As we get older the level of hyaluronic acid in our cells gradually decreases which causes the skin to become dehydrated and less firm. The effects of gravity also come into play as we age. As the popularity of these treatments grows, so does the risk involved for potential clients. Save Face—a national registry of accredited non-surgical aesthetic practitioners—discusses the dangers that exist for patients, the psychological effects of a botched job and how consumers can avoid such an experience.

Question: What impact do you hope Save Face will have on the non-surgical cosmetic industry as a whole?

Save Face: Save Face is a government-approved register of healthcare professionals who provide non-surgical cosmetic treatments. Entry to the register is only open to registered medical professionals who can demonstrate that they are insured and have received certified training. All premises are inspected. Practitioners must operate against best practice standards and have policies and procedures that demonstrate this. The ultimate goal is for the Save Face Kite Mark to become a default factor in the decision-making process for someone who is considering a non-surgical cosmetic treatment. Through our public awareness campaigns we hope to continue to educate the public about the unnecessary risks they may be taking by choosing a practitioner who isn’t accredited. The reality is, these sorts of procedures are becoming increasingly popular amongst men and women of all ages and consequently the number of unscrupulous practitioners is also rising. Q: Why do you think that this particular industry isn’t governed by the same rigorous regulations that others are?

SF: This field of practice is quite unique. Though medical in nature, there is no provision, nor ever has there been in the NHS, which has left training and standards to evolve organically and without recognised accreditation. The client base is healthy and treatment is a choice rather than a necessity. Unlike other medical procedures, it is possible to provide these in a variety of venues with relatively low capital cost and overheads. Because of the association with beauty, the vanguard of early adopters commonly provided services in association with beauty salons. This chartered new territory without reference to any expert authority to interpret and apply regulation developed with accountable institutions in mind. Given the exponential growth of the market and the commercial gains to be made, it is no wonder that providers have exploited the apparent loopholes in legislation and regulation and seized the opportunities to

practice with apparent impunity. The resulting diversity of practice and growing accessibility of services—left unchecked for over two decades—has led us to where we are today. Non-surgical cosmetic services may be provided by anyone, anywhere. Where legislation and regulation are breached, sanctions are not robustly applied and fail to deter. It is reliably estimated that there are some 10,000 providers in the UK alone. Approximately 800 belong to professional associations providing influence, guidance and political representation on standards and education related specifically to aesthetic medicine. This suggests there are thousands who don’t know what they don’t know and likely do not care.

Q: What are some of the potential risks of having a nonsurgical procedure at a clinic that is not accredited?

SF: As our investigations continue to demonstrate, finding a safe, reputable practitioner can quite simply be a lottery. Sadly, dotted in amongst all of the reputable practitioners, there are a mass of unscrupulous people masquerading as trained professionals. They have often had little or no training and purchase cheap illegitimate products over the internet. Frighteningly, these treatments are all too often considered and categorised as everyday beauty treatments like having your hair or nails done. In reality, it is a clinical procedure that can have disastrous consequences when it is administered by the untrained and unqualified. It’s because of this that the consumer is blind-sighted by cheap deals and how glossy the website and clinic décor might be. People so often neglect to pay any real attention to the fundamental thing that is essentially the difference between a good treatment and what can be a nightmare: the skills and qualifications of the person holding the needle. In the last 12 months, we have seen an alarming increase in the number of reports regarding procedures gone wrong and rogue practitioners. Figures have almost trebled in comparison to the previous year, with a total of 934. Over 83 percent of the reports we received were about treatments that were administered by beauticians, hairdressers and other non-healthcare professionals. Upon further investigation, a staggering 30 percent of all complaints we received were performed by people who had set themselves up with no relevant training whatsoever and were believed to be purchasing their products such as dermal fillers and botulinum toxin (Botox) over the internet. We received complaints relating to the following procedures (in order of the most numbers received): • Dermal fillers, which included lip fillers, cheek fillers, facial fillers to improve the appearance of skin folds and wrinkles, tear trough fillers to reduce eye bags and non-surgical nose jobs • Botulinum toxin • Laser treatments

Q: What are the psychological effects of having a ‘botched’ treatment?

SF: Being a victim of a botched procedure has so many ramifications both physically and mentally. Many people spend months contemplating a procedure to address a particular concern and restore their confidence, only to be left feeling worse and more self-conscious than they did before. It has a devastating effect on their state of mind. We have had people contact us who won’t leave the house or that cry when they look in the mirror and have had to be prescribed anti-depressants. In most cases they have no way of seeking redress from the person who treated them as they have ceased trading or have no insurance. In fact, 72 percent of all complaints we received last year were from patients who had found the practitioner on social media pages such as Facebook and Instagram. This is a major concern and something that has escalated enormously in the last few years. Social media has become a hotbed for unscrupulous practitioners to advertise unethical deals and offers, which flout advertising rules. These practitioners only have Facebook or

Q: Are there any success stories you can share with us that involve a patient finding a safe and reliable clinic through your service? SF: We have had over 5,000 positive reviews from patients that are published on our website. Q: You have recently received a considerable amount of press coverage regarding your involvement in exposing a fake doctor who had administered fake Botox treatments. Is this part of a wider initiative to stamp out dangerous practitioners? Do you have any plans in place to facilitate the discovery of other similar cases? SF: We have an endless list of unscrupulous practitioners we are currently investigating to ensure that they are exposed and are brought to justice—each has involved a breach in regulations. We are working with several patients who have been affected by practitioners who have left them with irreversible damage. We will ensure that they get the help and support they need and that these practitioners are stopped from operating in the future.

aricose veins are characterised as swollen or enlarged veins usually occurring in the arms and legs. In severe cases they can appear lumpy or bulging and may cause the area to cramp, throb or feel heavy and painful. Such cases will require medical treatment. Most times, however, varicose veins are not a severe medical condition and merely consist of blue or purple marks on the skin. Causes

Sclerotherapy Sclerotherapy is a procedure that works to improve the appearance of varicose veins by injecting a special type of foam solution called sclerosant into them. Sclerosant damages and scars the lining of the vein, ultimately closing it. As a result, the body will eventually rid itself of the veins. Whilst the management of spider and thread veins can be undertaken without any investigation of the deeper venous system, if any issue is present in the larger veins, it is unlikely that this superficial treatment will give a lasting effect. For the most complete and accurate management of venous problems of the legs, a detailed assessment of all the veins in the legs is advised, and this is considered the gold standard of care. Assessing the leg veins is performed by undertaking a duplex ultrasound examination, which is a non-invasive technique to map out and assess the function of the leg veins. From this, detailed treatment advice can be given to our clients, based upon this evidence gained. Lip lift Much like a facelift, a lip lift removes extra skin. This is done by making an incision right under the nose and shortening the skin to elevate the lips. There are small sutures involved with this procedure—they are removed after four or five days.

Permanent makeup Using tattoo techniques to enhance features that are not pronounced—like the lip outline—this non-surgical lip enhancement technique is most suitable for those who want a permanent solution without the need for surgery. Recovery Surgical lip enhancement will be carried out in a hospital, ambulatory facility or specialised surgical suite, will involve some preparation and take anywhere from 30 minutes to three hours to complete. General anaesthetic is also commonly used. After procedures, patients may experience swelling, bruising and mild to moderate pain. Years ago, makeup contouring was an obscure technique used only for the purpose of photo shoots. Now it's a worldwide phenomenon, made famous by reality TV star Kim Kardashian West when she tweeted a picture of herself practicing the art.

Step 1. Add lowlights and highlights using a concealer or foundation. A highlighter tends to be a shade lighter than your skin tone; a lowlighter (or contourer) tends to be a few shades darker.

Cheekbones Highlighter should be used on the apples of the cheeks to brighten and add a glow. A sweeping contour line should be applied to the hollow area below the cheekbone to create a subtle shadow, adding definition to the face.

Step 2. Using a beauty blender, merge the contouring lines together. Take time to fuse the differing shades so that you are left with a subtle gradient and flawless finish. This is, perhaps, the most important step.

A

lthough laser eye surgery isn’t currently available on the NHS for people that just want to improve their eyesight, it is one of the most popular procedures in the UK. A solution for improving eyesight and cancelling the need for glasses and contact lenses, vision correction can significantly enhance quality of life as well as boost self-esteem. Laser vision correction (LVC) aims to correct four major refractive errors—problems caused by an imperfectly shaped eyeball or cornea. Near-sightedness, also known as myopia, causes close objects to appear sharp and far objects to appear blurry. This condition is due to a curved cornea and a longer eyeball. Far-sightedness, known as hyperopia, causes the opposite and is due to a shorter eyeball or a cornea that is flatter than normal. Individuals affected by astigmatism will see objects blurry at any distance—in this scenario the cornea and lens are shaped so that images aren’t focused on the retina. Lastly, presbyopia, or ageing eye, occurs when the eye loses the ability to focus due to ageing. This usually happens between the ages of 40 and 50.

There are three main types of laser vision correction (LVC) procedures: LASIK, SMILE and surface laser treatments. LASIK (laser in-situ keratomileusis) is performed with two lasers: one opens up a thin ‘flap’ in the surface of the cornea while the other reshapes it. At the end of the procedure the flap is put back and stays in place without the need for stitches. Surgeons performing SMILE (small incision lenticule extraction) surgery, on the other hand, will reshape the cornea through a small, selfsealing hole. Other surface laser treatments—which include PRK, LASEK and TransPRK—will consist in removing the skin covering the cornea to allow the surgeon to reshape it with a laser. In this type of procedure, the cornea grows back naturally. Although all three types of surgeries yield similar results, it is important for patients to discuss with their surgeon which route is best for their individual needs. While they are relatively painless procedures, there are some stages which patients might find uncomfortable. To prepare, the surgeon will put anaesthetic eye drops in the eye to make it numb. The procedure Performed under general anaesthetic, the surgery can take up to an hour and a half. The surgeon will either make an incision two to five centimetres long in the crease below the breast, in the underarm or around the areola—preferences can be discussed prior to surgery during consultation sessions. There are two main types of implants that are commonly used in the UK: silicone gel and saline solution. Silicone gel implants contain soft or firm gel, are less likely to wrinkle and can give a more natural feel. However, if they rupture, they could cause siliconomas in the breast. Saline solution implants, on the other hand, are more likely to rupture or deflate over time but aren’t dangerous as the body can safely absorb saline in the event of a rupture. During the procedure, a pocket is created for the implant; this can be placed between the breast tissue and chest muscle or behind the chest wall muscle. Although both placement methods have their respective pros and cons, it is ultimately the size and shape of the breast and the looseness of the skin that will determine which option is best for each individual patient. When the surgery is complete, the incision will be closed up with stitches—these may be dissolvable—and covered with dressing.

Side effects As with all surgical procedures, breast enlargement may cause some side effects. It’s fairly common to experience temporary pain, swelling, bruising and a tight feeling in your chest for a few weeks. Consult your surgeon if these disturbances persist or if you’re in a lot of pain

The cause of your tooth loss will need to be understood and treated before you undertake implant treatment. The dentist will begin the process by fixing metal screws into the jaw at the site of the missing tooth. This metal fixing serves as a support for the artificial crown that is fitted. The procedure can take anywhere between 30 minutes and several hours depending on the number of implants needed and the complexity of the jaw structure. Full facelift A full facelift involves making a long surgical incision from the hairline, around the front of the ears and back into the hairline behind the ears. The surgeon will then separate the tissue beneath the skin in order to remove excess fat and reposition the facial muscles. Once this is completed, the skin will be pulled up and back and stitched in place carefully where the initial incision was made. The position of the incision means that the scar will sit comfortably in the hairline, making it almost invisible. The procedure typically takes between two to five hours to complete and patients are required to stay in hospital for a few days after. Patients are likely to experience bruising, swelling and fatigue following the procedure.

Eyelid surgery Eyelid surgery—also referred to as blepharoplasty—focuses specifically on tackling signs of ageing by correcting aesthetic abnormalities on the eyelids or eye bags. Although cosmetic surgery aims to be minimally invasive, even the slightest incision in your skin will send signals to your body to repair the injury. Sometimes the healing skin forms excessive, unorganised collagen bundles that push upwards and form a reddened, raised, hard scar.

KELO-COTE® should be applied to new scars as soon as the wound is closed or stitches have been removed, so be prepared and have some on hand before your procedure.

KELO-COTE® self dries within 5 minutes† forming a flexible and waterproof layer. Once dry you can apply make-up or sunblock on top of the scar.

1. Always keep your scar clean. Wash the area under running tap water, with saline (a salt water solution) or use an alcohol-free wipe.* 2. Avoid too much exposure to the sun. Scar tissue is very sensitive, this makes it very easy for scars to get sunburnt.**

What to expect

Aiding your recovery

Before undergoing cosmetic surgery, your doctor will explain the recovery process involved in your procedure. They will also provide helpful information on bathing techniques, changing dressings and wound cleanliness. Make sure to follow these instructions carefully. Generally, patients tend to experience varying degrees of bruising, swelling and tenderness of the skin after surgery. These effects may seem alarming—especially during the first few days of recovery. However, these will eventually subside and patients will be able to see the desired results of their procedure after some time.

After any procedure, the body will require time to heal. Allowing yourself an adequate period to recuperate will aid your recovery. Patients can play an active role in the healing process by following their post-operative guidelines to a tee and leading a healthy lifestyle. Aim to consume a balanced diet filled with whole foods, fruits and vegetables. Staying hydrated is also imperative—drinking plenty of fluids will aid the body’s natural healing process and regulate its overall temperature. Applying an ice pack to the affected areas of the body is an effective way to combat post-surgery bruising and relieve any discomfort. Try to elevate the area as directed by your surgeon; this will help to reduce swelling and blotching. There are a number of things that can hinder your skin’s ability to heal, including smoking and sun exposure. Sunscreen should be applied liberally before leaving the house and smoking should be avoided at all costs. Check with your doctor that any medications you are already taking—such as blood thinners or heart medication—won’t impede your recovery. Generally, sutures will be removed between five to 10 days after surgery. Your doctor will notify you of this during your follow-up appointments. These appointments are designed to allow your physician to track

Arrange a caretaker Immediately after your cosmetic procedure, you will need to arrange a caretaker; somebody to take you home and help you for the first 24 to 72 hours. During these initial stages of recovery, you will most likely suffer from fatigue—both from your medication and the surgery itself. As a result, simple tasks may seem difficult. Your assigned caretaker—usually a friend or relative—may need to assist in feeding you and redressing your bandages. Some clinics may include a personal caretaker as part of the package they offer—check this with your doctor beforehand. 64 | Flawless

Scar Management Seeking medical assistance If you experience any unusual side effects after surgery, report it immediately. Make yourself aware of the key signs of infection which may include pus, redness, inflammation, fever, chills and excessive drainage from the affected area. Additionally, seek medical advice if your pain isn’t alleviated by your prescribed medication, you have difficulty breathing or are suffering from persistent nausea

Cosmetic surgeons will always aim to keep scarring to a minimum, however, some scarring is usually unavoidable. Luckily, patients can assist with the reduction of post-surgery scars. After an elective surgery on the face, abdomen or breasts, a pressure garment should be applied to the scar site. To prevent any redness, discolouration or hypersensitivity, aim to use hypoallergenic products to moisturise the affected area. If you notice any flaking or scabbing of the skin, avoid touching it. Picking at the skin can result in infection or aggravate scars.

Consuming alcohol after surgery Most clinics will recommend that you reduce (or preferably eliminate) your alcohol intake for some time after your surgical procedure. Dry skin is more prone to stretch marks, cracks and scars than hydrated skin. It thins your blood. The thinning of your blood can wreak havoc on your recuperation. This can cause prolonged bleeding and infections to arise. It mixes badly with medication. Pain medication is almost always prescribed after a cosmetic procedure—the type and strength will depend on the circumstances. Mixing prescribed drugs with alcohol can have dangerous consequences and should be strictly avoided. It can increase risk of swelling.
