อาการวิทยา (Symptomatology) 1. หลักการและเหตุผล ผู้ป่วยจะมาหาแพทย์ด้วยปัญหาอาการต่าง ๆ จากอาการเหล่านี้ แพทย์ต้อง พยายามคิดหาทางสืบค้นให้ได้ต้นเหตุ คือ การวินิจฉัยโรค จึงจะให้การบําบัดรักษา ได้ ตรงเป้าหมาย กระบวนการนี้อาศัยการคิดอย่างสมเหตุสมผลในแต่ละขั้นตอน (clinical reasoning process) จึงเห็นสมควรจัดให้นักศึกษาได้ฝึกหัดคิดนำ�ความ รู้ทางทฤษฎีมาประยุกต์เมื่อเผชิญกับปัญหาอาการผู้ป่วย ซึ่งจะเป็นการเตรียมตัว ให้นักศึกษาคุ้นเคยกับแนวทางการปฏิบัติต่อผู้ป่วยในชั้นคลินิก เป็นการช่วยให้ นักศึกษาสามารถปฏิบัติงานด้านคลินิกได้ราบรื่นขึ้น

2. วัตถุประสงค์ เมื่อผ่านการเรียนรู้แล้ว นักศึกษาสามารถ 1.อธิบายกลไกการเกิดอาการในปัญหาตัวอย่าง (Pathophysiology, pathogenesis) 2.ตั้งสมมติฐาน จำ�แนกสาเหตุของอาการตัวอย่าง (Hypothesis generation) 3.บอกข้อมูลที่ต้องการการค้นหาเพิ่มเติม (Data gathering) เกี่ยวกับ 3.1รายละเอียดของอาการตัวอย่าง 3.2ข้อมูลอื่น ๆ จากประวัติและการตรวจร่างกาย 4.วิเคราะห์ข้อมูลในกรณีผู้ป่วยตัวอย่าง เพื่อสรุปการวินิจฉัยโรคของอาการนั้น (Hypothesis refinement) 1

ในแต่ละชุดจะมี 2 อาการ รวม 9 ชุด ดังนี้ 1. Fever / weight change & weight loss 2. Chest pain / syncope 3. Cough / hemoptysis & dyspnea / PND 4. Dysuria / hematuria & edema 5. Abdominal pain / abdominal swelling 6. Diarrhea / vomiting 7. Jaundice / GI bleeding 8. Delirium / insomnia 9. Headache / coma

แต่ละ symptom อาจมีกรณีผู้ป่วย 1 - 2 ราย เช่น Diarrhea

1. watery diarrhea

2. mucous bloody diarrhea

Weight change 1. weight loss 2. weight gain

Abdominal swelling 1. mass 2. fluid

Fever 1. acute febrile illness 2. prolonged fever

ในแต่ละ case จะประกอบด้วย ส่วนที่ 1 Scenario สั้น ๆ ตามด้วยคําสั่ง หรือ คําถามเกี่ยวกับ

1. Causes (hypothesis generation / differential diagnosis)

2. Pathogenesis / pathophysiology / mechanism

3.History questions & physical examination (data gathering) ส่วนที่ 2 ให้ข้อมูลเกี่ยวกับผู้ป่วย (ประวัติ และตรวจร่างกาย) แล้วให้นักศึกษาวิเคราะห์ ข้อมูลที่ได้ เพื่อนำ�มา refine hypotheses ที่ตั้งไว้

นักศึกษา 32 กลุ่ม ๆ ละ 5 - 6 คน แต่ละกลุ่มรับผิดชอบกรณีผู้ป่วย 1 ราย ในการค้นคว้า เพื่อให้บรรลุวัตถุประสงค์ และนําเสนอในที่ประชุมกลุ่มใหญ่ นศพ. 4 กลุ่มใหญ่ / อาจารย์ 4 คน

นศพ. กลุ่มละ 45 - 46 คน / อาจารย์ 1 คน

นักศึกษาจะได้รับแจก case ส่วนที่ 1 ล่วงหน้า 30 นาทีแรก นักศึกษาแต่ละกลุ่มลงทะเบียนเข้าเรียน วิเคราะห์ปัญหา

ส่วนที่ 1 ตามกลุ่ม ตอบคำ�ถามล่วงหน้า และรับ case ส่วนที่ 2

ให้วิเคราะห์ต่อไป เพื่อนำ�เสนอ นาทีที่ 31-210 นักศึกษานำ�เสนอในที่ประชุมกลุ่มใหญ่ Symptom ละ 60 นาที

โดยแบ่งเป็นเวลาในการนำ�เสนอ และอภิปราย 40 นาที

(case ละ 20 นาที ถ้ามี 2 cases)

แล้วสรุปโดยอาจารย์อีกประมาณ 20 นาที

4. การประเมินผล

อาจารย์ประจำ�กลุ่มเป็นผู้ประเมินการนำ�เสนอของนักศึกษา เพื่อคิดเป็น ส่วนหนึ่งของคะแนน ในการอภิปรายแต่ละ case อาจารย์จะสุ่มเลือกนักศึกษาทีละ 1 คน จากนักศึกษา 11-12 คน ที่รับผิดชอบ case นั้น เป็นผู้นำ�เสนอหรือตอบคำ�ถาม นักศึกษาทุกคนจะมีโอกาสได้รับเลือกให้มานำ�เสนอเนื้อหาส่วนใดก็ได้ จึงควรเตรียม พร้อมทุกคน อาจารย์จะประเมินการนำ�เสนอโดยดูจากเนื้อหาที่นำ�เสนอและการตอบ คำ�ถาม แล้วให้คะแนนตามกลุ่มย่อย โดยอิงตาม rubric score ดังนี้
