E
very January, our dental community has a unique opportunity to come together and experience excellent continuing education and enjoy collegiality throughout shared meals and social events. Your convention committee chair, Dr. Pat Kmet, and her army of dedicated volunteers have been planning tirelessly for many months to bring you the Manitoba Dental Association 133rd annual convention. The convention will be held January 27–28, 2017 at the RBC Convention Centre in Winnipeg. Our convention theme, “Dentistry and All that Jazz” will showcase the many genres of jazz music. This year we have an outstanding line up of speakers, featuring key opinion leaders from both Canada and the United States, and of course we will feature our respected local experts. Keynote speakers include: Dr. Michael DiTolla highlighting restorative dentistry, Dr. Steve Rasner focusing on clinical excellence, Dr. Tim Bizga presenting on dentistry for the oral health team, and Dr. Wenche Borgnakke speaking on dental hygiene. In addition to these, many more impressive speakers will be presenting cutting-edge education. There truly will be something for everyone. The trade show continues to grow, attracting more manufacturers every year. This year will be our largest trade show ever. The exhibitors are looking forward to showcasing the latest in dental technology and equipment. As always, great deals, incentives and great prizes can be found at the Manitoba Dental Association trade show. One of my favourite parts of the convention are always the social events. We have planned great food and fun entertainment. Friday night, we will be enjoying Papa Mambo and the sounds of their Latin jazz. Entertainment, food and spirits will be flowing in the Delta Hotel Ballroom at the annual social. Saturday night, the culmination of the convention, we will celebrate at the President’s Gala. A gourmet meal, fine wine and signature cocktails are on the menu. Nadia Douglas will provide entertainment throughout cocktails and dinner; and then we will dance the night away to the fabulous Danny Kramer Dance Band. The evening is always a highlight. Member engagement is an important aspect of the MDHA and we are pleased to offer exclusively to our members a networking opportunity on Friday, January 27th, from 8:30 am – 9:30 am. This informal event will give our members the opportunity to engage with their elected board members, connect with the Canadian Dental Hygienists Association through the Manitoba CDHA representative and enjoy connecting with colleagues from around the province. t is my sincere pleasure to welcome you to the 133rd annual MDA Meeting and Convention held at the RBC Convention Centre, January 27 – 28, 2017. This year we’ve adopted a colloquialism that we all use everyday‚ “all that jazz”, and applied it to our convention theme “Dentistry & All That Jazz”.
What is jazz you ask? Well, it seems that no one can agree on a working definition. Part of the reason is because jazz has always been and remains today a living art form, ever changing and ever growing. Some would argue that dentistry is a living art form balancing science, the human spirit, and art – ever growing and changing. Well, Ray Conniff said, “If you believe in your art and you love what you do, that energy will go out and people will respond.” So, we have combined the two, dentistry and jazz to inspire, motivate and entertain you. The convention committee has worked hard to create a dynamic and engaging weekend for the whole dental team. The MDA Convention also offers you those much-needed CE points to augment your professional development. A sample of what you can expect on Friday includes Dr. Michael DiTolla and Dr. Meredith August. The MDAA has lined up Dr. Tim Bizga and the MDHA is highlighting Dr. Wenche Borgnakke. Top 10 Crown and Bridge Mistakes: How to prevent and correct them Friday, January 27 • 8:30 am – 12 noon Dr. Michael DiTolla This seminar is a combination of photographs and live patient treatment video to illustrate methods to improve your crown and bridge restorations. This information applies to all-ceramic restorations as well as traditional ceramo-metal restorations. There were approximately 45 million crowns done in the U.S. last year, and according to Dr. Gordon Christensen, almost 90 per cent of the impressions that dental laboratories received did not have clearly visible margins around the entire preparation periphery! A procedure performed this often clearly needs to be more predictable, and thus more profitable. Because of his unique arrangement with Glidewell Laboratories, Dr. DiTolla has access to the thousands of impressions and models they receive on a weekly basis, and he has done an exhaustive study of the preparation and impression techniques of American dentists. You will see the good, the bad and the ugly; and the compromises a laboratory has to make in these situations. After completing this program, the dental professional should be able to recognize the various treatment modalities available to the modern practitioner to help them restore the mouth in an esthetic manner; be familiar with multiple preparation techniques that allow practitioners to prepare teeth in a predictable manner to achieve optimum reduction, esthetics and function; list the most acceptable impression technique for a variety of fixed prosthodontic scenarios, and recognize an optimal impression and how to produce one routinely; and, list the indications and specifications for the new class of all-ceramic restorative systems, and compare these to esthetic PFM restorations.
Digital Impressions and Restorative Dentistry Friday, January 27 • 2 pm – 4:30 pm Dr. Michael DiTolla This course is a combination of lecture and live patient clinical video to demonstrate the use of digital impressions for crown and bridge procedures. Is this technology “ready for prime-time,” or is it better to hold off on the purchase of a unit? Dr. DiTolla is the only dentist in the U.S. who owns examples of all of the commercially available digital impression units, in addition to one pre-market system. As a result, he is uniquely qualified to compare and contrast the features, benefits and shortcomings of the major systems. Dr. DiTolla shows real-time digital impressions being taken from start to finish to help dentists decide whether one of these systems is right for them. Model fabrication and chairside milling units will also be discussed. After completing this program, the dental professional should be able to recognize the various digital impression systems and list the features, benefits and shortcomings of each; be familiar with the hard and soft tissue requirements for making a digital impression versus making a conventional impression; be familiar with the time and cost requirements for these systems, and how a typical digital impression system would fit into their practice overhead structure; and, recognize how models are generated from these systems, and how a digital impression scanner can be used in a modeless system with a chairside milling unit.
Dr. Michael DiTolla is a 1988 graduate of University of the Pacific Arthur A. Dugoni School of Dentistry and was awarded his Fellowship in the AGD in 1995. In 2001, he became director of clinical education at Glidewell Labs and was instrumental in the clinical development of BruxZir Solid Zirconia. Dr. DiTolla has been the editor of Chairside magazine since 2006, a quarterly publication that frequently presents his own clinical case studies, and he also writes a bimonthly column on restorative dentistry for Dental Economics. Dr. DiTolla received the 2011 Dr. Bicuspid Dental Excellence Award for “Most Effective Dentist Educator”. He also became a clinical evaluator for CR in 2004, and became a project director in 2013. He is currently giving “Dentistry Update” lectures for CR and is an advisor to the CR Board of Directors.

Periodontitis and Diabetes: A Two-way Relationship Friday, January 27 • 2 pm – 4:30 pm Dr. Wenche Borgnakke This lecture will cover: the effect of diabetes on periodontal tissue and the effect of periodontitis on diabetes control. Periodontal treatment: the effect on general diseases and the links between the mouth and the rest of the body. What do I tell the patient?
Dr. Wenche Sylling Borgnakke is a senior research associate in health sciences/clinical lecturer in the Department of Periodontics and Oral Medicine at the School of Dentistry at the University of Michigan. She earned DDS and PhD degrees from the Royal Dental College in Copenhagen, Denmark, and a Master of Dental Public Health degree from the University of Michigan. Dr. Borgnakke has published seven book chapters and about 70 papers, and is a reviewer for 28 scientific journals. Her latest chapter is called The Travelling Oral Microbiome. Dr. Borgnakke has lectured nationally and internationally — in countries such as Canada, Cuba, The Netherlands, Norway, Pakistan, Saudi Arabia, and the United Arab Emirates. Dr. Borgnakke is a member of the CDC/AAP Workgroup on Periodontitis Surveillance, and the American Academy of Periodontology awarded Dr. Borgnakke a 2012 Special Merit award, the 2013 Clinical Research Award for a 2012 paper in the Journal of Dental Research on periodontitis in the U.S., and the 2016 Clinical Research Award for a 2015 paper in the Journal of Periodontology on the prevalence of periodontitis in the U.S.

Pearls and Tips for Oral Surgery Friday, January 27 • 8:30 am – 12 noon Dr. Meredith August The oral biopsy – pearls and perils. This discussion will focus on indications for biopsy, proper biopsy technique and how to avoid problems with artifact and sampling errors. We will discuss these topics and follow with illustrative cases that demonstrate both proper and improper techniques. At the conclusion of this hour, practitioners will have a better sense of when a biopsy is warranted, how to approach more diffuse lesions to ensure the biopsy is representative, how to biopsy intrabony lesions to improve diagnostic accuracy, and the best techniques to use in various anatomic sites. Human Papilloma Virus. What does the dental practitioner need to know about HPV infections? What is the approach when patients present with squamous papilloma lesions? What are the high-risk sites and the high-risk subtypes? How do we counsel patients in terms of sexual transmission and practices? What is the relationship between high-risk lesions and malignant transformation? How common is HPV-related squamous cell carcinoma and how is its treatment different from other lesions? A thorough discussion of HPV vaccination, indications and efficacy will also be included. When is a cyst not a cyst? We will review a spectrum of intrabony lesions and discuss features of each, both radiographically and clinically. We will focus on the KCOT and discuss genetic markers and newer treatment strategies. This will also be a case-based discussion looking at multiple radiolucent lesions, how they present and how they were treated. Update on medication-related osteonecrosis of the jaw. We will discuss updated classification schemes, treatment recommendations and general caveats in treating this patient population. We will expand the focus of discussion to include not just bisphosphonate preparations but also newer antiresorptive agents. Review of newer anticoagulants. Most of us have a comfortable approach to managing patients on Coumadin, but what about the host of newer oral agents? This discussion will try to unravel these medications, their mechanism of action, how they need to be managed and what drug interactions need to be considered.
Head and Neck Cancer Friday, January 27 • 2 pm – 4:30 pm Dr. Meredith August In terms of patient assessment regarding oral cancer, we will examine a general approach to these patients including history taking, physical examination, biopsy, imaging and treatment considerations. Special attention will be paid to the neck examination and how the finding of positive neck nodes changes treatment. We will also have full discussions of lesions with malignant potential and how to focus on the earliest possible diagnosis. This will be clinically-based with illustrations of dyplastic lesions, early carcinoma and keratinizing dysplasia. Conventional versus newer treatment approaches will be examined. What is on the horizon in terms of molecular markers for disease, photodynamic therapy, organ preservation and chemoprevention? We will also present cases to illustrate improvement in oral rehabilitation after ablative cancer surgery. These cases will show how large surgical defects were successfully ablated, reconstructed and, ultimately, dentally rehabilitated.
Dr. Meredith August is known for her body of work on diagnosis and reconstruction in patients with cancers of the oral and maxillofacial region. After receiving her BA summa cum laude in English literature from Union College, Dr. August attended the Harvard School of Dental Medicine, where she received her DMD with honours. She completed the Harvard-MGH residency in oral and maxillofacial surgery, earning her MD from Harvard Medical School. She is a diplomate of the American Board of Oral and Maxillofacial Surgery. She is currently associate professor of oral and maxillofacial surgery at Harvard School of Dental Medicine. Fluent in Mandarin, Dr. August maintains a clinical practice that spans the full scope of oral and maxillofacial surgery, including orthognathic surgery, dentoalveolar surgery, dental implants, and oral cancer. She was coeditor of Treatment of the Female Oral and Maxillofacial Surgery Patient, an issue of Oral and Maxillofacial Surgery Clinics (2007).

This dental assisting continuing education course is designed to teach dental assistants and EFDAs the skills necessary to better support their dentists in the everyday routines associated with a general dental practice. A lecture-based program, coupled with everyday practice essentials is the basis of this class. The topics covered will include basic material science, procedural protocols and topics related to patient care and treatment. Dr. Timothy M. Bizga is a general dentist practicing in Cleveland, Ohio. His practice focuses on comprehensive care, with special interests in implants, cosmetics and facial aesthetics. His background in dentistry is lengthy and diverse. Once a former chairside assistant, he also worked as a dental lab technician, making his perspective unique among others in the field of dentistry. He graduated from St. Louis University with a BA in biology, before receiving his DDS from the University of Michigan School of Dentistry, at which time he was the recipient of the Francis B. Vedder award for excellence in prosthodontics. He is currently a member of the American Dental Association and a Fellow in the Academy of General Dentistry. He is a graduating member of the Whitecap Institute, where he received advanced implant training. How to Steal From a Dentist Saturday, January 28 • 9 am – 12 noon Mr. David Harris “Nobody knows you like family”. Many dentists treat their staff like members of their extended family, and then are surprised and hurt when a member of this trusted “family” steals from them. If you are a dentist, the likelihood that you will be the target of embezzlement during your career is greater than 60 per cent. That’s right. GREATER than 60 per cent! In this interactive presentation, attendees will be given some alarming facts about this serious and personal crime, as well as proven strategies to empower them as they begin to recognize hotspots for embezzlement in their practice and how to effectively take back control of their office’s financial health and well-being. Using actual dental embezzlement case files, attendees will be able to evaluate the cases presented and then interact with the presenter as he discusses the myths and misunderstandings surrounding dental embezzlement and guides participants through basic, yet effective strategies to avoid being a part of the “60 per cent” statistic.
External Threats to a Dental Office Saturday, January 28 • 1:30 pm – 4:30 pm Mr. David Harris It’s a dangerous time to own a dental practice. Many people with criminal intent consider dental offices to present ideal targets, and in the “Internet era” an alarm system alone isn’t sufficient to protect you. This presentation examines often overlooked external threats to the dental office including: identity theft; medication scams; phishing, and Internet hacking (does not deal with staff embezzlement).
David Harris founded Prosperident, the world’s largest dental embezzlement investigations firm, in 1989 after what could only be called a “happy twist of fate”. Under Harris’ leadership, Prosperident has expanded over the past quarter-century to become a team of specialized investigators, consulting on hundreds of frauds annually. Harris’ business success, coupled with his own experiences as a miscreant youth, have given him a unique insight into the mindset and actions of embezzlers. He is passionate about sharing this ever-growing wealth of knowledge with dentists and dental specialists. He has lectured at several universities in both the U.S. and Canada in the faculties of business, law and dentistry. Harris is a CPA with a graduate degree in applied mathematics and a licensed private investigator. He is dual certified in fraud investigation, possessing both the Certified Fraud Examiner designation from the Association of Certified Fraud Examiners and he holds a Certified in Financial Forensics designation from the American Institute of CPAs.

Aging and Geriatric Oral Health Problems Saturday, January 28 • 1:30 pm – 4:30 pm Dr. Christian Caron This session will cover the following: • Aging processes of the mouth tissue and common oral health problems related to loss in autonomy; • Health issues in the elderly that impact dental care which include: main health issues, polypharmacy basic principles plus anticoagulants, anxiolytics, analgesics, antibiotics, management and use, Xérostomia; • Dementia, including behavioural management, plaque management and chemotherapeutic agents use (high-fluoride agents), dysphagia; and, • Long-term care, from oral health prevention to treatment: barriers, vision, organizing dental care to loss in autonomy, mobile care, interprofessionnal collaboration (early detection of oral disease with tools).
Dr. Christian Caron is associate professor in the faculty of dentistry at Laval University since 2001. He founded in September 2000 a specialized training program in geriatric dentistry. Caron holds a medical doctorate dental (1985), a bachelor of laws (2003) from Laval University, a fellowship in geriatric dentistry and a research doctorate from Harvard University (2000). Dr. Caron specializes in treating people with loss of autonomy and who are medically compromised, and offers a portable dental service for people with mobility difficulties. He is the founder and director of the Centre of Excellence on oral health and aging at Laval University and head of geriatric dentistry services in the health and social services of Quebec-South Centre. He is a researcher at the Geriatric Research Unit of the University of Laval (URGUL) in the research network in aging and oral health in Quebec.
Hand Disorders in the Dental Practice Saturday, January 28 • 10:30 am – 12 noon Ms. Lori De Pauw A variety of instruments are used in a dental practice on a daily basis but your hands are unquestionably your most important “tool”. This session will provide participants with tips and suggestions to keep their hands in working order. Participants will gain an understanding of common disorders affecting the hand as well as prevention and management strategies with a focus on the dental practice environment.
Lori De Pauw, BMRPT, MSc, is a physiotherapist with over 35 years experience in both hospital and private clinic settings. De Pauw’s special area of interest and the majority of her clinical practice experience as well as the focus of her master’s thesis research have been in the area of hand and arm rehabilitation. De Pauw is a former lecturer in biomechanics and in hand disorders at the College of Rehabilitation Sciences, University of Manitoba. She currently practices at Green Hand to Shoulder Physiotherapy.

Managing Thrombolytics, Bisphosphonates and Infections Saturday, January 28 • 9 am – 10:30 am Dr. Kurt Scherle There are an endless stream of new blood thinners on the market of which dentists and dental hygienists need to be aware. Depending on treatment, these medications may need to be stopped, reduced or maintained. This lecture will clarify the medications that the dental team should recognize and provide a simple approach to the management of these patients in office. Surgical technique and local measures of the dental extraction will also be discussed. Dr. Kurt Scherle graduated from the University of Manitoba Faculty of Dentistry in 2009 and immediately completed a hospital internship and residency in oral and maxillofacial surgery at Health Sciences Centre. He obtained fellowship from the Royal College of Dentists of Canada in 2014. Dr. Scherle maintains a private practice where he focuses on dentoalveolar surgery, dental implants, bone grafting and orthognathic surgery. He is involved with the National Dental Examining Board and instructs part time in the undergraduate oral and maxillofacial surgery clinic at the college of dentistry.

The Travelling Oral Microbiome Oral-Systemic Health in a Nutshell - A Preview of the Conference Presentation By Wenche S. Borgnakke, DDS, MPH, PhD
N
ovel technology and superfast computers are enabling exploration of the oral microbiome, the collective population of the oral cavity of bacteria, virus, fungi, and archaea, that has been referred to as the oral microflora or oral microbiota. Over 700 bacterial species — possibly as many as 1,000 — reside in the mouth; but identification of more than half is lacking and even less is known about their function. The crucial importance of the mutual effects of the oral microbiome and the rest of the body is drawing increasing attention by researchers, clinicians, and the public alike. This presentation will provide an overview of the scientific evidence for characteristics of the oral microbiome, including its constantly changing composition and abundance; its diversity within the oral cavity; and its importance relative to the total human microbiome. Mechanisms used when oral microbes travel within their host (domestic travel) and from one person to another (international travel) will be described, along with some of the consequences of travel and colonization of remote locations by members of the oral microbiome. Examples of such recently discovered or confirmed consequences will include bacteria that harbor antibiotics resistant variants of common oral microbes, or contribute to various cancers

Dr. Wenche Sylling Borgnakke is a senior research associate in health sciences and clinical lecturer in the Department of Periodontics and Oral Medicine at the School of Dentistry at the University of Michigan. She earned DDS and PhD degrees from the Royal Dental College in Copenhagen, Denmark, and a Master of Dental Public Health degree from the University of Michigan. Dr. Borgnakke has published seven book chapters and about 70 papers, and is a reviewer for 28 scientific journals. Her latest chapter is called The Travelling Oral Microbiome. Dr. Borgnakke has lectured nationally and internationally — in countries such as Canada, Cuba, The Netherlands, Norway, Pakistan, Saudi Arabia, and the United Arab Emirates. Dr. Borgnakke is a member of the CDC/AAP Workgroup on Periodontitis Surveillance, and The American Academy of Periodontology awarded Dr. Borgnakke a 2012 Special Merit award, the 2013 Clinical Research Award for a 2012 paper in the Journal of Dental Research on periodontitis in the US, and the 2016 Clinical Research Award for a 2015 paper in the Journal of Periodontology on the prevalence of periodontitis in the U.S.
also in organs distant from the oral cavity and adjacent areas. Naturally occurring compounds, that exhibit antimicrobial properties and thus might be used as alternatives to antibiotics, will also be mentioned. Such substances modify pathogenic effects of commensal oral bacteria that experience abundant overgrowth due to an unusually conducive environment during disturbance of the normal homeostasis. Development or discovery of such alternatives is important in the quest for reducing antibiotics resistance due to abuse and overuse of antibiotics while keeping the microbiome in its healthy equilibrium. PERIODONTITIS. WHAT IS IT, WHO HAS IT, AND WHY IS IT IMPORTANT?
When clinicians see periodontitis, they know it, most of the time. Any treatment plan will take into account all the information the dental professional has regarding the patient’s characteristics, adherence to home and professional oral hygiene measures, attitude and beliefs, preferences and wishes regarding keeping teeth and good oral health. However, when research is conducted and especially during data collection for population surveillance occurs, such knowledge is not available. Researchers define by their own criteria what they will classify as various categories of periodontitis, based on clinical and radiographic, and possibly other measures. This situation makes it difficult to compare results from different research teams, a fact that is often overlooked when interpreting scientific evidence. The periodontitis case definitions created by the Centers for Disease Control and Prevention (CDC) and the American Academy of Periodontology (AAP) have collaborated in a workgroup since 2003 with yours truly as a member. The group has designed periodontitis case definitions for no, mild, moderate, and severe periodontitis and these definitions are increasingly used as the global standard. Additionally, a set of eight self-reported periodontitis questions has been developed based on previous research, which correspond well to the clinically diagnosed periodontitis status, especially for the severe type. Six years (2009–2014) of data collection during the nationally representative population-based National Health and Nutrition Examination Survey (NHANES) in the U.S. included full-mouth periodontal probing at six sites per nonthird-molar teeth. When applying the CDC/AAP workgroup periodontitis case definitions to these data, it was for the first time possible to accurately assess the prevalence of periodontitis in adults 30 years or older. Results from these
NHANES data will be presented. Current scientific evidence for associations between periodontitis and systemic diseases and conditions will be summarized, including diabetes mellitus, obesity, metabolic syndrome, atherosclerotic cardiovascular disease, rheumatoid arthritis, kidney disease, certain types of cancers, possibly dementia/Alzheimer’s disease, and adverse pregnancy outcomes. It is increasingly realized that the common, underlying mechanisms for the links between these diseases are the host inflammatory responses. These responses can be triggered by oral microbes just as well as external infectious agents, pollutants, and toxins. Rather recently has it become evident that added dietary sugar triggers inflammatory responses and also acts like a toxin, contributing to non-alcoholic fatty liver disease – in addition to obesity and caries. Special attention will be given to the evidence based two-way associations between periodontitis and diabetes mellitus as both diseases are prevalent. In particular, the current controversy regarding the effect of non-surgical periodontal therapy on glucose control in Type 2 diabetes will be discussed. As well, the evidence for the systemic effects of such periodontal therapy will be summarized, as it pertains to each of the diseases and conditions mentioned. There are currently groups of well-meaning dental professionals who venture beyond what the science can support regarding the claimed effects of periodontal treatment on general health, and the public often is aware of some of these claims. Various examples of how and what to tell the dental patients about the links between oral health, mostly periodontitis, and general health will be provided.
