of BEAUTY As a plastic surgeon, I love performing surgery and I love being in the operating theatre. I love seeing the physical transformation that occurs virtually instantaneously in the operating room. Whether it be the change from a tummy tuck, where the overhang of excess skin and fat and bulging abdominal wall muscles are corrected. The creation of a flatter and taut tummy with a newly defined waistline. The transformation that can occur when a large prominent nose with a bump and a hanging tip is refined to a more feminine nose that is more proportionate to the face. I love being able to make a tangible difference to people’s lives through performing plastic and cosmetic surgery. However, I also enjoy my time outside the operating theatre when I am consulting from my rooms. It is in the rooms where I get the privilege of meeting people from all different walks of life and being able to share some of their most personal and intimate details about themselves, their insecurities and their bodies. The trust and confidence that patients and their loved ones place in me is something that means a lot to me, and which I try to return by doing the very best that I can do to help them achieve the results that they are after. I believe that the majority of people don’t actually want the popular ‘anti-ageing’

treatments. Seriously, who wants to look 21 again? I also don’t think they want to be rejuvenated like something worn and tired that has lost its vitality. I truly believe that most of us want to be more beautiful – we want to be the best version of ourselves for the stage of life we are at, and I believe that there are many ways to achieve this – not just through cosmetic surgery alone. Of course in some instances like breast reduction and reduction rhinoplasty for example, there are no nonsurgical alternatives to achieve the desired results that surgery can accomplish. However, in so many other areas of cosmetic enhancement, there are many non-surgical options available, and the choice of which pathway to choose to reach the desired endpoint will involve considerations such as desired results, downtime, cost and risks. This is why I offer my patients a range of choices and we spend a lot of time discussing the predictability, what is involved, the downtime and costs of each procedure, because ultimately I want to help people look their best, using the knowledge, skills and technology available today. And I encourage all my patients to consider their options.

If it is a facelift they are adamant they want and are a good candidate for it, then that’s what we do, but if it is flawless skin and subtle facial enhancement they are seeking, then we can achieve that with Fraxel and HydraFacial treatments, brow shaping, makeup tricks and maybe some fillers or Thermage. If better cleavage and enhanced breast volume is requested, then breast augmentation +/- a lift may be the only option available outside a push up bra. But if body contouring is requested to create more proportion and reduce fat in problem areas, then we can offer Liposuction or Liposonix (nonsurgical fat reduction) – both potentially achieving great results but with different predictabilities, costs and risks. As a female plastic surgeon, I understand how important it is to look and feel our best, and it is my vision to help as many people as possible to be their own beautiful version of themselves.

Q. You obviously have a busy life, being a trainee nurse, model and marathon runner. How do you manage to sustain a suitable health and beauty routine? Ever since I was a small child I have been heavily involved in sport. I was very competitive in surf lifesaving and netball and had to be very self disciplined to achieve my goals. I have carried that discipline into adulthood and with prioritising and good time management, I don’t find it too difficult to get everything done. Getting into a good routine helps too. Q. You’re obviously very fit. What led you to take up running as a competitive sport? I’ve always run as part of my fitness training. I began doing 2 km and 10 km soft sand runs in the sport of surf lifesaving and loved that, so taking up competitive running just seemed a natural progression. My trainer Neil Russell (ATLETA fitness) also thought I had some talent in this area so he has encouraged me to take it further. Being fit is so important to my wellbeing, both mentally and feeling good about myself physically. But at the end of the day, as important as fitness is, you have to love what you do. If it makes you happy it’s not a chore.

Q. A lot of people are turning to cosmeceuticals and non-surgical treatments now as an alternative to surgical enhancements. What are your thoughts on these products and procedures? I’m all for people doing what makes them feel better about themselves. As long as the treatments are safe and you’re realistic with your expectations I don’t see a problem. I think some people get into trouble when they get carried away and go too far with fillers etc.

Q. Competing in marathons obviously requires a lot of running in all types of weather conditions. Do you find this takes a toll on your skin? Yes it really can take its toll on your skin. I love to do most of my training outdoors, so I really have to be organised and prepared to avoid what the weather can do. Particularly in summer, getting burnt or too dehydrated is very common under the Australian sun. Even if its overcast I try to wear a hat and sunscreen at all times.

Q. We all have different strategies we use to help us ‘embrace ageing’. Do you think there is a place for surgical enhancement? I definitely think there’s a place for surgical enhancement. It’s every woman’s personal choice and definitely an individual thing. But I would encourage anyone thinking of doing this to do it for themselves and be realistic in what they would like to achieve. I also think getting fit and healthy is a good place to start because it gives you a good base level of self esteem to start with and that can help you make good choices.

Q. Can you tell us what your skin care routine consists of? When looking after my skin my go to routine consists of using the Elixxir skin care range daily. I love that it protects and rejuvenates my skin whilst being all natural and organic. I also swear by Ella Bache foundation sunscreen. As well as being a sunscreen it doubles as my foundation.

Q. Do you think you would ever be interested in entering the field of plastic surgery or cosmetic nursing? I’m not too sure where my nursing will take me at the moment, but the field of cosmetic nursing does interest me. If I do take that path, I would like to learn the skills necessary to be able to positively contribute and make women feel more confident, educated and assured with the choices they are making.

Q. Being part of the modelling industry, do you feel pressure to look a certain way? When I began modelling, I definitely felt the pressure to fit a certain mould, but coming from a background of sport, in particular surf lifesaving, I always appreciated the need to fuel my body for performance. I do try to eat a healthy and balanced diet when possible but I love my treats of course (hot chips are my weakness). I think like most girls and women, you have good and bad days with your body, and it’s all about making the choices that make you feel best.

Q. What is your top tip for young active women to help maintain a healthy lifestyle? If there is one tip I could give, it would be to set goals. Pick something that will challenge and motivate you. Get a friend and enter a fun run, an ocean swim or even a marathon. Once you have set a goal, you have something to work towards and keep you on a healthy path. And honestly there is nothing better than the feeling you get when you achieve your goal and long after its finished, you may even want to do another.

We are a nation that loves fun in the sun, and ironically this can be a deadly activity. Yet still thousands of us spend countless hours basking on our beaches every year. As our skin slowly drinks in UV radiation and our cells respond with a variety of gradual changes, we remain oblivious, never believing that we may be one of the tens of thousands of people who will be diagnosed in Australia over the next three years with melanoma1, or one of the 2000 or more people who die from it each year.2 Dr David Speakman: Chief Medical Officer and surgeon at Melbourne’s Peter MacCallum Cancer Centre says, while a mole may appear innocent, what lies beneath a melanoma can be truly frightening. “Melanoma is one of the fastest-developing cancers, with tumour cells mutating, growing and spreading at a rate up to 20 times faster than common cancers such as breast and prostate. MESSAGE FROM Plastic & Reconstructive Surgeon DR YUGESH CAPLASH DR CAPLASH SEES A LOT OF PATIENTS WHO HAVE EXPERIENCED LOSS OF FACIAL TISSUE DUE TO CANCER. After sections of the face have been removed, then the facial jigsaw puzzle begins. “I rebuild those lost anatomical components with tissue from other parts of the body and reconstruct the affected area so they can go back to looking and feeling like themselves again.” People can lose large portions of their facial features – cheeks, lips, noses and so on – after cancer removal. The key to rebuilding is to match the right tissue with the missing section of face so there is a blended effect in the repaired area. Creating the tissue building blocks involves sourcing skin, soft tissue and bone from another part of the body with a blood supply – such as the tummy or the leg. “I then reconnect the blood supply and shape the tissue taken from another area to look more like it’s an inherent part of the face” explains Dr Caplash. “For example tissue from the forehead might be used to repair the nose or tissue from the tummy or leg can rebuild sections of cheek and jaw that may have been lost or removed. I build up the section of cheek then thin and define it to improve it and make sure it matches the other side. However, there are limitations to our reconstructive abilities due to the complex nature of reconstruction. in some cases a face transplant may be the only solution.

Recent findings have revealed that our attention needs to be turned firmly toward the men in the room. Men over 45 are not only the most at risk3, but they are also the least likely to use sunscreen4. To generalize a little – many have a greater exposure to UV than women due to their often religious devotion to outdoor sporting passions and pursuits. It’s well known that many more men are engaged in sport and recreation activities than women, across a wider range of outdoor activities. Australia’s intense heat can often delude us into thinking that if we can’t feel it burning our skin, we are not at risk. Decades of advertising has focused on our need for caution on the nation’s playground; the beach, but recent studies have found that sporting events rival the beach, the lakes and rivers as high sunburn risk locations5. Australians love the great outdoors but every time we step out into the sun we need to be sun smart. Many hours are spent on the football field, the cricket pitch, the hockey ground, the tennis court, and more and whether they are players or spectators, only 29% of men are using sunscreen, compared to 45% of women6. Either way, still less than half of each gender is playing it safe. The Australian Institute of Health & Welfare’s Cancer Incidence Projections (2011-2020), state that over the next three years – from 2014 to 2016 - there will be 40 people a day told they have melanoma, and almost two out of three of them will be men. This means around 44,000 Australians will be diagnosed with melanoma in the next three years. Chair of Cancer Council Australia’s Skin Cancer Committee, Louise Baldwin puts these figures into perspective. “That number of people - 44,000 - is more than a full capacity crowd at the Gabba. The figures are startling.” With one of the highest incidences of melanoma in the world, Australia still has a lot of work to do. Many of the surgeons featured in this and previous issues of Nip Tuck magazine spend considerable amounts of time patching up those who have had moles excised, or tumours removed due to skin cancer. It is not a part of their practice they relish, but certainly a necessary part of the process for many cancer survivors. Smiling is the most natural of human behaviours. A smile is universal currency. It is a basic biologically consistent human expression, regardless of culture, age or background. Contrary to some grandparents’ insistence that the newborn baby is only experiencing ‘wind’, many of us have experienced the miracle of a 2 week old infant’s smile at loving cues and sounds from their parents. Research has proven that we are pretty much born to smile. 3-D womb ultrasounds have shown that we even smile in the womb in response to human voice. It’s in our programming well before any other basic human behaviour – but why? What role is this most basic of all human expressions destined to play in our lives?

SMILING FOR HEALTH One 30 year study out of Berkeley (University of California) looked at the smiles of students in an old yearbook, and compared it to their eventual wellbeing and success. Those with the widest yearbook smile ranked highest in wellbeing and general happiness scores, fulfilling and lasting marriages. They were also more inspirational to other people. Another university research project (Wayne State 2010) looked at a deck of professional baseball players’ trading cards from 1952. They found that the players who had flashed a beaming smile for their photo lived an average of 79.9 years, and those who hadn’t lived an average of 72.9 years! So there is obviously more to the smile than meets the eye. Research about its impact reveals some surprising and far reaching results. Good old Charlie Darwin could have told you this. As well as his dissertation on The Origin of the Species, he also

developed the Facial Feedback Response Theory, suggesting that smiling makes us feel better, rather than us smiling merely as a result of feeling good. Functional MRI studies out of Germany (Echnische Universitat 2009) have also shown that facial feedback by imitating a smile actually makes our brain cells process emotional content in a way that activates happiness. Subjects that were injected with Botox to suppress their ability to smile were unable to activate happiness feelings like those who were able to smile. Chocoholics: Beware of underestimating the power of the smile. British researchers have used electromagnetic brain scanners and heart rate monitors to find that smiling stimulates our brain’s reward and pleasure mechanisms at mood-boosting value equal to up to 2,000 that of chocolate! Smiling reduces stress hormone levels, increases mood enhancing hormone levels and lowers blood pressure.

SMILE ATTRACTIVENESS Studies show that the first thing people notice about you is your smile, and that it can make a profound impression. Studies show that when meeting someone, we can form surprisingly lasting impressions about their intelligence, success, confidence, competence, physical fitness, professionalism, their salary and lifestyle in as little as seven seconds. In forming this first impression, the smile and body language have been shown to have a much more profound impact than the content of what a person says or how they sound. A team from Radboud University Medical Centre in the Netherlands found that smile attractiveness has a penetrating psychosocial impact – affecting both how a person is judged and also how they judge themselves. Then it should come as no surprise that poll results have shown that an unattractive smile can hurt a person’s chances for career success (74%), that an attractive smile makes a person more appealing to the opposite sex (96%) and that a smile is an important social asset (99.7%). VALUE OF THE SMILE In the same British study, a smile was found to be potentially as stimulating as receiving up to £16,000 in cash. Who knew that a smile was worth $25,000? So how much should you smile? A third of us smile more than 20 times a day, and less than 14% of us smile less than 5 times a day. Kids can smile as much

as 400 times a day - and we wonder how they can soak things up and live so care free. There’s something to be said for keeping our child like sense of optimism and joy through the challenges of life. It’s proven to do us a world of good. And we know it’s contagious. Swedish studies (Uppsala University 2002, 2012) showed that another person’s smile suppresses our own ability to control our facial muscles, encouraging us to smile. We have trouble frowning if we see someone smile. So surrounding ourselves with smiling people also does us a whole lot of good. THE TAKE-AWAY MESSAGE: SMILE MORE! In a world obsessed with the latest diet, pill, procedure or fitness fad to encourage health, attractiveness, happiness, wellbeing and long life, the humble smile seems to be the original and the best panacea. When you smile it makes you happier and healthier, and it has the same effect on those who cross your path – partners, friends and strangers. And as you do this, your own neurological and emotional patterns change for the better so that before you know it, you’re all smiles.

So keep that smile healthy and active. But if you are hiding it because it could use an overhaul, seek the advice of a reputable and skilled dentist.

omen go through a multitude of changes throughout their lifetime, and one part of the body that really starts a conversation is the breast. Too big, too small, too flat, too pointy, too saggy, big nipples, small nipples, too far apart, one’s bigger than the other – it’s a continuing dialogue. It seems the perfect breast is an elusive thing for many of us.

However while most women just live with their imperfections, for many wanting to live a ‘normal’ life, there is little choice but to make some decisions about surgical interventions. Breast surgery is about more than just ‘boob jobs’ to increase cup size so you can wear gorgeous lingerie. Women who have breastfed children often end up with two, three or four cup sizes difference between each breast. Floppy, droopy breasts can also be an issue for women of all ages who have undergone dramatic weight loss. They now have a whole new body and a great new sense of self, but often are ashamed of the excess skin left behind. So after months and months of hard work, find themselves venturing into the world of body contouring surgery, which more often than not requires breast augmentation.


If you are going through any of these experiences right now, and not sure what to do first, we have put together a handy summary of the process and the things you need to consider where breast reconstructions and augmentations are concerned.

. CHOOSE YOUR SURGEON The first and most important thing you will do is choose your surgeon. It’s crucial that you feel comfortable with whoever you choose, and that you feel like they really understand your expectations. A good surgeon will also be strong enough to gently tell you when your goals are unrealistic and why, then guide you to the right choices, ultimately ensuring you are informed enough to make sensible decisions yourself.

. DO YOUR RESEARCH Once you have the surgeon, the work begins. It helps for you to have done some research first – know what training your surgeon should have had, get a sense of the results you are looking for, and be informed about whether there are any risks involved for you. The first consultation you have with your surgeon will be very comprehensive so the better informed you are, the less overwhelming it will be.

. BOOK A FIRST CONSULTATION In the consultation, usually the surgeon will take a full case history, and do a thorough examination, including taking measurements of the various anatomical landmarks of your chest. They will assess the existing breast tissue, check for lumps, scars, and abnormalities, and assess the current size and positioning of the nipples. Next they will discuss your ideas, and try to get a good sense of your expectations. A good surgeon will advise you on the best size and shape of breast for your frame; many even have 3D imaging software and can show you how different sizes will look on your body. Try to maintain a sense of scale when choosing breast size and make sure that your selection will result in a shape that will be in proportion with the rest of your body.

. CHOOSE YOUR IMPLANTS Choosing what you want your new breasts to look like means taking a number of things into consideration. Your surgeon will lead this discussion and you should not be afraid to ask questions. In this part of the consultation you will make decisions about how the implant will be placed, and therefore where the incisions will be made. This is also when you will discuss what kind of implant you would like. Depending on the implant type, incisions can be made under the breast, around the nipple and in the armpit. Implants are then placed under muscle and existing breast tissue, above the

muscle or a combination of both. Decisions about placement will be based on your body type, your existing shape and what breast tissue is available. The positioning is important as it determines how the finished breast will look, once the implant is in place. This is an example of when the 3D imaging options are useful so patients can see what the results will be, rather than just imagine. Implants are made of a variety of materials and come in a range of shapes, and textures. These features all dictate how the implant will sit and last over time, as well as how it will behave in the event of injury or impact. Implants are filled with saline or silicone. The main distinctions between them are the way they feel and that silicone implants can come with a textured external surface. Implants can be teardrop shaped to mimic the natural breast shape, or round to give more fullness to the upper breast. They also come in different profiles – moderate, moderate plus, or high profile which is a measure of how much the finished breast will project from the chest wall when the implant is in position. The implant you choose will determine the final result – so choose wisely and listen to the pros and cons that your surgeon will go through with you.

. KNOW THE PROCESS: RISKS, PROCEDURE AND RECOVERY

Finally you should understand the logistics of your surgery – how long will you stay in hospital for? Will there be much pain? How long is recovery immediately after surgery, and how many weeks later will you be able to fully resume normal activities? Make sure you ask questions about all the possible risks and be sure to tell your surgeon everything he needs to know about your medical history.

. GO SHOPPING! Your surgeon should have your satisfaction as his primary concern and be advising you on the best approach that tailors the surgery to your individual body shape and needs. It is your body and you need to live with the results, so don’t rush into anything you don’t feel happy about, change surgeons if you need to, and make sure that you understand what is about to happen in advance. Then get ready to go shopping and enjoy your new shape!

The surgical community has differing opinions on the relative benefits of the smooth and textured varieties. Discuss these with your surgeon. Saline implants are silicone shells that are filled with saline (salt water) at the time of surgery, and so smaller incisions are required than with pre-filled silicone implants. The volume of the implants can also be increased at a later date through a valve.

PROFILES

Moderate-profile implants give just that; a moderate amount of projection so they won’t “stick out” of your chest so much. They are good for wide chests. Moderate-plus implants are ideal for smaller chests as they are slightly tapered and won’t create excessive width on the side.

Sub-glandular breast augmentation is where the implant is placed just below the breast tissue or over under the pectoralis muscle of the chest. This method is often used in women who are quite thin and lacking in breast tissue. This tends to soften the edge of the implant so it is not obvious.

It is a modern take on implant placement and is used in a variety of settings. One advantage of this technique is that positioning the upper portion of the implant under the muscle helps to hide the obvious “step” at the top of the implant, especially in a woman with thin tissue coverage, while positioning the lower portion under the glandular tissue of the breast helps take up any breast laxity or sag.

INCISION SITES

Most commonly, world over, breast implants are inserted through the inframammary fold, the natural crease under the breast. An incision of 3-5cm is made and the scar remains hidden under the breast. A periareolar incision is made half way around the nipple. The resulting scar may be virtually undetectable, but if you’ve not finished having your family, this technique may interfere with breast-feeding. Inserting implants through the armpit may be an option for women who are prone to keloid (raised, thick) scarring or for those who don’t want any trace of a scar on their chest, if you are an appropriate candidate of course. The surgeon may use an endoscope, which allows them to see through the whole cavity and guide the implant into the right place.

BREAST RECONSTRUCTION POST MASTECTOMY

Many women experience changes to their breasts during pregnancy and after breastfeeding. Some more dramatic than others. In Lorraine’s case, her breasts increased from a 34A before her first pregnancy to a 44FF after her second child was conceived. “It’s as if my breasts were built for breastfeeding, but after I stopped, I was left with this strange mass of saggy skin on my chest so when I was 40 years old, I decided to have a breast augmentation.” After feeding two children and ‘rewarding’ herself with new breasts, Lorraine was feeling pretty happy about everything, then in January 2009 Lorraine was diagnosed with breast cancer. “They took out the lump first, then I had an MRI. It was then that they located the primary that was a splattering of cancer cells on my right side. It was decided I would have chemotherapy first and then a mastectomy.” The chemotherapy finished in July and the mastectomy was scheduled for August. Lorraine was certain of her next step. “They wanted to remove only one, but I knew I would live with constant anxiety about the cancer returning so I thought it would be better to have them both removed. Also Dr Timms advised he could do a better job reconstruction if both breasts were removed.” Lorraine then underwent a radical bilateral mastectomy. As soon as that surgery was completed, Dr Timms began work on Lorraine’s reconstruction. He had seen her breasts before her mastectomy and was unhappy with the original augmentation she had received. Lorraine’s breasts were quite low set and the implants weren’t suitably placed. Dr Timms removed a section of Lorraine’s latissumis dorsi from her back

and brought the muscle around to the front to make the mound. The implants were then placed and volume added to the size agreed upon. Now Lorraine thinks her breasts look better than they ever have! Of course it took Lorraine some time to recover from her cancer treatment, and she took some time off work to get her health and emotional well being back to normal. “I was quite a fit person anyway so I think my body tolerated the chemotherapy really well but it’s still a challenging thing to have to go through. It’s been 5 years now this January and I feel great.” Lorraine recently underwent some ‘finishing off ’ surgery. After a recent visit with Dr Timms, he felt her breasts needed a little adjustment. Residual scar tissue was removed and because her tumour had been on the right side, there had been some shifting in that area, so he tidied that up also. Lorraine’s optimistic attitude surely helped with her recovery. “I wouldn’t have it any other way. I feel like a much stronger person from all of this. I met so many nice friends, and it has really made me tougher. I feel like I can conquer anything now. Nothing else can phase you when you’ve been through something like that.”

EXPERT What are the most common types of scars that you are presented with in your work as a dermatologist at Skin and Cancer Foundation Australia? In my capacity as a dermatologist specializing in the surgical management of skin cancers almost all the scars I encounter are surgical in nature. The most common scars that are of concern to the patient are either hypertrophic/ keloid scars and less commonly stretched atrophic scars. What issues do these present for the patient? The most consistent issue of concern for patients is the cosmetic appearance. Less commonly patients experience discomfort such as pain, tenderness or an itch that can last for quite a long time after surgery. How important is scar minimisation as part of your treatment protocol? (Ie. do you perform treatments where associated scarring is a potential consequence? Is scarring a concern expressed by clients? For a good number of patients, concern expressed regarding scarring is greater than discussions about the importance of removing their skin cancer, which is the reason for having surgery. Consequently, many patients are eager to engage in any measures at the pre-operative, intra-operative and post-operative stages. Postoperatively this can range from the least invasive measures, such as massage and the use of silicon-based gels, to more invasive procedures including various laser treatments and surgical scar revision. I routinely recommend massage and the use of a siliconbased gel as they frequently improve healing and hasten the process. Fewer patients elect to pursue more invasive post-operative procedures because of the risks associated with these procedures including the worsening or production of new scars.

How do you currently address scarring issues in-clinic? I approach the minimization of scarring by addressing the issues from before the patient has surgery to their post operative care. This includes: 1. Pre-operative issues: addressing intercurrent illnesses that delay healing or lifestyle choices such as smoking. 2. Intra-operative issues which largely relate to surgical technique. 3. Post-operative issues: including massage, silicon based gels, avoiding excessive physical exertion (many patients expect to exercise or work the day after surgery), wound dressing choices and corticosteroid injections into scars.

Dr Simon Lee is a Visiting Medical Officer performing Mohs’ surgery and laser therapy at The Skin and Cancer Foundation Australia, Darlinghurst and Westmead day surgeries. He also consults at Sydney Skin dermatology practice in the RPAH Medical Centre, Camperdown.

How does the use of Strataderm and Stratamed fit into your practice? I use Stratamed in patients at the earliest stage after surgery, usually within days while their wounds are compromised (ie not completely healed over), as is recommended by Stratpharma, though also in instances where secondary intention healing (ie like a graze) is the method of healing. I recommend Strataderm in all my patients once the wound has healed and usually from the time sutures are removed.

I’ve recommended silicon based gels or sheeting for many years though specifically Stratamed and Strataderm in the last 2 years because of greater ease of use over other silicon gels and less cosmetic impact than silicon sheeting. What feedback have you received from patients about their experience using Stratamed and/or Strataderm? Although not universal, the majority of patients report faster and better healing (as determined by reduced scarring) after using these products when compared with previous surgical experiences when not using these products.

Despite being the sacred vessel that brought most of us into the world, there is still a surprising level of shame and embarrassment where discussion about the vagina is concerned. Your mum has one, your sister, your daughter and your female friends. Guess what? Even your grandma has one. Yet there is still a stigma attached to it. Maybe we talk about it with our friends, but for the most part, vaginal aesthetics is not commonly a water cooler discussion topic. This patient was a young lady who had a labiaplasty for both functional and aesthetic reasons. A modified wedge excision of the enlarged labial minora was performed with lateral extensions to include folds from the paraclitoral hood. She had an excellent result, with minimal visible postoperative scarring and an improvement in symptoms and her overall confidence - which are the goals of a labiaplasty.

Although women may not necessarily see their own labia that often, sometimes the presence of something that ‘doesn’t feel right’ is enough to make us realise maybe there’s an issue. Anh’s patient realised when she was about 14 years old that there was something different. Her sister would comment that she could see her labia and she gathered from a few conversations that hers were different to other girls. This of course was not a huge issue during puberty, however as she got older, it started to become a problem. “I had the procedure mainly for cosmetic purposes, but as I got older the reasons became practical as well. I had lots of excess skin. The inside labia hung outside of my vulva about 1 inch on both sides. I also had some excess skin folds around my clitoris.” The patient had to be particularly attentive with hygiene as there was so much excess skin, and once she became sexually active, her frustration about the situation grew. “I was in a long term relationship and when we started having sex, we would regularly have to stop so I could maneuver myself and rearrange things. The excess skin really got in the way. When we had sex it would pull. It also affected my confidence as I was self conscious about it.” Anh’s patient had held off on surgery previously because she didn’t know enough about it and didn’t feel like it was an option for her. “I thought I was stuck like that. I had done my research and about 6 months before went to my doctor to discuss it. She gave me the speech about how everybody’s different, and how I was normal so then I hesitated again. I

waited another 4 months and was still unhappy so went back to my doctor for a referral.” Once she had decided on Anh Nguyen as her surgeon, the patient made contact with the clinic to discuss her options. She had a consultation with Anh who examined her and took some photos for reference, and then she had another consult with Anh 2 weeks before the surgery. “The surgery itself was quick and simple. It was a day procedure under general anaesthetic. I went in first thing in the morning and was out by 3pm. Anh removed the length off my labia and she also reduced excess skin around the clitoris. There was some pain after the surgery but not as much as I had expected. It was uncomfortable ore than painful.” The patient had stitches, which came out after a month and was able to resume sexual activity 6 weeks after the surgery. “I noticed the difference immediately. I didn’t have to spend so much time cleaning myself in the shower, and sex was much better. I was no longer self conscious so could relax and enjoy it for a change!” NIPTUCK Magazine

MOST OF US WANT TO LOSE WEIGHT BUT AS THE OBESITY EPIDEMIC WORSENS, WE SEE AN INCREASING NUMBER OF PEOPLE, AT INCREASINGLY YOUNGER AGES DEALING WITH WEIGHT ISSUES. The weight loss journey requires lifestyle changes, emotional engagement and diet and exercise adjustments, all of which mean huge changes in someone’s life. Surgery is often required, and few are prepared for the body they end up with – large folds of floppy excess skin and lots of stretch marks. Dramatic weight loss is life changing and with the right support you can achieve the results you want. Recently we sat down with Lindy Mumme who shared her emotional journey with us, and showed us that with the right support system of specialists and people who love you, you can completely change your own life. She married her childhood sweetheart, became a nurse and had two children who are now teenage boys. Lindy lived a mostly happy existence, until a family ski trip to NSW changed everything. Lindy Mumme had been big most of her life, and was a size 16 as a teenager when she first met her husband Sean. As she got older she put on more weight gradually, but never really felt it was an issue. It was a hurtful encounter with a ski instructor that shone a light on her situation and Lindy’s thoughts about her weight changed.

“I had never skied before and we spent thousands of dollars on a trip to NSW with 2 other families, to enjoy a skiing holiday together. We paid for some lessons and things took a turn for the worse on the 2nd day when I fell over yet again after many attempts to stay up on the skis.” It was the first time she felt her weight was a barrier to doing something she wanted to do. “I didn’t want the instructor to help me up because I didn’t think it was fair for him to have to lift my 108 kilos off the ground, but my consideration was lost on him, as he looked down on me in the snow and said ‘You know this takes effort’. I was humiliated and spent the rest of the holiday inside drinking lattes while my friends and family enjoyed the holiday.” It was on her return to Perth after that trip that Lindy quickly took action. She and Sean attended an information night about bariatric procedures, (gastric band insertion) and on contacting and meeting Dr Steve Watson, she was in surgery 6 weeks later. After a couple of nights in hospital and a few days recovery at home. Lindy was back at work the next week and living on a whole new learning curve. NIPTUCK Magazine

The band has a small tube inside that is increased or decreased in size by the addition or removal of fluid. The fluid volume inside the band determines the level of constriction and this is regulated to the individual and their gastric requirements and biological responses. A new position for the belly button is then made and the belly button is brought back through the skin.

The band is a ring, and within the ring is a balloon that gets liquid put into it to determine the level of constriction. Fluid is not added to the band for the first month but it still provides physical constriction. The band is fitted around the top part of the stomach and limits the volume of food that can pass into the stomach. “For the first week I ate only fluids – soups and broth. Then for the next few weeks I could manage thicker foods like scrambled eggs. By the end of the first month I was back to eating most normal foods, but much smaller portions. I do remember sitting there after my first month thinking ‘what have I done? This is too hard!’ I was constantly thinking about what I could eat. The reasons I was so big before were that I was eating too much. I’d eat while preparing dinner, while serving it, I’d eat what the boys didn’t eat, and then I’d sit down to the same size meal as my husband, who at 6 foot 2 is much better able to metabolise that volume of food. He’s in the navy and I would always lose weight when he went to sea because then my meals were the same size as my kids’ meals”. After a month of learning new eating behaviours, Lindy returned to Dr Watson and had her band filled for the first time. She also went in every couple of weeks afterwards and the constriction was assessed and adjusted to suit her individual gastric responses. The band constriction is managed through a metal port with a cover that connects to a tube. This in turn connects to the band, which is filled via a needle. Once things are regulated, the band constriction should be checked annually and patients can continue to live a normal life. Although in most cases - a new one! For many people who have undergone dramatic weight loss, shifting the kilos is only the first part of the journey. The emotional transformation and the body they are often left with are not always what they signed up for. Usually this means another round of work, with body lifting or contouring as the final stage of the process of truly ‘getting in shape’.

Lindy had the gastric band surgery in mid 2006 and by Christmas had dropped from 108 kilos to 91 kilos - she had lost 17 kilos in almost 6 months. The weight kept falling off, and once she had lost around 30 – 40 kilos, Lindy’s focus shifted from her weight loss to what her body was starting to look like. Her pre surgery jeans were a size 24 and she was now shopping for a size 10. Although she felt smaller, there were large folds of skin left behind that had to be folded into clothes to be contained. “When I was naked, I couldn’t look in the mirror. I hated it – everything hung and sagged. Pre weight loss my breasts were an E-F cup. Afterwards they were floppy empty sacks that I could roll up and fold into my bra. I still wore loose t-shirts over jeans to hide the rolls of excess skin and when I’d go swimming all the excess would float to the surface. I hated it and had to do something.” Lindy went back to Dr Watson and discussed her concerns with him and he recommended some surgeons to her that he thought might be able to help. Lindy worked through the list and by early 2009 she had an appointment with Dr Mark Duncan-Smith. Because she had stopped losing weight, he was able to address her surgical needs. “He asked me what I wanted done, and he asked Sean too! He assessed my stomach excess, my breasts and talked me through the different options and techniques. We decided on a radical abdominoplasty and a breast lift.” Lindy’s suture line ran from hip to hip and from the bottom of her rib cage to the pubic line. Her belly button was moved and her abdominal wall tightened. Excess skin was removed from her stomach and breasts, and her nipples were repositioned to higher on her newly shaped breasts. The whole procedure took around 6 hours and Dr Duncan-Smith removed 1.5 kilos of excess skin in total. Lindy was in hospital for about 5 days and though it was uncomfortable and there was a fair amount of pain, she knew immediately that the results were amazing. “I’ve been really lucky I had no post operative complications. After the surgery, I did everything I was told to do and my experience has really been a success story. I lost 60 kilos overall and aside from marrying my husband and having our boys, it’s the best thing I ever did.” Lindy knew she needed to make the changes but didn’t know how completely life changing it would be. “My confidence is much better, I’m not as withdrawn as I used to be when it comes to talking to people. Now that I can look at people in the face, I realised how much shame I was carrying around. I used to hate going into clothes shops and often would be ashamed of myself on behalf of Sean – believing his friends and colleagues were judging him because of me and my size. I’m very blessed – my family has been incredible. Sean has loved and supported me no matter what size I’ve been and one of the best moments ever was when my son gave me a hug and said ‘oh Mum - my hands go all the way around!’ It made it all worthwhile.”

Permanent Hair Reduction by David Ginges

Permanent hair reduction technology has given hope to people who’ve struggled with excess or unwanted hair on the face and body. Laser, Intense Pulsed Light (IPL) and Broadband Light (BBL) devices have provided the ability to permanently reduce – even totally eliminate - hair where it’s not wanted, on areas such as legs, bikini line, underarms, upper lip and chin.

David Ginges has enjoyed a variety of professions, from pharmacist to rock promoter and now software development CEO, but in 2004 he came across an opportunity that he has found one of his most rewarding. At the time permanent hair technology was usually performed by doctors and expensive. David found a way to make it affordable for everyone – now available at clinics around Australia.

Both laser and IPL treatments use light to cauterize the blood supply to the follicle but the light source with each is different so has a different action. “Hairfree has chosen intense pulse light for their removals, due to the painless nature of the treatment,” says David Ginges. “All of the Hairfree therapists are laser safety certified and their machines are medical grade IPL. Clients undergo repeated treatments until eventually the follicle is killed off and the hair no longer grows in that place.” Laser uses what’s called coherent light. This is a focused light with a single wavelength. IPL is non monochromatic (not single wavelength) and non focused. “There is a difference in the action of the two in that laser is much slower because it is acting on individual follicles, where IPL treats multiple follicles at once,” explains David. “Both are doing exactly the same job, but clearance rates can vary considerably depending on the type of machine being used and the type of treatment.” Clearance rate is a measure of the efficacy of the treatment in terms of what percentage is cleared during the course of treatment, or the number of hairs removed per area of skin. For significant results, several or more treatments spaced around a month apart are recommended so that hair follicles are targeted in their active (anagen) growth phase. Hair on

the face and body will be variously in one of three cycles – anagen, catagen (transitional) and telogen (resting) - and it’s only in the anagen phase that treatment is effective. But as with all scientific/medical technologies that provide powerful results, they are not to be toyed with. To seek permanent hair reduction from practitioners who are not adequately qualified, trained and experienced is to risk disappointing results at the very least and injury at worst. There has recently been some talk about burns from laser treatments. These usually occur because the intensity can be too high, or due to operator error. It could also be that the patient has not complied with the after treatment care requirements - not to exercise afterwards and not to go out in the sun and so on. In many cases it is client error that is to blame because assuming you have the right machine and a skilled operator, there is no reason a burn should occur. “If the setting [of lasers] is too high, it can burn the upper layer of the skin and cause blistering, perhaps eventually scarring and hyper- (dark discolouration) or hypopigmentation (loss of colour).” Mr Ginges reinforces the need for Australians to do their homework before seeking permanent hair reduction treatment, advocating IPL over laser. Only IPL is available at his clinics. “Therapists should be experienced and must hold current laser safety certificates to be properly qualified,” he says.

MICRODERMABRASION My experience with microdermabrasion: Microdermabrasion is a non-invasive, deep exfoliation technique used to remove the outermost layer of the skin. It has been proven to help with a wide variety of skin problems, including acne scars, hyperpigmentation, sun damage and diminishing fine lines and wrinkles. The most common treatment usually involves tiny aluminum oxide crystals, which are blasted across the skin and then sucked up through a vacuum, removing dead skin cells along the way.

I decided to undergo a course of microdermabrasion treatments to reduce the appearance of hyper-pigmentation caused by acne scarring. I had a consultation with Dr Ash Sankarayya and was told that 6 treatments 1-2 weeks apart would be the best course of action. The photos below were taken 7 weeks apart and the most obvious change in my skin was the reduction in redness and discolouration. Additionally, I found the treatments reduced my active acne, decreased my pore size and allowed my skincare products to work much more effectively as they were able to penetrate the skin deeper. I have continued to undergo maintenance sessions once a month to keep my skin looking clear.

LASER/ IPL HAIR REMOVAL Laser hair removal or Intense Pulsed Light (IPL) removes hair by causing thermal damage to the hair follicle. It involves using wavelengths of light to target the hair during an active growth cycle without causing damage to the skin. The treatment promises permanent hair reduction not necessarily complete hair removal. Over time you can expect to see up to 20% hair growth although much finer and at a slower pace. Success will largely depend on your colouring however it’s usually most effective on fair skin and dark hair.

The term “Mummy Makeover” has become a catch cry following pregnancy. Undeniably, the two body areas significantly affected after pregnancy and childbirth are the breasts and the abdomen. A woman is often told “not to worry” about their tummy after childbirth as “it will shrink” with breast feeding and exercise. In some cases this is true, but I see many women in my practice where this simply fails to happen. Pregnancy and breast feeding can leave women with a bulging abdomen, stretched skin, slumped posture and a broadened waist. They also often present with back pain, stress incontinence, and their breasts are sagging and have lost all upper fullness. Mummy Makeovers come in many and forms and depend entirely on the specific need of the woman. Abdominoplasty or liposuction, breast lift or augmentation or any combination of the same can be required to restore the body to the pre-pregnancy state. In nearly all of cases I will strongly recommend an abdominoplasty over liposuction. This is simply because the abdomen is significantly stretched through pregnancy and the excess skin and bulging of the lower abdomen is not fat but is instead caused by a lax muscle wall. Yes, there is a significant scar associated with an abdominoplasty, but I believe the long term benefits of this procedure far outweigh any procedure which does not address the crux of the problem. The abdominoplasty provides the surgeon the opportunity to do a functional repair of the muscle wall restoring core strength, back pain and incontinence issues. Conversely, liposuction is simply removing fat and hoping the skin will retract appropriately which has no functional benefit. Breast are a different story. Some women’s breasts shrink to an A or AA cup while others lose the majority of breast material and their breast sag like flat, fried eggs on the chest. Some manage to maintain their breast tissue but the whole structure of the breast is lost leaving the breast still a reasonable size but sitting way too low.  read more

At this point it is important to be mindful of whole body proportioning. Making the abdomen flatter and producing a waist makes the breasts automatically look larger. In other words, reducing the abdomen is like getting a breast augmentation for free. Obviously, an A cup breast is never going to look like a C cup, and in that situation an implant is required to restore fullness and projection to the breast. Larger breasts may require a breast reduction to restore shape and position. However, if the nipple sits low and the breast is quite collapsed, a breast lift is needed to elevate the nipple and tighten the breast mound. Although no volume is added or removed during this surgery it does produce a better, more projecting breast. Whatever size you were in a bra pre surgery, you will be the same post surgery.

It is my personal preference not to perform an augmentation and a breast lift (mastopexy) at the same time. Augmentation Mastopexy is an operation with quite variable results. I believe it is better to tighten a breast and wait six months to let it soften before augmentation. In my experience, the majority of my patients are happy with firmer, tighter and better positioned breasts and decide against the extra volume and weight. There is no point having an “S” lift, walk in walk out, one hour procedure done under a local anaesthetic in the rooms, if you need a more longer lasting outcome. Similarly, there is little point in have an upper eyelid blepharoplasty alone if you also need a brow lift as the results will be disappointing.

At consultation, a relevant history needs to be taken, paying particular attention to the words of the patients and what physical attributes are discussed. The medication the patient takes is very relevant to facial surgery. A bleed and a haematoma will detract from a good result. A thorough examination is carried out. I sit patients down in front of a mirror and show them what can be achieved with surgery. You can see the position of the skin, the direction of the tightening, the appearance of the neck, the position of the ears, the eyebrows and eyelids. What will happen to the hair, where the scar lines will be. Then follows a discussion of the operative procedure, it’s length of time, the placement of the scars, suturing and dressings. Is the operation carried out in hospital. under a local or general anaesthesic, and the duration of hospital stay. Your recovery period is discussed as well as what you can and cannot do, like driving, housework, return to work and exercising. Finally, you need to be informed about the potential complications of surgery before making any informed decisions. I take photographs to help me prepare my thoughts and outline my plans for surgery if and when the time arrives.

W

hen we were at school there were almost always one or two children who copped the nickname ‘wingnut’. Whilst some of us may have thought this was a term of endearment, for the individual concerned it was invariably not a positive nickname. It was, in all probability the start of an ongoing element of self-consciousness related to their prominent ears, sometimes stretching into and affecting their young adulthood. So what can be done - ears can be simply ‘pinned back’. I see a significant number of people who are concerned about the prominence of their ears and these patients all present at various stages of their life – I see all ages. Probably the most common presentation is during primary school when kids are perhaps, not so much given a hard time about their ears, but their ears are noticed by their school friends and their parents become concerned. The children become aware of the prominence of their ears and surprisingly, even at that young age, some are prepared to think about having something done.

In children this young, it’s important the child has an understanding of what is being done before proceeding. Sometimes parents can be a little keen (particularly if they have had their own ears done as children), but it is nice for a child to develop their own awareness so that he or she can more readily tolerate the procedure and the recovery period afterwards. Adolescence is another period when young people are very self-conscious and I see a number of them looking for otoplasty in that age group. It is not to say that adults do not come in looking for correction of their prominent ears. Adults come in through the full range of adulthood to talk about their ears and in fact I have seen and treated people in their 60’s. The operation itself is not simply the excision of skin but involves reshaping of the cartilage. It involves in particular reshaping of the fold in the cartilage that lies between the hole in the ear and the rim of the ear. This restructures the skeleton of the ear. The skin excision and or some sutures behind the ear hold the ear in place whilst the ear resets itself.

Once the operation is over, there is a period of time where a bandage needs to be worn on the head. This is almost turban-like in its extent and, in general, is worn for about two weeks. After that, simple care has to be taken of the ears for about another 4-6 weeks. The results of the procedure are generally very satisfactory. Complications are uncommon. The ears should look very natural and not appear as if they have had an operation. Personally I find it a very rewarding operation to perform. The results are instantaneous on the operating table and the patient’s excitement about the results is usually obvious immediately after the bandages are taken off. Girls who are shy to wear their hair up, or go swimming, are much happier to do so and young boys who don’t really want to grow their hair long often end up getting it cut reasonably short after the surgery.

Rachel had gone from a size 22 to a size 14 which she was thrilled about, however she’d been left with a big pocket of stomach and empty saggy breasts. “I’ve been really lucky to not have wobbly arms, and to get my waist back, but I was left with this big hanging stomach that I needed to get rid of ” It was just a short time ago that Rachel underwent a radical abdominoplasty and a breast lift with Dr Ian Timms, who removed 25 centimetres of skin from her stomach, repositioned her breasts and made sure her upper abdomen was generally tighter and more contained. Rachel had all of the surgery in one visit. The procedure took about 5 hours and she stayed in hospital for about 3 days. The pain was minimal and she was surprised to be up and walking around the day after the surgery. “I noticed the physical difference as soon as I woke up. I woke up and looked where my boobs used to be and I thought ‘oh hello!’ I feel great now. I just feel much lighter in myself overall. After the surgery there is some gradual recovery required before resuming normal activities such as driving or vacuuming, but Rachel is back to general household tasks and now feels she is as close to normal as she can be. “I really couldn’t believe that just 3 or 4 days out of the surgery I was showered, upright, and mobile. I now have a really natural womanly shape and feel like I did when I was 18. I’m a mother of a 12 year old and voluntarily putting myself under the knife was a massive decision because of the risks. I’m so glad it all worked and I’m really thankful that I’ve been given back an opportunity that I thought I’d thrown away. I never thought I’d look and feel as good as I do.”

Following a CT scan, photographic records and detailed 3D computerized planning, the sharp areas in Asanka’s upper jaw were corrected and four dental implants in accordance with the All-on-4 method were placed. This was all performed during one comfortable sedation visit. On Day 2 Asanka had a cosmetic fitting and on Day 3 he walked away with his new smile, never looking back.

After having a hysterectomy in her late 40’s, and starting menopause shortly after, Lesley started to experience hormonal changes. Labiaplasty, Vulvoplasty, Vaginaplasty, Designer Vaginas are all terms that come up in consultations from women of many different age groups who seek some form of enhancement and rejuvenation to the female genital area. The desire to look good and feel good extends today to areas other than our skin, breasts, face and body. For many women, feeling comfortable about how their female intimate parts look is just as important, and in some cases, more important, than how their breasts sit and how flat their tummy is for example. From my experience of seeing many women who seek cosmetic surgery of the female genital region, the majority are not looking for a designer vagina. The main reason for considering a labiaplasty is not vanity, but to feel less self conscious about how their vulva appears and so they can feel comfortable doing the activities they love doing. Certain sporting and physical activities, and wearing the clothes they love to wear are all hindered by discomfort and irritation that can occur for some women with enlarged labia minora. The most common procedure in the field of Female Genital Cosmetic Surgery is labiaplasty (or labia minora

reduction and reshaping), followed by labia majora reduction, labia majora enlargement, vaginal tightening and skin treatments to address pigmentation in the area. A Labiaplasty is surgery to change the labia minora. A labiaplasty can involve a trim excision (which is trimming off excess edge skin of the inner lips), a wedge excision (taking a triangular piece of excess inner lips and rearranging the remaining tissue to also improve the shape of the inner lips), or variations of both techniques, and also S-shaped excisions of excess labia minora to name a few more common ways to reduce the inner lips. Labia majora rejuvenation can either be to reduce the size of the outer lips by direct excision, or to enlarge the labia majora by restoring volume loss in that area that occurs with ageing and hence can restore proportion to the genital region (balance between larger outer lips and smaller inner lips is an aesthetic

ideal). Volumisation of the labia majora can be achieved with fat grafting or with dermal fillers. Vaginal tightening can be performed surgically or with the use of laser therapy to stimulate tissue contraction and tightening. Surgery of this nature is usually performed by a gynaecologist but the laser can be performed by a qualified doctor. Increasingly now, women and men are seeking treatments to lighten and brighten the skin in the genital region, particularly requesting the discoloration be corrected. AM I A CANDIDATE FOR GASTRIC BAND SURGERY?

Gastric band surgery is a common procedure that has benefited millions of people across the globe since it’s development in the late 1970’s. Its function is to restrict the amount of food that can be ingested by reducing the capacity of the stomach. This is done by placement of a band over a section of the upper quadrant of the stomach. The band has a small tube inside that is increased or decreased in size by the addition or removal of fluid. The fluid volume inside the band determines the level of constriction and this is regulated to the individual and their gastric requirements and biological responses. Gastric band surgery has been highly successful in treatment of the morbidly obese.

Gastric band recipients must meet certain criteria for this surgery to be considered as a solution. • Must be 18 years or over • Have a BMI of 30 or higher • Have been overweight for more than 5 years • Have made serious attempts to lose weight but have had only short term success • Do not have any other disease that may have caused your obesity • Are prepared to make substantial changes in eating habits and lifestyle • Are willing to continue being monitored by the specialist who is treating you • Do not drink alcohol in excess. Saline (water filled) implants have a greater risk of rippling where a wave like irregularity can either be seen or felt. The reason? She didn’t like wearing her hair up as she was self-conscious about her ears. After much thought and soul-searching, her parents allowed Justine to have her ears set back. The difference was immediate, and as you can, see hasn’t changed as the years have gone on. I read a lot of press about cosmetic surgery. There are a lot of clichés thrown about. ‘Be happy with who you are’. ‘Grow old gracefully.’ ‘Beauty is on the inside.’ I’ve been a plastic surgeon for over twenty years and I know a fair bit about it. I think about what I am doing, I try to help. My foundations lie in a very thorough surgical training in both reconstructive and cosmetic surgery, and over twenty years of experience. I have thought long and deeply about everything I do. I think about the effect it has on function, on cosmesis, and on the longevity of the results. It disappoints me when I read things about cosmetic surgery that come from people whose expertise is in

other areas, but who feel they are informed about something with which they have minimal contact and no training. When offering opinions on areas outside their own area of expertise, some influential or well-known people suffer from the ‘DunningKruger effect’. This basically states that ‘the less you know, the more certain you are’. Cosmetic surgery is an important part of medical practice. It genuinely helps people and can improve their confidence and sense of well-being. Like anything it can be serious when done poorly or when things go wrong. But this is unusual (although thought to be more newsworthy and therefore a misrepresentation of the truth). I do cosmetic surgery. I love it and I love the effect it has on people’s lives. I take it seriously. Done well on the right person, cosmetic surgery is a sound part of medical practice.

What are the issues for Massive Weight Loss (MWL) patients?

Successful massive weight loss is a life-changing journey. After years, even decades, of obesity, the patient has a new body and the psychological benefits are incredible. What is involved in Liposuction? Liposuction is the removal of fat from underneath the skin through a thin metal tube. The tube is introduced under the skin through small incisions, usually about 5mm in length. Different types of liposuction are simply variations on this theme. The fat may be removed by suction applied to the tube, or can be liquefied by ultrasonic wave energy converted to heat, before it is sucked out. What is Tumescent Liposuction? Tumescent liposuction involves injecting a saline solution (with local anaesthetic added) into the area prior to the liposuction. This assists in the technique, reduces bleeding and tissue damage and helps with post-operative pain relief. Tumescent liposuction is the most commonly used liposuction technique and is the standard method for liposuction used by ARC Plastic Surgeons. What are the benefits of Liposuction? Liposuction is an excellent treatment for removing small fat deposits, which diet and a healthy lifestyle won’t shift. It is best defined as a recontouring or

refining procedure, as opposed to a way of getting thinner. Once the fat is removed, the degree of skin contraction in the area is critical. What can’t Liposuction do? Liposuction does not remove any skin; tighten the overlying skin, or correct cellulite. If the overlying skin is firm, it will remain firm. If the overlying skin is loose, it may come looser. While this may be acceptable for some, it won’t be for others, so the possibility must be discussed. If the skin is likely to loosen or sag unacceptably, a skin-removing body contouring procedure may be preferable. What do you recommend? A consultation with a plastic surgeon is the first place to start. In liposuction, all techniques work well. It is the quality of the surgeon that makes the difference. The method chosen should be the surgeon’s preference, as the accuracy of fat removal is the key to a smooth result. And, of course, maintaining a sensible diet and healthy lifestyle are essential to a successful long-term outcome after liposuction.

‘What was it the surgeon said about….?’ How often do you attend a doctor’s office and forget half of what was said? Maybe it’s not such an issue when being prescribed medicines, as the instructions are printed on the label, but what about when considering surgery? That’s a big decision. Given the inherent risks with any surgery I would want to ensure I am fully informed before undertaking any procedure, and have the recall to weigh up the benefits against the risks. When it comes to elective cosmetic surgery, I would also want to know my plastic surgeon understands what outcomes I desire, and that we are able to have a clear discussion about what’s possible. Still, any consult can offer a lot to take in. Will I remember half of the discussion when I walk out the door? I’m well aware of possible ‘information overload’ with any cosmetic surgery consultation, and am always seeking ways to improve the patient experience. When it comes to breast implants, what do women want? Most women want breast fullness and projection, and breasts that complement their overall figure. For women who have always had small breasts, or for those who have lost breast volume through time, weight loss or pregnancy, breast implants reliably create fuller, larger breasts.

As bone loss starts to occur with ageing, the face can take on a more sunken appearance. This process affects all parts of the face and neck - it applies to the brow, the eyelids, the mid face, the lower face and neck, and the nose. Even the ears change as we age! I have a particular interest in facial surgery and rhinoplasty and my skills in facial trauma really help me to manage issues related to these areas.” Working from the foundations up, Mr Morgan believes in a dynamic approach. “We can add things on top of bone so implants, for example, can be used to change the bony shape. Or we can strip everything back from the bone, move it and reattach it, to create a pulling effect on the structures of face to make them more youthful. The correct result can be achieved by separating out the different levels of the face and getting them to pull in different directions and create different tensions.”

An important skill you want your surgeon to have is the ability to maintain harmony between all of the parts of the face and neck. There’s no point having a face that looks 40 with a neck that looks 60. “The lower face and neck should be considered as one region but you can do a procedure isolated to one area. I am lucky to have been able to meet and train with and collaborate with experts all over the world in different aspects of female pelvic reconstructive surgery. There are a range of pelvic disorders affecting millions of women, and I am motivated in my work to strive to improve the quality of life of women. Many women are self conscious about their gynaecological issues. How would you encourage them to seek treatment? Every individual gets to a point where things are getting worse rather than better. It’s normal to feel embarrassed and afraid, but % % $ you are already on the way to a solution. Patients often comment after surgery that they wish they had done it years ago. How do you make women feel at ease in consultation with you, when discussing issues of an intimate nature? It’s important to listen to patients without judgement and communicate with them in a way that lets them know you are on their side. I have to ask very intimate questions to get to the source

I always explain this and let them know that only ! ! % that women, though often initially hesitant to volunteer intimate information, will respond very positively to direct, well targeted questions. With Pelvic Floor Disorders (PFD) affecting so many women, how do women know what they should seek treatment for and what is ‘normal’ or ‘just part of being a woman’? The term PFD covers a range of disorders all relating to the muscles and connective tissue of the pelvis, and resulting from the impact of physiological and traumatic change to pelvic organs. It’s important to be familiar with what your symptoms are and how they bother you. Do not let others patronise you and tell you ‘it’s part of being a woman’. You need to recognise what is normal for you and ask yourself if it is affecting your quality of life. For example, leakage of urine during exercise is not normal but many women put up with it for years before seeking help. During this time they may rationalize the abnormality as their new ‘norm’ until they have an accident in an embarrassing situation.

Many patients with unsightly veins are keen to have treatments for cosmetic reasons. Varicose veins usually look aesthetically unattractive to the eye – they can bulge out, be deep red or dark blue in colour and in most cases become highly visible. Some people’s varicose veins come from genetic causes. In other cases, they have received a GP referral after presenting to a clinic with aching legs. This is a classic symptom experienced by patients with varicose veins. However there are also serious health risks to consider with untreated varicose veins. Complications such as ulceration can occur and symptoms preceding this include changes to the appearance, size or shape of the veins, as well as itchiness, swelling, and eczema. Any changes like these should be checked and treated. Treatments can address the superficial spider veins that are commonly seen as thin coloured lines close to the surface of the skin, but there are also focused treatments such as UGS or Ultrasound Guided Sclerotherapy for the larger bulging veins.

Sophie had always been disappointed in her breast size, and for many years thought about getting an augmentation. Eventually, after seeing a few of her friends go through the process with great success, she decided to take the plunge in September 2013.

That size is much better than what I expected and they feel really natural.”

“I had always wanted an augmentation. I was a 10D before so I had decent boobs but I like big breasts.” Sophie’s existing breasts were teardrop shaped and the look she wanted was one that had more fullness in the top. “I wanted more roundness at the top of my breast and I also wanted to go bigger. I knew I had to do my research, so started looking for the right surgeon. I had heard a lot of positive feedback about Dr Phil Richardson so decided to go with him.” Sophie’s choice of surgeon was important, as she really wanted someone who understood what she wanted and supported her choice. “I felt a couple of other surgeons were quite conservative when I talked about my dream breasts. When I said what I wanted they commented that it might be too big, usually without discussion. I wanted a surgeon who would listen to me and discuss the possibilities, who could articulate my wishes and understand my needs.” Sophie had discussions with Dr Richardson and together they decided on a size 10F. This was within the size range Sophie was looking for, but was also a realistic choice for her frame and stature. Sophie’s implant was round, and textured, with a moderate profile. “I was 100% happy with the results.

Because Sophie had a reasonable amount of breast tissue to begin with, the addition of the implants did not result in a hard breast. “My breasts were not this hard before, but now I feel like they are nice and firm which is what I wanted. It was very exciting shopping for new bras and I love that full feeling in my bikini now. Before the augmentation I wouldn’t wear backless tops, but now I have greater options with clothing as I don’t always have to wear a bra. I can wear strapless tops and shoestring dresses comfortably.” Sophie came through the surgery very well, with sensation returning at 90% almost immediately. She had no pain at all after a few weeks and returned to exercise 6 weeks post operatively. “I was able to do everything – weights, cardio, boxing, running. I just worked back into it gradually and increased the intensity week by week until I was back to 100%” Sophie has received a lot of great feedback since having her procedure. “I get compliments all the time, especially on the feel of them – other women want to see what they are like and when they touch them are always surprised at how natural they feel. I’m really happy with the results and have so much more confidence now. Maybe after I have children I will go back for a lift if the fullness drops after breastfeeding but if they stay the way they are I’ll be thrilled.”

The second technique is for people with a little crowding in their front teeth. Many adults had braces when they were teenagers, only to find that some misalignment of the front teeth has started to creep back over the years. There was also a huge difference between full American-style facelifts and the much lesser S-Lift procedure which cosmetic surgeons were doing, so something was needed between the two. A lot of people used to think of a facelift as major surgery where you came out looking like you had been in a wind tunnel. When loose skin occurs around the eyes, eyebrows, jowls and neck, surgical approaches have become necessary to effect maximum improvement. Loose skin in the neck, jowls, mid-face and cheeks is best improved using face-lifting techniques. A full face-lift procedure has been the standard operation for many years, but when only the neck and lower face needs lifting the less radical procedure MIFACE in the lower face can be performed under a general anesthetic at a Day stay hospital. Since the MIFACE procedure was first developed 10 years ago it has been expanded to include the MIFACE Plus, Extended MIFACE and Extended MIFACE Plus to cater for individual needs.
