hoever installed the floor furnaces in my grandparents’ old house clearly did not have children. Usually floor vents line the edges of rooms where only dust bunnies dare to tread. Not the furnace vents in that house. They were smack-dab in the middle of the hallways and bigger than the average Texan’s backyard grill. (When my mother was young, she would hang her head over them to blow-dry her hair.) To a barefoot 4-year-old, those vents were like magnets: No matter how carefully I traipsed around them, I’d somehow end up with a toe or a heel on the cooking surface. It became obvious why my grandmother kept an Aloe vera plant in the kitchen. Aloe vera juice fresh from the leaf is one of several home remedies I remember from childhood. My parents had no qualms about dosing me with Motrin, but they also had a cabinet of cures that weren’t so clinical: tea tree oil for the dozens of mosquito bites I’d sport after exploring my grandparents’ backyard (visiting my grandparents was a hazardous venture), scrambled eggs and ginger ale for an upset stomach, sugar for hiccups, and Highlights magazine for splinter removals—because distraction can be an effective form of pain relief. I’m sure you have an equally robust arsenal for the boo-boos that befall your own kiddos, as sometimes it takes more than a kiss to make it all better. Perhaps the most powerful thing in your parental medicine kit is the roster of people you turn to for help with your kids’ health, from the pediatrician to other parents who’ve been there, done that. After all, mother knows best—which sometimes means knowing exactly where to go for the answers you don’t have. Symptoms of post-partum depression and strategies to fight it ALEXIS MANRODT

I

t’s normal for new moms to call the pediatrician over every little thing baby does, but Jessica Miller (name changed for privacy) found herself on the phone with her doctor for a very different reason: She feared that she might harm her 4-monthold daughter.

In the weeks after her baby arrived, Miller could not understand why it was so difficult to connect with her new role. “I felt exhausted all of the time. I would have crying fits while eating and driving,” she admits. “I thought I was a terrible mother.” The North Dallas mom felt trapped in traumatic “thought loops” while interacting with her daughter during bathtime, or even walking down the stairs. “I would feel queasy in the kitchen, like around knives,” she explains. “I don’t know what would’ve happened but I worried something would.” 10

Convinced that some part of her must want to hurt her child, Miller admitted herself for a psychiatric evaluation. “I was worried there was something wrong with me fundamentally, as a person,” she says. Intrusive thoughts are a common symptom of postpartum depression (PPD), explains Ann Dunnewold, a Dallas psychologist who was one of the first health professionals to help identify these “postpartum obsessions” in the early 1990s. According to her research, these thought patterns are related to high levels of oxytocin, the hor-

mone associated with, among other things, mothering behavior. “Postpartum obsessions are linked to the mom’s ability to sense danger—it doesn’t mean you’re going to hurt your baby,” she explains. What Miller believed were signs she was an unfit parent, Dunnewold asserts are positive behaviors distorted by hormonal and environmental changes. “If you know these thoughts are wrong, you shouldn’t feel afraid. It means that you are an attentive mother,” she says. NOT ONE-SIZE-FITS-ALL

Though shrouded in mystery and misconceptions, PPD affects up to 1 in 7 women, according to the American Psychological Association. Unlike the baby blues, PPD impacts a mother’s day-to-day functioning, says Katie Sardone,

a licensed psychologist and founder of the mental wellness practice Behavioral Health Dallas. Typical symptoms include depressive episodes, loss of pleasure in interests and activities, feelings of guilt or anxiety and negative thoughts about yourself or your baby. But as with other mood disorders, PPD symptoms manifest in highly personalized ways. There’s no one-size-fits-all diagnosis. Miller once believed that she would be able to recognize the symptoms in herself. “But going through it, it was so terrifying,” she admits. “My first thought wasn’t that I had postpartum depression. It was that I was a horrible person.” THE OTHER SIDE

After Miller’s diagnosis, she sought help through therapy and a small dosage of a selective serotonin reuptake inhibitor (SSRI), an antidepressant that would still allow her to breastfeed. Many mothers, like Kathryn Rock, find strength after emerging from their PPD experiences. After the birth of her son, the Lakewood mom felt crippling loneliness, listlessness and irritability. “I spiraled. I felt so alone,” she says. “There’s a feeling that you’re the only one, which itself triggers a cycle of being alone,” explains Sardone. “There’s a shame that compels people to withhold sharing this information with other people.” Thankfully, PPD is becoming destigmatized, in part due to a growing number of celebrities— among them Chrissy Teigen, Gwyneth Paltrow and Brooke Shields—who have come forward with their own PPD battles. “The normalization of mood disorders like PPD has grown so much over the past decade,” notes Andrew Carlton, a registered nurse and an administrator at Springwood Hospital in Bedford who frequently works with mood disorders. He stresses that every postpartum adjustment experience is normal—even the troubling ones. Just as there is no one root cause of PPD, the disorder requires a highly individualized treatment plan. When Jennifer’s self-esteem and friendships began to suffer, Patterson decided to give medication a try. Unfortunately, drug therapy definitely didn’t prove to be the easiest—or the most effective—antidote. Jennifer was first prescribed Strattera, a non-stimulant class medication that is commonly offered for ADHD treatment because it is considered less addictive and offers fewer side effects than stimulant drugs. Almost immediately, she began experiencing muscle weakness in her legs that made it difficult to stand, as well as lethargy that left her sleepy—or asleep—for most of the day. While drowsiness is a common side effect with Strattera, Jennifer’s daytime sleepiness was borderline narcolepsy. “She was calmer but only because she was too sleepy to do much of anything else,” Patterson says. So the frustrated mom went back to Jennifer’s psychiatrist. This time, she was prescribed 18

Concerta. But Patterson had reservations about giving Jennifer this new drug. She didn’t like the trial-and-error approach to helping her daughter cope. Patterson’s not alone. Doctors typically choose a medication to prescribe for ADHD largely based upon their own professional experience with it, meaning if most of their patients have had favorable results with it, your child likely will too. But that’s not entirely true. And many of the families affected by ADHD find themselves stuck in a system that relies heavily on an experimental approach to find the right prescription for their kids. Which begs the question: Why do some medications work for some kids and not for yours? THE RISE OF PERSONALIZED MEDICINE

Whether a medication will work depends on several factors including age, other medications the child might be taking, kidney function and more. But

it also depends on whether or not the prescribed medication is best matched to a child’s genetic makeup. That’s right: Genes that kids inherit from their parents are also responsible for how the body processes medications. A rapidly growing field in health care called pharmacogenetic testing aims to provide this genetic information to physicians so they can select the right medication and dosage for each child—and adult. The promise of pharmacogenetic testing is that people get the right dose of the right drug with one pain-free swab on the inside of the cheek. For patients with ADHD, this could define a better starting point when looking at drug therapy. Dr. Michael Jann, professor and department chair of pharmacotherapy at University of North Texas Health Science Center in Fort Worth, offers Strattera as an example: “Strattera is metabolized by a specific liver enzyme, and it turns out that 10 percent of the population is a poor metabolizer of this enzyme. The poor metabolizer will have almost four times the amount of the drug in their bloodstream as a fast metabolizer. In other words, [the slow metabolizer] could have more side effects. TESTING THE LIMITS

So why isn’t every doctor in the

hen 11-year-old Jennifer was diagnosed with ADHD at the age of 8, her mom, Katherine Patterson knew for certain that she didn’t want to use medication, except as a last resort. The Denton mom, whose name has been changed for privacy, tried everything—from major diet changes to therapy—to cope with the daily emotional outbursts and struggling grades.


Dallas-Fort Worth area subscribing to this? There’s the question of what pharmacogenetic testing actually measures. Today the most frequently used tests look for variations in the genes that carry instructions for making the enzymes in the liver that metabolize drugs. Researchers say these tests don’t look at anything other than metabolic rate. Representatives from the companies making tests, like GeneSight, which offers ADHD pharmacogenetic testing, claim the test measures metabolic rate and efficacy. At one time, you could have picked up a Harmonyx ADHD genetic test from the community pharmacy for under $100. But the FDA ordered the removal of the Harmonyx tests from pharmacy shelves in November 2015, citing that the tests were considered “direct-to-consumer” and therefore required special approval. GeneSight isn’t FDAapproved either, but it hasn’t been pulled from the system. The test is ordered by a doctor’s office and processed in a laboratory. The results filter medications into categories, showing which drugs a particular body is programmed to break down in a normal fashion and which drugs the body is not programmed to break down in a normal fashion. It’s important to note, however, that a “normal” result doesn’t necessarily mean the drug is going to be effective or free of side effects. TESTING THE WATERS

With the science of pharmacogenetics still finding its footing, the patient response is mixed. For Linda Ferrell-Brooksbank, pharmacogenetic testing helped her figure out what medication would finally work for her son Alex, who was diagnosed with ADHD in the first grade. “We had tried so many different ADHD medications, and I was feeling very frustrated that after several years of trial and error we still had not yet landed on the right one,” she says. “My feelings of exasperation led me to the GeneSight testing.” When the test results came back, they showed that only Strattera would metabolize effectively in Alex’s body. He began taking

the non-stimulant, and Mom was thrilled with the results. “I am more comfortable with the nonstimulants because I have noticed a dramatic decrease in side effects in my son,” she says. Penny Williams, who has written several books on ADHD and children, had an opposite experience when her son tried two different pharmacogenetic tests. Luke was diagnosed with ADHD in 2008 before genetic testing for drug therapy was offered. Like many kids, he was sensitive to several medications and had serious side effects, including psychosis and deep depression.

talk to your doctor Since it’s still a relatively new science, your child’s doctor may or may not be familiar with genetic testing for drug therapy, so it might actually be up to you as the parent to help educate your practitioner about why you feel it would be a good fit for your child. Find out if the doctor has any experience with pharmacogenetic testing and whether they think a test like that might help find the right medication, especially if you’re dealing with a disorder, condition or illness that typically uses a trial-and-error formula to find the right drug and dose. Then call your insurance company. Coverage varies from total coverage to nothing at all.

Knowing this history, a therapist recommended pharmacogenetic testing when it became available. “We had gotten to the point where we were fearful to try anything else because it was so bad when it was bad,” Williams recalls. The results? Even more frustrating. It turns out that several of the drugs Luke had already taken— and had significant side effects with—were green-lighted for use. “Obviously, the reason that he is sensitive to medication was

not found in the genetic markers these tests looked for,” Williams explains. “That tells me that the test isn’t covering as much as they want you to believe it does. THE FUTURE OF PHARMACOGENETICS

So there’s a reason pharmacogenetic testing isn’t routinely offered by your child’s physician yet: The usefulness of the test results are still a bit murky, and experts disagree on the efficacy of the pharmacogenetic testing currently on the market. But it seems clear to all parties that genetic testing for drug therapy looks promising. In his 2015 State of the Union address, former President Obama announced a Precision Medicine Initiative (PMI) that would use pharmacogenetic tests “to bring us closer to curing diseases like cancer and diabetes—and to give all of us access to the personalized information we need to keep ourselves and our families healthier.” The PMI was launched in 2016 and includes the All of Us research program, which seeks to gather data from at least a million Americans in order to further more individualized research into health conditions. But Jann says we’re likely years away from routine genetic testing for drugs, explaining that we first have to have to better understand the genetics behind disorders like ADHD. “We don’t have a genetic test for autism, and we don’t have a genetic test for ADHD,” he points out. “We don’t really understand the disease at the genetic level, and until we come up with a genetic marker for those disorders, we will never be able to learn everything we need to from these tests.” Until then, parents like Patterson, who still hasn’t found the right medication for her daughter, will have to make do with the limits of genetic testing and the standard trial-and-error drug therapy. “It would be a dream come true to have an affordable test that would tell me exactly what medicine my daughter needs,” Patterson laments. “Until that happens, we will keep hoping that the next thing we try will be the right answer.” dfwchild.com / 2019

ine-year-old Evangeline was at the dentist to have two baby teeth removed before her braces could be put on the following week. Her mom, Michelle Warren, wasn’t asking the dentist to give the teeth back — so that they could be placed under Evangeline’s pillow for the tooth fairy.

No, the Fort Worth mom came to the office with a kit where the teeth would be placed, which would be picked up by a courier and sent to a lab. Warren is banking her daughter’s baby teeth because the living dental pulp inside those teeth contains stem cells. And banking these cells is becoming a growing trend. You’re likely familiar with the concept of banking. You were probably asked if you wanted to store the stem cell-rich umbilical cord blood if you gave birth in a hospital or birthing center. Well, those same stem cells that exist in cord blood have also been found in a variety of

human tissues such as bone marrow, fat and baby teeth too— good for parents who missed the boat on banking their child’s cord blood. Stem cells are essentially biological wild cards. Research shows that stem cells harvested from baby teeth may have the potential to be manipulated into a variety of cells to repair damaged teeth, induce the regeneration of bone or restore damaged or malfunctioning cells and help treat future cancers, diabetes, heart disease and neural conditions like Parkinson’s, depending on ongoing technology and medical advancements. baby teeth might hold the key to a lifesaving treatment in your child’s adulthood. And according to the National Institutes of Health, stem cells remain one of the most promising frontiers in medical research. “Dental stem cell preservation offers an abundance of benefits for a patient’s future,” says Dr. Maxwell Finn, who is licensed in both dentistry and medicine and practices at Oral Surgery Associates of North Texas in Dallas. “Because research and technology are advancing so rapidly, it’s a worthwhile investment.” In a University of Pennsylvania study published last year, stem cells from baby teeth were used to help regrow tissue in permanent teeth that had been injured. Despite this exciting potential, however, some experts say it’s premature to consider dental pulp stem cells as a source to replace or regenerate other tissue. To collect dental stem cells, a dentist must extract the baby tooth when it starts to get wiggly (which takes some of the fun out of the process for kids) and then prep it with materials from a special kit provided by the chosen stem cell bank, where it’s sent and cryogenically frozen until it’s needed. Initial processing fees range from $475 to nearly $2,000 plus annual storage fees of $99 or more. So do the potential benefits of preserving your child’s baby teeth justify the expense?

With dental stem cells, you have at least 24 chances (that’s the number of baby teeth plus wisdom teeth) to gather cells that can potentially be transformed into other cells, including cardiac, muscle, bone, cartilage, nerve and fat tissue. Though research is ongoing and the future looks promising, the U.S. Food and Drug Administration has yet to approve the use of dental stem cells in any medical procedure. Advocates, however, are hopeful that the successes with umbilical cord stem cells will hasten the approval process for dental stem cell therapies in the next several years. At this point, “storing dental stem cells is essentially a bet against future problems,” Finn says. Which might be an advantage for young people today, who are projected to have life spans of 100-plus years. The American Academy of Pediatric Dentistry recommends that parents talk to their pediatric dentist about dental stem cell banking. Then if you’re interested, read up on all your options to find the one that best fits your needs and budget. That’s what Warren did. She and her husband opted out of banking the umbilical cord blood when Evangeline was born because of the expense. “And I regretted it almost immediately,” she admits. “Diabetes runs in my family. By banking Evangeline’s dental stem cells, I feel like I’m giving her biological insurance for her future.”

ailer Simmons doesn’t think about alcohol and drugs all the time. No, when the little girl from North Oak Cliff is participating in one of her favorite pastimes like dancing or singing, she doesn’t think about those things at all. But when she’s home, alcohol and drugs creep into her thoughts often. “When is Daddy coming home?” she asks her mother. Sailer’s daddy has struggled with substance abuse since his little girl’s birth. Louise Simmons, Sailer’s mom (names changed for privacy), knew that addiction ran in her now ex-husband’s family even before she married him. “I had the misperception that because I was aware of the dangers of drugs and alcohol, I was educated and we had a certain socioeconomic status, we were safe,” Simmons reveals. “But that is wildly untrue. Addiction can impact anyone, regardless of their background.” Shortly after Sailer was born, Simmons discovered her husband wasn’t only struggling with an alcohol addiction but also with over-the-counter pills and medications prescribed by doctors, all of which he was unsuccessfully trying to hide from his family. These substances enslaved him, and she’d had enough. 36

PARENTS ON DRUGS

One in three children goes home to a family member using drugs or alcohol daily. According to the National Center on Addiction and Substance Abuse, alcohol and drug use knows no age, gender, race, ethnicity or status when it comes to its victims. But those paying the biggest price for substance addiction are the children. “The use of drugs and alcohol monopolizes the motivation and attention for the parent,” says Dr. David Atkinson, medical director of the Teen Recovery Program at Children’s Health and assistant professor of psychiatry at University of Texas Southwestern Medical Center in Dallas. “The child tends to be neglected because things that are naturally motivating for most people are not there for people using drugs and alcohol.” A parent who is an addict may have difficulty with emotional regulation and get overly

angry or overly sad, Atkinson says. These emotional elements contribute to a decreased ability to engage with their kids, discipline effectively and be an overall effective parent. “Many people think that if the addict would just stop drinking or using drugs, then everything would be OK,” says Emily Van Pelt, a licensed professional counselor supervisor at Southwest Clinical and Forensics in Dallas. “But it can’t be turned on or off [like a switch]. It’s a daily battle.” THE SIDE EFFECTS

Long term, children with parents who suffer from addiction have an increased incidence of academic problems, mental illness, criminal behavior and early use— and abuse—of the same substances that bind their parents. “Kids will pick up on something being wrong with the parent,” Atkinson explains. “Even if the parent can hide the substance use, they can’t hide all the consequences that go with it.” Some kids feel ashamed and keep the secret from others, causing them to isolate themselves socially, explains Vanessa M. Sanford, a licensed professional counselor supervisor and registered play therapist supervisor who specializes in addiction and families at Support System in Frisco. They may avoid going to events where a parent has to go, or lie that an absent family member is sick or traveling. Kids with parents who have an addiction feel like they live two separate lives—the embarrassing one in emotional upheaval at home and the façade they put on in front of others.


“[These kids] may sit at the lunch table at school and listen to friends talking about weekend plans or birthday parties and not contribute to the conversation because they’ve been trained not to share their home life,” Sanford says. Other kids blame themselves, thinking their bad grades, untidy room or poor behavior causes Dad to stay away for days at a time or their parents to fight. “Developmentally, kids can be egocentric, so oftentimes, they think it’s their fault,” says Erin Williams, a children’s counselor who previously worked in the Children’s Program at the Betty Ford Center in Irving, a prevention and education curriculum for kiddos with an addict in their lives. (The location closed in September.) These children feel insecure, frustrated and lonely as a result of the unstable environment at home. But they don’t know how to process these emotions and often act out; struggle with school; and experience anxiety, depression and sleeping issues. And then there are other kids who hide an unpredictable family life well. Sam McCage, manager of the behavioral health clinic in the psychology department at Cook Children’s in Fort Worth, explains that these children may excel academically, behave well in class and participate in numerous extracurricular activities. But inside they harbor a tremendous amount of guilt and a sense that no matter what they do it’s never enough. “The kids quietly struggling often go unnoticed,” Williams says. “They don’t appear to be affected and aren’t helped.” It’s not uncommon for children of substance abusers to lose trust in all people who hold positions of power—teachers, grandparents and other wellmeaning adults. As the kids get older, meaningful relationships become harder to sustain, and instead these children seek out relationships in which the dynamics feel familiar—with people who are addicted to drugs or alcohol, for instance, Williams cautions. There’s also the danger that they too might spiral into

the world of addiction since genetic and environmental factors affect who suffers from the disease. If it’s already in the bloodline and it’s being modeled at home, kids are frighteningly more likely to develop an addiction to a substance or substances.
