Required medical information for new students
Medical requirements are completed through your My Wellness Portal under the My Apps tab on MyDenison.
ACCESSING THE MY WELLNESS PORTAL:
1. Log in to MyDenison (my.denison.edu)
2. Click the My Apps drop-down at the top of the page
3. In the drop-down list, select My Wellness Portal
4. To log in, enter your MyDenison username (BigRedID) and password
5. Required student forms can be found by clicking Medical Clearance on the left
Wellness Examination Form
This form must be completed by a licensed Physician, Nurse Practitioner (NP), or PHASE Physician’s Assistant (PA). This form cannot be completed by a family member. Once completed, upload your form in your My Wellness Portal. 2
Note that this form requires that a physical examination needs to have taken place on or after November 1, 2021. If you have had a physical within this time frame, most ofces will allow you to drop of the forms to be completed without the need for another appointment. The medical clinician should be able to complete the forms and date it for when the physical examination actually took place.
Authorizations
18 years of age or older?
R Sign the electronic Authorization Form through the My Wellness Portal
17 years of age or younger?
R Print the Minor Authorization Form to be signed by a parent or legal guardian
R Upload the signed Minor Authorization Form to the My Wellness Portal
Tuberculosis Screening
This electronic form helps students understand their risk for contracting tuberculosis, and determines whether further testing is required.
Tuberculosis Testing
International students are required to complete TB testing. Some domestic students will be required to submit TB testing as a result of prior medical history. The Wellness Center will notify you of this requirement by email.
Immunizations
The health and safety of the university community is our top priority, and Denison makes every efort to ensure that the university is as proactive as possible at protecting the health and well-being of our community. While the vaccination requirement alone cannot always stop outbreaks, the medical evidence shows that higher vaccination levels among a population lead to greater protection and less severe symptoms should an outbreak occur.
With this in mind, all students must complete Denison University vaccination requirements before the start of the student’s frst semester. The university selected the required vaccines based on the Centers for Disease Control and Prevention (CDC) recommendations, which the State of Ohio also follows for grades K-12. Additionally, Denison added the meningococcal conjugate (ACWY) vaccination for students who are new to residence halls because college students are at higher risk for the disease, particularly those living in dense living quarters such as residence halls.
PROOF OF COVID-19 IMMUNIZATION
All students are required to be fully vaccinated and, if eligible, to receive their booster for COVID-19. Upload proof of your COVID-19 vaccination to your My Wellness Portal.
Proof Of Insurance
If you are planning to use 3rd party 5 PHASE insurance (e.g., parent’s insurance, Ohio Medicaid) during your time at Denison:
R Please upload a copy of the front and back of your insurance card to the My Wellness Portal.
R A NOTE IF YOU HAVE OUT-OF-STATE MEDICAID: Medicaid is partially funded by state taxes, therefore the money does not travel with you to other states. Keep in mind that if you have Out-of-State Medicaid (not Ohio), you will likely NOT have health coverage unless it is an emergency. Take into consideration that emergencies are defned very narrowly. This means that while in Ohio if you have Out-of-State Medicaid and you need anything like an annual dentist check-up, x-rays, lab work, a vaccine, a refll for a prescription medication, referral to a specialist, or new glasses you will not be covered. If you have Medicaid in another state, you should verify this according to your specifc plan.
If you are planning to purchase the optional Denison Student Health Insurance plan:
R There is no need to upload an insurance card. This requirement will be satisfed upon your purchase of Denison insurance. See more information in the Finances section of this guide (see page 41).
