A Strategic Alliance

The purpose of this publication is to introduce and present a new Australian strategic consultancy service in International Health Design and Planning. This alliance between Medical Architecture and Swanbury Penglase is able to add international and local value to all stages of health and well being initiatives by both public and private enterprise. Through our unified and established working relationship this alliance realises the value of true collaboration where the respective strengths of our organisations are combined to provide an integrated service that meshes international experience with local knowledge. As well as their specialised expertise in the health sector, Medical Architecture bring extensive experience in managing large and complex projects to the alliance, complementing the substantial resources and broad experience base of Swanbury Penglase. Our collaborative approach combines active listening with strong leadership as proponents of design as a healing therapy. Our active and effective consultation skills ensure that the process creates and supports committed advocates for the solution especially when it involves “changes in culture” and “operations”. Our approach is centred on people, underpinned by design skills from strategic analysis from master planning down to the fine grain design of individual facilities. We ensure our designs promote the integration of the local community which ultimately leads to the pride that comes from making aspirations a reality.

Local Awareness & Global Perspective The alliance draws on over 20 years of experience that Swanbury Penglase has working with both private sector providers and government agencies. Our work encompasses Health Facilities, Medical Centres and Aged Care projects including consultation for the South East Region Health Services in South Australia over 10 years as well as national and international work in dialysis and a wide range of institutional and commercial work. Medical Architecture (MA) brings to the alliance experience of various health care procurement initiatives including Private Public Partnership starting in 1996 when they successfully embarked upon one of the first NHS PFI (PPP) projects reconfiguring services at Highcroft and All Saints Hospital in Birmingham, UK. This led to further participation in over 15 PFI/ PPP projects worldwide. Our most recent PPP work includes: Roseberry Park UK, completed 2010, the £75m PPP project is the largest mental health facility to be constructed in the UK in recent years;

WHO WE ARE

Medical Architecture Medical Architecture (MA) is an award-winning practice that works exclusively on health care design. Established in 1991, the founding directors are former members of the Medical Architecture Research Unit (MARU) at North London University, UK. Our ethos is centred on people and the belief that research and excellence in design can create high quality and flexible medical and therapeutic settings for the benefit of patients, staff and the wider community. We design buildings, undertake research, consultancy and provide advice to professional bodies in the health care sector. Our work spans two decades and includes projects in the United Kingdom as well as in Canada, Europe and Australasia. We have offices in London and Newcastle in the United Kingdom as well as subsidiary branches in Australia and Canada. We have international benchmarking knowledge of recent projects and exchange the latest thinking in health care environments through an established network of international specialists, conferences and industry forums. We are committed to improved outcomes through research and the application of evidence based design. MA have the capacity to work on projects from small-scale clinics to large hospitals. Our teams of architects and designers have experience in working on acute, primary care, mental health, and community social care facilities. We draw together a range of skills including health care planning, architecture, interior design, landscape design and research. We acknowledge the success of a project relies on teamwork and through collaborative relationships with consultant teams have delivered numerous projects within budget and on time through a range of construction techniques and procurement routes. Medical Architecture Raechal Ferguson, Director Raechal grew up in New Zealand and moved to London in 1987. She has 25 years experience in Architecture previously in designing private housing in New Zealand and later community projects and public housing before joining MA two years after it was formed in 1993. A directorship followed in 2000. Raechal specialises in the design and medical planning of health facilities, particularly mental health, including as a consultant to other architectural practices. She is responsible for the delivery of all the major projects in the office, including national and international award winning health facilities in the UK. She now regularly visits Australia as the Director in Charge of a new health facility in Adelaide a commission for MA to carry out full architectural services with local architects Swanbury Penglase.

Mungo Smith, Director Mungo has 30 years of experience in the health care sector. During the 1980’s he was project architect for the pioneering Community Care Centre in London with Edward Cullinan Architects. He then became Head of Design at the Medical Architecture Research Unit (MARU), University of North London. In 1991 Mungo became a founding Director of MA. Whilst he continues to be actively involved in design projects he leads MA’s research consultancy undertaking work with the UK Department of Health and is a member of their Design Review Panel. He is also regularly invited to speak on the design of health care environments in the UK, Europe and Australasia. Recently, he has been involved in developing the reference design for the New Royal Adelaide Hospital, South Australia and the latest master plan for the new Sunshine Coast University Hospital in Queensland.

Amanda Hervey, Director Amanda is a New Zealand Registered Architect (NZRAB) and has over 6 years experience in the architecture and building industry working with CCM Architects, Wellington. She has extensive experience in contract administration and project management working on numerous building types including commercial, residential, education and health care. She was involved in projects for both public and private health care facilities in Wellington, these include the redevelopment of Wellington Regional Hospital and Boulcott Hospital, Lower Hutt. Since moving to London and joining MA in 2009, Amanda has worked on the Glenside Campus Redevelopment in Adelaide, South Australia and the mental health developments in the UK ranging from internal refurbishment to new build developments.

Swanbury Penglase David Bagshaw - Director David has over 25 years experience as a senior architect, associate and director and has been responsible for designing and managing a full range of projects from preschool to tertiary, research, community, health and aged care, offices, residential; covering adaptive reuse, new build and heritage contexts. Many of the projects have been large and complex, involving a high level of consultation, negotiation and coordination. The outcomes always provide a sensitive and creative solution, responding to the specific context. This is a clear and recurring theme in David’s projects. Many projects have been recognised by awards, but more importantly the user feedback is always positive.

Witold Generowicz - Senior Associate: Architecture Witold has been with the firm since 1994 and has been involved in a wide range of educational, health care, residential and recreational projects. Witold has experience across all aspects of architectural practice, with a particular interest in brief formulation and concept development. He is highly consultative in his approach and very effective in dealing with complex client groups. Experience over recent years includes a new Early Learning Centre and a Performing Arts Centre at St Andrews School, new Mental Health and Substance Abuse Facilities at Glenside, aged care developments and large scale residential / recreational projects both locally and in China. Tim Conybeare - Senior Associate: Landscape Tim joined Swanbury Penglase in 2005 as a senior landscape architect. He has contributed to building a strong collaborative team environment in landscape and urban design within the office and continues to expand the breadth of landscape and urban design projects managed by the practice. He has particular strengths in working on complex urban projects and is committed to delivering outstanding design outcomes for a range of government and corporate clients. His leadership on the Glenside Campus Master Plan is highly valued as is his facilitation of the urban design and landscape design in the Health Precinct. He is an active member of the Institute of Landscape Architects and is a guest critic with the University of Adelaide. Tim is joint co-ordinator of the office training program and was appointed as a Senior Associate in 2010. Felicity Hope - Senior Associate: Interior Design Felicity has a diverse portfolio in both large and small scale projects delivering design and management expertise in Interior Design and facilities planning. Felicity is involved with both Government and Private Sector projects with a focus on education, hospitality and health. With a collaborative client approach, Felicity’s involvement in a project from concept design through to completion ensures continuity throughout all phases of the project and ensuring successful design outcomes. Joining Swanbury Penglase in 1999 and appointed as a Senior Associate in 2010, Felicity is committed to encouraging exploration and development within the Interior Design team to create functional and innovative design solutions.

Strategic Planning Medical Architecture has a great deal of experience in strategic planning of health care buildings and sites. We are engaged in planning and designing facilities for leading health care providers worldwide and collaborate with a network of consultants to draw on the latest thinking and international best practice as well as our research to deliver high quality and effective medical architecture. We have been engaged as lead consultant and trust advisor roles for PPP and other major health care initiatives in the UK and Australia including public sector comparator development. Director Mungo Smith sits on Department of Health design review panels to assess the quality of design proposals for health care developments in the UK. Our work spans from small, leading edge work on single bedroom ward layouts, through whole hospital building developments, up to hospital site development control plans and wider master planning strategies. Our experience in these diverse fields allows us to approach new projects informed from the very beginning both by broad site concerns as well as the detail of medical planning and strategy. It seems likely that more and more will be done at a local level. In an age with increasing citizen awareness of the need for sustainability and carbon emission reduction, the days of bringing patients onto the hospital site from some distance for events that could easily be staged locally may be numbered. Swanbury Penglase has wide experience working for both private and public sector clients on strategic planning, master planning and compliance audits for individual facilities as well as regional strategic planning. In particular, we have consulted for the South East Region Health Services in South Australia for over 10 years, carrying out numerous commissions including master plans for Naracoorte and Mount Gambier Health Services.

MA’s commitment to evidence based design stems from the Directors’ former involvement with the Design Projects Group at the Medical Architecture Research Unit (MARU), in London University. Since the company’s formation, MA has remained at the leading edge of health care design thinking and built a mature and wide-ranging health design knowledge base. Our background in research and innovation has maintained a 20 year on-going relationship with the UK’s National Health Service. We work with the Department of Health on the preparation of numerous health building notes, health technical manuals and guidance documents for acute hospital ward design, primary care centres and mental health facilities. For example we designed a single-bed pilot ward at Hillingdon Hospital. Commissioned by the Department of Health, to aid research as part of HBN04-01 ‘Adult In-patient Accommodation’ and by the Hillingdon Hospital NHS Trust. The pilot ward comprises of 3 different configured wings housing a total of 24 bedrooms with separate ensuite and focuses on single bedroom occupation as part of clinical research and monitoring by the Department of Health. Central to the initiative is the plan to test the prototype accommodation in use through an evidence based programme of research. The research evaluated the prototype unit alongside wards in the present hospital and other NHS hospitals for comparable groups of patients.

Fundamental principles included in the bedroom and ensuite design are accessibility and space for moving and handling patients. The project has incorporated evidence based design principles for increased patient safety drawn on research from the US. Space for visitors, carers and family has been incorporated into all bedrooms.


DESIGN

Master Planning Medical Architecture approaches the master planning of hospitals from both a health planner and architectural perspective. We keep abreast of emerging health care trends influencing hospital design and are regularly invited to evaluate new hospitals such as the St Olav’s Hospital in Trondheim, Norway. We draw upon director Mungo Smith’s international experience and knowledge of precedent and developing best practice. Mungo worked on the Sunshine Coast University Hospital master plan in Queensland, Australia (as pictured below). We advocate and promote master planning which integrates heath facilities into the community. A guiding principle of the SCUH plan was to develop a series of separate buildings on one site which connects with the local communities. Swanbury Penglase have extensive experience in master planning across a wide range of projects, drawing on substantial landscape and urban design expertise within the office to facilitate comprehensive master plan solutions. In 2008, under the leadership of Tim Conybeare, we prepared the Glenside Campus Redevelopment Master Plan, organising the site into 5 precincts. Precinct 1 is dedicated to integrated health facilities covering mental health and substance abuse. Swanbury Penglase and MA went on in 2009 -10 to develop a concept master plan for the health facilities themselves, adopting a ‘Village’ configuration whereby a group of separate buildings are arranged around a common central shared garden. A strong relationship has been achieved with the adjoining retail and commercial precincts by creation of a Village Green which connects the three precincts and preserves views through the site to significant trees. Public places are linked to form a continuous series of spaces complementary to the adjacent Adelaide Parklands and the facilities are configured to provide a continuous gradient of privacy and security for consumers, fostering autonomy and supporting a recovery focussed model of health care.


Urban Design Swanbury Penglase has extensive experience in the master planning and detailed design of major health and public domain precincts. We have a specialist design team that is capable of synthesising multiple user needs and requirements to achieve outcomes that are sustainable, elegant and which simply work. Of particular importance, our design philosophy is underpinned by a highly consultative design process and a commitment to sustainability and site appropriateness. Our approach draws on the latest national and international design thinking and we regularly collaborate with specialist urban planning consultancies for these often complex projects. The Glenside Campus Redevelopment Master Plan proposed a visionary step forward from the routine upgrades that had been made to the campus over the past 50 years. Taking into consideration the important heritage assets of the site, the master plan proposed a village style redevelopment of the campus that aligned with South Australia’s broader objectives for the delivery of a decentralised and community integrated mental health services network. The plan proposes new land uses being brought onto the site including medium density residential development, commercial and retail development and transforms the core heritage listed buildings into an arts and cultural precinct. These new activities, linked by a network of public roads, footpaths and parklands create a vibrant and stimulating hub to complement the health facilities that are to remain on the campus. Working closely with urban design specialists from Sydney, Swanbury Penglase has completed the detailed design of all public domain components of the master plan with an emphasis on the creation of real streets that tie into the surrounding residential street network and simple functional parks that provide amenity for both health consumers and the general public.

Landscape We aim to create landscaped environments that support and enhance healing while ensuring that staff can work effectively. Visual and physical access to nature has been proven to significantly reduce stress levels with beneficial effects in many areas of health recovery and is therefore the priority of our external environment design approach. We are committed to contemporary and environmentally appropriate landscape architectural and urban design services. In particular, we apply our expertise in horticultural knowledge to suit prevailing site conditions, approaching design problems using contemporary sustainable design technologies. As every site and client are unique, our project methodology is always tailored to the individualities of each, founded on a detailed and complete approach to site assessment and analysis, brief development and client liaison. This comprehensive approach to design has yielded a diverse client base and a track record of significant landscape design projects in the private residential and commercial sector as well as the public domain for both Local and State Government agencies.

DESIGN

Interior Design The interior design of our health care facilities reflects a balance between the clinical function and social purpose. Spatial legibility and way-finding is pivotal. Health care building design is often burdened by issues of security, zoning and functional compartmentalisation. We aim to create special places that are attractive, calming and memorable to the individual to assist orientation and promote healing. We create interior spaces that are both intuitive and functional whilst being reflective of the user’s aspirations. Our interiors are developed hand-inhand with the architecture and landscape, each discipline informing the other, providing continuity from the outside in and vice versa. The interior philosophy for the new Glenside Mental Health Facilities has been developed to complement the orientation of each building on the site and translates the colour philosophy to reflect its Adelaide region i.e. Adelaide Hills theme, Coastal theme, Northern Plains theme. The colour schemes for each building will be selected to represent the region and create a unique sense of place and identity for each building with a theme that distinguishes one from the other. The concept for each building is to have a neutral/natural canvas with colour introduced in subtle ways to fixed furniture, loose furniture, walls, floors and graphics. To help distinguish between the various zones within each unit, the level of colour saturation will be used to help define the nature of the spaces, stronger bursts of colour in more public and active zones scaling down to softer colour use in residential zones. We adopt a collaborative process from the outset, and acknowledge the direct interaction the user has with their interior spaces.

Heritage Swanbury Penglase have widely acknowledged expertise in heritage architecture, both in conservation and adaptive reuse. We have a solid body of award winning work to our credit, including a number of prominent buildings in Adelaide such as Bonython Hall, Elder Hall, the Torrens Building and Carclew. We have demonstrated outstanding ability to reconcile clients’ requirements with heritage constraints by way of sensitive interventions. The University City Project in the Torrens Building in central Adelaide exemplifies our approach. The project involved working closely with SA Government through the Department of Premier & Cabinet and the Department of Transport Energy and Infrastructure. The facility needed to be delivered within a short timeframe and was completed in 2 phases utilising many remote access technology sessions and fast track managing contractor procurement. By highlighting and working within the existing heritage context, while introducing sympathetic contemporary elements in stark contrast to the original features, an ideal environment has been created for educational purposes that reflect forward looking contemporary thinking in a traditional context. The finished project reflects the history and tradition of both the Torrens Building, Carnegie Mellon and University College London who have established boutique post graduate facilities on the Campus.

Acute Hospitals Medical Architecture is engaged in planning and designing facilities for leading health care providers worldwide. We collaborate with a network of consultants to draw on the latest thinking and international best practice as well as our research to deliver high quality and effective medical architecture. As an example, MA was engaged by the South Australian government as the international architects in the exemplar design for the replacement of the existing Royal Adelaide Hospital that will be delivered under a Public Private Partnership model. Opening in 2016, it will become the cornerstone of South Australia’s Health Care Plan and the largest hospital in South Australia. The new RAH will lead cultural change in the South Australian health system and promote innovation in health care delivery, education and training. The aim has been to bring together a modern approach to departmental organisation and workforce planning with a very high quality patient environment and low energy, ecologically conscious design. This work represents ‘state of the art’ current hospital design. This is the result of benchmarking against global Acute hospital designs and brings together a leading team of specialists from around the world.

Local Hospitals The creation of the Treatment Centre in Kidderminster in the UK formed an essential element of national and local strategy to meet the aim of delivering high quality modern health care and was the 1st hospital in the UK with 100% single bedrooms. The Centre provides a ‘walk-in, walk-out’ service that includes a one-stop clinic; high-tech diagnostic facilities; outpatient clinics; day surgery and a primary care centre; a local injury centre; and a rehabilitation service. New design concepts often emerge from our involvement in international design competitions, speaking at conferences and engagement in peer review. One example is our award-winning design concept for a Local Hyper Modern Hospital. This theoretical project was based on an examination of the configuration of health services in Barnstaple, North Devon, UK. The aim was to regenerate an affordable and sustainable “gold standard” of acute health care service situated in a smaller local community. This led to winning the Best Future Design Concept award at the UK’s Building Better Healthcare Awards in 2007. This project now forms the basis of our new publication “Providing Acute Care Locally - The (Small) Hypermodern Local Acute Hospital”. Co-authored with Durrow Management and NSW Architects, the book offers an in-depth look into the challenges faced by the health service and the acute hospital concept and proposes a solution. The concept focuses on organising medical staff to use space in a more flexible way, creating a smaller, more efficient hospital that is affordable, where an otherwise traditional local Acute Hospital would not be viable. Primary Care MA has a long history of involvement in Primary Care, both in research and practical implementation. We have built since 1990 a number of health centres, surgeries and primary care centres within larger hospitals, based on the health care planning principles included in UK published guidance, of which research MA was involved in. We have been engaged as consultant to the NHS for the preparation of standardised design guidelines (HBN) for Local Health Care Facilities. During the development of the documentation in 2000 our role expanded to develop guidelines for the new LIFT initiative a key component of shifting from a hospital driven system to a primary care led health service. The UK’s NHS LIFT is a vehicle for improving and developing frontline primary and community care facilities allowing Primary Care providers to invest in new premises in new locations, giving patients modern integrated health services in high quality, fit for purpose primary care premises. The one-stop-shop principle is an important component of NHS LIFT - allowing the patient to be treated in their locality. The commissioning of a new family health care centre in the Birmingham, UK suburb of Small Heath offered an imaginative opportunity to create a new kind of facility. This brought together, for the first time in England, a health centre, flexible space for use by other medical services, including GPs and a community mental health facility, offering both day and residential care with 14 acute beds each in single rooms. Small Heath is a residential area with a large Asian population, many with origins in Bangladesh or southern India. This required a particular understanding of the special health and cultural needs of the Muslim community. Neighbourhood consultation, including discussions with the local Imam, led to greater local commitment to the new centre.

Mental Health MA has been designing award winning mental health units since 1991, over 35 built or on site, ranging from older adult, acute including assessment, intensive treatment units (PICU), forensic including medium and low secure, learning disabilities, autism, children and adolescent services. In order to achieve therapeutic effectiveness, we seek to create an environment that is: •

Protective - emphasis on individual and group safety and security and providing an environment for people who are vulnerable, at risk from self harm or a danger to others and allowing flexible separation of different patient/client groups.

Homely - providing opportunities for privacy as well as social contact, indoors and out, in order to facilitate self-development.

Secure - a structured and supportive setting. The challenge for designers is to reconcile the essential safety and security measures within a domestic setting, without resorting to bland or superficial architectural references.

Key to our innovation in design is that we generate high quality, bespoke, robust and affordable buildings which enable and support clinical excellence; allowing these buildings to be flexible and adaptable in response to future changes in guidance and clinical practice. Our mental health buildings continue to attract international interest for their flexible layouts, use of natural daylight and ventilation and integration of safe, secure gardens and courtyards and sustainability.

Traditional inpatient units result in bedrooms that are disaggregated with no flexibility between bed areas and a centralised day space with potential restricted outlook. Internal corners are difficult to plan and dead end corridors compromise security and safety. Dispersed outdoor space is difficult to observe and privacy in bedrooms compromised. The same accommodation can be reconfigured by placing bedrooms at the perimeter to form a protective edge while integrating gardens into the heart of the building. This can produce a more flexible layout, while introducing greater levels of natural light into the building and increasing views outwards.

Community Health & Social Care Community health care buildings are designed with the need to tailor services to a specific locality whilst providing flexible accommodation to cater for future changes in health and social care. Careful planning and good design are essential if the most is to be made of limited resources. Solutions continue to be brought to bear as a complement to new developments in health care policies. MA’s experience in community health projects stems back to the early nineties, when we designed the Small Heath Health Centre, Birmingham. Since then we have worked on numerous community health projects including De Crespigny Park, 10 flats designed for life, a supported accommodation project and Carrickore Children’s Home, in Newry, Northern Ireland, designed by Donnelly O Neill Architects with a health care planning role by MA. A recent new-build treatment centre, award winning Rose Lodge, provides specialist short-term inpatient assessment and treatment for working-age adults with a range of learning disabilities and mental health problems. The scheme offers 12 bedrooms for three different user groups: Acute, Complex Needs and Ready-to-Go.

Swanbury Penglase have recently completed the Port Hedland hostel development for Aboriginal Hostels Limited which will provide medium to long term accommodation for people from indigenous communities in the Pilbara Region receiving renal dialysis treatment at Port Hedland Regional Hospital. The hostel will provide supported accommodation for 40 residents, as well as accommodation for staff, visitors and families and all associated service facilities including kitchen, laundries and administration.

Aged Care Swanbury Penglase has over 15 years experience in aged care, designing numerous successful facilities across the full range of the sector, including dementia and high care facilities as well as low care and independent living units. Aged care facilities must have a high degree of legibility and afford opportunities for a wide range of behaviours and activities. Effective layouts are built on a combination of walking routes and sitting places that flow seamlessly between indoor and outdoor environments. We frequently take on projects involving facilities that remain fully operational throughout the renewal process, which requires careful planning of the project both in terms of design and implementation. The Allambi Aged Care Facility for instance was carefully staged over a four year period to ensure minimal disruption to residents and staff. Completed early in 2011, it provides 117 beds, ranging from High Care Dementia through to Low Care. Aged Care facilities need to cater to a wide range of resident needs and capabilities. We are ever vigilant for opportunities to create varied and stimulating environments. We seek to complement generous central outdoor and indoor living spaces with a range of small spaces and secluded areas, dispersed along continuous walking loops to provide a wide choice of experience. We also use differentiation of garden spaces to add environmental richness and opportunities for views or visual and tactile enjoyment.
