my role involves travelling across the 15 wholly owned and Joint Venture Hospitals in the Southern Cross network, providing technical advice with the set-up of sterile services units in our new hospitals and the refurbishment of departments in our existing hospitals. My travel enables me to support, mentor and promote on-going education within all of our teams. It is both a challenging and rewarding position and my aim is to raise the awareness of our profession and encourage new recruits to make Sterile Service a viable career choice.
Executive Member: Tracey Kereopa My name is Tracey Kereopa and I have been involved in sterilising now for 12 years. I am the manager, SSD at Wairarapa DHB. I have been a member of the NZSSA executive for some time now and have learnt so much in my journey of sterilising from all of the passionate people that work in the industry. I am the current editor of Supplyline, which is a challenging job, however also very rewarding. I live with my partner and 13 year old son on a rural lifestyle block at Mt Bruce. My passion outside of work is rongoa Maori, which is developing slowly and a huge learning curve, however natural remedies are the way forward for my whanau and a lot of my friends. We have also purchased a campervan so spend a lot of time away in that at the moment, where I can be found either in the water fishing or diving or wandering around the ngahere gathering either food or medicine. Executive Member: Aileen Derby Hi, my name is Aileen Derby and I have worked in the sterilising technology field for 40 years now. During that time I have had a variety of roles, including Technician, Deputy Manager, Educator and Manager. I’ve also seen a huge number of changes, particularly with the technology that we use for processing RMDs; but also with the technology that our customers use. The sterilising field has certainly changed from when I first started working in a CSSD. Currently I am the Manager of the CSSD at Counties Manukau DHB in Auckland. There are two CSSD’s in CMDHB, one at Middlemore Hospital and one at Manukau Health Park. I have 60 staff members over both sites. I’m looking forward to being a NZSSA Executive Member. It is certainly a time of change for sterilising units around the country at the moment along with a number of other challenges. Executive Member: Karen McCormick Hi I’m Karen McCormick and recently moved to Wellington as I am now a proud grandmother. Most of my life I have worked within the Medical/ Hospital environment and becoming a Community/Enrolled nurse working mainly the Theatre areas. Approximately 6 years ago I became interested in the processing of medical devices and trained at Royston Hospital. During my stay I recognised how passionate and excited I was to learn about the complexity of the reprocessing of instruments. Last September I took on a huge roll here at Capital and Coast Hospital as Sterile Service Manager. What drives me is providing the best care and outcome for our patient’s and also try to improve the quality and safety of care in our department and Organisation. Being selected to sit on the Sterile Services Executive means that I can share my knowledge, experience and support the Organisation plus, hopefully develop a strong safety culture where quality and safety is paramount.

Why the health system needs to be more sustainable, and how you can help Oliver Hunt, Medsalv, www.medsalv.com
For a number of years now, consumer distaste for waste, carbon emissions, and unsustainable practices have had some very real impacts on the way all kinds of industries deliver products. Businesses have realised pollution is not so cool, have found ways to count their carbon calories, devised carbon reduction schemes, and figured out the various paths to becoming more sustainable operators. These are all things that consumers expect from organisations. While this environmentalist revolution has been brewing across industries, our healthcare sector has, for the most part, been immune from any real change. How bad is the sustainability of healthcare in the first place? Okay, first the bad news, then the good news. If the world’s healthcare sector was a single country, it would be the world’s fifth-largest biggest polluter by the amount of greenhouse gases it emits. Worldwide, the healthcare sector generates almost 5% of the world’s carbon emissions and is showing no signs of slowing down.
These energy-intensive processes are often conducted in parts of the world that do not use clean energy to power their factories - and this is often combined with low-cost labour. Not only is the total emissions footprint of a medical device a glaring issue, but the monumental amount of waste and pollution has become a widely accepted by-product of keeping our people healthy. Single use devices are a standard mode of operation, and hospitals are continuing to send masses of the stuff to the landfill on a daily basis. This is not a New Zealand specific issue. Even in the United States, data shows that the average patient is producing 10 kilograms of waste, even among top performing hospitals that earn environmental awards from Practice Greenhealth.
In fact, analysis in a Lancet Planet Health paper last year shows that the greenhouse gas emissions produced by the industry have not only shot up dramatically since the turn of the century, but are continually increasing to this day.
Much of this objectively bad practise can be attributed to a lack of consumer demand and understanding — how could a hospital patient know about the amount of waste, let alone demand change. Another part is the problem of NZ’s healthcare ‘monopsony’.
Zooming in on one problem in New Zealand, procurement, it is estimated that the importing of millions of dollars of devices, pharmaceuticals, and associated services - accounts for around 61 per cent of the sector’s emissions. This corresponds to around 6.6 million tonnes of CO2 equivalent emissions, per year. To give you a better idea, 1.1 billion tree seedlings are needed to grow for 10 years to sequester this amount of CO2 emissions (United States Environmental Protection Agency, 2021). Sadly, almost none of this is being captured by current reporting measurements.
Are we living in a healthcare ‘monopsony’, and what’s that got to do with sustainability?
The emissions profile of your medical devices Manufacturing, and processes leading up to it, is where most of a device‘s carbon footprint comes from. This comes from a combination of the processes used to extract and refine raw materials, manufacture plastics, and mine metals to make up these devices. One example of this would be in nylon, a common plastic that can be found in the majority of all medical devices. Manufacturing this stuff is an intensive process, requiring large amounts of energy and water. It also emits nitrous oxide, a greenhouse gas that is incredibly potent at nearly 300 times the equivalent impact of CO2.

Once materials are sourced for manufacturing, energyintensive processes must then be used to shape them and finally clean the resulting device. Processes applied to raw materials to change their shape or join them include, thermoforming, molding, welding, and forging.

In economic terms, a ‘monopsony’ is when there is a single (or outsized) buyer in a market. With a dominant public healthcare system and relatively small private counterpart, New Zealand’s healthcare is, in no uncertain terms, a monopsony. Because the public health system is the dominant buyer, they are able to set the terms for the market. Right now we’re seeing that monopsony play out on Queen Street as nurses strike to try and get more money from an employer that sets the rate, knowing full well the nurses have very few other places to go to get better pay. This can result in situations like we’re seeing, with nurses feeling underpaid and undervalued, but it also shows up from a sustainability perspective, too. As the chief purchaser of goods, the New Zealand healthcare system also has an outsized impact on the sustainability of the stuff they buy. In a world dictated by price, this often means that we’re buying container loads of cheaply made stuff from all around the world — things so poorly made that they can only be used once before they get sent to the bin. This is true across the board. Every decision made by New Zealand’s healthcare system will have huge environmental consequences, which are often minimised or overlooked. For many of the readers of the NZSSA, learning that New Zealand healthcare doesn’t have the greenest reputation won’t come as a shock. Understanding where your hospital stands That was a lot of bad news all at once, but if we play it right, there are blue skies on the horizon. And the first step for any hospital to play it right, is to understand where the hospital stands in the first place. By measuring your organisation’s greenhouse gas emissions and carbon footprint, you can understand not just if things get worse, but also if you’re getting better as you try to actively reduce your hospital’s emissions. “Measuring against third-party standards provides a framework for reporting and adds rigour to sustainability initiatives as well as quantifying savings. When measuring your organisation’s greenhouse gas emissions, it is important to follow the accepted international standard for carbon accounting (ISO 14064 / GHG Protocol). In this way, your measurements will be robust and consistent with international standards. A number of companies offer greenhouse gas accounting as a service in New Zealand.” - Ministry of Health. After capturing your baseline vitals, you’re ready to make some changes. Choose medical devices which are able to be remanufactured Of all the potential changes, choosing to re-manufacture medical devices is currently the best bang for your buck in seeing a meaningful reduction in a hospital’s carbon footprint. In general, where reusable medical devices are used, they create a significantly smaller carbon footprint per use than when the equivalent single-use devices are used. The exception is when the re-usable device has a far more energy-intensive manufacturing process - which may be required to create or shape a more robust material. Carbon emissions from a single-use medical device (SUD) are typically significantly higher per use as the majority of a device’s carbon footprint is generated in the manufacturing process, while the carbon emissions created when cleaning a reusable device are typically negligible in comparison. Through remanufacturing, hospitals can get the benefits (Convenience, Infection Prevention, and Fewer Cleaning Requirements) of single-use devices, with less of the financial or environmental costs. If the device is damaged and rendered unusable, which can happen to reusable devices too, the comparative carbon emissions per use are likely to be lower than for an equivalent reusable one should it be damaged.
Choose clean energy At Medsalv, we choose Meridian energy to power us as they use wind and water to produce energy instead of fossil fuels. Where possible, use clean energy and invest in technologies that will reduce your carbon output. In the long term, these will have a financial benefit too. For many New Zealand hospitals these changes are already underway as the healthcare sector undergoes a radical shift from coal boilers into more progressive forms of energy. Opt for reusable items Instead of using and providing plastic, single-use items such as plastic water bottles, pill cups and drinking cups, choose reusable items or consider using biodegradable or compostable. New Zealand has an increasing number of great suppliers of eco packaging and supplies such as ecoware. Incentivise staff to consider alternative transport options Encourage active transport such as walking or cycling through staff competitions and incentives. These not only reduce emissions but also have positive health impacts. By identifying safe routes and providing infrastructure such as bike racks, plugs for e-bikes and scooters, e-bikes for staff to use and changing facilities, your hospital can dramatically reduce its overall impact. The use of electric vehicles reduces fuel costs and carbon emissions compared with fossil fuel powered transport fleets. Encouraging staff to carpool also has the power to reduce emissions. - Sustainability and the Health Sector. Reduce the number of flights taken, and offset the carbon emissions on all flights that have to be. Air travel is sometimes unavoidable with no equal alternatives - sometimes Zoom won’t cut the mustard. Airlines like Air New Zealand have implemented an optional initiative for you to offset the carbon emission you will produce by taking the flight. Usually, it costs a few dollars at most and your carbon emissions will be offset by the planting of trees. Reduce meat and dairy options It is now widely known the vast environmental impacts that the meat and dairy industry generate. Take inspiration from Hayek Hospital in Beirut, Lebanon who are the first hospital in the world to go fully vegan. Not only this, the frequency of people who are reducing their animal product intake or turning to a plant based lifestyle is greatly increasing. Ultrasonic Cleaners – When do I do the daily performance test? Terry McAuley, MSc Medical Device Decontamination.
With the release of the new edition of the Australian Standard for Ultrasonic Cleaners, AS2773 in 2019, some queries have been raised regarding the testing of ultrasonic cleaners. Here’s one example: I purchased and read the new ultrasonic standard yesterday and came across a potential ‘typo?’ which I am hoping you can clarify as it is very important.
My query is regarding the following two clauses: 6.2 Performance testing (normative) (b) “Daily functionality test after each day of use” and B.5 Operating Procedures (informative) “The performance of ultrasonic cleaners should be monitored each day of use [See Clause 6.2(b)].” The word ‘after’ has been only inserted in 6.2(b) and left out of the statement in B.5. Is the correct? In my view, it is counter-intuitive to performance test an ultrasonic cleaner at the end of the day.
Terry’s Response It is not a typo as such – basically ultrasonic cleaners are a ‘pre-treatment’ for RMDs prior to definitive cleaning i.e. in a WD or if a WD is not available – then ultrasonic cleaning followed by manual cleaning. Typically, when performing a test on the ultrasonic cleaner, people use a pencil load test or aluminium foil test. If you do the test in the morning and don’t empty the tank you pose a risk of galvanic corrosion or particulate contamination to the RMDs from residual graphite from the pencil or from the aluminium foil. Thus, best practice is to perform the test at the end of the day, prior to emptying the tank and leaving it clean and dry overnight. This also saves water and time. Alternatively, you can do the test at the start of the day, knowing that you will need to empty, refill, and degas the ultrasonic cleaner afterwards. People are going to ask – what if we find the ultrasonic wasn’t working at the end of the day? The answer is – as long as a further cleaning process [manual or automated] was performed after the RMDs were removed from the ultrasonic cleaner, then there is little risk to the patients. It would be the same situation if you tested an ultrasonic cleaner in the morning and found it not to be working on the morning of the following day, as you cannot say at what stage in the previous day the machine stopped working.
Please note that some RMDs don’t actually need the ultrasonic cleaning pre-treatment step unless this has been specifically stated in the manufacturer’s IFU as a requirement. For those facilities that do not use an ultrasonic cleaner at all, you should ensure that you have read all the RMDs IFU to be sure that none of you inventory require ultrasonic cleaning as a pre-treatment / pre-cleaning step prior to further processing. A classic example is that some brands / models of ophthalmic lens folders / inserters need to undergo ultrasonic cleaning according to the manufacturer’s IFU. Are you using your ultrasonic cleaner correctly? General rules of thumb are to (1) rinse gross debris off the open or dismantled RMD, (2) immerse RMDs in a single layer in the ultrasonic cleaner basket without overcrowding, ensuring air bubbles are removed from the lumens of cannulated RMDs or attaching cannulated RMDs to the cannulated adaptors in your ultrasonic cleaner, (3) close the lid of the ultrasonic cleaner and then (4) expose RMDs for the specified length of time and at the appropriate frequency as specified in either the RMD or ultrasonic cleaner manufacturer’s IFU and (5) remove the RMDs from the ultrasonic cleaner and subject to a further manual and / or automated cleaning process in accordance with the RMD manufacturer’s IFU, noting that the

RMDs may need to be rinsed off after removal from the ultrasonic cleaner if cleaning agent chemistries in the washer-disinfector are not compatible. You might be thinking to yourself “that’s not how you were taught to use the ultrasonic cleaner”, for example I often see people manually clean the RMDs first, then expose them to the ultrasonic cleaner. After this process has finished, most people would rinse the RMDs in running water and then put them in the drying cabinet, for those RMDs that are manually cleaned. If you think I’m wrong, then this paper by Vassey et al. clearly demonstrated that “manual washing combined with ultrasonic cleaning was significantly less effective” than processing using manual cleaning alone or processing through a washer-disinfector.
However, when you think about it, most facilities only change the water once or twice a day and when soiled. So, by the time you have processed the 10th tray through the ultrasonic, all that debris that you have dislodged from the RMDs must still be in the solution in the ultrasonic cleaner’s tank, so by the time you have come to the 10th tray, rinsing may not be enough to remove the accumulated materials that have contaminated the water and detergent that remains on the RMDs surfaces.
