The Endo Crown: Dr Paul Moore BDS Gate Clinic Galway 091 547 592 The upper right first molar has been root treated and crown recommended to prevent structural collapse of the remaining tooth structure. A root treated tooth is more likely to fracture unless protected with a full “biting� surface to dissipate biting forces.

Here the filling has been removed and the tooth prepared, reducing the height of the tooth to allow adequate thickness of porcelain. With a large internal surface area to bond the “E-Max� porcelain restoration to we can preserve the outer and inner natural walls of enamel and avoid: 1. Entering the delicate root structure with traditional metal posts which may weaken the roots. 2. Removing more circumferential tooth structure than necessary. Normally all the enamel is removed in a full crown preparation. 3. Taking the edge of the crown below the gum margin and compromising oral hygiene. Dr Paul Moore BDS

Care is taken to keep smooth margins and a digital image is taken of the teeth to generate a “Virtual Model” on the Cerec computer screen. This allows us instant review of the preparation and we can then build our new tooth with reference to the neighbouring and opposing teeth. The “impression” is accurate to a margin of 10 microns. ( the average human hair is 90 microns).

The software allows us to visualise the internal fitting surface, the pattern of bite against the opposing teeth and the “contact point� ( where the new crown will meet the neighbours ). Giving us an accurate bite and flossing point between teeth prevents food trapping and allows good bacteria control.

The shape of the tooth established, the new porcelain crown is “cut” from a solid block of dense porcelain. E-max ( Lithium Di-Silicate). This is FOUR times stronger than the traditional porcelain. The extra strength allows us to combine the “post and core” and crown into a one piece restoration. As you can see from the x ray the “cement” is highly visible and the crown is translucent to the x rays. This allows us to monitor the restoration over the following years. Dr Paul Moore BDS

Before and After. Keeping the shape and providing a new durable “table-top� to the tooth, this restoration should have a 95% success rate over the next 25 years!. The colour blends with the preserved natural tooth and the root canal treatment is protected. Completed in one visit, two hours.
